To recognize the problems in malocclusion by roentgenocephalograms, the author designed a new pentagonal frame based on maxillary and mandibular bones. The subjects consisted of 44 normal occlusions (20 male and 24 female), 44 Class II division 1 malocclusions (15 male and 29 female) and 67 Class III malocclusions (31 male and 36 female). The results are as follows; 1. In normal group, the maxillary and mandibular skeletons of female we placed more closely to FH plane, so more anteriorly and upward than those of male. 2. In normal group, the posterior vertical height is longer in male than in female and the upper anterior teeth of female are in more labioversion than those of male. 3. By the X, Y coordinate values in pentagonal frame, it is helpful to recognize certain problems in malocclusions. 4. The posterior vertical height is a good indicator in detecting Angle's Class III malocclusion. 5. The maxillary and mandibular body length, the anterior point of maxillary and mandibular body length and the axial inclination of upper and lower anterior teeth can be useful in discerning Angle's Class II & Class III malocclusion.
Skeletal class III had been classified by the position of the maxilla, the mandible, the maxillary alveolus, the mandibular alveolus and vertical development. This morphologic approach is simple and useful for clinical use, but it is insufficient to permit understanding of the pathophysiology of dysmorphoses. The author hypothesizes that there are different patterns of mutual relation of the skeletal components which have contributed pathologic equilibrium of skeletal class III. The purpose of this study are threefold: 1) to classify skeletal class III in subgroups, which can show the architectural characteristics of the deformity, 2) to analyse the craniofacial architecture of each subgroup on etio-pathogenic basis, and 3) to characterize and visualize the pattern as a prototype. Materials used in this study were lateral cephalograms of 106 skeletal class III adults, which were analysed with modified Delaire's architectural and structural analysis. Linear and angular measurements of the individual subject were obtained and cluster analysis was used for the subgrouping. Data were evaluated for verification of the statistical significances. The following results were obtained. 1. By the modified Delaire's architectural and structural analysis and cluster analysis, skeletal class III adults were classified into 7 clusters and presented as prototypes, which could show the pathophysiology of the skeletal architecture 2. There was significant relationship in measurement variables of each cluster, which could reflect characteristics of the skeletal pattern of growth. 3. The flexure of cranial base had a close relationship to the anterior rotational growth of the maxilla and contributes to understand the etio-pathology of skeletal class III. 4. The proportion of craniospinal area in cranial depth, craniocervical angle and vertical position of point Om had a close relationship to rotational growth of the mandible and direction of condylar growth. They contribute to understand the etio-pathology of skeletal class III. In summary, the cranium and the craniocervical area must be considered in diagnosis and treatment planning of dentofacial deformity. And the occlusal plane can be considered as a representative which shows the mutual relationships of the skeletal components.
Purpose: Craniofacial structure form results from the adaptation to morphologic and functional changes in their neighboring structures for a mutual balance. The purpose of this study is classification of maxillomandibular complex growth pattern follow by cranial base growth pattern. And this study is identifying the correlation between maxilla-mandibular complex growth pattern and orthodontic criteria. Methods: 142 Class III malocclusion patients had orthognathic surgery at Wonkwang University Dental Hospital during April 2004 to October 2010. Patients were divided into 4 groups and the correlation between cranial base and maxillomandibular growth patterns were evaluated. Results: There was a correlation between cranial base and maxillomandibular growth patterns. Positive relationships were found between the occlusal plane, Incisor mandibular plane angle, mandibular plane, positioning of pogonion and the saddle angle, indicating maxillary growth patterns. Negative relationships were found between SNA, SNB, maxillary incisor angle and saddle angle. Positive relationships were found between the ratio of the anterior and posterior cranium, positioning of pogonion and the percentage of cranial depth indicating mandibular growth patterns. Negative relationships were found between the occlusal plane, maxillary incisor angle, mandibular plane, mandibular angle and cranial depth. Conclusion: Cranial base and maxillofacial growth patterns were correlated and the classification should be adjusted before orthognathic surgery.
This study was undertaken to find out the factor highly correlated to the depth of overbite among the skeletal factors of the craniofacial complex using lateral roentgenocephalograms. The subjects cconsited of fifty normal occlusions, sixty Class I malocclusions, sixty Class II division I malocclustions and sixty Class III malocclusions. The results were as follows: 1. Ans-Go-Me angle and lower genial angle showed high correlation to the depth of overbite in the total malocclusion sample. 2. The mean values of Ans-Go-Me angle and lower goinal angle for the normal sample were $49.8^{\circ}\;and\;75.6^{\circ}$, respectively. 3. Ans-Go-Me angle above $56^{\circ}$ or lower gonial angle above $84^{\circ}$ indicated a tendency toward an openbite. Conversely, Ans-Go-Me angle below $48^{\circ}$ or lower goinal angle below $73^{\circ}$ indicated a tendency toward a deepbite.
The purpose of this study was to investigate the difference between normal and malocclusion subjects in Temporomandibular joint. This study was based on the 44 subjects with normal occlusion, 30 subjects with Class II malocclusion, 30 subjects with Class III malocclusion before treatment. After submental vertex view analysis, each subject was given the TMJ Tomogram in centric relation and centric occlusion and the Cephalogram was taken with Quint Sectograph. The TMJ spaces were measured and analyzed statistically. Following results were obtained. 1. When centric relation was compared to centric occlusion, The condyles were positioned more posteriorly and superiorly in centric relation position of the normal occlusion group and the class II malocclusion group. In the Class III malocclusion group. There was no significant difference in the condylar position between centric occlusion and centric relation. 2. The condyles of the Class III malocclusion group were positioned more superiorly than the normal occlusion group and the Class II malocclusion group. 3. In the correlation between articular eminence posterior slope angle and lingual slope angle of the upper anterior central incisor, there was significant correlation in the normal occlusion group. But no significant correlation was found in the malocclusion group. 4. The mean value of the horizontal angulation of condylar head to the transear rod axis plane was $20.32^{\circ}{\pm}8.12^{\circ}$ in the normal occlusion group, $25.08^{\circ}{\pm}4.83^{\circ}$ in the class II malocclusion group, $14.68^{\circ}{\pm}4.08^{\circ}$ in the class III malocclusion group.
This study was designed to investigate the attrition pattern in Angle Class III malocclusion with facial asymmetry. The sample consisted of three groups, the 20 subjects of normal occlusion group(Group I), the 12 subjects of class III malocclusion without facial asymmetry group(Group II) and 17 subjects of Class III malocclusion with facial asymmetry group(Group III). Attrition areas from canine to second molar on both sides in upper and lower arch, totally twenty, was marked by pencil and mesured by computer system(INTERGRAPH CO. USA) 2 times and the average value was used for date processing. In Group I, Maxillary attrition area was larger than mandibular attrition area, but in Group ll and III, there was no significant difference in attrition area between maxilla and mandible. 4. In Group III, the attrition area of deviated side was target than undeviated side 5. There was no significant difference in attrition area between chewing side and non-chewing side in each group. 6. The total attrition area was unaffected by gender.

Purpose : The purpose of this study is to evaluate the clinical availability of submentoplasty for esthethic improvement of the cervico-facial region of patients with obtuse chin-neck angle. Materials and methods : Case 1. We evaluate the changes of submental line length and chin-neck angle of 35-year-old woman with skeletal Class III and mandibular excess with excessive submental fat before and after surgery: Bilateral sagittal split ramus osteotomy(BSSRO) setback(5mm), Mandibular Angle Reduction, Reduction Malarplasty and Submentoplasty. In this case, It was done simultaneously with orthognathic surgery. Case 2. The changes of submental line length and chin-neck angle of 20-year-old man with skeletal class III and maxillary defiency were evaluated before surgery, at first surgery : Lefort I osteotomy(6mm posterior Impaction), BSSRO setback(9mm), Paranasal Augmentation and at second surgery: genioplasty(6mm advanced) with submentoplasty. In this case, submentoplasty and advancement genioplasty were done after 2 months post-operative periods. Results : Case 1. In case of the Skeletal Class III mandibular excess with submental fat deposit, It showed the improvement of submental angle and length of submental line after simultaneous submentoplasty. Submental angle is changed from $177^{\circ}$ (pre-op) to $151^{\circ}$ (post-op) and submental line length is changed from 8mm(pre-op) to 36mm(post-op). Case 2. The improvement of submental angle and length of submental line after delayed submentoplasty was aquired in case of the skeletal class III maxillary defiency. Submental angle is changed from $154^{\circ}$ (pre-op) to $161^{\circ}$ (first surgery) and to $153^{\circ}$ (second surgery) and submental line length is changed from 25mm(pre-op) to 19mm(first surgery) and to 23mm(second surgery). Conclusion : The results suggest that Submentoplasty is useful surgical procedure for esthethic improvement of the cervicofacial region of patients with obtuse chin-neck angle.
This study was performed to investigate the characteristics of soft tissue profile of the class III malocclusion and to test the yardstick far differential diagnosis between surgical and orthodontic patients. Initial lateral cephalograms of orthodontic group(30 patients) that have acceptable occlusion and profile by orthodontic treatment alone and surgical group(30 patients) that have favorable occlusion and profile by combined surgical-orthodontic treatment were selected in Ajou university hospital. Powell and Burstone II analysis were made on the tracing. Descriptive, comparative, factor, cluster, and discriminant analysis were carried out with computer program. The results were as followings : 1. Patients who received surgery had a more concave profile and a longer lower facial height than patients who received orthodontic treatment alone. 2. Nasolabial angle, ratio of vertical height, and mentolabial sulcus were significantly different at the 5% level. And facial protuberance, upper lip protuberance, mentocervical angle, nasofrontal angle, nasomental angle, mandibular vertical height, angle between cervix and lower face, ratio of mandibular vertical height divided by cervical depth, ratio of vertical height between upper and lower lip, and maxillary protuberance were significantly different at the 1% level. 3. 8 factors were extracted and factor 2, 3, and 8 showed significant differences by factor analysis. 4. Orthodontic group (25) and surgical group (35) were classified by cluster analysis. 5. Discriminant function was D = 0.079Nasomental angle + 0.081Sn-Gn + 3.343Sn-Gn/C-Gn + 1.734Sn-St/St-Me' -26.460, and cutting score was 0, so we can discriminate that orthodontic group has the score above 0, and surgery group below 0. And 91.7% of original grouped cases were correctly classified.
Objective: This study investigated whether temporomandibular joint (TMJ) condyle-fossa relationships are bilaterally symmetric in class III malocclusion patients with and without asymmetry and compared to those with normal occlusion. The hypothesis was a difference in condyle-fossa relationships exists in asymmetric patients. Methods: Group 1 comprised 40 Korean normal occlusion subjects. Groups 2 and 3 comprised patients diagnosed with skeletal class III malocclusion, who were grouped according to the presence of mandibular asymmetry: Group 2 included symmetric mandibles, while group 3 included asymmetric mandibles. Pretreatment three-dimensional cone-beam computed tomography (3D CBCT) images were obtained. Right- and left-sided TMJ spaces in groups 1 and 2 or deviated and non-deviated sides in group 3 were evaluated, and the axial condylar angle was compared. Results: The TMJ spaces demonstrated no significant bilateral differences in any group. Only group 3 had slightly narrower superior spaces (p < 0.001). The axial condylar angles between group 1 and 2 were not significant. However, group 3 showed a statistically significant bilateral difference (p < 0.001); toward the deviated side, the axial condylar angle was steeper. Conclusions: Even in the asymmetric group, the TMJ spaces were similar between deviated and non-deviated sides, indicating a bilateral condyle-fossa relationship in patients with asymmetry that may be as symmetrical as that in patients with symmetry. However, the axial condylar angle had bilateral differences only in asymmetric groups. The mean TMJ space value and the bilateral difference may be used for evaluating condyle-fossa relationships with CBCT.
This study was aimed to investigate the characteristics & the causative areas of the adult skeletal class III malocclusions with different facial divergency. The lateral cephalograms of 80 subjects with skeletal class III malocclusion from 17 to 29 years of age were classified into 3 groups according to SN-MP angle; hypodivergent group $(21.65{\pm}3.52^{\circ})$, neutrodivergent group $(30.50{\pm}2.29^{\circ})$ and hyperdivergent group $(40.02{\pm}3.98^{\circ})$. The data were gathered by digitizing of the traced cephalograms and were statistically analyzed. The results were as follows: 1. The anterior cranial base of the hyperdivergent group was shortest & tipped upwardly to the FH plane. 2. The maxilla of hyperdivergent group was shortest anteroposteriorly and positioned posteriorly to the anterior cranial base. 3. The degree of the mandibular prognathism in hyperdivergent group was less than the hypodivergent group. The hyperdivergent group showed the downward & backward rotated mandible. 4. The mandibular ramus & body was short & slender in the hyperdivergent group and the gonial angle was greatest in the hyperdivergent group. 5. The temporomandibular joint was positioned more superiorly to the anterior cranial base in the hyperdivergent group. 6. The cranial base, palatal plane, occlusal plane and mandibular plane were diverged in the hyperdivergent group. And this group had a great anterior total facial height, especially anterior lower facial height. 7. The craniofacial characteristics of skeletal class III malocclusion were critical in the vertical structure than the horizontal.
