Resibufogenin induces cardiac arrhythmia

  • Resibufogenin is a single compound isolated from the skin venom gland of the toad (Bufo bufo gargarizans cantor). Formulations containing toad venom have been widely used as complementary and alternative medicines. However, like digitalis, resibufogenin possesses both pharmacological and toxicological activities. Our previous data indicated that resibufogenin induces electro-toxicity, including delayed afterdepolarization and triggered arrhythmias at high concentration, both in cardiac fiber in vitro and in beating heart in vivo.

  • The venom gland of Toad contains large quantities of cardiac glycosides, and toad venom poisoning is similar to digitalis toxicity and carries a high mortality. Sometimes after ingestion of aphrodisiac pills which contain dried toad, a patient develops gastrointestinal symptoms and bradycardia, psychoneurologic symptoms. We have experienced 2 cases of toad venom intoxication, who ingested asian toads. Patients were presented to our ED with nausea, vomiting, and abdominal pain. The patients were peformed monitoring and conservative treatment and were fully recovered. Toad venom intoxication should be considered in patients with clinical manifestation of gastrointestinal irritation, cardiac arrhythmias, hyperkalemia, and detectable serum digoxin level without current medication of digoxin.

  • The plant Korean rosebay are members of the Ericaceae(Heath) family and may contain grayanotoxins, which exert toxic effects by binding to sodium channels in cell membranes. We had experience of 2 cases of Korean rose bay intoxication, who exposed to Rhododendron mucronulatum. Patients were presented to our ED with nausea and vomiting, hypotension, and dizziness or depressed mentation. The patients were performed monitoring and supportive treatment and were fully recovered within 48hours. Korean rose bay intoxication should be considered in patients with clinical evidence of gastrointestinal irritation, cardiac arrhythmias, and/or neurologic signs without identifiable causes, especially in spring or early summer.

Therapy for Postoperative Cardiac Arrhythmia in Patient with Mitral Valve Surgery (승모판막 수술 환자에서 발생한 부정맥의 치료)

  • This is a clinical review of the results from electric cardioversion and pharmacological therapy used in our hospital for reverting cardiac arrythmia in patients with mitral valve surgery between Jan. 1990 and Jun. 1991. Of 62 evaluated patients, 16 patients had regular sinus rhythm and the other 46 had arrhythmias [42; atrial fibrillation 1; atrial flutter 1; premature ventricular contraction] preoperatively. In 2 of patients with sinus rhythm, atrial fibrillation newly developed after surgery and was converted into sinus rhythm soon by intravenous administration of digoxin. Remaining 14 patient resumed sinus rhythm spontaneously. In patients with preoperative arrythmia, 3 patients reverted into sinus rhythm from atrial fibrillation by electric cardioversion at operative field, 1 patient by lidocain and mexiletine, 4 patients by combined use of digoxin and verapamil, 4 patients by 2 times of oral quinidine and 9 patient by long term use of oral amiodarone. Throughout this consecutive trials of anti-arrhythmic drugs and electric cardioversion, Conversion into normal sinus rhythm occurred in 48% of patients with arrhythmia developed after mitral valve surgery.

  • Modified coaxial-slot antenna for minimally invasive microwave thermal therapy for liver tumor is studied in this paper. Minimally invasive microwave antenna in medicine are applied for hyperthermia for medical treatment for cancer, cardiac catheter ablation for ventricular arrhythmias treatments, microwave treatment of Benign prostatic hypertrophy, and so on. Microwave hyperthermal ablation for liver tumors is expected for enthusiasts as an alternative to curative surgical resection. Tumors have to heated up to 60 degree C to coagulate .cancer cells but less than 100 degree C to avoid evaporation. Temperature dependence of properties of the tissues should be considered for wide range of treatment. Electrical properties of liver tissue were measured for different temperatures. SAR distribution around the antenna into the liver are simulated using Remcom's XFDTD.

  • Data collected to date indicate that sleep apnea syndrome is a risk factor in the development of cardiovascular dysfunction such as systemic hypertension, cardiac arrhythmias, pulmonary hypertension, right heart failure, angina, myocardial infarction, and stroke. Several reports suggest that untreated patients with sleep apnea syndrome have a higher long-term mortality than treated patients and than normal subjects. But there are many controversies concerning the treatment of sleep apnea syndrome, and there is no universally accepted standard for effective treatment. We reviewed the current approaches and options in the medical treatment of sleep apnea syndrome.

  • Although amiodarone is generally regarded as safe with a low incidence of associated arrhythmias, torsade de pointes (TdP) has been observed usually in the presence of predisposing factors. We report a case of amiodarone-induced TdP after long-term administration of alow dose of oral amiodarone in the absence of predisposing factors.

  • Isolated congenital aneurysm of the left atrium with intact pericardium is a rate anomaly, which usually presents with arrhythmia, cerebral embolism or abnormalities on routine chest X-ray. Surgery is indicated in most cases to eliminate a potential source of systemic emboli and arrhythmias. A 42-year-old woman having cervical cancer, she was suspected of having a left atrial aneurysm on review of chest X-ray and confirmed by echocardiography and cardiac catheterization. Surgical resection of Left atrial aneurysm was achieved without complication using median sternotomy with cardiopulmonary bypass. The postoperative course was uneventful.

A study of the electrical cardiac pacemaker model (폐순환계의 모델화에 관한 연구 2)

  • The electrical cardiac pacemaker model described in this paper simulates the most important functional properies of cardiac pacemaker cells. It is a minimum-parameter model which has a simple relaxation oscillator circuit as its main element. The electrical cardiac pacemaker model is analyzed in detail in order to show that its characteristic is similar to that of cardiac pacemaker cells. The main feature of the model is the possibility of controlling the time course phase 4 depolarization, the threshold level and the maximum level of repolarization, the rate of cardiac pacemaker. Emphasis is placed on phenomena of acceleration and frequency entrainment. This particular pacemaker model is very useful for the study of interactions between cardiac pacemakers and the description of the mechanism of arrhythmias.

  • Obstructive Sleep Apnea(OSA) is a form of sleep disordered breathing(SDB) characterized by the occurrence of episodes of complete or partial upper airway obstruction during sleep that is often quantified as the apnea-hyponea index(AHI). It is increasingly being recognized that OSA is a public health hazard and there is increasing evidence that it is associated with an increase in morbidity. Early recognition and diagnosis of this condition may lead to earlier treatments (eg, CPAP, Oral appliances) with reduction of the risk of metabolic disease, cardiovascular diseases, such as hypertension, ischemic heart disease, arrhythmias and pulmonary hypertension.
