수술 중 음악청취가 척수마취 환자의 불안, 진정 및 활력징후에 미치는 효과 (The Effect of Listening to Music on Anxiety, Sedation, and Vital Signs of Patients Undergoing Spinal Anesthesia)

  • Purpose: The aim of this study was to examine the effect of listening to music on the level of anxiety, sedation, and vital signs of patients undergoing surgery with spinal anesthesia. Methods: A convenience sample of 70 adult patients, ASA physical status I-II, scheduled for elective urologic or orthopedic surgery with spinal anesthesia, was included in this study. They were assigned to either an experimental group (n=35), listening to music during surgery, or a control group (n=35), not listening to music. Their anxiety was measured with the Spielberg's Trait and State Anxiety Inventory (STAI-KYZ). Sedation level was monitored with the Bispectral Index, and the vital signs at beginning, 10, 20, 30, and 45 min after operation. Results: The music group reported significantly lower state anxiety level during surgery as compared with the control group (t=3.91, p<.001). Repeated measures of ANOVA indicated a significant by group interaction on BIS index scores of sedation (F=4.23, p=.006). Among the vital signs, only heart rate was a significant by group interaction (F=5.529, p=.004). Conclusion: These findings indicate that listening to music during surgery with spinal anesthesia is a useful and effective nursing intervention to reduce anxiety and to maintain proper sedation.

초등학생의 수학 인식론적 신념과 시험불안의 관계에서 수학 시험준비전략의 조절효과 분석 (The Moderated Effects of Mathematics Test-preparation Strategies in the Relation between Elementary School Students' Epistemological Beliefs about Mathematics and Test Anxiety)

  • 이 연구의 목적은 초등학생의 수학 인식론적 신념과 시험불안의 관계에서 수학 시험준비전략의 조절효과를 검증하는데 있다. 이를 위해 수학 인식론적 신념을 예측변인, 수학 시험준비전략을 조절변인, 시험불안을 결과변인으로 설정하여 구조방정식 모형을 활용한 조절효과 검정을 시도하였다. 이 연구는 G광역시에 소재한 13개 초등학교 6학년 810명(남학생 411명, 여학생 399명)을 대상으로 수학 인식론적 신념 검사, 시험불안 검사, 수학 시험준비전략 검사를 실시하여 자료를 분석하였다. 분석 결과, 초등학생의 수학 인식론적 신념과 시험불안의 관계에서 수학 시험준비전략의 조절효과가 유의하였다. 즉, 수학 인식론적 신념이 높을수록 시험불안이 낮았고, 수학 인식론적 신념이 낮을수록 수학 시험준비전략 수준이 시험불안에 미치는 영향력이 큰 것으로 나타났다. 이 분석결과는 수학 인식론적 신념이 낮은 학생들의 경우 수학 시험준비전략에 대한 강조가 오히려 시험불안에 대한 영향력을 가중시키는 반면, 수학 인식론적 신념 수준이 높은 학생들에게는 수학 시험준비전략을 높이기 위한 노력이 시험불안을 낮추는데 효과적임을 시사한다.

환아의 질병특성에 따른 불안에 대한 탐색적 연구 (An Exploratory Study on Children's Anxiety Related to Disease Characteristics)

  • The purpose of this study is to explore the level of children's anxiety related to disease characteristics The subjects of this study were 72 childrens (6year-10 year) who were admitted to the pediatric wards at University Hospital and 24 healthy children, and 24 mothers of children with fatal & chronic illness. The data were collected with sarason's Anxiety scale for children & spielberger's State Trait Anxiety Inventory for mothers and analyzed of One way Anova, Duncun Test, Pearson correlation, and T-test. The results of this study were as follows : 1) There was significant difference in children's anxiety related to disease characteristics(P<0.01) Fatally ill children showed highest level of anxiety and chronically, acutely ill children in order. And there was no significant difference in anxiety between acutely ill children and healthy children. There was significant difference in anxiety between groups of ill and healthy children(P<0.01) .The anxiety of ill children was higher than that of healthy children. 2) There was no significant correlation between children's anxiety and their age. 3) There was no significant difference in children's anxiety related to frequency of hospital admission. 4) There was significant correlation between children's anxiety and duration of illness(P<0.01). The longer the duration of illness is, the higher children's anxiety is. 5) There was significant difference in mothers' anxiety between groups of fatally ill and chronically ill children(P<0.05). The anxiety of mothers of fatally ill children was higher than that of mothers of chronically ill children. 6) There was no significant correlation between children's anxiety of fatal and chronic illness and their mothers' anxiety.

이완술 정보제공이 심도자 검사환자의 불안에 미치는 영향 (The Effects of Relaxation Informativeness upon the Anxiety Level of Adult Patients with Cardiac Catheterization)

  • This study was done in order to help alleviate or prevent the anxiety resulting from cardiac catheterization among adult patients. This goal may be re-ached through providing relaxation techenique to the patients. Such an informativeness would make it possible to establish a basis for comprehensive nursing intervention. The results of this study are summarized as fellows: 1. The first hypothesis:“The experimental group with relaxation informativeness will have less score of state anxiety level before cardiac catheterization than the control group without relaxation informativeness”was accepted. (t=3.72, p=.001). 2. The second hypothesis: “The experimental group with relaxation technique informativeness will have less score of distress level during the procedure than the control group without relaxation technique informativeness”was accepted. (t=2.36, p=.023) 3. Additional analysis; It is seen that most cardiac patients were satisfied with precardiac catheterization procedure information provided by medical teams. (experimental group: 90%, control group: 85%) The relaxation technique informativeness contributed to the decrease of anxiety level. Patients showed interest in reusing the relaxation technique informativeness in the event of further need. In conclusion, the researcher thinks that it is necessary that nurses provide Patients with relaxation technique to reduce the anxiety level with cardiac catheterigation. This will enable them to practice effective comprehensive nursing.

Detection speed of negative information in anxious participants

  • A modifiedPosner cue-target paradigm in which neutral, positive and negative (threat) words were presented in peripheral location for cue was used to investigate the difference of engagement component of attention across emotional valence and anxiety level of participants. Results showed an interaction effect between anxiety level of participants and emotional valence of cue in valid trial. This indicates that the engage component of attention is not encapsulated and influenced by anxiety level of participant.

유방절제술 환자의 불확실성, 불안 및 대처방식 (Uncertainty, Anxiety and Coping with Mastectomy for Breast Cancer)

  • The purpose of this study was to provide basic information for developing a nursing intervention that helps patients learn how to acquire coping to reduce post operation uncertainty and anxiety by investigating the level of uncertainty and anxiety experienced by mastectomy patients. The subjects were 134 patients selected from St. Mary's Kangnam and St. Mary's hospital, and the data collection period was from October to December of 1998. Uncertainty was measured by using Mishel Uncertainty in Illness Scale (MUIS), anxiety measured by using State-Trait Anxiety Inventory(STAI), and coping by using a questionnaire developed by Kim & Yoo (1996). Data were analyzed with SAS program by t-test, ANOVA, Duncan's multiple range test, Pearson correlation coefficients, and stepwise multiple regression. The results were as follows : 1. The mean uncertainty score was 57.6. The results of the correlation between the compliance of medical regimen and demographic characteristics were as follows ; Those who monthly income over 2,010,000 won had lower than above 1,000,000 won, and those with the experience of chemotherapy had higher than those without, and the patient who has 7∼12 months(1 yr.) post operation period had higher than the one below 6months, 25∼36 months(3 yrs.), 37∼60 months(5 yrs.), and over 61 months. 2. The mean anxiety score was 45.9. Anxiety tended to be increased slightly in subjects with low educational background, poor monthly income, experience of chemotherapy, and 7∼12 months(1 yr.) post operation period, but there was no significant difference by general characteristics 3. The mean value of the coping score was 100.7. The study revealed higher score in problem-focused coping than emotion-focused coping. In regard to coping by demographic characteristics were as follows ; those who had monthly income over 2,010,000 won had higher level of coping than those whose monthly income was between 1,010,000 and 2,000,000 won. In terms of problem- focused coping, those who had 25∼36 months of post operation period showed significantly lower level of coping than those below 6 months or 37∼60 months(5 yrs.) or over 61 months of post operation period. Regarding the emotion-focused coping, those with the christianity had significantly lower level of coping than those without it. Also, those whose monthly income over 2,010,000 won had significantly higher coping level than those with income of between 1,010,000 and 2,000,000 won. 4. A positive relationship was found between uncertainty and anxiety. Patients who experienced more uncertainty also showed more anxiety. Problem-focused coping was inversely related to uncertainty and anxiety. 5. The major variable that affected uncertainty was anxiety, explaining 63.3% of the uncertainty. In addition to this, it would explain 66.4% in total when experience of chemotherapy was added.

고혈압 환자에서 불안증상에 영향을 미치는 정신심리학적 요인 (Psychosocial Factors Influencing Anxiety Symptoms in Patients With Hypertension)

  • Objective : The aim of this study was to investigate the prevalence of anxiety symptoms and its association with psychological factors in patients with hypertension. Methods : The Participants included 124 patients with hypertension. Anxiety symptoms were evaluated by the Hospital Anxiety and Depression Scale. Socio-demographics, perceived stress, state-trait anger, life satisfaction, and ego resiliency were assessed. Stepwise multiple regression analyses were carried out to examine the impact of perceived stress, state-trait anger, and life satisfaction on anxiety symptoms. In addition, moderated regression analysis was performed to explore the moderating effect of ego resiliency between perceived stress and anxiety symptoms. Results : A total of 17 subjects (13.7%) were identified as having anxiety symptoms. Higher perceived stress and state-trait anger, and lower life satisfaction were found to be significant correlates of anxiety symptoms. In the final model, higher levels of perceived stress (β=0.378, p<0.001) and trait anger (β=0.320, p<0.001) were related to higher levels of anxiety symptoms, while a higher level of life satisfaction (β=-0.166, p=0.025) was associated with a lower level of anxiety symptoms. Further, ego resiliency buffered the negative effect of perceived stress on anxiety symptoms. Conclusion : This study demonstrated the prevalence of anxiety symptoms and associated psychological factors among patients with hypertension. Our findings suggest that improving life satisfaction and ego-resiliency as well as controlling stress and anger may be important in the management of anxiety symptoms in patients with hypertension.

뉴욕 거주 한국인의 불안, 자아존중감 및 건강증진 생활양식 (Health Promoting Lifestyle, Anxiety and Self-Esteem : Korean Immigrants in New York)

  • Objectives : To examine the relationships among health promoting lifestyle, anxiety, and self-esteem for korean immigrants in New York. Methods : The study included 425 adults aged 18 and over, who were chosen from Korean church and social organizations located in New York. The data were collected from April to July, 1996 using self-administered questionnaires. Data analyses were conducted using stepwise multiple regression, Pearson's correlation coefficients, 1-test, and ANOVA. Results : Health promoting lifestyle was significantly different by age, religion, occupation, and the length of residence. Those insured and those with no current medial diagnosis revealed significantly higher score in health promoting lifestyle than their counterparts. As for the level of anxiety, significant differences were found by educational level, marital status, occupation, income, and the length of residence. Self-esteem varied significantly by education, marital status, occupation, income, and the length of residence. Those with no current medical diagnosis and those insured experienced significantly higher level of self-esteem than their counterparts. Negative correlations were observed between health promoting lifestyle and anxiety. Perceived health status at present was positively correlated to perceived health status in the future. Health promoting lifestyle was significantly predicted by anxiety, age, and insurance. Conclusions : The above findings suggested that it is necessary to develop programs improving the perceived health status and lowering the level of anxiety. In this context, further researches on the relation between health promoting lifestyle and anxiety is required.

Mental Stress from Animal Experiments: a Survey with Korean Researchers

  • Animal experiments have been widely conducted in the life sciences for more than a century, and have long been a subject of ethical and societal controversy due to the deliberate infliction of harm upon sentient animals. However, the harmful use of animals may also negatively impact the mental health of researchers themselves. We sought to evaluate the anxiety level of researchers engaged in animal use to analyse the mental stress from animal testing. The State Anxiety Scale of the State-Trait Anxiety Inventory (STAI) was used to evaluate how researchers feel when they conduct animal, as opposed to non-animal, based experiments (95 non-animal and 98 animal testing researchers). The Trait Anxiety Scale of STAI was employed to measure proneness to anxiety, namely the base trait of the researchers. Additionally, the information on sex, age, education, income, and total working periods was collected. While the Trait Anxiety scores were comparable ($41.5{\pm}10.9$ versus $42.9{\pm}10.1$, p = 0.3682, t-test), the State Anxiety scores were statistically significantly higher for animal users than non-animal users ($45.1{\pm}10.7$ versus $41.3{\pm}9.4$, p = 0.011). This trend was consistent for both male and female. Notably, younger animal testers (${\leq}30$ years of age) with less work experience (${\leq}2$ years) and lower income level (${\leq}27,000$ USD) exhibited higher anxiety scores, whereas these factors did not affect the anxiety level of non-animal users. The present study demonstrated that participation in animal experiments can negatively impact the mental health of researchers.

일부 치위생과 학생의 시험불안, 자아존중감과 정신건강과의 관계 (Impact of Self-Esteem and Test Anxiety on Mental Health in students of Dental Hygienics)

  • This study is a descriptive paper addressing the relationship between test anxiety, self-esteem and the mental health levels of freshmen and third year students of Dental Hygienics. In late 2011, we received a total of 298 questionnaires from freshmen and third year Dental Hygienics students of three South Korean universities, located specifically in Seoul and Gyeonggi-do. The timing of this questionnaire was significant because it was one week prior to midterm examinations. The results were as follows:1. There was a statistically significant relationship between age (t=3.017, p<.001) and grade(t=2.665, p<0.05), subjective health status(t=27.513, p<.001). 2. The lower the participants' reported mental health in general, the higher their level of test anxiety (r=.565, p<.001), broken down into two subdivisions: cognitive text anxiety (r=526, p<.001) and emotional test anxiety (r=.534, p<.001). 3. When considering the factors that influence the general mental health of participants, there were age differences. For first year students, their subjeetive health status (${\ss}$=-.300, p<.001) and test anxiety (${\ss}$=.530, p<.001) were significant. For third year students, this was also true (subjective mental condition: ${\ss}$=-.242, p<.001 and test anxiety: ${\ss}$=.350, p<.001) but self-worth was also significant (${\ss}$=.377, p<.001). The results of this study suggest that the key factors that influence the mental health level of students Dental Hygienics are test anxiety, subjective appraisal of health level, and general self-worth. A systematic approach is therefore needed to improve students' mental health and reduce test anxiety. Health management programs which address and test students' physical health will also be important in improving student performance and welfare.
