From April, 1981, to April, 1990, 20 male and 7 female patients ranging in age from 17 to 63, were operated on for aortic insufficiency with an aneurysm of the ascending aorta. Ten patients were in New York Heart Association functional class II, 7 in class III, and ten in class IV. The surgical treatment in all cases consisted of total replacement of the ascending aorta with composite graft containing a prosthetic aortic valve and reimplantation of the coronary arteries by an intermediate tube graft. In 15 patients an uncomplicated annulo-aortic ectasia existed, and in 12 an aortic dissection; three of the latter group were operated during the acute phase. 17 patients showed typical Marfan syndrome, and 3 patients showed severe ascending aortic aneurysm secondary to the aortic valve disease. The overall operative mortality was 7%[2 deaths]. Those 2 deaths occurred following emergency operation due to associated aortic dissection, but no death during elective operation. All survivors have been followed-up during a period ranging 1 to 108 month[average 34 months]. There was no late mortality. Among the survivors, clinical improvement is readily apparent[2,3 in class I, 2 in class II ]. In conclusion, the treatment of aortic insufficiency associated with an aneurysm of the ascending aorta by insertion of a composite graft and reimplantation of the coronary arteries through an intermediate Dacron tube is a reliable method with low mortality and excellent results.
Background: The aim of this study was to compare the early outcomes of octogenarians undergoing minimally invasive right anterior mini-thoracotomy aortic valve replacement (RAT-AVR) with those undergoing transcatheter aortic valve implantation (TAVI) for aortic valve disease. Methods: In this single-center retrospective study, data were collected from octogenarians before and after RAT-AVR and TAVI between January 2021 and July 2022. Short-term outcomes, including the length of hospital stay, in-hospital mortality, all-cause mortality, and other major postoperative complications, were compared and analyzed. Results: There were no significant differences in in-hospital mortality, stroke, acute kidney dysfunction requiring renal replacement therapy, length of intensive care unit stay, or length of hospital stay. However, the TAVI group had a higher incidence of permanent pacemaker insertion (10% vs. 0%, p=0.54) and paravalvular leaks (75% vs. 0%, p<0.001). Conclusion: In the present study on octogenarians, both TAVI and RAT-AVR showed comparable short-term results. Although both procedures were considered safe and effective in the selected group, RAT-AVR had a lower incidence of complete atrioventricular block and paravalvular regurgitation.
Between November, 1984, and May, 1986, 93 patients underwent combined valvular and coronary artery operation. They were 70 male and 23 female, the age ranging from 29 to 82. From this population 89 patients underwent single valve replacement and 4 patients underwent double valve replacement. Patients with mitral valve disease were in the majority present in the age group between 50 till 70, where as in the group after 60 years, patients with aortic valve disease were dominant. The main indication for aortic valve replacement was aortic stenosis and the indication for mitral valve replacement was equal between mitral stenosis and mitral incompetence, the later was due to papillary dysfunction after myocardial infarction. Dyspnea was a very frequent symptom and it was found in nearly all patients. 28 patients had a previous myocardial infarction and severe left ventricular dysfunction. The grafts were placed prior to valve replacement and periods of myocardial ischemia were kept at a minimum by maintaining coronary perfusion throughout the operation. It is our opinion that simultaneous valve replacement and myocardial revascularization does not increase the risk of cardiac valve replacement substantially.
From October 1990 to May 1993, 19 patients underwent replacement of the transverse aortic arch. [10 men, 9 women, mean age 52.5 years] Underlying diseases were acute aortic dissection [10 cases], chronic aortic dissection [4 cases],and aortic arch aneurysm [ 5 cases]. In 19 patients, 10 underwent partial replacement and 9 underwent total arch replacement. The cerebral protection was achieved by profound hypothermia [rectal temperature,16$^{\circ}$ to 2$0^{\circ}C$] associated with total circulatory arrest [mean 35.5 minutes]. In one patient, the aortic arch distal to the left common carotid artery was resected with the distal arch being cross-clamped and in another two patients, the selective cerebral perfusion was also applied during the period of total circulatory arrest via innominate artery and left common carotid artery because of longer total circulatory arrest time. Among 14 patients of aortic dissecton, 10 presented hypertension, 1 presented Marfan syndrome, 1 presented pregnancy-induced hypertension and 2 revealed no evidence of hypertension. All of the above 14 patients complained chest pain. Among 5 patients of aortic arch aneurysm, Be het disease was suspected in only one patient and atherosclerotic aneurysm was proved in another 4 patients. The overall hospital mortality was 32% [6/19]. In aortic dissection, the mortality was 43% [Acute aortic dissection 30%, chronic aortic dissection 75%] and in aortic arch aneurysm, the mortality was 0%. Follow-up was done in all survivors for from 7 months to 36 months[mean,17.3%].
배경: 좌상엽에 발생한 비소세포형 폐암에서 aortic node전이 양상에 따른 생존율을 분석하여 임상적 의의를 알아보고자 하였다. 대상 및 방법: 1991년 1월부터 2000년 12월까지 좌상엽에 발생한 비소세포형 폐암으로 페엽절제술 이상의 수술을 받은 환자 중 수술 전 항암 화학요법 또는 방사선 치료를 받은 환자와 수술 사망 환자를 제외한 156명을 대상으로 후향적 연구를 하였다. 결과: 종격동임파절 전이 중 aortic node로의 전이는 52.7%였으며, 단일 부위(single station) 전이 중 aortic node로만 전이된 경우는 64.3%였다. N 병기에 따른 5년 생존율은 N0 65.0%, Nl 30.4%, N2 17.9%였으며, Nl 병기와 N2 병기 간에는 생존율의 차이를 관찰할 수 없었다(p=0.06). N2 병기 중 aortic node에만 전이된 경우 5년 생존율 35.6%로 다른 종격동임파절에 전이된 경우(5년 생존율 4.6%)보다 생존율이 높았으며 (p=0.01), Nl 병기와 비교 시 비슷한 생존율을 보였다(p=0.97). Aortic node를 Nl 임파절로 재분류하였을 때 N 병기별 5년 생존율은 N0 65.0%, Nl 11.2%, N2 4.6%로 Nl 병기와 N2 병기 간에 뚜렷한 생존율의 차이를 관찰할 수 있었다(p=0.00). 다변량 분석 시 aortic node에만 전이된 경우(hazard ratio 2.799, p=0.009), 남자(hazard ratio 6.892, p=0.011)가 술 후 생존에 영향을 미치는 의미있는 인자였다. 결론: 좌상엽 종양에서 aortic node에만 전이된 경우 외과적 절제 시 Nl 병기와 비슷한 생존율을 가지며, 따라서 치료 방침의 결정 시 이를 Nl 임파절로 간주하는 것이 타당하다고 생각한다.
배경: 심한 대동맥 판막 질환을 가진 환자에서 많은 경우에 승모판막 폐쇄부전을 동반한다. 이런 환자들에서 대동맥 판막 수술 후 남겨지는 승모판막 폐쇄부전의 변화는 수술 등의 치료 과정을 결정하는 데 중요하다. 그러나 대동맥 판막 형태에 따른 대동맥 판막 치환술 후 중등도 이하의 승모판막 폐쇄부전의 변화는 잘 알려져 있지 않다. 본 연구에서는 중등도 이하의 승모판막 폐쇄부전을 동반한 대동맥 판막 협착(Group S)과 폐쇄부전(Group R)을 갖는 두 환자군에서 대동맥 판막 치환술 후 승모판막 폐쇄부전의 변화를 추적 비교해 보았다. 대상 및 방법: 연구 대상은 본 병원에서 1996년 1월에서 2005년 5월까지 대동맥 판막 치환술을 받고 중등도 이하의 승모판막 폐쇄부전을 수술을 하지 않은 환자 43명을 대상으로 하였다. 대상 환자들은 대동맥 판막 협착군(n=29)과 대동맥판막 폐쇄부전군(n=14)으로 나뉘었다. 추적검사 방법은 수술 후 7일, 수술 후 $6{\sim}10$개월 그리고 18개월 이후에 시행한 경흉부 심초음파 결과로 하였으며 평균 추적기간은 38개월이었다. 결과: 평균나이는 60.9세(Group 5=62세, Group R=52.5세)였으며 60% (Group S=55%, Group R=71%)가 남자 환자였다. 수술 전 승모판막 폐쇄부전의 정도는 경도가 29 (67.5%)명이었고 경도와 중등도 사이가 11 (25.5%)명이었으며 중등도가 3 (6.9%)명이었다 Group S에서 승모판 폐쇄부전 정도가 수술 후 수 일 내에 16 (55%)명에서 만 호전을 보였고 수술 후 18개월 후에 시행된 검사에서는 17 (59%)에서 호전을 보였다. 반면에 Group R의 모든 환자에서 조기에 승모판막 폐쇄부전의 호전을 나타냈다. 좌심방 크기감소는 승모판막 폐쇄부전의 호전에 따라 감소하였으나 좌심실 구출률은 두 군에서 의미 있는 차이가 없었다. 결론: 심한 대동맥 판막 질환과 동반된 중등도 이하의 승모판막 폐쇄부전이 있는 환자에서 대동맥 판막치환술 후 승모판막 폐쇄부전의 호전은 대동맥 판막 협착증의 환자보다 대동맥 판막 폐쇄부전 환자에서 보다 조기에 잘 이루어진다.
A 4 year old mixed female dog was presented dyspnea, anorexia and syncope. In blood smear test, a lot of heartworm larvae were observed. In thoracic radiography, "reverse D" shape was observed. So, this case was considered with heartworm disease. In echocardiography, marked left heart hypertrophy was presented. In M-mode view, aortic valvular stenosis was found. After mongrel dog died, aortic valvular stenosis was reconfirmed in necropsy. After all, the dog died of pulmonary edema because of aortic valvular stenosis caused left heart failure.t failure.
A 23-year-old male patient complained dyspnea on exertion and orthopnea since December 1977. On examination, he was tall and slender. There was grade IV/VI to-and-fro murmur on the left sternal border especially on Erb`s point. The liver was descended 2 fingers breadth below right costal margin. There were no signs of Marfan`s syndrome. Echocardiography demonstrated partial closure of aortic valve and dilated aortic root with enlargement of ascending aorta. Left heart cardiac catheterization revealed moderately elevated pulmonary wedge pressure and right ventricular pressure. The left ventricular end diastolic pressure was markedly elevated to 26 mmHg. On aortography, the aortic regurgitation was severe and it was belonged to angiographically Grade IV. The aortic valve was replaced with Carpentier-Edwards valve without excision and replacement of ascending aorta, under the impression of rheumatic valvular heart disease. After closure of aortotomy, blood pressure was transiently elevated and bleeding from the site of inserting air vent needle of ascending aorta was developed. The bleeding was not controlled by any means. On postmortem microscopic study, the histologic changes were strikingly limited to the ascending aorta from the region of the aortic valve ring.
Extensive thoracic aortic disease involving the ascending aorta, the aortic arch, and the descending thoracic aorta may require multiple surgical and interventional managements, which impose a burden in terms of cumulative surgical trauma and the risk of interval mortality. Herein, we describe a single-stage arch and descending thoracic aorta replacement via sternotomy in a patient with multiple comorbidities presenting with an extensive thoracic aortic aneurysm.
From February 1985 to February 1993, 18 operations were performed in 17 patients for treatment of aneurysmal disease [n=12] and/or dissection of the ascending aorta [n=6]. The ages ranged from 26 to 69 years [mean 44.3 $\pm$ 11.0 years].The proposed operations include composite graft replacement of aortic valve and ascending aorta with coronary reimplantation in 11, graft replacement of ascending aorta alone in 5, aortic valve replacement and supracoronary graft replacement in 1 and ascending aorta to abdominal aorta bypass with thromboexclusion of descending aorta in one patient. Both Bentall [n=6] and Cabrol [n=5] technique were utilized for reimplantation of coronary arteries.Concomitant replacement of aortic arch and arch vessel reconstruction was necessary in two patients. Hypothermic circulatory arrest was utilized in 6 patients. Recently, four patients were managed on warm blood continuous cardioplegia via retrograde route. There were no operative deaths. No significant postoperative complications were noted. Postoperative follow up was complete in 15 patients from 1 month to 72 months. Redo operation was necessary in one patient who had suffered from distal recurrence of dissection 5 years after successful Bentall operation. The other patients are all in excellent clinical condition. From our early experience with those 17 cases, we assume that satisfactory operative result could be achieved with a variety of surgical technique including hypothermic circulatory arrest. In addition, continuous perfusion of warm blood cardioplegia via retrograde route is supposed to be beneficial in selected cases.
