Prevalence of Toxocariasis and Its Risk Factors in Patients with Eosinophilia in Korea

  • Eosinophilia occurs commonly in many diseases including allergic diseases and helminthic infections. Toxocariasis has been suggested as one cause of eosinophilia. The present study was undertaken to examine the prevalence of toxocariasis in patients with eosinophilia and to identify the risk factors for toxocariasis. This prospective cohort study recruited a total of 81 patients with eosinophilia (34 males and 47 females) who visited the outpatient clinic at Seoul National University Hospital from January 2017 to February 2018 and agreed to participate in this study. The prevalence of toxocariasis was examined by T. canis-specific ELISA, and the various risk factors for toxocariasis were evaluated by a questionnaire survey. Among 81 patients with eosinophilia, 18 were positive for anti-T. canis antibodies (22.2%); 88.9% were male (16/18) and 11.1% were female (2/18). Multivariate statistical analysis revealed that males (OR 21.876, 95% CI: 1.667-287.144) with a history of consuming the raw meat or livers of animals (OR 5.899, 95% CI: 1.004-34.669) and a heavy alcohol-drinking habit (OR 8.767, 95% CI: 1.018-75.497) were at higher risk of toxocariasis in patients with eosinophilia. Toxocariasis should be considered a potential cause of eosinophilia when the patient has a history of eating the raw meat or livers of animals in Korea. A single course of albendazole is recommended to reduce the migration of Toxocara larvae in serologically positive cases with eosinophilia.

Study on Anti-allergic Effect and Safety of Bangpung-galgeun-tang (방풍갈근탕(防風葛根湯)의 항알레르기효과에 관한 연구)

  • The purpose of this study was to examine the anti allergic effect in vivo and in vitro, and to observe single and four weeks repeated toxicity in mice of Bangpung-galgeun-tang (BGT). We investigated anti DNP IgE-mediated passive cutaneous anaphylaxis in rodents and compound 48/80-induced active systemic anaphylatic shock in mice after oral administration with BGT of 0.4 g/kg and 0.8 g/kg for 8 days, and also examined MTT assay, ${\beta}-hexosaminidase$ activity, IL-4 and $TNF-{\alpha}$ from RBL-2H3 and $TNF-{\alpha}$ from Raw264.7 after pre-treatment with BGT of 0.25 mg/ml, 0.5 mg/ml, 1 mg/ml and 2 mg/ml. To ascertain safety and toxicity of BGT, we divided into single and four weeks repeated administration test. In single test, three groups were administrated different dosages and routes (2 g/kg/i.p., 4 g/kg/i.p. and 15 g/kg /p.o.) of BGT, and in four weeks repeated test, 0.8 g/kg BGT was administrated. Control groups were administrated with only saline according to on Korean Food and Drug Administration, respectively. We observed attentively motality, abnormal clinical sign, body weight change, organ weight, AST and ALT of mice after BGT administration. BGT inhibited passive cutaneous anaphylaxis and active systemic anaphylatic shock by oral administration. All the concentrations of BGT from 0.25 to 2 mg/ml didn't have an effect on cell viability and cytotoxicity. In RBL-2H3, ${\beta}-hexosaminidase$ release, IL-4 and $TNF-{\alpha}$, and in Raw264.7, $TNF-{\alpha}$ were significantly reduced by treated all concentrations of BGT. During toxicity experiment period, there was no difference in body weight change, organ weight, AST and ALT among different dose groups. Death were found 3 mice from day 2 to day 3 in single test i.p. group. (2 g/kg, 4 g/kg). Several individuals of single test i.p. group were observed that decreased locomotor activity, exophthalmos, bloodshot eyes, loss of eyesight and so on in early period after administration. But there was no difference in clinical signs among p.o. group. These results indicate that BGT have inhibition effects on allergy and suggest that no observable effect level of the test orally administration was considered to be more than 2 g/kg in mice under the conditions employed in this study.

Grape Pruning Stem Extract (GPSE) Suppresses Allergy and Skin Proliferation Inhibition against UVB Induced Skin Damage (포도전정가지 추출물이 UVB에 유도된 피부 손상에 대한 알레르기와 피부증식 억제효과)

  • This study examined the effects of polyphenols in grape pruning stem extracts (GPSE) using grape stems discarded after harvest. The inhibitory effects on allergy, proliferation, and apoptosis in UVB-induced HR-1 hairless mice were analyzed. The applicability as a material for functional food and functional cosmetics was evaluated. The contents of the active ingredients of GPSE were analyzed by HPLC. After UVB irradiation on the dermis of HR-1 hairless mice, the number of mast cells was determined by toluidine blue staining to confirm that the skin allergic reaction was caused by GPSE. The results were similar to the normal group in the group applied GPSE, and there was no allergic reaction in the GPSE application group and a significant decreased compared to the sun cream control. PCNA immunohistochemical staining of the epidermal proliferation factor had an inhibitory effect on epidermal epithelial cell proliferation in all concentrations of GPSE and serum base mixture as an average of 42% compared to the control group. The mixture of GPSE and serum base suppressed the apoptosis inhibition rate by 27% on average compared to the control. These results confirmed the inhibitory effects GPSE on the allergic, proliferation, and apoptosis activities by with a serum base on UVB-induced skin damage. GPSE is a functional ingredient with potential skin protection effects, and has a high utilization as an ingredient for functional cosmetics.

Effect of Fritillariae Thunbergii Bulbus for Allergic Inflammation on HaCaT and RBL2H3 Cells (HaCaT 세포와 RBL2H3 세포에서 패모 추출물의 알레르기 염증 완화 효과)

  • Objectives : The aim of this study was to investigate the effect for allergic-inflammation of Fritillariae Thunbergii Bulbus (FTB) on HaCaT cells and RBL2H3 cells. Methods : To investigate the effects of FTB for anti-inflammation in HaCaT cells, the cells were pretreated with FTB for 1h and then stimulated with $TNF-{\alpha}/IFN-{\beta}$ for 24h. Then thymus and activation-regulated chemokine (TARC) and Macrophage-derived chemokine (MDC) levels were analyzed with ELISA kit. Also to investigate the effect of skin barrier protein, the cells were treated with FTB of various concentrations, and then cells were harvested, expressions of skin barrier protein were measured with RT-PCR. To investigate the effects of FTB for anti-allergy in RBL2H3 cells, the cells were pre-treated with FTB for 1h, and then stimulated with A23187 for 30 min. ${\beta}$-hexosaminidase, IL-4 and $TNF-{\alpha}$ were measured using cultured media. The cells were harvested to analyze the mechanism of the effect for FTB via Western blot. Results : FTB did not show cytotoxicity in HaCaT and RBL2H3. In HaCaT cells, FTB significantly suppressed the expression of TARC, MDC at a dose-dependent manner and markedly increased formation of the skin barrier proteins. In RBL2H3 cells, FTB decreased release of the ${\beta}$-hexosaminidase, IL-4 and $TNF-{\alpha}$ in RBL2H3 through inhibition of the phosphorylation of JNK and p38, which are include in the signaling mechanism of MAPK Conclusion : These results indicate that FTB has an anti-inflammatory effect on the allergic response through blocking MAPK pathway. This suggest that FTB could be a therapeutic agent for allergic response.

Anti-allergic Effects of Chungshimbohyeoltang in RBL-2H3 Mast Cells and OVA/alum Sensitized Mice (RBL-2H3 비만세포와 OVA/alum 감작 생쥐에서 청심보혈탕(淸心補血湯)의 항알레르기 효과)

  • Objectives The suppressive effect of CSBHT has been mysterious. Thus, the present study is designed to investigate the suppressive effect and its mechanism. Methods To investigate the anti-allergy effect from ChungShimBoHyeolTang(CSBHT), RBL-2H3 cell was used and examined by Real-Time PCR, and IL-4 and IL-13 from RBL-2H3 was examined by ELIS. In addition, GATA-1, GATA-2, NFAT-1, NFAT-2, c-Fos, c-Jun, NF-${\kappa}B$ p65 transcription factors of RBL-2H3 mast cell were examined by Western Blotting. Also, OVA/alum-sensitized mice were orally administrated CSBHT and serum OVA-specific IgE production, IL-4, and IL-13 production in splenocytes supernatant were examined. Results As a result of treating with CSBHT extract, RBL-2H3 mast cells significantly suppressed the IL-4 and IL-13 mRNA expression and IL-4 and IL-13 production. Western blot analysis of transcription factors involving IL-4 and IL-13 expression also revealed a prominent decreases of mast cell's specific transcription factors including GATA-1, GATA-2, NFAT-1, NFAT-2, c-Fos, and NF-${\kappa}B$ p65. Also, examining the mice, administration of CSBHT suppressed the amount of OVA-specific IgE in OVA/alum-sensitized mice and IL-4 and IL-13 production in splenocytes supernatant. Conclusions The study suggested that the anti-allergic activities of CSBHT suppresses IL-4 and IL-13 production from the Th2 cytokines by suppressing transcription factors as GATA-1, GATA-2, NFAT-1, NFAT-2, c-Fos and NF-${\kappa}B$ p65 in mast cells.

Effects of Mix-1 on Anti-CD40 Antibody and Recombinant IL4- Induced Cytokine Production and Immunoglobulin E in Highly Purified Mouse B Cells

  • In the oriental medicine, a mixture of herbs has been commonly used as important components to control allergic and inflammatory diseases. In the present study, we prepared a mixture of Dictamni Radicis Cortex(Baiksunpee), Houttuyniae Herba(Uhsungcho), and Aurantii Immaturus Fructus(Jisil) to examine its anti-allergic effects in activated mouse splenic cells and found that Mix-1 is involved in regulating levels of B cell activating factors (CD23 and CD11a), IL-1β, IL-6, IL-10, TNF-α, and 1gE as well as HRF expression. It was observed that Mix-1 did not have cytotoxic effects on mLFC. Mix-1 showed inhibition of CD23 and CD11 alpaha expression in mouse B cells, and also decreased the production of IL-6, TNF-α, and 1gE. Both RT-PCR and ELISA analyses indicated that IL-6 and TNF alpha production were regulated at the gene expression level. In contrast, IL-10 mRNA and protein levels were increased in activated B cells by Mix-1 treatment. We also found that Mix-1 inhibited B cell proliferation and inhibited histamine releasing factor(HRF) expression, suggesting its inhibitory effect on histamine secretion. These data indicated that Mix-1 has an anti-allergic effect in activated macrophages and further suggest the possible application of Mix-1 as a therapeutic agent for the treatment of allergy-related diseases.

Ocular complications in patients with anti-adhesion barrier agent during arthroscopic shoulder surgery (견관절경 수술에서 사용된 유착방지제와 수술 후 안구 합병증의 연관성)

  • Background: Intraoperative and postoperative ocular complications are rare. We retrospectively investigated the abnormal ocular findings in patients who underwent arthroscopic shoulder surgery. Methods: The records were investigated of patients who underwent shoulder arthroscopic surgery at our hospital between January 2018 and March 2019. Ocular complications were confirmed by review of medical records. We performed logistic regression analysis to defined the main factors associated with ocular complications. Results: The overall prevalence of postoperative ocular complications was 8.5% (18/211). A significant association was found between anti-adhesion barrier agent and ocular complications (p=0.020). Conclusions: Anti-adhesion barrier agent used during surgery may be the cause of postoperative ocular complication.

The Inhibitory Effect of Premature Citrus unshiu Extract on Atopic Dermatitis In Vitro and In Vivo

  • Atopic dermatitis (AD) is a chronic, recurrent inflammatory skin disease that is associated with Th2 cell-mediated allergy. The process that leads to infiltration of inflammatory cells into an AD lesion is remarkably dependent on various chemokines, especially TARC (thymus and activation-regulated chemokine/CCL17) and MDC (macrophage-derived chemokine/CCL22). Serum levels of these chemokines are over-expressed in AD patients. Citrus unshiu, which is known as Satsuma mandarin, has anti-oxidative, anti-inflammation, and anti-microviral activity. Therefore, we investigated the effect of EtOH extract of premature C. unshiu on AD. We did this using a DNCB-induced AD mouse model. We also tried to confirm an inhibitory effect for premature C. unshiu on the expression of inflammatory chemokines in IFN-${\gamma}$ and TNF-${\alpha}$ stimulated HaCaT human keratinocytes. We found that extract of premature C. unshiu reduced DNCB-induced symptoms such as hyperkeratosis, increased skin thickness, and infiltrated mast cells, in our AD-like animal model. The extract decreased levels of IFN-${\gamma}$ and IL-4 in ConA-stimulated splenocytes isolated from DNCB-treated mice. Also, extract of premature C. unshiu inhibited mRNA expression and protein production of TARC and MDC through the inhibition of STAT1 phosphorylation. These results suggest that C. unshiu has anti-atopic activity by regulating inflammatory chemokines such as TARC and MDC.

The Rhizomes of Acorus gramineus and the Constituents Inhibit Allergic Response In vitro and In vivo

  • The rhizomes of Acorus gramineus have frequently been used in traditional medicine mainly for sedation as well as enhancing brain function. In this study, the anti-allergic activity of A. gramineus was investigated. The 70% ethanol extract of the rhizomes of A. gramineus was found to inhibit the allergic response against 5-lipoxygenase (5-LOX)-catalyzed leukotriene (LT) production from rat basophilic leukemia (RBL)-1 cells and ${\beta}$-hexosaminidase release from RBL-2H3 cells with $IC_{50}$'s of 48.9 and > $200{\mu}g/ml$, respectively. Among the 9 major constituents isolated, ${\beta}$-asarone, (2R,3R,4S,5S)-2,4-dimethyl-1,3-bis (2',4',5'-trimethoxyphenyl)tetrahydrofuran (AF) and 2,3-dihydro-4,5,7-trimethoxy-1-ethyl-2-methyl-3-(2,4,5-trimethoxyphenyl)indene (AI) strongly inhibited 5-LOX-catalyzed LT production in A23187-treated RBL-1 cells, AI being the most potent ($IC_{50}=6.7{\mu}M$). Against ${\beta}$-hexosaminidase release by antigen-stimulated RBL-2H3 cells, only AI exhibited strong inhibition ($IC_{50}=7.3{\mu}M$) while ${\beta}$-asarone and AF showed 26.0% and 39.9% inhibition at $50{\mu}M$, respectively. In addition, the ethanol extract of A. gramineus showed significant inhibitory action against the hapten-induced delayed hypersensitivity reaction in mice by oral administration at 200 mg/kg. Therefore, it is suggested that A. gramineus possesses anti-allergic activity and the constituents including ${\beta}$-asarone and AI certainly contribute to the anti-allergic activity of the rhizomes of A. gramineus.

Gamipaidok-san Possesses Antiallergic and Anti-inflammatory Activities

  • Gamipaidok-san(GPDS) is the Hyungbangpaidok-san prescription fortified with the additional ingredients known to be effective for dermatitis. So it has been used for atopic dermatitis in the clinic work actually. In this study, we investigated the effects of GPDS on in vitro and in vivo anti-allergic effect on RBL-2H3 rat basophilic leukemia Cells and on IgE-induced passive cutaneous anaphylaxis (PCA) in mice. The in vitro anti-inflammatory activity of GPDS in RAW 264.7 cells was investigated. GPDS potently inhibited $\beta$-hexosaminidase release from RBL-2H3 and the IgE-mediated PCA reaction in mice. GPDS inhibited LPS-induced NO and PGE2 production in a dose-dependent manner Furthermore, It also inhibited inducible nitric oxide synthase (iNOS) and cyclooxygenase-2 (COX-2) protein expression in RAW 264.7 cells, and the activation of the transcription factor, NF-kB, in nuclear fraction. The antiallergic action of GPDS may originate from anti-inflammatory activities, and can improve the inflammation caused by allergies.
