9주간의 자조관리.수중운동 프로그램이 골관절염환자의 통증, 유연성, 균형감, 피로감 및 자기효능감에 미치는 효과 (Effects of a 9-week Self-help Management.Aquatic Exercise Program on Pain, Flexibility, Balance, Fatigue and Self-efficacy in the Patients with Osteoarthritis)

  • Purpose: The purpose of the study was to identify the effects of a 9-week self-help management aquatic exercise program on pain, flexibility, balance, fatigue and self-efficacy in the patients with osteoarthritis living in the suburban area. Methods: Utilizing a quasi-experimental design, 43 osteoarthritis patients consisted of the experimental group (n=20) and the control group (n=23). Due to the busy schedule of the patients working in the farm, the program was provided for 9 weeks instead of 12 week program under the guidance of professional advice. The questionnaires and physical measurements were used to measure the outcome variables before and after the program. Data were analyzed with the SPSS WIN using descriptive statistics, t-test and ANCOVA. Results: The age was significantly different between the groups. After controlling for the age, pain and fatigue in the experimental group decreased significantly more than the control group. Flexibility, balance and self-efficacy in the experimental group improved significantly more than the control group. Conclusion: This 9-week program could be an effective nursing intervention to be used especially for osteoarthritis patients in the suburban area during the leisure seasons of the farmers to improve their arthritic symptoms and self-efficacy.

Effect of aquatic exercise on gait in persons with chronic stroke: a meta-analysis study in Korea

  • Objective: Based on the results of previous studies, it is necessary to analyze gait and discuss and present the effects of aquatic exercise for chronic stroke. The purpose of this study was to present objective data on the effect of aquatic exercise on the gait of persons with stroke by performing a meta-analysis. Design: A systematic review and meta-analysis. Methods: We performed a meta-analysis of 23 studies that investigated the effects of aquatic exercise performed between 2006 and 2017. The studies were searched on the basis of the participants, intervention, comparison, outcomes standard. The quality of the research method was assessed using a tool that can assess the risks posed by each study design. A meta-analysis software program was used to calculate the mean effect size, effect size by intervention, and effect size by outcome. We also performed a meta-regression analysis and an analysis of publication bias. Results: The mean effect size of the patients' gait was 0.65 (p<0.05). The largest effect size by outcome was observed at the 6-m walk test, followed by the 6-minutes walk test, 10-m walk test, and the walking equipment test (p<0.05). The meta-regression analysis showed that the effect size increased with increased duration, number, and length of sessions. Conclusions: Aquatic exercise appears to show a moderate effect on the gait of chronic stroke survivors. Meta-analyses on the effects of aquatic exercise in other patient populations are needed. This study suggests standard criteria establishments for the effect of aquatic exercise on the walking ability of persons with chronic stroke.

수중운동 프로그램의 효과에 대한 논문분석 (Analysis of Studies on the Effect of Aquatic Exercise Program)

  • Purpose: The purpose of this study was to analyze the effect of aquatic exercise and to identify directions for future research. Methods: Twenty one articles from 2 sites of article database (1990-2010) and 6 from Korean nursing journals were reviewed. Results: There were 20 quasi experimental design and 1 randomized controlled design. Major subjects of research were older women with arthritis. The most common type of exercise was Kim (1994)'s form. The most frequent duration of exercise was over 6 weeks. Outcome measurement was classified to physical and psychosocial variables. For physical outcomes, pain release was found in 13 articles among 15 articles, muscle strength improvement in 5 articles among 10 articles, flexibility improvement in 5 articles among 9 articles, and balance improvement in 3 articles among 6 articles. For psychosocial outcomes, improvement of quality of life was found in 3 articles among 7 articles and self-efficacy improvement in 5 articles among 7 articles. Conclusion : The effect of aquatic exercise over 6 weeks improves the physical and psychosocial functions. It is suggested that meta analysis needs to be done to integrate the various results.

Effects of Aquatic Exercise on Upper Extremity Function and Postural Control During Reaching in Children With Cerebral Palsy

  • Background: Despite the fact that aquatic exercise is one of the most popular alternative treatment methods for children with cerebral palsy (CP), there are few research regarding its effectiveness. Objects: The purpose of this study was to examine the effects of aquatic exercise on upper extremity function and postural control during reaching in children with CP. Methods: Ten participants (eight males and two females; 4-10 years; Gross Motor Function Classification System levels II-IV) with spastic diplegia were recruited to this study. The aquatic exercise program consisted of four modified movements that were selected from the Halliwick 10-point program to enhance upper extremity and trunk movements. The participants attended treatment two times a week for 6 weeks, averaging 35 minutes each session. The Box and Block Test (BBT), transferring pennies in the Bruininks-Oseretsky Test (BOT), and pediatric reaching test (PRT) scores were used as clinical measures. Three-dimensional motion analysis system was used to collect and analyze kinematic data. Differences in BBT and BOT values among pre-treatment, post-treatment, and retention (after 3 weeks) were analyzed using a Friedman test. In addition, the PRT scores and variables (movement time, hand velocity, straightness ratio, and number of movement units) from the three-dimensional motion analysis were tested using a Wilcoxon signed-rank test. The significance level was established at p < 0.05. When the results appeared to be statistically significant, a post-hoc test for multiple comparisons was performed with the Wilcoxon signed-rank test. Results: All clinical measures, which included BBT, transferring pennies of BOT, and PRT, were significantly increased between pre-intervention and post-intervention scores and between pre-intervention and retention scores after treatment (p = 0.001). Three-dimensional motion analysis mostly were significantly improved after treatment (p = 0.001). Conclusion: Aquatic exercise may help to improve body function, activity, and participation in children with varying types of physical disabilities.

수중걷기운동이 여성노인의 보행 및 평형능력에 미치는 영향 (Effect of Aquatic Walking Exercise on Gait and Balance Parameters of Elderly Women)

  • Objective: The purpose of this study is to analyze the effects of aquatic walking exercise on gait and balance parameters of elderly women. Method: 15 elderly people were recruited for this study (age: 73.20±5.19 yrs, height: 153.87±3.36 cm, mass: 60.33±5.73 kg). All variables were measured using Gaitview AFA-50. The variables were the heel contact time ratio, gait angle, and M/P change ratio for gait patterns and ENV, REC, RMS, Total Length, TLC, Sway velocity, and Length/ENV for balance abilities. A paired t-test and the Wilcoxon signed-rank test were carried out to verify the differences in the test scores after participating in the water walking program. The significance level for all statistical analyses was set to α=.05. Results: As for the changes in their walking function after the exercise, heel contact time ratio (p<.01) showed a statistical significance, while gait angle and M/P change ratio did not reveal statistically significant differences. In the test of balance ability on both feet and with eyes opened, statistical significance was found in ENV, REC, RMS, TLC (p<.01), and sway velocity (p<.05), while the test with eyes closed showed statistical significance in length/ENV as well as ENV, REC, RMS, sway velocity (p<.01) TLC, and total length (p<.05). As for the single-leg stance balance ability, ENV and REC revealed statistically significant differences. Conclusion: These results show that water walking is effective for improving the function of the ankle flexor muscles, providing stability to the ankle joint during walking and helping efficient walk. In addition, it is also expected to help prevent falls due to loss of balance by improving the stability of lower extremity muscles and trunk.

12주간의 수중운동이 노인 골관절염 환자의 동적평형성과 통증요인에 미치는 영향 (The Effect of 12-weeks Aquatic Exercise on Dynamic Balance and Pain Factors in Elderly Osteoarthritis Patients)

  • 본 논문은 관절염을 앓고 있는 노인여성(환자군; $65.25{\pm}5.9$세, 11명)과 일반 노인여성(통제군; $64.50{\pm}7$세, 11명)을 대상으로 12주간의 수중운동(주 3회)을 실시 한 후 동적평형성과 통증(통증자각도)의 변화를 알아보고자 실시되었다. 동적평형성은 시각정보의 유무에 따라 개안 시와 폐안 시로 나누어 측정하였다. 12주간의 수중운동에 따른 개안 시 동적평형성은 관절염 환자군(p<.001)과 통제군(p<.05)에서 유의하게 향상되었고, 폐안 시 관절염 환자군에서도 유의하게 향상된 결과를 보였다(p<.05). 또한, 통증자각도(VAS) 평가에 의한 통증 변화는 관절염 환자군의 10요인(푹신한 의자에 똑바로 앉아있기 힘든가, p<.01), 11요인(누워있을 때 통증이 덜해지는가, p<.05), 12요인(통증때문에 일상생활에 지장이 있는가, p<.05), 13요인(통증 때문에 직장일 또는 집안일에 지장이 있는가, p<.01)에서 유의한 통증 감소가 나타났다. 본 연구 결과를 통해 12주간의 수중 운동은 골관절염 노인 환자에게 개안 시와 폐안 시 모두 동적평형 능력을 향상 시키고, 일부 통증요인 감소에 효과적인 운동임을 알 수 있었다.

Effects of an Aquatic Exercise Program on Body Composition, Blood Components and Physical Fitness in the Elderly Women

  • 본 연구는 수중운동 프로그램이 지역사회 거주 노인여성에게 신체구성, 혈액성분 및 기초체력에 미치는 효과를 파악하기 위해 시도되었다. 수중운동 프로그램은 23명의 노인여성을 대상으로 회당 50분, 주 2회, 12주간 중정도의 운동 강도로 시행되었다. 자료 분석은 SPSS 프로그램을 사용하여 기술적 통계, 대응 표본 t 검정을 하였다. 본 연구 결과 수중운동 프로그램 적용 전에 비해 적용 후 체질량 지수(t=2.42, p=.024), 복부둘레(t=4.36, p<.001), 혈중 혈당(t=3.19, p=.004), LDL 콜레스테롤(t=3.33, p=.003) 감소에 유의한 효과를 나타내었다. 이러한 결과는 수중운동 프로그램이 노인여성에게 신체구성, 혈액성분에 긍정적인 효과를 보일 수 있는 효과적인 중재 방안임을 의미하며, 연령, 운동 강도를 고려한 수중운동 프로그램은 노인여성의 건강증진을 위한 방안으로 활용될 수 있겠다.

류마티스 관절염 환자의 통증 감소를 위한 프로그램의 효과: 체계적 문헌고찰 (Effect of Program for Pain Reduction in Patients with Rheumatoid Arthritis: A Systematic Review)

  • Purpose: The purpose of this study was to establish the foundations for the application of appropriate programs for pain reduction in patients with rheumatoid arthritis. Methods: The literature on patients with rheumatoid arthritis were found on an electronic search site for South Korean academic papers. The search terms "rheumatism" and "rheumatoid arthritis" were used independently. The combined terms "rheumatism and pain" and "rheumatoid arthritis and pain" were also used. For the meta-analysis, the R version 3.5.1 program was utilized. Results: The meta-analysis of eight papers showed a large effect size of -4.11. The programs were most effective in the order of aquatic exercise, aromatherapy, self-help education, muscle strength exercises, and tai chi. Conclusion: This study could provide the basis for presenting appropriate programs for pain management in patients with rheumatoid arthritis.
