아주 드문 항문직장 악성 흑색종 (A Rare Entity: Primary Malignant Melanoma of the Anorectum )

  • Malignant melanomas, which are rarely found in the Asian population, are malignant tumors or melanocytes that manifest in the skin mucosa. Malignant melanomas of the anorectum are very rare and account for approximately 1% of all malignant melanomas in the Asian population. Here, we present a rare case presenting a malignant melanoma of the anorectum. An 85-year-old woman visited the hospital with bloody stools and an anal mass. Sigmoidoscopy revealed a black mass protruding from the anus, and the scope was able to penetrate the anorectal mass. Close-up endoscopy revealed black moles of different sizes scattered across the rectal mucosa. PET-CT indicated multiple FDG uptakes in the liver, indicating multiple metastases. Pathologic examination led to the detection of malignant melanocytes with dark brown deposits. The patient's immunohistochemical markers were positive for melanin-A antibodies and HMB-45, indicating a malignant melanoma. As there was no evidence of malignant melanomas on the skin, the patient was diagnosed with primary malignant anorectal melanoma with liver metastases.

Can we omit prophylactic inguinal nodal irradiation in anal cancer patients?

  • Purpose: To evaluate the appropriateness of prophylactic inguinal nodal irradiation (PINI), we analyzed patterns of failure in anal cancer patients who were inguinal node-negative at presentation and did not receive PINI. Materials and Methods: We retrospectively reviewed the records of 33 anal cancer patients treated by definitive concurrent chemoradiation therapy (CCRT) between 1994 and 2013. Radiotherapy consisted of a total dose of 44-45 Gy (22-25 fractions in 5 weeks) on the whole pelvis, anus, and perineum. Except inguinal lymphadenopathy was present at initial diagnosis, the entire inguinal chain was not included in the radiation field. In other words, there was no PINI. Results: The median follow-up duration was 50 months (range, 4 to 218 months). Median survival and progression-free survival (PFS) were 57 months (range, 10 to 218 months) and 50 months (range, 4 to 218 months), respectively. Among the survival, the median follow-up duration was 51 months (range, 12 to 218 months). The 5-year overall survival and PFS rates were 93.4% and 88.8%, respectively. Although none of the patients received inguinal node irradiation for prophylactic purposes, there was no inguinal recurrence. Conclusion: Treatment of anal cancer by omitting PINI might be considered in selected patients with clinically uninvolved inguinal nodes.

Comparison of elective inguinal node irradiation techniques in anal cancer

  • Purpose: To compare photon thunderbird with deep match (technique 1) with 3-field technique with electron inguinal boost (technique 2) in acute skin toxicity, toxicity-related treatment breaks and patterns of failure in elective inguinal radiation therapy (RT) for curative chemoradiation in anal cancer. Materials and Methods: Seventeen patients treated between January 2008 and September 2010 without evidence of inguinal and distant metastasis were retrospectively reviewed. In 9 patients with technique 1, dose to inguinal and whole pelvis area was 41.4 to 45 Gy and total dose was 59.4 Gy. In 8 patients with technique 2, doses to inguinal, whole pelvis, gross tumor were 36 to 41.4 Gy, 36 to 41.4 Gy, and 45 to 54 Gy, respectively. The median follow-up period was 27.6 and 14.8 months in group technique 1 and 2, respectively. Results: The incidences of grade 3 radiation dermatitis were 56% (5 patients) and 50% (4 patients), dose ranges grade 3 dermatitis appeared were 41.4 to 50.4 Gy and 45 to 54 Gy in group technique 1 and 2, respectively (p = 0.819). The areas affected by grade 3 dermatitis in 2 groups were as follow: perianal and perineal areas in 40% and 25%, perianal and inguinal areas in 0% and 50%, and perianal area only in 60% and 25%, respectively (p = 0.196). No inguinal failure has been observed. Conclusion: Photon thunderbird with deep match technique and 3-field technique with electron inguinal boost showed similar incidence of radiation dermatitis. However, photon thunderbird with deep match seems to increase the possibility of severe perineal dermatitis.

Dosimetric advantages and clinical outcomes of simultaneous integrated boost intensity-modulated radiotherapy for anal squamous cell carcinoma

  • Purpose: The purpose of this study was to explore the dosimetric difference between simultaneous integrated boost intensity-modulated radiotherapy (SIB-IMRT) and three-dimensional conformal radiotherapy (3DCRT), and the clinical outcomes of anal squamous cell carcinoma (ASCC) chemoradiotherapy featuring SIB-IMRT. Materials and Methods: This study included ten patients with ASCC who underwent chemoradiotherapy using SIB-IMRT with 5-fluorouracil and mitomycin C. SIB-IMRT delivered 54 Gy to each primary tumor plus metastatic lymph nodes and 45 Gy to regional lymph nodes, in 30 fractions. Four patients received additional boosts to the primary tumors and metastatic lymph nodes; the median total dose was 54 Gy (range, 54 to 60 Gy). We additionally created 3DCRT plans following the Radiation Therapy Oncology Group 9811 protocol to allow dosimetric comparisons with SIB-IMRT. Locoregional control, overall survival, and toxicity were calculated for the clinical outcome evaluation. Results: Compared to 3DCRT, SIB-IMRT significantly reduced doses to the external genitalia, bladder, and intestine, delivering the doses to target and elective nodal region. At a median follow-up time of 46 months, 3-year locoregional control and overall survival rates were 88.9% and 100%, respectively. Acute toxicities were treated conservatively. All patients completed radiotherapy with brief interruptions (range, 0 to 2 days). No patient experienced ${\geq}grade$ 3 late toxicity during the follow-up period. Conclusion: The dosimetric advantages of SIB-IMRT appeared to reduce the toxicity of chemoradiotherapy for ASCC achieving high locoregional control in the extended period.

항문암 환자에서 근치적 목적의 동시 항암화학 방사선치료의 결과 (The Results of Curative Concurrent Chemoradiotherapy for Anal Carcinoma)

  • 목적: 항문암으로 근치적 목적의 동시 항암화학 방사선치료를 받은 환자들을 대상으로 치료반응의 예측인자, 생존율과 관련된 예후인자를 알아보고자 하였다. 대상 및 방법: 전남대학교병원에서 1993년부터 2008년까지 병리조직학적으로 항문의 편평상피세포암으로 진단받고 근치적 목적의 동시 항암화학 방사선치료를 받은 42명의 환자를 대상으로 후향적으로 분석하였다. 환자들 나이의 중앙값은 61.5세 (39~89에)였고 방사선량의 중앙값은 50.4 Gy (30.0~64.0 Gy)였다. T병기 상 2기 이하였던 환자는 총 36명 (85.7%)이었다. 영역림프절전이는 14명(33.3%)에서 있었다. 항암화학요법으로는 5-fluorouracil, mltomycin 병행요법으로 치료받은 환자가 36명 (85.7%)이었고 나머지는 5-fluorouracil, cisplatin 병행요법을 받았다. 결과: 전체 환자의 추적관찰기간의 중앙값은 62개월 (2~202개월)이었다. 5년 전체생존율, 국소영역제어율, 무병생존율, 무장루생존율은 각각 86.0%. 71.7%. 71.7%. 78.2%였다. 전체생존율과 관련된 인자로 단변량분석에서는 Eastem Cooperative Oncology Group (ECOG) 수행능력과 완전관해여부가 유의하였고 다변량분석에서는 ECOG 수행능력만이 유의하였다. 국소영역제어율, 무병생존율과 무장루생존률에 관련된 인자는 단변량이나 다변량분석 모두 유의한 인자는 없었다. 치료 후 완전관해 여부와 관련하여 유의하게 예측할 수 있는 인자는 없었다. 국소영역재발은 7명에서, 원격전이는 1명에서 발생하였다. 전체 환자에서 평가가 가능하였던 급성 부작용으로 3도의 피부염이 4명(9.5%), 2도는 22명(52.4%)에서 발생하였다. 결론: 본 연구에서 항문의 편평상피세포암에 대한 근치적 목적의 동시 항암화학 방사선치료로 양호한 생존율을 얻을 수 있었고, 특히 ECOG 수행능력이 1 이하인 환자의 생존율이 유의하게 높았다. 그러나 치료 후 종양반응을 예측할 수 있는 유의한 인자는 없었다.
