우울증 및 불안장애 환자에서 특성불안 차이에 따른 긍정심리요인의 특징 (Characteristic of Trait Anxiety and Positive Psychological Factors in Patients with Depression and/or Anxiety Disorders)

  • Objective : The aim of this study was to investigate the relationship between trait anxiety and positive psychological factors. Methods : A total of 429 outpatients diagnosed with depression and/or anxiety disorders surveyed with questionnaires measuring socio-demographic, clinical, and positive psychological factors. The relationships of trait anxiety with clinical symptoms and positive psychological factors were evaluated. Results : Patients with low trait anxiety showed high optimism, gratitude, hope, spirituality and purpose in life. After adjusting age, scores of BDI and STAI-S, the results showed low trait anxiety was associated with high spirituality and purpose in life. Conclusion : The result revealed that trait anxiety had negative-relationship with spirituality and purpose in life in patients with depressive and/or anxiety disorders. Although trait anxiety was known as a general tendency which is stable, approach to trait anxiety by modulating these associated positive psychological factors would be newer approach of treatment to patients with depressive and/or anxiety disorders.

불안장애로 진단된 환자의 심(心) 대소(大小) 관련성에 관한 연구 (The Study about Correlations of Heart Sizes in Patients with Anxiety Disorder)

  • Objective : The relations of the heart size and seven emotions were described in Dong Yi Bo Gam, and we aimed to identify the correlations of the patients with anxiety disorder and their heart sizes. Methods : The subjects were 15 patients with anxiety disorder and 20 control groups. Chest radiography were performed. We measured patients' heart sizes and compared with control groups. Results : 1. The signigicant difference of somatization, anxiety, depression scale and Panic anxiety scale in SCL-90-R in patients with anxiety disorder from controls was confirmed. 2. The difference of thoracic sizes in patients with anxiety disorder and controls had no correlations. 3. The difference of heart sizes in patients with anxiety disorder and controls had no correlations. 4. The difference of cardiothoracic ratio in patients with anxiety disorder and controls had negative correlations. Conclusions : Based on these results, the patients with anxiety disorders and their heart sizes had no correlations. But in cardiothoracic ratio had negative correlations. So heart size and seven emotions have significant correlation.

치아상실 환자들의 보철치료 전과 후의 불안 및 우울에 영향을 미치는 요인 (Influencing factors on anxiety and depression before and after dental prosthetic treatment in the patients)

  • Objectives: The purpose of the study was to investigate the influencing factors on anxiety and depression before and after prosthetic treatment in the patients. Methods: A self-reported questionnaire was completed by 248 patients with tooth loss and over 20 years old. The questionnaire was carried out before and after oral examination and dental prosthetic treatment from July to December, 2013. The study instruments included Beck's anxiety inventory and self-rating depression scale by Zung. The questionnaire was adapted and modified from Kwon's anxiety inventory and Lee's depression scale. Results: While the anxiety level increased from 69.4% to 78.2% in the normal people, the depression level changed from 53.2% to 64.1% in the normal people. The gender variable had a positive effect on anxiety and depression while educational level had a negative effect on them. The dental fear was closely related to anxiety and depression before and after the prosthetic treatment. The entire body health perception showed a negative effect on anxiety and depression. The distrust towards the dentist had an influence on the anxiety and depression in the prosthetic patients. Conclusions: The influencing factors on anxiety and depression in the prosthetic patients included gender, dental fear, entire body perception, and distrust towards the dentist.

국소마취에 의한 척추수술 환자가 지각한 수술 중 돌봄과 불안정도 (Intraoperative Caring Behavior and Anxiety as Perceived by Patients Undergoing Spinal Surgery under Local Anesthesia)

  • Purpose: The purpose of this study was to identify the relationship between intraoperative level of caring and anxiety among patients who underwent spinal surgery under local anesthesia. Methods: The study participants included 162 patients who had spinal surgery under local anesthesia at a general hospital in B metropolitan city between July 20 and September 20, 2014. To measure the caring behaviors of patients under local anesthesia, we used the Caring Behavior Measurement, and state anxiety was measured by the State Trait Anxiety Inventory. The collected data were analyzed using a SAS program. The differences in anxiety levels based on participants' characteristics were analyzed with t-test and an analysis of variance. The correlation between levels of caring and anxiety was tested with Pearson's correlation coefficient. Results: Education and caring levels showed significant relationship with the anxiety level of patients with spinal surgery under local anesthesia. Conclusion: To decrease the level of anxiety in patients undergoing spinal surgery under local anesthesia, it is necessary to educate and train operating nurses about the intra-operative caring behavior.

불안 장애 환자와 우울 장애 환자의 감정표현불능증

  • Objective : The aim of the study was to investigate the difference in alexithymia between anxiety disorder and depressive disorder. The second was to evaluate the effect of alexithymia on quality of life in patients with anxiety disorder and depressive disorder. Methods : A total of 175 patients with diagnoses of anxiety disorder or depressive disorder were recruited. Demographic, psychosocial, and clinical data were analyzed, as well as results on the 20-item Toronto alexithymia Scale (TAS-20K), the Symptom Checklist-90-Re-vised (SCL-90-R), a quality of life scale, the Beck Depression scale, and Beck Anxiety Inventory. Results : As compared with the patients with anxiety disorder, patients with depressive disorder showed significantly higher total score on the TAS-20K and for factor 1 (difficulties identifying feelings) and factor 2 (difficulties describing feeling) scales of the TAS-20K and showed significantly lower scores of psychosocial well-being on the quality of life scale. Total scores on the TAS-20K correlated significantly with scores for some subscale on the quality of life scale. Conclusion : This study suggest that patients with depressive disorder had more alexithymic symptoms and worse quality of life compared with those with anxiety disorder. Also, alexithymic symptoms are found to be associated with quality of life. Therefore, clinicians should try to focus on relieving symptoms to help patients restore their psychological well-being and improve their quality of life.

유방암 환자의 피로, 통증, 불안, 우울이 삶의 질에 미치는 영향 (Impacts of Fatigue, Pain, Anxiety, and Depression on the Quality of Life in Patients with Breast Cancer)

  • Purpose: This study was conducted to identify the impact of fatigue, pain, anxiety and depression on the quality of life (QOL) among breast cancer patients. Methods: This study was conducted from July 12th to July 20th, 2010. One hundred and fifty breast cancer patients were recruited from D city in Korea. The instruments used in this study were the fatigue, pain, anxiety, depression and the quality of life scales for patients with breast cancer. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficients and multiple regression with the SPSS/WIN 12.0 program. Results: The quality of life for cancer patients showed a significant relationship with fatigue, pain, anxiety and depression. The significant factors influencing quality of life were fatigue, pain, anxiety and depression that explained 65.6% of the variance. Conclusion: Patients with breast cancer experienced fatigue, pain, anxiety and depression which led to a negative effect on quality of life. The results suggest that intervention programs to reduce fatigue, pain, anxiety and depression could improve the quality of life for breast cancer patients.

공황장애 환자에서 범불안장애 공존 유무에 따른 임상적 특징 비교 (A Comparison of the Clinical Characteristics of Panic Disorder with and without Generalized Anxiety Disorder)

  • Objective : This study aimed to investigate differences in demographic, clinical characteristics, and quality of life between panic-disorder patients with generalized anxiety disorder (PD+GAD) and without generalized anxiety disorder (PD-GAD). Methods : We examined data from 218 patients diagnosed with PD+GAD (150 patients) and PD-GAD (68patients). The following instruments were applied: Stress coping strategies, Beck Depression Inventory (BDI), Beck Anxiety Inventory (BAI), Panic Disorder Severity Scale (PDSS), Anxiety Sensitivity Index-Revised (ASI-R), Albany Panic and Phobia Questionnaire (APPQ), NEO-neuroticism(NEO-N), Short Form health survey-36 (SF-36). Results : Compared to the PD-GAD group, the PD+GAD group had higher scores in emotion-focused coping strategies and clinical severity, such as BDI, BAI, PDSS, ASI, APPQ, and neuroticism. The PD+ GAD group showed lower scores in most scales in SF-36 status than PD-GAD group. Conclusions : This study shows that PD+GAD patients are different from PD-GAD patients in coping strategies, clinical severity and quality of life. It emphasizes the need of personalized therapy in clinical approach among patients with PD+GAD.

Sedative methods used during extraction of wisdom teeth in patients with a high level of dental anxiety

  • Introduction: Intravenous sedation is performed to ensure smooth and safe surgery. Dental anxiety is a reaction to an unknown danger. The Spielberger's state-trait anxiety inventory (STAI) can be used to simultaneously evaluate the levels of state and trait anxiety. State anxiety is defined as subjective feelings of nervousness. This study assessed the presurgical anxiety using STAI and performed intravenous sedation for patients whose level of state anxiety was > stage IV. Based on our clinical experience, it is believed that higher doses of sedatives are needed to induce the desired levels of sedation in patients with a high level of state anxiety. Objectives: This study examined whether the sedative consumption of the patient with a high anxiety level increased. Patients and Methods: Patients with state anxiety scores of ${\geq}$51 were included in Group V, and those with state anxiety scores ranging from 42 to 50 were placed in Group IV. To induce sedation, intravenous access was established, and a bolus dose of 3.0 mg midazolam was administered intravenously. Sedation was maintained by administering a continuous infusion of propofol, which was aimed at achieving an Observer's Assessment of Alertness/Sedation scale of 10-12/20. In this study, midazolam was initially administered when the body movements appeared to occur or the blood pressure increased. This was followed by the administration of higher doses of propofol if low sedation was observed. Results: There were no significant differences in the patient demographics, duration of sedation, and doses of local anaesthetic agents between Groups IV and V. The midazolam dose and mean propofol dose needed to maintain comparable levels of sedation were significantly higher in Group V than in Group IV. Conclusion: In female patients, whose level of preoperative state anxiety is more than Stage V of STAI, a large quantity of sedatives is needed for intravenous sedation.

사랑니 발치환자에서 불안에 관한 연구 (THE ANXIETY IN PATIENTS UNDERGOING THIRD MOLAR EXTRACTION)

  • The purpose of this study is to understand anxiety in patients undergoing third molar extraction and care patients of third molar extraction. The subjects in the study were Sixty-three patients who visited the department of dentistry at Pusan Paik Hospital from May 2006 to September 2006 and who underwent third molar extraction. Patients' anxiety(state anxiety and trait anxiety) to use Spielberger's state-trait anxiety inventory(STAI) were measured. They were asked to fill out STAI question paper immediately before third molar extraction(pre-extraction) and on the day after the extraction(post-extraction). And visual analog scale(VAS) to measure patient's postoperative pain on the day after extraction(postextraction day) was used. Then Differences in anxiety before and after extraction, between men and women, between the first and second extraction, with impacted versus non impacted third molar extraction, between extraction time and anxiety, and between postoperative pain and anxiety were evaluated. The anxiety status of patients undergoing third molar extraction could be quantitatively evaluated using the STAI-KYZ. There were significant difference before and after third molar extraction. Especially women and patients of more severe postoperative pain was more anxious. We need to alleviate patient's anxiety(especially women) and to control postoperative pain throughout the tooth removal process.

Anxiety and Symptom Assessment in Turkish Gynecologic Cancer Patients Receiving Chemotherapy

  • Diagnosis and treatment procedures in cancers and resulting anxiety negatively affect the individual and the family. Particularly treatment methods may generate psychological symptoms. The aim of this study was to determine the level of such symptoms in Turkish gynecologic cancer patients receiving chemotherapy. A total of 41 patients who were referred to our gynecologic oncology research clinic between January-March 2012, receiving 3 months or more chemotherapy and who agreed to participate were enrolled in study. All the data were collected using a personal information form, Edmonton Symptom Assesment System and State-Trait Anxiety Inventory. Patients received highest point average from fatigue symptom ($6.53{\pm}2.67$) and lowest point average from dyspnea ($1.53{\pm}3.03$) according to Edmonton Symptom Assesment System. The mean State Anxiety score of patients was $43.1{\pm}9.77$ and mean Trait Anxiety score was $46.7{\pm}7.01$. Comparing symptoms of patients and mean State Anxiety score it was found that there was a statistically significant corelation with symptoms like pain (p<0.05), sadness (p<0.001), insomnia (p<0.05), state of well being (p<0.001) and dyspnea (p<0.05). Similarly comparing symptoms of patients and mean Trait Anxiety score demonstrated significant correlations for fatigue (p<0.05), sadness (p<0.01), insomnia (p<0.01) and state of well-being (p<0.01). As a result, patients with gynecological cancers experienced symptoms related to chemotherapy and a moderate level of anxiety. In accordance, appropriate interventions should recommended for the evaluation and improvement of anxiety and symptoms related to treatment in cancer patients.
