Epidemiological Characteristics of Bicycle-Injured Patients (자전거손상 환자의 역학적 특성)

  • Backgrounds: According to a high interest in health these days, a bicycle is being highlighted as a means of leisure activity and auxiliary transportation means. Along with an increase in this interest, there is tendency that even the width of bicycle utilization gets diverse and that bicycle accident is also rapidly growing. Purpose: The purpose of this study was to suggest a prevention measure available for reducing occurrence of bicycle accidents by analyzing the Epidemiological characteristics according to age and injury severity in bicycle-injured patients. Methods: This study analyzed 115 materials suitable for this study by examining a medical record of patients, who received the hospitalized treatment owing to bicycle accident from January 1, 2011 to December 31 at one general hospital in S city. Results: The male-and-female ratio of bicycle-injured patients stood at about 7:3. The average age was 38.14(${\pm}21.59$) years old. The largest injury happened in 40-59 years old(31.3%). The averagely hospitalized period was 17.33(${\pm}18.12$) days. Injury severity was the ratio of 2:8 in a mild case versus a serious case. As for drinking and smoking characteristics according to age, the injury occurred the most in between 40 and 59 years old for people with drinking and smoking and in under 19 years for people without drinking and smoking. According to injury severity, the ratio of serious case got higher in the higher age. This was indicated(${\chi}^2$=15.675, p=0.001) to be statistically significant. Conclusion: In this study, a bicycle accident was indicated to have high injury risk in young children, adolescents, and old-age group. Prevention measure and publicity are demanded for reducing injury occurrence such as the prevention education for bicycle accident and wearing protective gear focusing on the ages of corresponding to groups with risk of injury occurrence. In addition, the security of safe movement route is necessarily demanded in order to increase application level as an auxiliary transportation means.

Biomarkers in Acute Kidney Injury (급성 신손상의 생물학적 표지자)

  • Acute kidney injury (AKI) can result in mortality or progress to chronic kidney disease in hospitalized patients. Although serum creatinine has long been used as the best biomarker for diagnosis of AKI, it has some clinical limitations, especially in children. New biomarkers are needed for early diagnosis, differential diagnosis, and reliable prediction of prognosis in AKI. Up to the present, candidate AKI biomarkers include neutrophil gelatinase-associated lipocalin (NGAL), kidney injury molecule-1 (KIM-1), interleukin-18 (IL-18), livertype fatty acid-binding protein (L-FABP), matrix metalloproteinase-9 (MMP-9), and Nacetyl-$\ss$-D-glucosaminidase (NAG). However, whether these are superior to serum creatinine in the confirmation of diagnosis and prediction of prognosis in AKI is unclear. Further studies are needed for clinical application of these new biomarkers in AKI.

A Case Report of Prescribing Gamisopunghwalhyeal-tang (jiameishufenghuoxue-tang) for the Three Patients with Traumatic Knee Joint Injury (가미소풍활혈탕(加味疎風活血湯)을 투여한 외상성 슬부 손상 환자 치험 3례)

  • Objectives: The purpose of this study is to investigate the clinical application of Gamisopunghwalhyeal-tang to three patients traumatic knee joint injury. Methods: Patients are hospitalized at Dept. of Oriental Rehabilitation, Bu-Chun Jaseng Oriental Medicine Hospital, diagnosed as traumatic knee joint injury and treated mainly with herbal medicine ; Gamisopunghwalhyeal-tang. This study was measured by VAS(Visual Analogue Scale) score and walking time and Western Ontario and McMaster Universities(WOMAC) index score. Results: After taking Gamisopunghwalhyeal-tang, the patient's pain was controlled and increased time of walking on floor after treatment. VAS & WOMAC score were decreased. Conclusions: As seen in this three cases of traumatic knee joint injury, Gamisopunghwalhyeal-tang has a positive effect to control pain with traumatic knee joint injury.

The Effects of c-Fos Expression on Ultrasound Treatment in Sciatic Nerve Crush Damaged Rats (초음파 치료가 좌골신경 압좌 손상된 흰쥐의 c-Fos 발현에 미치는 영향)

  • This study was performed to evaluate the effects of low-intensity ultrasound application to the peripheral nerve injury animal model on enhancement of nerve regeneration and functional recovery. Using aseptic microsurgical techniques, the sciatic nerve of adult male Sprague-Dawley rats was crushed at the outside of right mid-thigh for 30 seconds with fine forceps. Beginning just after surgery, various continuous-wave ultrasound treatments with intensities of 0.2 W/$cm^2$, 0.5 W /$cm^2$ and 1.0 W /$cm^2$ operated at 1 MHz or sham treatment were applied to the opposite inside of the crush site for 1 minute every other day with a transducer moving speed of 2cm/sec. For evaluation of the progress of sciatic nerve regeneration, c-Fos expression in the lumbar spinal cord (L4-5) dorsal horn was investigated. c-fos expression was markedly increased at 1hour after sciatic nerve crush injury, then gradually decreased thereafter. The c-fos expressions were significantly decreased (p<0.05) in all the experimental groups in comparison with the control group until 3days post-crush, and the degrees of decrease were higher in 0.5 W/$cm^2$ and 1 W/$cm^2$ intensity ultrasound application groups. It is suggested that low-intensity ultrasound application to an animal model of sciatic crush injury may suppress pain transmission and promote nerve regeneration, and which may result in delayed progress of muscle atrophy and accelerated progress of muscle recovery and eventually may result in accelerated and improved foot function recovery.

The comparative study of treatment method on adriamycin-induced extravasation injury (Adriamycin의 혈관외 유출 손상 후 치료 방법의 비교)

  • Purpose: Local skin necrosis after extravasation of adriamycin, a widely used chemotherapeutic agent, is a common problem in cancer patients. The extravasation of chemotherapeutic agents yields severe inflammatory responses, crust formation, skin necrosis, and ulceration. Even though several treatment options have been proposed for extravasation injury, there is still controversy regarding the management of such lesions. Thus the aim of this study was to compare the efficacy of saline injection(Group 1), hydrocortisone injection(Group 2), propranolol injection(Group 3) and early surgical excision as a treatment(Group 4) in a rat extravasation model. Methods: The authors planned forty mature male Sprague - Dawley rats were divided into 4 groups and each group contained 10 rats. Administration of adriamycin($1.0mg/m{\ell}$) $1.5m{\ell}$ by subcutaneous injection on the dorsal side of the rats was followed by protocol. The treatment options were applied 2 hours after adriamycin injection. At the end of the 5th days, the presence and size of ulcers at the injection site were measured. 3 weeks after injection, a histopathologic examination was performed for each treatment and control group. T - tests were used to analyze the differences between the measurements. Results: Propranolol significantly improved tissue recovery compared with control group and other groups. These data suggest that there is little role for saline and hydrocortisone in the treatment of adriamycin extravasation injury. Conclusion: In this study, we compared some treatment methods in adriamycin extravasation model. The findings support the propranolol injection may prevent extravasation injury. However this study was performed in the laboratory using rats, and the results could be different in clinical application. Thus, more needs further investigations and clinical application.

The Effect of Electrical Stimulation on MAP2 Expression in the Cerebral Cortex following Sciatic Nerve Crush Injury in Rat (흰쥐 좌골신경손상 후 전기 자극이 대뇌피질에서의 MAP2 발현에 미치는 영향)

  • The purpose of this study was to investigate the effect of electrical stimulation(EST) on MAP2(Microtubule Associated Protein 2) expression in cerebral cortex following sciatic nerve crush injury in rats. Twelve Sprague-Dawley adult female rats, six for control and six for experimental, were anesthetized and their sciatic nerves were crushed. The electrical stimulation (EST) was applicated with 3 Hz for 10 minuties in a day for muscles innervated sciatic nerve. The MAP2 expression in cerebral cortex was identified from immunohistochemistry against MAP2. The result of this study were as follow: 1) In control group, MAP2 immunoreactive neurons were observed but there no significant increase for 3 days. 2) MAP2 immunoreactive neurons were increased markably in experimental group than control group. 3) MAP2 immunoreactive neurons were increased markably after applicating with EST in sciatic nerve crush injury induced group from 2nd day. This study showed that the application of EST for muscles after sciatic nerve crushed injury made MAP2 immunoreactive neurons in the cerebral cortex increased. Therefore, the electrical stimulation on the peripheral site, denervated muscle, may facilitate MAP2 expression in the cerebral cortex.

Interpretation of Domestic Industry under Safeguard Agreement (세이프가드협정상의 국내산업 범위에 대한 해석)

  • This paper discusses the definition of the term "domestic industry" in relation to the application of the safeguards provisions of the WTO through the judicial interpretation made by the WTO Appellate Body and panel. The requirements for the imposition of safeguards include a rapid increase in import quantity, the existence of serious injury or threat of serious injury to the domestic industry, and a causal relationship between the increase in imports and the industrial injury. The domestic industry refers to the producers that account for a considerable portion of the total national production, or the national producers who produce articles "like" or "directly competitive" with the specific imports. Chronically, there have been controversial disputes relating to the interpretation of the term "like" or "directly competitive". Reviewing the disputes relating to the term "domestic industry" in application of the safeguards provisions since the establishment of the WTO, the interpretation of the term "like" has been made imposing weight on the physical characteristics of the products. This interpretation is in contrast with that of the interpretation of the term "directly competitive" which has been interpreted with imposed weight on the commercial elasticity of substitution which can be measured by the final use or consumer's taste.

Protective Effects of Seonpyejeongcheon-tang on Elastase-Induced Lung Injury in Mice (Elastase 매개성 폐조직 손상에 대한 선폐정천탕(宣肺定喘湯)의 보호효과)

  • Objectives : This study aimed to evaluate the protective effects of Seonpyejeongcheon-tang (SJT) on elastase-induced lung injury. Materials and Methods : The extract of SJT was treated to A549 cells and an elastase-induced lung injury mouse model. Then, various parameters such as cell-based cytoprotective activity and histopathological findings were analyzed. Results : SJT showed a protective effect on elastase-induced cytotoxicity in A549 cells. This effect was correlated with analysis for caspase 3 levels, collagen and elastin contents, protein level of cyclin B 1, Cdk1, and Erk1/2, and gene expression of TNF-$\alpha$ and IL-$1{\beta}$ in A549 cells. SJT treatment also revealed a protective effect on elastase-induced lung injury in mouse model. This effect was evidenced via histopathological findings, including immunofluoresence stains against elastin, collagen, and caspase 3, and protein levels of cyclin B1, Cdc2, and Erk1/2 in lung tissue. Conclusion : These data suggest that SJT has pharmaceutical properties on lung injury. This study thus provides scientific evidence for the efficacy of SJT for clinical application to patients with chronic obstructive pulmonary disease.

Regulatory Effects of Samul-tang on Axonal Recovery after Spinal Cord Injury in Rats

  • In oriental medicine, Samul-tang (SMT) has been used for the treatment of cardiovascular diseases and neuronal disorders. Here, possible effects of SMT on axonal regeneration after the spinal cord injury were examined. SMT treatment induced increases in regeneration-related proteins GAP-43, cell division cycle 2 (Cdc2) and phospho-Erk1/2 in the peripheral sciatic nerves after crush injury. Increased levels of Cdc2 and phospho-Erk1/2 were observe mostly in the gray matter area and some in the dorsomedial white matter. These increases correlated with increased cell numbers in affected areas. Moreover, axons of corticospinal tract (CST) showed increased sprouting in the injured spinal cord when administrated with SMT compared with saline-treated control. Thus, the present data indicate that SMT may be useful for identifying active components and for therapeutic application toward the treatment of spinal cord disorders after injury.
