본 논문은 전동 보행보조기의 편의성 향상을 위한 최적의 제어 이득을 찾고 제어 알고리즘을 설계하였다. 최근 노인의 인구가 의료 기술의 발달로 급속히 증가되고 있으며, 다양한 이동형 보행보조기구는 삶의 질을 개선하기 위해 개발되고 있다. 이러한 이동형 제품 중 수동형 보행보조기는 노인의 보행의 힘을 도와주는 전동모터를 가지고 있지 않아 노인들이 충분한 근력이 없기 때문에 경사로 및 문의 문턱 등 고르지 않는 지형에서 이동하는데 제안사항이 있다. 이러한 상황을 극복하기 위해 전동타입의 보행보조기를 개발하였다. 전동형 보행보조기는 수동형과 다르게 사용자가 조작을 해야 모터가 구동되는 구조이다. 이러한 구조는 사용자가 조작을 잘못 하였을 경우 상당한 불편함을 사용자에게 준다. 이러한 전동형 보행보조기를 수동형 보행보조기와 비교하여 조작의 편의성을 판단하고 기준을 만들어 사용자의 편의성에 대하여 제어 값을 변경하면서 사용자 편의성을 개선하였다. 본 논문에서는 보행의지를 인식하고 편의성의 성능을 측정할 수 있는 방법인 햅틱 센서를 소개하며 편의를 개선하는 제어 알고리즘을 제안한다. 또한 편의성의 평가는 COV(Center of Vehicle)와 사용자의 COP(Center of Position)과의 차이의 변화를 통해 상대적인 편의성을 평가하였다. 전동 타입 보행보조기와 새로운 측정방법을 도입하여 모든 과정을 실험을 통하여 확인 하였다.
본 논문에서는 전동휠체어용 압력분포 측정시스템을 개발하고, 실제 휠체어 사용자들을 대상으로 동작분석과 연계한 압력분포 변화실험을 통해 유의성을 검증하고자 하였다. 측정시스템은 휠체어 이동방향에 따른 압력분포 변화 측정을 위해 64ch의 시트쿠션 FSR 센서신호 및 좌 우 각 30ch의 팔걸이 FSR 센서신호를 2ch의 좌 우 바퀴형 엔코더 신호와 동시에 12bit, 10Hz의 샘플링 속도로 획득할 수 있도록 개발하였으며, 이 신호들을 USB 인터페이스를 통해 휠체어 뒤쪽에 장착 가능한 노트북PC로 전송 후 실시간으로 화면표시, 저장, 재생을 수행할 수 있는 실시간 신호처리 소프트웨어를 개발하였다. 개발된 측정시스템을 사용한 압력분포 변화 실험은 실제 휠체어 사용자 10인을 대상으로 적외선 마커 기반의 동작분석실에서 3D 동작분석과 연계하여 실시하였다. 실험결과, 휠체어 진행 중 회전구간에서 몸이 회전의 바깥 방향으로 기울어지는 것을 확인할 수 있었으며 실험결과 및 실험 후 인터뷰를 통해 전동휠체어 설계 시 시트의 형태에 대한 개선요구사항을 획득할 수 있었다.
목적 : 본 연구의 목적은 국립재활원에서 운전인지평가(CPAD: Cognitive Percpetual Assessment for Driving)를 받은 뇌졸중 환자들의 현재 운전실태를 조사하고 CPAD와 안전운전과의 연관성을 알아보기 위함이다. 방법 : 국립재활원에서 CPAD평가를 받은 뒤 전화인터뷰에 응한 48명의 뇌졸중 환자를 대상으로 하였다. 의무기록자료를 통해 기본적인 정보와 CPAD점수, KMMSE점수 등을 취득하였으며 전화설문을 통해 운전실태에 대하여 조사하였다. 결과 : 48명의 응답자 중 12명이 현재 운전을 하고 있다고 응답하였고, 36명은 운전을 하지 않고 있었다. 운전을 하고 있는 사람의 CPAD 평균 점수는 하지 않는 사람의 CPAD 평균 점수에 비해 더 높았다(평균=54.13 vs 47.80, p<0.05). CPAD의 합격군이 불합격군보다 현재 운전을 하고 있을 확률이 더 높았다(OR=8.3, 95%CI=1.9331-35.558). 결론 : CPAD의 합격군에 속하는 사람이 불합격군에 속한 사람보다 현재 운전을 하고 있을 확률이 더 높았으며, 사고를 경험할 확률이 더 낮았다. 따라서 CPAD는 운전과 관련이 있는 시지각 및 기타 인지기능 평가도구로 사용될 수 있을 것이다.
본 연구는 장애인의 스마트폰 중독 실태 및 중독 특성을 알아보고 장애인의 스마트폰 중독 예방 및 해소를 위한 대응책을 제안하고자 하였다. 장애인 20~59세 총494명을 대상으로 스마트폰 중독 특성을 분석한 결과, 장애인 스마트폰중독위험군 비율은 12.55%였다. 장애인의 스마트폰, 모바일메신저, SNS 및 게임 사용시간은 2015년 인터넷 과의존 실태조사의 스마트폰 과의존 잠재위험군과 비슷하거나 더 높게 나타났다. 스마트폰중독에 관한 인식 분석 결과, 조사대상자의 2/3 정도가 스마트폰 중독이 심각하다고 생각하고 있었고, 중독관련 상담기관을 알고는 있었으나, 실제 도움을 받을 의향이 있는 사람은 그 절반 수준으로 나타났다. 이는 장애인을 대상으로 한 정책 홍보와 장애인을 위한 제도적, 구조적 기능을 갖추는 것이 필요함을 시사한다.

Objective: Sperm must be properly prepared in in vitro fertilization (IVF)-embryo transfer (ET) programs in order to control the fertilization rate and ensure that embryos are of high quality and have appropriate developmental abilities. The objective of this study was to determine the most optimal sperm preparation method for IVF. Methods: Patients less than 40 years of age who participated in a fresh IVF-ET cycle from November 2012 to March 2013 were included in this study. Poor responders with less than three mature oocytes were excluded. Ham's F-10 medium or sperm-washing medium (SWM) was used in combination with the density-gradient centrifugation/swim-up (DGC-SUP) or SUP methods for sperm preparation. A total of 429 fresh IVF-ET cycles were grouped according to the media and methods used for sperm preparation and retrospectively analyzed (DGC-SUP/Ham's F-10, n=82; DGC-SUP/SWM, n=43; SUP/Ham's F-10, n=181; SUP/SWM, n=123). Results: There were no significant differences among these four groups with respect to the mean age of the female partners, duration of infertility, number of previous IVF cycles, and retrieved oocytes. We determined that both the DGC-SUP and SUP methods for sperm preparation from whole semen, using either Ham's F-10 or SWM media, result in comparable clinical outcomes, including fertilization and pregnancy rates. Conclusion: We suggest that both media and both methods for sperm preparation can be used for selecting high-quality sperm for assistive reproductive technology programs.
Background: The purpose of this study was to suggest agenda priorities for research and development with physical therapy technology. Design: Cross-sectional study. Methods: A survey was consecutively done to 62 experts working in physical therapy research areas to investigate the priority of the categories. Paired T-tests and Importance-Performance Analysis using SPSS 21.0 package were conducted to find the differences between importance and satisfaction of the categories and the order of priority. Results: In results, it was developed that the research and development with physical therapy technology were divided with 18 categories. And the importance were significantly higher than satisfaction in 18 categories. Specifically, categories about assistive technology and robot rehabilitation were found to be on the high importance and lower performance quadrant. Conclusion: In sum, this study was meaningful in finding usage categories of research and development with physical therapy technology and investigating Importance-Performance differences for deriving research and development with physical therapy technology.
Purpose: The development of assistive devices has allowed for the performance of capsule endoscopy in children. Anticipating the capsule's transit time could affect the efficacy of the investigation and potentially minimize the fasting period. This study determined the predictors of small bowel transit time for small-bowel capsule endoscopy in children and adolescents with inflammatory bowel disease. Methods: We retrospectively examined children and adolescents with inflammatory bowel disease who underwent capsule endoscopy by the age 18 at a Japanese tertiary care children's hospital. Small bowel transit time predictors were analyzed using multiple regression with explanatory variables. Results: Overall, 92 patients, aged 1-17 years, with inflammatory bowel disease (63 Crohn's disease and 29 ulcerative colitis cases) were examined for factors affecting small bowel transit time. In the simple regression analysis, diagnosis, age, height, weight, serum albumin, general anesthesia, and small intestine lesions were significantly associated with small bowel transit time. In the multiple regression analyses, serum albumin (partial regression coefficient: -58.9, p=0.008), general anesthesia (partial regression coefficient: 127, p<0.001), and small intestine lesions (partial regression coefficient: 30.1, p=0.037) showed significant associations with small bowel transit time. Conclusion: Hypoalbuminemia, the use of general anesthesia for endoscopic delivery of the capsule, and small intestine lesions appeared to be predictors of prolonged small bowel transit time in children and adolescents with inflammatory bowel disease. Expecting the finishing time may improve examination with a fasting period reduction, which benefits both patients and caregivers.
Objective: Dual-task interference is defined as decrements in performance observed when people attempt to perform two tasks concurrently, such as a verbal task and walking. The purpose of this study was to investigate the changes of gait ability according to the dual task interference in chronic stroke survivors. Design: Cross-sectional study. Methods: Ten chronic stroke survivors (9 male, 1 female; mean age, 55.30 years; mini mental state examination, 19.60; onset duration, 56.90 months) recruited from the local community participated in this study. Gait ability (velocity, paretic side step, and stride time and length) under the single- and dual-task conditions at a self-selected comfortable walking speed was measured using the motion analysis system. In the dual task conditions, subjects performed three types of cognitive tasks (controlled oral word association test, auditory clock test, and counting backwards) while walking on the track. Results: For velocity, step and stride length, there was a significant decrease in the dual-task walking condition compared to the single walking condition (p<0.05). In particular, higher reduction of walking ability was observed when applying the counting backward task. Conclusions: Our results revealed that the addition of cognitive tasks while walking may lead to decrements of gait ability in stroke survivors. In particular, the difficulty level was the highest for the calculating task. We believe that these results provide basic information for improvements in gait ability and may be useful in gait training to prevent falls after a stroke incident.
난청인의 청력 손실은 개인별로 다양하므로 기존의 대칭적으로 주파수 밴드별 청각 필터를 구현하는 방법은 다양한 형태의 난청인의 청력 손실을 적절하게 모사해주지 못한다. 각 중심주파수와 음성의 입력크기에 따라 청각 필터의 형태가 비대칭적으로 바뀌고 청력손실이 있는 난청인은 청력 손실에 따라 청각필터의 형태가 정상인들과는 다른 형태로 바뀌게 되며 음질에도 차이가 있다. 본 연구에서는 난청인의 난청 정도에 따라 변하는 비대칭 청각 특성을 잘 반영한 청각필터를 구현하여 몇 가지 실험을 통해 각 구현된 청각 필터의 성능을 객관적으로 평가하였다. 실험은 구현된 청각 필터를 통한 음성의 perceptual evaluation of speech quality (PESQ) 와 log likelihood ratio (LLR)를 사용하였으며 그 값을 통해 처리된 음성의 객관적인 음질과 왜곡정도를 평가 하였다. 청력 손실을 주었을 때 대칭과 비대칭 청각 필터사이의 PESQ 와 LLR 값을 실험해 본 결과 청각 필터 간의 큰 차이를 보였다. 위 실험 결과들로 대칭과 비대칭 청각 필터의 형태에 따라서 음성의 음질에 영향을 받는다는 것을 알 수 있었다. 특히, 난청이 있을 때 중심 주파수별 청각 필터의 비대칭적 형태 변화가 난청인이 받아들이는 음질에 영향이 있었다.
무릎 슬리브는 재활뿐만 아니라 스포츠 활동 시 손상 예방의 목적으로도 사용되고 있다. 무릎 슬리브의 스포츠 손상 예방을 정의하기 위해서는 고유수용성감각으로부터 기인하는 균형을 평가하는 것이 중요한 요소이다. 본 연구의 목적은 골지 건 기관 자극 패드 무릎 슬리브의 효과를 분석하는 것이다. 5명의 건강한 성인 남성이 본 연구의 실험대상자로 참여하였다. 실험대상자들은 일반 무릎 슬리브와 자극 패드 슬리브를 착용한 상태에서 각각 BIODEX에서 설정된 타겟(Target) 각도를 재연하였고, 한 발 서기 동작을 수행하였다. 각도 재연성 평가의 결과를 보면, 5명의 실험대상자 중 4명의 실험대상자는 무릎 슬리브를 착용했을 때 보다 자극 패드 슬리브 착용 시 %Target angle이 증가하는 결과를 보였다. 한 발 서기 동작 수행 시 무릎 슬리브 착용 시 보다 자극 패드 슬리브 착용 시 유지 시간(duration time)이 증가했으며 x, y 방향에서 COP 길이가 모두 감소했다(COPx - 무릎 슬리브: $162.06{\pm}58.99mm$, 자극 패드 무릎 슬리브: $149.03{\pm}45.30mm$; COPy - 무릎 슬리브: $310.79{\pm}115.89mm$, 자극 패드 무릎 슬리브: $291.57{\pm}76.53mm$). 초기 전이구간, 정적 구간, 말기 전이구간이 차지하는 비율은 무릎 슬리브 착용 시와 자극 패드 무릎 슬리브 착용 시에 유의한 차이를 보였다 (all, p < 0.05). 본 연구의 결과로부터 골지 건 기관 자극 패드 무릎 슬리브 착용이 고유수용성 감각과 균형에 긍정적인 영향을 미치는 것을 확인할 수 있었다.
