농촌지역의 B형, C형 간염에 관한 혈청역학적 연구 (Seroepidemiologic Study of Hepatitis B Virus (HBV) and Hepatitis C Virus (HCV) Infection among Koreans in Rural Area, Korea)

  • To estimate the prevalence of hepatitis B virus (HBV) and hepatitis C virus (HCV) infection and to determine associated risk factors, a population-based seroepidemiologic study was carried out. In 1993, a health examination survey of the population was carried out in rural area known to have a high incidence of liver cancer. The study population were those who volunteered to participate in a health survey over 10 years of age. Examinees were interviewed by specially trained staffs. Sera from 1,033 study subjects were tested for hepatitis B surface antigen (HBsAg) by .everse passive hemagglutinin (RPHA) estimation and for hepatitis C virus antibody (anti-HCV) by 2nd generation passive hemagglutinin (PHA) estimation. The age and sex standardized prevalence of HBsAg was 6.3% which was similar to national average, but that of anti-HCV was 5.1% which was 4 to 5 times higher than that of blood donors or other health examinees in Korea. In a multivariate analysis, transfusion history, surgical operative history, and acupuncture history were not associated with HBsAg positivity. In contrast, acupuncture history (adjusted odds ratio[OR]=2.2 : 95% Confidence interval[CI] 1.0-4.7) and surgical operative history(adjusted OR=2.0 : 95% CI 1.0-4.1) were associated with anti-HCV positivity. The present study suggest that there is an highly endemic area of HCV infection in Korea and probably this endemicity is associated with a parenteral source of HCV infection other than blood transfusion.

Synthesis and Antiviral Activity of 2'(β)-Hydroxymethylated Carbodine Analogues Against Hepatitis C Virus

  • 2'($\beta$)-Hydroxymethylated adenosine is a potent and selective inhibitor of hepatitis C virus (HCV) replication. It targets the RNA-dependent RNA polymerase of HCV, NS5B. Synthesis and antiviral evaluation of carbocyclic versions are described. The cyclopentene intermediate ($9\beta$) was successfully synthesized through sequential Johnson-Claisen orthoester rearrangement and ring-closing metathesis (RCM). Coupling of bases via a Pd(0) catalyst, selective dihydroxylation, and desilylation yielded the target nucleoside analogues. The compounds 17 and 18 were assayed for their ability to inhibit HCV RNA replication in a subgenomic replicon Huh7 cell line and showed moderate antiviral activity with toxicity up to 20.0 and 24.7 ${\mu}g/mL$, respectively.

  • In this paper, we study a within-host mathematical model of HCV infection and carry out mathematical analysis of the global dynamics and bifurcations of the model in different parameter regimes. We explore the effect of reverse transcriptase inhibitors (RTI) on spontaneous HCV clearance. The model can produce all clinically observed patient profiles for realistic parameter values; it can also be used to estimate the efficacy and/or duration of treatment that will ensure permanent cure for a particular patient. From the results of the model, we infer possible measures that could be implemented in order to reduce the number of infected individuals.

Methamphetamin 남용입원환자들의 C형간염 항체 양성률에 관한 조사 (Seroprevalence of antibody to the hepatitis C virus in methamphetamine abusers)

  • 약물남용자들의 C형간염 감염 상태를 파악하고자 methamphetamine 남용에 의한 중독 증상으로 입원하고 있는 141명을 대상으로 하여 약물남용의 빈도와 기간, 약물 투여경로 등에 관한 조사와 함께 혈청내 C형간염 항체, B형간염 표식자 및 간기능 검사를 실시하고 이들 사이의 관련성을 검토하여 다음과 같은 결과를 얻었다. 1. C형간염 항체 양성률은 60.3%(85/141)이었으며 성별로는 차이가 없었으나 연령군별로는 40세 이상군에서 증가되는 경향이 있었다. 2. C형간염 항체의 양성률은 약물남용의 빈도가 높았던 군과 약물남용의 기간이 길었던 군에서 유의하게 높았으나 B형간염 표면항원의 양성률은 약물남용의 빈도 및 기간과 유의한 차이가 없었다. 이상의 결과로 보아 methamphetamine 남용자에서 C형간염 유병률은 매우 높을 것이며 감염 전파경로에 있어서 C형간염은 반복적인 경주적 폭로가 중요 전파 요인일 것으로 추측된다.

Purification and Characterization of HCV RNA-dependent RNA Polymerase from Korean Genotype 1b Isolate: Implications for Discovery of HCV Polymerase Inhibitors

  • The nonstructural protein 5B (NS5B) of hepatitis C virus (HCV) is the viral RNA-dependent RNA polymerase (RdRp), which is the essential catalytic enzyme for the viral replication and is an appealing target for the development of new therapeutic agents against HCV infection. A small amount of serum from a single patient with hepatitis C was used to get the genome of a Korean HCV isolate. Sequence analysis of NS5B 1701 nucleotides showed the genotype of a Korean isolate to be subtype 1b. The soluble recombinant HCV NS5B polymerase lacking the C-terminal 24 amino acids was expressed and purified to homogeneity. With the highly purified NS5B protein, we established in vitro systems for RdRp activity to identify potential polymerase inhibitors. The rhodanine family compounds were found to be potent and specific inhibitors of NS5B from high throughput screening (HTS) assay utilizing the scintillation proximity assay (SPA) system. The binding mode of an inhibitor was analyzed by measuring various kinetic parameters. Lineweaver-Burk plots of the inhibitor suggested it binds not to the active site of NS5B polymerase, but to an allosteric site of the enzyme. The activity of NS5B in in vitro polymerase reactions with homopolymeric RNA requires interaction with multiple substrates that include a template/primer and ribonucleotide triphosphate. Steady-state kinetic parameter, such as Km, was determined for the ribonucleotide triphosphate. One of compounds found interacts directly with the viral polymerase and inhibits RNA synthesis in a manner noncompetitively with respect to UTP. Furthermore, we also investigated the ability of the compound to inhibit NS5B-directed viral RNA replication using the Huh7 cell-based HCV replicon system. The investigation is potentially very useful for the utility of such compounds as anti-hepatitic agents.

  • HCV의 가족 내 집단 감염은 드물게 발생하며 배우자에서 감염의 위험이 가장 높고 수직 감염의 빈도는 매우 낮게 보고되고 있다. 저자들은 환자와 환자의 어머니, 외할머니, 이모, 이종 사촌 언니에서 HCV 감염이 확인되어 가족 구성원의 50%가 HCV에 감염된 극히 드문 가족 내 집단 감염을 경험하였기에 문헌 고찰과 함께 보고한다.

의료종사자의 주사침 손상 사고 후 감염예방처치와 혈액매개바이러스 혈청양성전환 (Post-exposure Treatment and Seroconversion to Blood-borne Viruses after Needlestick Injuries among Healthcare Personnel)

  • Purpose: Needlestick injuries (NSI) is the most frequent occupational hazard for healthcare personnel (HCP), and immediate report and adequate post-exposure prophylaxis (PEP) is essential in preventing occupational transmission of blood-borne pathogens. Methods: From June 2010 to October 2010, 544 NSI were reported through websites from 21 general hospitals in Korea. Among those, 499 cases of NSI were analyzed to identify the rate of follow-up treatment completion and for seroconversion. Results: 88.2% of the cases were completed with follow-up treatment, 8.8% of the NSI were not completed with follow-up treatment, and 5 cases were unavailable to trace. 4.2% cases of NSI required a hepatitis B vaccination concurrent with hepatitis B immunoglobulin. 41.1% of the cases and 31.1% of the cases needed to be tested for anti HCV and anti HIV, respectively. Prophylaxis medication for HIV was prescribed in 3 cases, and all cases completed required 1 month of medication. There was 1 case (0.2%) of seroconversion to HCV. Conclusion: The PEP completion rate was not satisfactory, and the importance of completion of PEP treatment should be emphasized through education and counseling. Also, a careful risk assessment is needed for HCP who are exposed to HCV or HIV.

C형 간염바이러스(HCV) 유전체를 특이적으로 변형할 수 있는 Trans-Splicing Aptazyme 발굴 (Development of Trans-Splicing Aptazyme Which Can Specifically Modify Hepatitis C Virus Genome)

  • C형 간염바이러스(hepatitis C virus; HCV) 복제를 효과적이며 특이적으로 제어할 수 있는 유전산물을 개발하기 위하여 특정 리간드 존재에 의해 allosteric하게 그 활성이 조절될 수 있는 HCV 유전체 표적 trans-splicing 리보자임(trans-splicing aptazyme)을 발굴하였다. 이러한 trans-splicing aptazyme은 특정 리간드와 특이적으로 결합하는 RNA aptamer 부위, aptamer와 리간드와의 결합에 의해 리보자임 활성을 유도할 수 있도록 구조적 변이를 전달할 수 있는 communication module부위 및 HCV IRES의 +199 nt를 인지하는trans-splicing리보자임 등으로 구성되도록 설계하였다. 특히 trans-splicing 리보자임의 catalytic core의 P6과 P8 부위에 aptamer와 communication module을 삽입하였을 때 가장 allosteric하게 리보자임 활성이 유도되었다. 이러한 리보자임은 리간드가 없거나 대조 리간드가 존재할 때에는 trans-splicing 반응을 유도하지 못하였으나 특정 리간드가 존재할 때에만 효과적이며 특이적으로 trans-splicing 반응을 유도하여 표적 RNA를 변형시킬 수 있음을 관찰하였다. 이러한 aptazyme은 HCV 증식에 대해 특이적이며 효과적인 억제를 위한 선도물질로 이용 가능할 뿐 아니라 HCV 치료선도 물질의 스크리닝용 도구로서도 활용될 수 있을 것이다.
