The heart disease taking the second place of the cause of the death of modern people is a terrible disease that makes sudden death without noticing. To judge the aortic valve disease of heart diseases a name of disease was diagnosed using psychological data provided from physioNet. Aortic valve is a valve of the area that blood is spilled from left ventricle to aorta. Aortic stenosis of heart troubles is a disease when the valve does not open appropriately in contracting the left ventricle to aorta due to narrowed aortic valve. In this paper, 3126 samples of cardiac sound data were used as an experiment data composed of 180 characteristics including normal people and aortic valve stenosis patients. To diagnose normal and aortic valve stenosis patients, NEWFM was utilized. By using an average method of weight as an feature selection method of NEWFM, the result shows 91.0871% accuracy.
The incidence of abdominal aortic aneurysm is very rare in this country as other disease of the aorta. Aneurysm can be cased by a variable cause but mainly by arteriosclerosis. It is the disease of aged and degeneration. So recently it may be prevalent due to increase of aged people. In general the natural course of an abdominal aortic aneurysm is very grave so surgical intervention is indicated as soon as possible after the diagnosis. Recently a case of abdominal aortic aneurysm was seen in this clinic with abdominal pain and pulsating mass on the abdomen. This case was confirmed by aortography and treated by graft replacement of the aneurysm with favorable result.

Background: PWV is determined by dividing the distance by the time taken for the pulses traveling between two measuring sites, used as a marker of arterial stiffness and an important indicator for cardiovascular disease. Methods: A PWV measurement system, which offers a non-invasive, simple method of measurement, and simultaneous recording of six signlas(ECG, PCG and four pulse waves from carotid, femoral, radial and dorsalis pedis arteries) was developed. Seventeen healthy subjects with a mean age of 33 years(22 to 52) without any cardiovascular disease were participated for the experiment. Two observers(A and B) performed two consecutive measurements from the same subject in a random order. For the evaluation of stability and accuracy of the PWV measurement system, reproducibility of PWV from between-observer were also evaluated. Results: PWV $values(Mean{\pm}SD)$ measured by A were $7.07{\pm}1.48m/s$, $8.43{\pm}1.14m/s$ , $8.09{\pm}0.98m/s$ for aorta, arm, and leg, respectively. The values obtained from B were $6.76{\pm}1.00m/s$, $7.97{\pm}0.80m/s$, and $7.97{\pm}0.72m/s$ for aorta, arm, and leg, respectively. Between-observer $differences(mean{\pm}SEM)$ from the aorta, arm and leg were $0.14{\pm}0.15m/s$, $0.18{\pm}0.10m/s$ and $0.07{\pm}0.10m/s$. Reproducibility coefficients(2SD) from the aorta, arm, and leg were 0.62m/s, 0.84m/s and 0.86m/s. Correlation coefficients were significantly higher in aortic PWV, 0.93, compared to the coefficients for arm and leg. Coefficient of variance which reflects the reproducibility of the system ranged from $4.4{\sim}5.8%$ in all regional PWV. , Conclusion: Reproducibility of PWV in the study shows that the developed system has reliable and reproducible characteristics. The PWV measurement system used for the study offers comfortable and simple operation and provides accurate analysis and results with high reproducibility. Results of the PWV measurement system could contribute to various clinical applications in the future.
Truncus ateriosus is one of the cyanotic congenital heart disease. The incidence is relatively uncommon, as 0.4% of totoal congenital heart disease. Embryologically the defect is due to a lack of partitioning of the embryonic truncus and conus during the first few weeks of fetal life. The ventricular septal defect is invariable present. A single arterial vessel arises from the heart and supplies blood to the aorta, the lung, and the coronary arteries. In 1949, collett and Edwards classified this defect according to anatomic variation to four major types, such as type I, II, III, and IV. Type IV is defined that pulmonary arteries are absent, and the pulmonary arterial supply arises from the descending thoracic aorta. This patients often have a continuous murmur head particularly well in the interscapular area. No effective surgical treatment is available. We have experienced one case of truncus arteriosus, type IV of Collett and Edwards in the Department of Thoracic and Cardiovascular Surgery, Kyungbook National University Hospital. This patient was 10 year-old girl. The chief complaints were cyanosis and dyspnea on exertion since birth. She was admitted at this hospital on April 16, 1980. The continous machinery murmur was heard loudest at the interscapular area. The chest X-ray films revealed cardiomegaly with an increase in pulmonaryvascular markings. The pulmonary secotr was significantly concave. No filling of pulmonary arteries noticed by the right ventriculogram. There was possible biventricular hypertrophy in EKG. The echocardiogram showed that the demension of the aortic root was larger than normal and minimal increase of the left ventricular internal dimension. The cardiac catheterization data was obtained by use of the great saphenus vein approach. The systolic pressure of the right ventricular outflow tract was 80 mmHg and was similar to that of the aorta. The oxygen saturation data revealed the evidence of the left to right shunt at the level of ventricular septum. The patient was operated and the diagnosis was confirmed as trucus arteriosus, type IV. No effective surgical interventins were performed.
There is tendency of increasing number and decreasing age of patients who are indicated for Rastelli operation for their cyanotic congenital heart disease. So there is the need to find the criterion which saves the patients from early postoperative hemodynamic disturbances. We reviewed the 26 patients who had been performed Rastelli operation at Seoul national University Hospital from January 1981 to June 1988. mean age of the patients was 7.8*3.4 years[range 2.5-15years], mean body surface area[BSA] 0.79*0.25m2[range 0.49-1.51m2] and mean hematocrit 57.95*12%[range 48-80%]. We divided these patients into survived group and died group before postoperative 72 hours, and analyzed preoperative arterial oxygen saturation[SaO2], the ratio of diameter of right pulmonary artery to ascending aorta[RPA/AA], the ratio of both right and left pulmonary artery diameter to descending thoracic aorta[RPA+LPA/DTA], pulmonary artery index[PA index], cardiopulmonary bypass time, aorta cross-clamping time, postoperative perfusion state and total amount of dopamine infused postoperatively. The results showed that RPA+LPA/DTA and PA index were statistically significant factors to influence early postoperative cardiac death rate[P< 0.05]. Especially there were good linear correlations between PA index[X] and peripheral perfusion index[Y][Y= - 1.15+0.02 X, r=0.86, P<0.01]and between PA index[X] and total amount of dopamine infused before postoperative 72 hours[mg/kg, Y][Y=61.94 - 0.15 X, r=-0.80, P < 0.01]. Also there were tendencies that the higher RPA+LPA/DTA[Y], the better peripheral perfusion [X] and the lower need of dopamine[X], but no statistical significance.[Y=0.78+1.60 X, r =0. 49, P >0.05] And the discriminate analysis showed that patients with PA index over 221 mm2/BSA could undergo correction with 25 per cent of error rate. In conclusion, early postoperative hemodynamic states could be predicted by preoperatively measured PA index, and which can be used as a criterion for Rastelli operation performed on cyanotic congenital heart disease.

Background: The surgical threshold for bicuspid aortic valve (BAV)-related aortopathy is a matter of debate due to its uncertain etiology and prognosis. This study investigated the prognosis of unrepaired BAV aortopathy in patients undergoing surgical aortic valve replacement (SAVR). Methods: We retrospectively analyzed data from 720 patients (age, 60.8±11.5 years; 246 women) who underwent SAVR for BAV disease without aortic repair between 2005 and 2020 at Asan Medical Center. The clinical endpoints were defined as occurrences of sudden death, aortic dissection or rupture, and elective aortic repair. To estimate postoperative changes in the dimensions of the unrepaired aorta, the individual annual aortic expansion rate was calculated. Multiple linear regression models were used to evaluate the risk of aortic expansion. Results: The mean ascending aortic diameter was 39.5±4.6 mm, and 299 patients (41.5%) had a baseline ascending aorta diameter >40 mm. During 70.0±68.3 months of follow-up, the mean annual aortic expansion rate was 0.39±1.96 mm/yr, no aortic dissection or rupture was observed, and sudden deaths were reported in 12 patients (0.34% per person-year). Linear regression analysis revealed no significant correlation between the baseline ascending aortic diameter and postoperative aortic expansion (R2=0.004, β=-0.84, p=0.082). Conclusion: In selected patients undergoing SAVR for a BAV (<55 mm), the risk of adverse aortic events was very low. As this observation contradicts current practice guidelines advocating for proactive aortic replacement in dilated ascending aortas measuring >45 mm, the study results need further validation by studies involving larger populations or randomized controlled trials.
We retrospectively studied 34 patients who underwent operations of ascending aortic aneurysm and aortic valve replacement from August 1979 to July 1992 at the Yonsei Cardiovascular Center. Eight patients underwent supracoroanry non-composite graft replacement and separate aortic valve replacement[group I]. Twenty six patients underwent valved composite graft replacement with reimplantation of coronary arteries[group II]. Two cases in group II died within 1 month after the operation. Among the 32 survivors 28 patients have been followed up for an average of 59 months ranging from 1 months to 159 months. During the follow up periods, a pseudoaneurysm around the ascending aorta and a newly developed dissecting aneurysm in remaining aorta were noted in group II. There were 6 late deaths: 2 cases in group I and 4 cases in group II. Three cases among the 6 late deaths have stigmata of Marfan`s syndrome[1 cases in group I and 2 case in group II]. There was no statistically significant difference in actuarial survival rates between group I and group II[p > 0.05]. This study suggests that non-composite supracoronary graft interposition with separate aortic valve replacement is a safe surgical technique in patients who have normal aortic annulus and normal position of coronary ostia. However in degenerative disease such as cystic medial necrosis, composite graft replacement is recommended because this procedure eliminates entire abnormal tissue.But it seems to be important that the suture technique and strict follow-up in patients with diseased aortic wall.
Systemic arterial supply from the aorta to the lung is a rare congenital anomaly within the spectrum of bronchopulmonary sequestration according to Pryce's terminology. We describe our experience of this anomaly in an infant with congenital cardiac disease confirmed by multidetector CT scan. We found a systemic arterial supply from the aorta to the right lower lobe of lung without right lower lobar pulmonary artery and bronchopulmonary sequestration. This combination of congenital anomaly is most rare form.
From January 1991 to January 1995, 11 patients with aortic diseases underwent various surgical repairs. The age at operation ranged from 26 years to 63 years[ mean=50.9 years . The disease entities included 8 aortic dissections[ type I in 4, type II in 2 and type III in 2 cases , 2 Marfan`s syndrome with annuloaortic ectasia and 1 desecending thoracic aortic aneurysm The operative procedures we tried were 3 Bentall`s operation, 5 graft replacement of ascending aorta, and 3 graft interposition in descending thoracic aorta.Overall hospital mortality rate is 36.3%[4/11 . And causes of death are pump weaning failure in 2 cases and multiorgan failure in 2 cases. It was that 2 sternal dehiscence & mediastinitis, 1 acute renal failure, 2 hypoxic brain damages and 2 postoperative psychosis were complicated. Recently we tried surgical repair of aortic dissection five out of 6 cases using total circulatory arrest with deep hypothermia at 14$^{\circ}C$. Total circulatory arrest time ranged from 18 to 26 minutes[ mean 22.2 minutes , and mean aortic cross-clamping time was 48.2 minutes. One of 5 patient died on the 7th postoperative day due to multiorgan failure. Mortality of patients with TCA was 20%[1/5 , and it of remainders was 50%[3/6 . Our result for surgical repair using total circulatory arrest with deep hypothermia is satisfactory on the basis of our clinical data.
The identification of COX-2 (cyclooxygenase-2) has led to potential novel insights on disease pathogenesis (atherosclerosis, cancer, Alzheimer's disease) and the regulation of normal organ function. The present in vivo study with estrogen or soy-isoflavones has provided evidence for the association between COX-2 and ICAM-1 (Intercellular adhersion molecule-1). In the system of mature female rats, soy-isoflavones exerted more pronounced effect on ICAM-1 inhibitory and COX-2 stimulatory effect than estrogen. In the system of ovariectomized estrogen deficient rats, the down-regulatory properties of soy-isoflavones on ICAM-1 was less evident, whereas estrogen exerted the inhibitory activity. These results demonstrate that COX-2 limits adhersion molecule expression on rat aorta cells and suggest that COX-2 may play a protective role in cardiovascular system in mature female rats. Soy-isoflavones appear to have beneficial effect on vascular systems through modulation of ICAM-1 and COX-2, and these molecules appeared to be closely associated.
