당뇨병성 족부 궤양에 의한 절단술후 상처 치유와 발목-상완 지수, 족지-상완 지수, 족지압의 관계 (Prognostic Factors of Wound Healing after Diabetic Foot Amputation; ABI, TBI, and Toe Pressure)

  • Purpose: The purpose of this study is to establish guidelines for ankle-brachial index (ABI), toe-brachial index (TBI) and toe pressure with regard to healing of diabetic foot amputation wound. Material and Methods: We designed a retrospective study that included patients with diabetic foot ulcer. From 2008 to 2011, 46 patients who had suffered from amputation of a foot due to diabetic foot ulcer were included in this study. We divided them into amputation-success group and amputation-revision group, and compared their ankle-brachial index (ABI), toe-brachial index (TBI) and toe pressure between two groups. Amputation-revision group is that first forefoot amputation is failed to heal successfully and need to have another proximal amputation. Results: Toe pressure was 78 mmHg (54~107) in the amputation success group, 0 mmHg (0~43) in the amputation revision group (p=0.000). Ankle-brachial index was 1.1650(1.0475~1.1975) in the amputation success group, 0.92(0.5275~1.0750) in the amputation revision group (p=0.05), and toe-brachial index was 0.6100(0.4050~0.7575) in the amputation success group, 0.00(0.00~0.4150) in the amputation revision group (p=0.04), respectively. Conclusion: ABI, TBI, toe pressure of amputation success group were significantly higher than those of amputation revision group.

대사증후군을 동반한 허혈성 뇌혈관 질환 환자의 맥파속도에 관한 연구 (Determinants of the Brachial-ankle Pulse Wave Velocity in Patients with Metabolic Syndrome and Ischemic Stroke)

  • The purpose of this study is to investigate the relation between brachial-ankle pulse wave velocity and the features of metabolic syndrome, and differences according to sex in patients diagnosed cerebral infarction. The study group comprised 61 patients over age 30 who were diagnosed cerebral infarction, accompanied with metabolic syndrome. The brachial-ankle pulse wave velocity, blood pressure, lipid profiles, fasting blood glucose, body mass index were measured. Also we checked past history of patients. Then we analyzed the association between brachial-ankle pulse wave velocity and the features of metabolic syndrome. Pearson correlation analysis reflected the variables affecting the brachial-ankle pulse wave velocity as follows : Age, SBP(systolic blood pressure), DBP(diastolic blood pressure), FBS(fasting blood glucose) were positively correlated. As a result of regression analysis, in patients with cerebral infarction accompanied with metabolic syndrome, the brachial-ankle pulse wave velocity is affected by age to men, SBP, FBS, DBP to women. The brachial-ankle pulse wave velocity is not affected by the components of metabolic syndrome, except blood pressure, FBS, in patients with ischemic stroke.

Cardio-Ankle Vascular Index(CAVI), Ankle-Brachial Index(ABI)와 동맥경화 관련 요인과의 상관관계 연구 (The Relationship between Cardio-Ankle Vascular Index (CAVI), Ankle-Brachial Index (ABI), and Factors Related to Arteriosclerosis)

  • Objectives: The cardio-ankle vascular index (CAVI) and ankle-brachial index (ABI) are non-invasive methods to evaluate cardiovascular disease and arteriosclerosis. This study investigated the relationship between CAVI, ABI, and factors related to arteriosclerosis. Methods: This study included 535 healthy adults who underwent health examinations in 2019. We analyzed the correlation between CAVI, ABI and clinical variables. Multiple regression analysis was performed on the independent clinical variables associated with CAVI and ABI. Results: The correlation analysis of CAVI showed that body mass index (BMI) and HDL-cholesterol (HDL-C) had a negative correlation, and the other variables had a significant positive correlation. The correlation analysis with ABI on the right side showed that age, diastolic blood pressure (DBP), gender, and LDL-cholesterol (LDL-C) had a significant positive correlation, while HDL-C had a significant negative correlation. There was no statistical significance on the left. In the multiple regression analysis, DBP, age, BMI, gender, and HDL-C were proved to be independent factors in CAVI (right) (R2=0.365); DBP, age, gender, BMI, fasting blood sugar, and total cholesterol in CAVI (left) (R2=0.357); and age, gender, DBP, and systolic blood pressure in ABI (right) (R2=0.133). There were no statistically significant factors in ABI (left). It could be inferred that smoking and drinking are determinants that play an important role in CAVI. Conclusion: CAVI showed a high correlation with gender, age, and blood pressure. A significant correlation between CAVI and serum lipid values could be observed, but this showed a low correlation coefficient. ABI showed a high correlation with age and DBP. These results support the use of CAVI and ABI as primary diagnostic devices in medical treatment.

난치성 당뇨 족부 궤양에 대한 임상적 고찰: 치료에 영향을 미치는 인자 분석 (Clinical Analysis of Intractable Diabetic Foot Ulcers: Accessing Risk Factors)

  • Purpose: Diabetic foot ulcer is one of the most important diabetic complications because it increases the risk of amputations. Moreover, it lowers the quality of patients' life and increases the social medical expenses. Authors analyzed risk factors of intractable diabetic foot ulcer using retrospective study. Materials and Methods: From January 2007 to December 2010, 40 patients who could not achieve complete healing despite more than 12 weeks of proper management among who had been diagnosed and treated as diabetic foot ulcer at our hospital were included and evaluated retrospectively. We compared the risk factors between two groups who were finally treated by amputation and non-amputation. Results: The sample was composed of 31 male patients (77.5%) and 9 female patients (22.5%). Comorbidity including hypertension and hyperlipidemia were 77.5% and 80% each. By Wagner classification, 30 patients (80%) had ulcerative lesion over the grade 3. From bacteriology results, 29 patients (72.5%) had polybacteria infection. 35 patients (87.5%) had neuropathy and 26 patients (65%) had vascular stenosis at least one level. The mean initial ankle-brachial index and toe-brachial index were 0.982 and 0.439. In comparison between amputation group and non-amputation group, ulcer severity, number of stenotic vessel and initial ankle-brachial index/toe-brachial index had statistical significance. Conclusion: The most commonly risk factor of intractable diabetic foot ulcer was peripheral neuropathy reaching 87.5% of cases. In comparison with non-amputation group, ulcer severity according to Wagner classification, number of stenotic vessel and initial ankle-brachial index/toe-brachial index were demonstrated as a risk factor of amputation in intractable diabetic foot ulcer.

Utility of Toe-brachial Index for Diagnosis of Peripheral Artery Disease

  • Background : The ankle brachial pressure index (ABI) is a simple, useful method for diagnosing peripheral artery disease (PAD). Although the ABI is an objective diagnostic method, it has limited reliability in certain scenarios. The aim of the present study was to determine the accuracy and reliability of the toe brachial index (TBI) as a diagnostic tool for detecting stenosis in PAD, associated with normal or low ABI values. Methods : ABI and TBI values were measured in 15 patients with diabetic gangrene who were suspected of having lower extremity arterial insufficiency. The ABI and TBI values were measured using a device that allowed the simultaneous measurement of systolic blood pressure in the upper and lower extremities. In addition, the ABI and TBI values were compared pre- and post-angiography. Results : Patients with an ABI of 0.9-1.3 showed almost no difference between the 2 measurements. The patients with TBI >0.6 had no arterial insufficiency. The patients with TBI <0.6 required vascular intervention with ballooning. After the angiography, the gangrenous wounds decreased in size more rapidly than they did prior to the intervention. Conclusions : Our findings suggest that TBI is the method of choice for evaluating lower limb perfusion disorders. This result requires further studies of TBI in a larger number of patients. Future long-term studies should therefore evaluate the utility of TBI as a means of screening for PAD and the present findings should be regarded as preliminary outcomes.

지첨-족지 지수에 의한 동맥 혈관 특성화 연구 (A Study on Arterial Characterization using Finger-Toe Index (FTI))

  • In this paper, Finger-Toe Index (FTI) is proposed as an analytic parameter for the characterization of arterial vessel. Different from the currently being employed pulse wave velocity (PWV) information of the volume pulse wave measured from 4 arterial channels, the proposed FTI uses the ratio of the shorter of the two up-stroke time of PPG from fingers ($UT_{finger}$) and that of PPG (Photoplethysmography) from toes ($UT_{toe}$). To verify the usefulness of the proposed method, Finger-Toe Indexes were derived from the volume pulse waves acquired from 50 people under examination aged from 12 to 81 years old, and they were then compared with blood pressure ankle-brachial index (ABI). It was successfully demonstrated that the arterial stiffness can be estimated with respect to age and FTI is more strongly correlated with the pulse transit time than ABI. From the regression analysis, we also found that FTI has significant correlation PWV for a quantitative index of arterial stiffness and provides more accurate information than ABI for the characterization of arterial vessel.

심혈관 질환자의 발목-상완 지수에 따른 Vessel disease 및 Gensini score 비교 융복합 연구 (Comparison of Vessel Disease and Gensini Score according to Ankle-Brachial Index in Patients with Cardiovascular Disease)

  • 본 연구는 심혈관 질환자의 발목-상완 지수(ankle-brachial index, ABI)에 따른 심혈관 질환 중증도를 분석하여 ABI의 임상적 유용성을 확인하기 위함이다. 연구 대상자는 일개 종합병원의 ABI를 측정한 심장내과 환자 441명 이었으며, 전자 의무 기록을 통해 ABI와 심혈관 질환 중증도를 나타내는 vessel disease 및 Gensini score를 조사하였다. 수집된 자료는 SPSS 21.0을 이용하여 분석하였다. ABI 0.90 이하인 비정상군과 0.90 초과인 정상군으로 나누어 비교하였을 때, vessel disease 분포 비율의 차이가 있었다($x^2=4.731$, p=.030). ABI 두 군과 Gensini score에 대한 비교분석에서 비정상군 $39.1{\pm}32.7$점, 정상군 $27.4{\pm}27.4$점으로 유의한 차이가 있었다(t=2.351, p=.019). 본 연구의 결과에서 ABI 비정상군이 중증도 높은 심혈관 질환자들의 분포가 많았으며, 평균 Gensini score도 높은 것을 확인할 수 있었다. 따라서 ABI를 시행하는 것은 비침습적인 방법으로 중증도 높은 심혈관 질환자를 예측하는데 도움이 되어 허혈성 심장 질환의 조기 발견에 유용하게 활용 가능할 것으로 기대된다.

본태성 고혈압 청소년에서 pulse wave velocity와 ankle brachial index에 대한 연구 (Pulse wave velocity and ankle brachial index in adolescents with essential hypertension)

  • 목 적 : 성인에서 PWV와 ABI는 동맥 경직을 측정할 수 있는 비침습적 방법으로 성인에서 고혈압, 말기 신장병, 동맥경화증의 정도를 평가할 수 있는 유용하고 간단한 방법으로 알려져 있다. 그러나 소아를 비롯한 청소년에서의 연구는 많지 않은 실정이다. 동맥경화증을 포함한 혈관의 변화가 무증상의 상태로 어린 나이에 시작해서 진행되기 때문에 청소년 고혈압, 비만 환자에서 동맥 경직의 평가가 절실하게 필요하다. 본 연구에서는 고혈압 청소년을 정상 청소년과 비교함으로 청소년에서 PWV의 유용성을 평가하고자 하였다. 방 법 : 15-16세의 청소년 413명을 고혈압군(1군 : 23명), 고혈압과 비만이 같이 있는군(2군 : 17명), 정상 대조군(3군 : 328명) 3개의 군으로 나누었다. 신장, 체중, 체질량지수를 측정하였고, Colin사의 VP-1000을 이용하여 사지의 수축기, 이완기, 평균 혈압과 양측 PWV, ABI, ET, PEP를 측정하여 비교분석 하였다. 결 과 : 우측 상완과 발목 사이의 PWV(RbaPWV)는 1군($1,186.4{\pm}134.9cm/sec$)과 2군($1,214.4{\pm}45.8cm/sec$) 모두에서 3군($939.1{\pm}84.3cm/sec$)에 비해 유의하게 높았으며(P<0.05), 2군에서 1군에 비해 유의하게 높았다(P<0.05). 좌측 상완과 발목사이의 PWV(LbaPWV) 또한 1군($1,198.4{\pm}99.8cm/sec$)과 2군($1,219.7{\pm}43.2cm/sec$) 모두에서 3군($942.1{\pm}75.8cm/sec$)에 비해 유의하게 높았으며(P<0.05), 2군에서 1군에 비해 유의하게 높았다(P<0.05). PEP/ET는 각 군 사이에 유의한 차이가 없었다. 사지의 수축기, 이완기, 평균 혈압과 PWV는 유의한 양의 상관 관계가 있었다. 사지의 수축기, 이완기, 평균 혈압은 모두 체중, 체질량지수와 유의한 양의 상관 관계가 있었으며 ABI는 체중, 체질량지수 어느 것과도 유의한 상관 관계가 없었다. 결 론 : 본태성 고혈압 청소년에서 정상 청소년에 비해 PWV가 높게 측정되었으므로 예후를 평가하기 위해서 추적 관찰이 필요하다.

The Correlation of Pulse Wave Velocity and Atherosclerotic Risk Factor in Stroke Patients

  • Pulse wave velocity (PWV) is used to non-invasively estimate the severity of arteriosclerosis by measuring arterial stiffness. Increased arterial stiffness measured by PWV stands for progressive arteriosclerosis and is caused by atherosclerotic risk factors. This study is focused on how brachial-ankle pulse wave velocity (baPWV) is related to the leading risk factors for arteriosclerosis or atherosclerosis. Subjects were 114, 69 males and 45 females who are in 60's and had baPWV test for acute stroke. The results are as follows: the group with increased arterial stiffness showed significant increase in HbA1c, total cholesterol, BSBP (brachial artery systolic blood pressure), BDBP (brachial artery diastolic blood pressure), CSBP (central artery systolic blood pressure), CDBP (central artery diastolic blood pressure), augmentation index (AIx) and diabetes mellitus. Correlation analysis between baPWV and atherosclerotic risk factor showed significant relationship in age, HbA1c, LDL cholesterol, BSBP, BDBP, CSBP, CDBP and augmentation index. baPWV was independently related to age and BSBP in multiple linear regression analysis. The group with increased arterial stiffness was independently related to BDBP in multiple logistic regression analysis. This study might be meaningful in evaluating the relationship between arterial stiffness and atherosclerotic risk factor in a new way, and be helped to make various studies for cardiovascular disease.
