The effect of balance training with plantar flexor stretching on range of motion, balance, and gait in stroke patients: a randomized controlled pilot trial

  • Objective: The aim of this study was to investigate the effect of balance training with plantar flexor stretching on ankle dorsi flexion range of motion (ROM), balance, and gait ability in stroke patients. Design: A randomized controlled pilot trial. Methods: Thirty stroke patients volunteered to participate in this study. The subjects were randomly allocated to two groups: the experimental group (n=15) received the neurodevelopment therapy plus balance training with plantar flexor stretching for 20 minutes in one session. The control group (n=15) received the same neurodevelopment therapy plus plantar flexor static stretching for 20 minutes in one session. Both groups underwent sessions four times a week, for a total of 4 weeks. Measurements included passive range of motion (PROM), active range of motion (AROM) of ankle dorsiflexion using a goniometer, timed up and go (TUG), the functional reaching test (FRT), and the 10 m walk test (10 MWT). Results: There were significant improvements in AROM and PROM of ankle dorsiflexion, TUG, and FRT scores after the intervention in the experimental group (p<0.05). However, the control group showed no statistically significant differences except for PROM of ankle dorsiflexion. The experimental group showed a significant improvement in PROM, TUG, and FRT scores compared to the control group (p<0.05). Conclusions: Balance training with plantar flexor stretching improves ankle dorsiflexion ROM and balance ability in patients with stroke. Therefore, this therapeutic intervention will be effective for rehabilitation of stroke patients in the clinical setting.

발목 근 피로 시 테이핑, 스트레칭 그리고 초음파 중재가 융합적 균형에 미치는 효과 (The Effect of Taping, Stretching and Ultrasound Intervention on Convergent Balance with Ankle Muscle Fatigue)

  • 본 연구의 목적은 유도된 발목 근피로 시 테이핑, 스트레칭 그리고 초음파가 융합적 균형능력에 미치는 효과를 알아보는 데 있다. 건강한 성인 남녀 39명이 본 연구에 자발적으로 참여하였다. 발목 근피로 유발 이후 대상자들은 테이핑, 정적 스트레칭 또는 초음파 그룹으로 무작위 선정되었다. 그리고 각 그룹에 해당하는 중재방법이 적용되었다. 균형 능력은 Space Balance 3D를 이용하여 발목 근피로 전과 후 그리고 중재 후에 측정되었다. 균형능력은 그룹 간 유의한 차이는 없었지만(p>.05), 그룹 내 모두에서 유의하게 증가하였다(p<.001). 그러므로 발목 근 피로에 테이핑, 정적 스트레칭 그리고 초음파는 융합적 균형능력에 효과적일 것이다. 향후 연구에서는 다양한 측정변수 측정과 치료적 중재를 적용하여 치료적 효과를 증명할 필요가 있다.

자가 스트레칭과 도수치료가 런지 테스트에 미치는 영향 (Self stretching and manual therapy influence on lunge test)

  • Purpose : To determined the effect of self-stretching and manual therapy on ankle joint about ROM and persisting effect. Method : Twenty participants(6 male, 14 female) ramdomly divided into self-stretching and manual therapy group. Each participant started with lunge test then self-stretching and manual therapy intervention was performed. After intervention lunge test was performed again. Last lunge test was performed after 30 minute break. Result : Result of lunge test showed both self-stretching and manual therapy group have significant difference in immediate increase of ROM and persistence effect(p<0.05). However, persistence effect of each group did not show significant difference(p>0.05). Conclusion : Manual therapy and well-designed self-stretching by therapist showed the effect in increase ROM.

Acute Effects of Dynamic Stretching and Self-Mobilization of the Ankle Joint on Dorsiflexion Range of Motion, Muscle Strength, and Balance in Healthy Adults

  • Purpose : Several studies have investigated the effects of dynamic stretching (DS) and self-mobilization (SM), however, studies comparing the two interventions are rare. Therefore, the purpose of this study was to compare the effects of DS and SM on ankle strength, dorsiflexion range of motion (DFROM), and balance to determine which is superior. Methods : Thirty-two healthy young adults participated in this study. Participants were randomly assigned to two groups (SM and DS). DS was performed for the purpose of stretching the medial gastrocnemius muscle. For the SM group, ankle joint SM was performed in three ways. For all participants, the following measurements were performed as pre- and post-tests: isometric strength of dorsiflexor and plantar flexor, weight-bearing lunge test (WBLT) to evaluate DFROM, Tetrax system to evaluate static balance, and y balance test (YBT) to evaluate dynamic balance. Differences before and after the intervention within each group were compared using paired t-test. Also, the variable's variation was compared between groups using an independent t-test. Results : Significant differences were found in ankle dorsiflexor strength, WBLT, YBT, weight distribution index (WDI) (pillow and opened eyes; PO), and stability index (ST) (normal and closed eyes; NC) before and after intervention in the SM group (p<.05). In the DS group, significant differences were found in ankle dorsiflexor and plantar flexor strength, WBLT, YBT anterior, WDI (normal and opened eyes; NO, PO), and ST (NO, NC, PO, pillow and closed eyes) before and after the intervention (p<.05). Ankle plantar flexor strength and WDI (PO) were significantly different between groups. Conclusion : Based on the results of this study, DS or SM can be considered as a possibility for selective use according to variables for improving ankle joint function (DFROM, muscle strength, balance).

Effect of foam roller, kinesiotaping and dynamic stretching on gait parameters with induced ankle muscle fatigue

  • Objective: The purpose of this study was to investigate the effects of foam roller (FR) stretching, kinesiotaping (KT), and dynamic stretching (DS) on gait parameters after inducing muscle fatigue in the ankle joint. Design: Cross-sectional study. Methods: The subjects were thirty healthy young adults between the ages of 20 and 31 years at Baekseok University who voluntarily participated in this study. The participants were randomly assigned to either the FR group, KT group, or the DS group after inducing muscle fatigue of the ankle joint. Fatigue induction of the ankle joint muscles was performed by alternating a heel up and down exercise with the standing posture on the ground. The speed was maintained at 40 beats/minute using a metronome. Subsequently, the respective intervention was applied to each group. Gait parameters were measured before and after ankle muscle fatigue induction, and after intervention using the GAITRite system. One-way ANOVA was used to compare gait parameters among groups, while repeated measures ANOVA was used to compare gait parameters within each intervention group. Results: The FR group increased significantly in velocity, step length, and stride length except for cadence after intervention compared to after ankle muscle fatigue induction (p<0.01). Furthermore, the KT group showed significant increases in velocity, cadence, step length, and stride length after intervention, especially in cadence group (p<0.05). All intervention groups showed significant increases in stride length after intervention, especially the DS group (p<0.05). Conclusions: Therefore, we suggest that KT, FR, and DS can be an effective intervention on gait parameters when the ankle joint is unstable and injured.

Comparison of the Duration of Maintained Calf Muscle Flexibility After Static Stretching, Eccentric Training on Stable Surface, and Eccentric Training on Unstable Surfaces in Young Adults With Calf Muscle Tightness

  • The objective of this study was to determine the duration of maintained calf muscle flexibility gained in young adults with calf muscle tightness, as measured by increases in ankle active and passive dorsiflexion range of motion (DFROM) after three stretching interventions. Twenty subjects (5 men and 15 women) with calf muscle tightness received the following three stretching interventions in one leg (assigned at random): static stretching (SS), eccentric training on stable surface (ETS), and eccentric training on unstable surfaces (ETU). The subjects received all three interventions to the same leg, applied in a random order. Each intervention had a break of at least 24 h in-between, in order to minimize any carryover effect. Each intervention used two types of stretching: with the calf muscle stretched and both knees straight, and with the knee slightly bent in order to maximize the activation of the soleus muscle. All three interventions were performed for 200 seconds. We measured the duration of maintained calf muscle flexibility through active and passive ankle DFROM before intervention, immediately after intervention (time 0), and then 3, 6, 9, 15, and 30 min after intervention. We found a difference in the duration of maintained calf muscle flexibility between the three interventions. In the ETS and ETU interventions, a significant improvement in calf muscle flexibility, both ankle active and passive dorsiflexion ranges of motion (ADFROM and PDFROM), was maintained for 30 min. In the SS intervention, however, ADFROM before 9 min and PDFROM before 6 min were statistically different from the baseline. Our results suggest that ETS and ETU may be more effective than SS for maintaining calf muscle flexibility in young adults.

A Comparative Evaluation of Closed and Open Kinetic Exercises in the Management of Chronic Ankle Instability

  • Background: Repetitive damage to the ankle joint causes chronic ankle instability, and studies comparing the effects of exercise in open and closed chains as a treatment method are very rare. Objectives: To investigate the effects of open and closed kinetic exercises on muscle activity and dynamic balance of ankle joint in adults with chronic ankle instability. Design: Single-blind randomized controlled trial. Methods: The selected 30 subjects are randomly divided into open kinetic chain exercise experimental group (EGI, n=10), closed kinetic chain exercise experimental group (EGII, n=10), and stretching control group (CG, n=10). Open and closed kinetic exercises lasted 30 minutes three times a week for six weeks and stretching exercises performed four actions for 20 seconds and five sets. The measurement tools using surface electromyography to measure muscle activity in the ankle joint. The dynamic balance of the ankle was evaluated using the Y-Balance test. Results: Following the intervention, closed and open kinetic chain exercise group showed significant difference in tibialis anterior and gastrocnemius muscle activity and dynamic balance (P<.05). However, no significant difference in tibialis anterior and gastrocnemius muscle activity and dynamic balance between closed and open kinetic chain exercise group (P<.05). Conclusion: This study provides evidence that closed and open kinetic chain exercise can be presented as an effective exercise for the muscle activity of ankle muscle and dynamic balance of the subject with chronic ankle instability.

스쿼트 동작 시 정적 스트레칭과 근막이완기법이 하지 관절의 운동학적 요인에 미치는 영향 (Effect of Static Stretching and Myofascial Release Techniques on Kinematic Factors of Lower Extremity Joints during Squat )

  • Objective: The purpose of this study was to analyze the differences in kinematic factors according to stretching treatment, myofascial release treatment, and static stretching treatment conditions during squat. Method: Twelve males with resistance training experience participated in this study. Participants performed squats without treatment (Pre-Test), and performed squats after treatment with the myofascial release technique (MRT) and static stretching (SS) on different days (post-test). Squat movements were captured using eight motion capture cameras (sampling rate: 250 Hz), and the peak joint angles of the ankle, knee, hip, and pelvis were calculated for each direction. One-way repeated ANOVA and Bonferroni post hoc analyses using SPSS 27 (IBM Corp. Armonk NY, USA) were used to compare the peak joint angle of the lower extremity joints and pelvis among the normal condition (squat without treatment), MRT condition (squat after MRT treatment) and SS condition (squat after static stretching). The statistical significance level was set at .05. Results: It was observed that the maximum ankle joint flexion angle during squats was statistically reduced under conditions of myofascial release and static stretching (p<.05), in comparison to the scenario where no stretching was performed. Furthermore, static stretching was found to enhance the maximum hip flexion angle during squat (p<.05), whereas the myofascial release stretching technique resulted in the minimal posterior pelvic tilt angle (p<.05). Conclusion: Employing myofascial release stretching as a preparatory exercise proved to be more efficacious in maintaining body stability throughout the execution of high-intensity squat movements by effectively managing the posterior tilt of the pelvis, as opposed to foregoing stretching or engaging in static stretching.

  • 본 연구의 목적은 뇌졸중 환자의 발목관절 가동범위를 증가시키기 위한 중재를 실시한 후 발목가동범위 변화를 비교한 것이다. 본 연구는 총 45명의 뇌졸중 환자를 경사대, PNF 스트레칭 그리고 관절가동술 방법에 따라 각 3군으로 나누어 8주간 실시한 후 발바닥 굽힘과 발등 굽힘을 측정하였다. 발바닥 굽힘은 집단 간에 차이가 유의하게 있었고(p<.001), 시기별 집단 간 차이에서도 차이가 유의하였다. 발등 굽힘은 관절가동술을 적용한 군 내에서 유의하게 증가하였으며(p<.05), 집단 간, 시기별 유의하였다(p<.05). 본 연구를 통해 발목의 가동범위를 증가시키기 위한 방법으로 스트레칭과 관절가동술이 효과가 있음을 알 수 있었다. 향후 연구에서는 뇌졸중 환자의 발목가동범위 증가가 미치는 또 다른 영향에 대한 연구가 있기를 기대한다.

Effect of Elastic Band Exercise on the Plantar Pressure and Maximum Muscle Strength in Female University Students Wearing High Heels

  • PURPOSE: The purpose of this study was to investigate the effect of the elastic band and stretching exercise program on ankle joint maximal voluntary isometric contraction (MVIC) and plantar pressure in high-heel wearing women in their 20s. METHODS: Twenty women in their twenties were randomly assigned to the experimental group (n=10) and the control group (n=10). The experimental group (n=10) performed the elastic band exercise program, while the control group (n=10) performed the stretching exercise program. Both exercise programs were performed three times a week for a total of four weeks. The BTE Primus RS was used in order to measure the ankle joint MVIC during dorsiflexion, plantar flexion, inversion, and eversion. Maximum pressure ($N/cm^2$), average pressure ($N/cm^2$) were measured using the Pedoscan. SPSS v. 21.0 software was used for all statistical analyses in this study. RESULTS: The measurement of the ankle joint's MVIC revealed that in inversion, a significant change in both feet was seen in both the experimental and control groups. In eversion, there was a significant change in both feet only in the experimental group. In terms of the rest of the results, no significant changes were visible. With regard to the plantar pressure, no significant results were seen for either foot in the comparison between or within the groups. CONCLUSION: Exercise program using elastic band and exercise program using stretching were effective on MVIC of ankle joint muscles, although it had no effect on changes of plantar pressure.
