The Carcinogenic Liver Fluke Opisthorchis viverrini is a Reservoir for Species of Helicobacter

  • There has been a strong, positive correlation between opisthorchiasis-associated cholangiocarcinoma and infection with Helicobacter. Here a rodent model of human infection with Opisthorchis viverrini was utilized to further investigate relationships of apparent co-infections with O. viverrini and H. pylori. A total of 150 hamsters were assigned to five groups: i) Control hamsters not infected with O. viverrini; ii) O. viverrini-infected hamsters; iii) non-O. viverrini infected hamsters treated with antibiotics (ABx); iv) O. viverrini-infected hamsters treated with ABx; and v) O. viverrini-infected hamsters treated both with ABx and praziquantel (PZQ). Stomach, gallbladder, liver, colonic tissue, colorectal feces and O. viverrini worms were collected and the presence of species of Helicobacter determined by PCR-based approaches. In addition, O. viverrini worms were cultured in vitro with and without ABx for four weeks, after which the presence of Helicobacter spp. was determined. In situ localization of H. pylori and Helicobacter-like species was performed using a combination of histochemistry and immunohistochemistry. The prevalence of H. pylori infection in O. viverrini-infected hamsters was significantly higher than that of O. viverrini-uninfected hamsters ($p{\leq}0.001$). Interestingly, O. viverrini-infected hamsters treated with ABx and PZQ (to remove the flukes) had a significantly lower frequency of H. pylori than either O. viverr-iniinfected hamsters treated only with ABx or O. viverrini-infected hamsters, respectively ($p{\leq}0.001$). Quantitative RT-PCR strongly confirmed the correlation between intensity H. pylori infection and the presence of liver fluke infection. In vitro, H. pylori could be detected in the O. viverrini worms cultured with ABx over four weeks. In situ localization revealed H. pylori and other Helicobacter-like bacteria in worm gut. The findings indicate that the liver fluke O. viverrini in the biliary tree of the hamsters harbors H. pylori and Helicobacter-like bacteria. Accordingly, the association between O. viverrini and H. pylori may be an obligatory mutualism.

A Fifteen-year Epidemiological Study of Ventriculoperitoneal Shunt Infections in Pediatric Patients: A Single Center Experience (소아 환자에서의 뇌실-복강 단락 감염의 역학적 고찰: 15년 간의 단일 기관 연구)

  • Purpose : Ventriculoperitoneal (VP) shunt insertion is an important treatment modality in children with hydrocephalus. VP shunt infection is a major complication and an important factor that determines the surgery outcome. This 15-year study was performed to evaluate the epidemiology of VP shunt infections in pediatric patients treated at our center. Methods : A retrospective review of medical records was performed in patients 18 years old or younger who underwent VP shunt insertion surgery from April 1995 to June 2010. Results : Three hundred twenty-seven VP shunt surgeries were performed in a total of 190 pediatric patients (83 females, 107 males). The median age of the patients was 2.4 years (range, 0.02-17.9 years). Having a malignant brain tumor was the most frequent cause for VP shunt insertion. The shunt infection rate was 6.7% (22/327) per 100 operations and 9.5% (18/190) per 100 patients, and the incidence rate was 0.45 infection cases per 100 shunt operations-year. The most common pathogen was coagulase-negative staphylococcus (n=7) followed by methicillin resistant Staphylococcus aureus (n=1). Ten cases were treated with vancomycin and beta-lactam antibiotic (cephalosporin or carbapenem) combination therapy and 7 cases were treated with vancomycin monotherapy. The median duration of antibiotic treatment was 26 days (range, 7 to 58 days). Surgical intervention was performed in 18 cases (18/22, 81.8%). Conclusion : Epidemiologic information regarding VP shunt infections in pediatric patients is valuable that will help guide proper antibiotic management. Additional studies on the risk factors for developing VP shunt infections are also warranted.

Surgical Correction of a Vesicourachal Diverticulum in a Cat (고양이에서 방광요막관 게실의 외과적 치료 증례)

  • A 4-year-old female Korean short hair cat weighing 6.5 kg presented for evaluation of urinary incontinence and stranguria. On physical examination, stranguria was observed concurrently with urinary bladder distention. Abdominal radiographs revealed two small uroliths in the urinary bladder. Urinary bladder sludge was detected on abdominal ultrasound. Urine analysis indicated hematuria and bacteriuria. The cat was treated with a 4-week course of a combination of antibiotics and urinary bladder irrigation using normal saline; however, response to treatment was minimal. Excretory urography was performed to identify a congenital disorder. A small diverticulum, located to the urinary bladder apex, was identified. A tentative diagnosis of a vesicourachal diverticulum was made. Surgical exploration of the abdomen was performed and a triangular-shaped diverticulum was apparent at the urinary bladder apex. Cystotomy was performed to remove two small uroliths. Partial cystectomy was then performed for diverticulectomy. Approximately 2 cm diameter of a part of the apex was removed. Normal urination was regained 5 days postoperatively. The follow-up was completed by physical examination 2 years after surgery. There was no evidence of stranguria and urinary incontinence.

A Case of Feline Proliferative Eosinophilic Keratitis Treated by Topical Cyclosporine and Corticosteroids (고양이 증식성 호산구성 각막염에서 국소적인 사이클로스포린과 코르티코스테로이드 점안 1례)

  • A 6-year-old spayed female Persian cat presented with a 3-month history of recurrent ulcerative keratitis with noticeable opacification and vascularization of the right cornea. The lesion was nonresponsive to topical antibiotics and to nonsteroidal anti-inflammatory drugs. Ophthalmic examination showed signs of ocular discomfort, such as epiphora and blepharospasm, in the right eye. Biomicroscopic examination revealed an irregular, edematous, vascularized mass with pink to white tissue on the entire cornea and mild conjunctivitis. A tentative diagnosis of feline proliferative eosinophilic keratitis (FPEK) was made on the basis of clinical appearance. Cytologic examination of the cornea showed a mixture of numerous eosinophils and mast cells, which confirmed the original diagnosis of FPEK. The cat was treated with a topical antibiotic-corticosteroid combination, cyclosporine ointment, trifluridine eye drops, and oral Llysine. The clinical signs improved remarkably 18 days after the cat was first examined. The short-term use of corticosteroids and long-term use of cyclosporine and an anti-viral agent resolved the lesion without recurrence of the disease for 1 year.

Applicability of UV and UV/$H_2O_2$ Processes in the Control of Pharmaceuticals and Personal Care Products and Microbiological Safety for Water Reuse (잔류 의약품류의 제거 및 미생물학적 안전성을 고려한 하수 재이용 기술로서의 UV 및 UV/$H_2O_2$ 공정의 적용성)

  • Over the last decades, much consideration has been given to microbiological and chemical risks, especially when wastewater was reclaimed as water resources for urban water, irrigation water and recreational water etc. We investigated the performance of UV-based processes such as UV and UV/$H_2O_2$ for both the removal of pharmaceuticals and personal care products (PPCPs) as an emerging chemical and the inactivation of pathogen with bench-scale experimental study. 38 kinds of PPCPs including antibiotics and analgesics were detected from secondary effluent used as tested water. Bench-scale experimental study showed that UV process would require considerable UV dose for the effective PPCPs removal. Contrarily, PPCPs removal efficiency significantly improved by the combination of $H_2O_2$ with UV even at a lower UV dose and, moreover, their removal efficiency increased with the increased initial $H_2O_2$ concentration. Besides naproxen (>89%), concentrations of all the investigated PPCPs decreased by more than 90% of their initial concentrations under $923\;mJ/cm^3$ of UV dose and 6.2 mg/L of $H_2O_2$. Previous studies showed that this operational condition could get 4~5 log inactivation for Total coliform, indicating that UV/$H_2O_2$ process will be appropriate to comply with the criteria of California Title 22 for Total coliform.

Antibacterial Activity of Herbal Medicine Extracts against Edwardsiella tarda (어병세균 Edwardsiella tarda에 대한 한약재 추출물의 항균활성)

  • The methanol extracts of 19 commercial herb medicines was analyzed to antibacterial activities against Edwardsiella tarda, causing several fish diseases. Rhus javanica showed most strong antibacterial activity against E. tarda and Escherichia coli. Methanol extract of R. javanica was further extracted using several organic solvents having different polarity. Extract from ethyl acetate fraction showed strong activity against E. tarda as well as E. coli. Minimal inhibitory concentration, MIC of R. javanica extract was measured and resulted showing $64\;{\mu}g/m{\ell}$ for E. tarda and $256\;{\mu}g/m{\ell}$ for E. coli. It is needed that, from these results, further purification and isolation of reposible compound of these activities and further study on the synergy effect using combination with antibiotics against pathogenic bacteria.

A Case of Churg-Strauss Syndrome with Multiple Tracheobronchial Mucosal Lesions (기관과 기관지내 다발성 점막 병변을 동반한 Churg-Strauss 증후군 1예)

  • Churg-Strauss syndrome is a rare form of systemic necrotizing vasculitis that occurs exclusively in patients with asthma, and is associated with blood and tissue eosinophilia. The classic pathology findings in the lung include a combination of eosinophilic pneumonia, granulomatous inflammation and necrotizing vasculitis. However, there are few reports of tracheobronchial mucosal lesions in Churg-Strauss syndrome. We report a case of Churg-Strauss syndrome with multiple tracheobronchial mucosal lesions in a 33-year-old man with a history of bronchial asthma and allergic rhinitis. He had been diagnosed with community acquired pneumonia at another hospital and was treated with antibiotics. However, the chest radiographic findings were aggravated and showed multifocal consolidations in the whole lung fields. He was transferred to the Asan Medical Center. Fiberoptic bronchoscopy revealed multiple nodular mucosal lesions of the trachea and bronchi. The histopathology of the mucosal lesions revealed necrotizing bronchial inflammation with eosinophilic infiltration. Video Assisted Thoracic Surgery was performed. The wedge resected lung tissue revealed chronic eosinophilic pneumonia that was consistent with Churg-Strauss syndrome. Methylprednisolone (1 mg/kg q 8 hr) was prescribed and his symptoms resolved gradually. The chest radiographic findings improved significantly, and a follow-up fiberoptic bronchoscopy performed eight days later showed that the tracheobronchial mucosal lesions had resolved. The patient was prescribed oral prednisolone for 20 months after discharge. Currently, the patient is not taking steroids and is being followed up.

The Effect of Platelet Rich Plasma Combined with Bovine Bone on the Treatment of Grade II Furcation Defects in Beagle Dogs (성견 2급 치근이개부 병변 치료시 이종골 이식 및 혈소판 농축 혈장의 골재생에 관한 효과)

  • New techniques for regenerating the destructed periodontal tissue have been studied for many years. Current acceptable methods of promoting periodontal regeneration are basis of removal of diseased soft tissue, root treatment, guided tissue regeneration, graft materials, and biological mediators. Platelet Rich Plasma has been reported as a biological mediator which regulates activities of wound healing progress including cell proliferation, migration, and metabolism. The purpose of this study is to evaluate the effects of using the Platelet Rich Plasma as a regeneration promoting agent for furcation involvement defect. Five adult beagle dogs were used in this experiment. The dogs were anesthetized with Ketamin HCl(0.1 ml/kg, IV)and Xylazine hydrochloride($Rompun^{(R)}$, Bayer, 0.1 ml/kg, IM) and conventional periodontal prophylaxis were performed with ultrasonic scaler and hand instruments. With intrasulcular and crestal incision, mucoperiosteal flap was elevated. Following decortication with 1/2 high speed round bur, degree II furcation defect was made on mandibular third(P3), forth(P4) and fifth(P5) premolar, and stopping was inserted. After 4 weeks, stopping was removed, and bone graft was performed. Ca-P was grafted in P3(experimental group I), Combination of Ca-P and plasma rich platelet were grafted in P4(experimental group II), and P5 was remained at control group.Systemic antibiotics(gentamicin sulfate)and anlgesics(phenyl butazone) were administrated intramuscular for 2 weeks after surgery. Irrigation with 0.1% Chlorhexidine Gluconate around operate sites was performed during the whole experimental period except one day immediate after surgery. Soft diets were fed through the whole experiment period. After 4, 8 weeks, the animals were sacrificed by perfusion technique. Tissue block was excised including the tooth and prepared for light microscope with Gomori's trichrome staining. At 4 weeks after surgery, there were rapid osteogenesis phenomenon on the defected area of the Platelet Rich Plasma plus Ca-P BBP group and early trabeculation pattern was made with new osteoid tissue produced by activated osteoblast. Bone formation was almost completed to the fornix of furcation by 8 weeks after surgery. In conclusion, Platelet Rich Plasma can promote rapid osteogenesis during healing of periodontalregeneration.

CTX-M β-lactamase and plasmid-mediated quinolone resistance genes in cefotaxime-resistant gram-negative bacteria isolated from companion animals (반려동물에서 분리된 cefotaxime 내성 그람 음성균에서 CTX-M β-lactamase와 plasmid 매개 퀴놀론 내성 유전자)

  • The aim of this study was to investigate the prevalence of CTX-M β-lactamase and plasmid-mediated quinolone resistance (PMQR) genes, and the pattern of antibiotic resistance in cefotaxime-resistant gramnegative bacteria. A total 126 gram-negative bacteria were isolated from hospitalized dogs and cats between 2018 and 2019. The most predominant isolates were E. coli (n=41), followed by Pseudomonas aeruginosa (n=25), Proteus mirabilis (n=14), Klebsiella pneumoniae (n=9), Sphingomonas paucimobilis (n=7), and Enterobacter cloacae and Serratia marcescens (respectively, n=5). Cefotaxime-resistant isolates were identified in 26.2% (33 isolates) of 126 gram-negative bacteria. CTX-M type β-lactamase were found in 15 isolates (10 E. coli, 1 Ent, cloacae and 4 K. pneumoniae, respectively). Among the CTX-M producing gram-negative bacteria, CTX-M-1 and CTX-M-9 were detected in 10 (66.7%) and 5 (33.3%) isolates, respectively. While, CTX-M-2 and CTX-M-8 were not found. PMQR genes were detected in 12 (36.4%) isolates (4 E. coli, 2 Ent, cloacae and 6 K. pneumoniae, respectively), and the predominant PMQR gene was aac(6')-lb-cr (n=9), followed by qnrB (n=8) and qnrS (n=1) alone or in combination. qnrA and qepA were not found. Additionally, 9 (60%) of 12 PMQR positive isolates were co-existence with CTX-M-1 or CTX-M-9. CTX-M or PMQR producing isolates showed highly resistance to penicillins (100%), cephalosporins (100~66.7%), monobactams (72.2%), and non-β-lactam antibiotics (94.4~61.1%) such as quinolones, trimethoprim/sulfamethoxazole, tetracycline and gentamicin. These findings showed CTX-M-1, CTX-M-9, aac(6')-lb-cr and qnrB were highly prevalent in cefotaxime-resistant Enterobacteriaceae isolates from companion animals in our region. Moreover, PMQR genes were closely associated with CTX-M type β-lactamase.

Effects of the Combination Feeding of β-glucan and Multi-species Probiotics on Growth Performance and Microflora in Broiler Chickens (효모변이균주 유래 β-글루칸과 복합균종 생균제의 혼합급여가 육계의 생산성 및 장내 균총에 미치는 영향)

  • This experiment was carried out to investigate the combination feeding of β-glucan and multi-species probiotics on growth performance, various lipid concentrations of serum, antibody production and cecal microbial profiles in broiler chicks. A total of six hundred 1-d-old male broiler chicks were divided into five groups, placed into four pens per group(30 birds per pen) and fed one of five non-medicated experimental diets(T1, 0.15% multi-species probiotics; T2, 0.1% β-glucan+0.15% multi-species probiotics; T3, 0.3% multi-species probiotics; T4, 0.1% β-glucan+0.3% probiotics or devoid them as control) for 5 wk. There was no significant difference in feed intake among the groups. The average weight gains and FCR in groups fed diet containing 0.3% probiotics were significantly improved(p<0.05) than control in finisher period(22-35d). The concentration of serum cholesterol ester in groups fed 0.3% probiotics were significantly lowered(p<0.05) as compared to that of the control. Relative weights of liver, spleen, bursa of Fabricius, breast and leg were not influenced by the dietary treatments. The average ND or IB antibody titers in groups fed diets containing β-glucan and probiotics were tended to be increased, but not significantly. The number of cecal lactic acid bacteria was significantly increased(p<0.05) by the dietary β- glucan and probiotics. These results indicated that dietary β-glucan and probiotics exerted a growth- promoting and immune-enhancing effects on broiler chicks. In addition, yeast derived β-glucan, and multi-species probiotics modulated the profiles of cecal microflora, reflecting potential alternative substances to replace antibiotics for feeding broiler.
