Parasplenial arteriovenous malformations(AVMs) are rare vascular malformations which have distinct clinical and anatomical features. They are situated at the confluence of the hippocampus, isthmus of the cingulate gyrus and the gyrus occipitotemporalis medialis. These lesions are anterior to the calcarine sulcus and their apex extends towards the medial surface of the trigonum. Posterolaterally, these lesions are in close proximity to the visual cortex and optic radiation. The objectives in the surgery of parasplenial AVMs are complete resection of the lesions and preservation of vision. These objectives must be achieved with comprehensive understanding of the following anatomical features :1) the deep central location of the lesions within eloquent brain tissue ; 2) the lack of cortical representation of the AVMs that requires retraction of visual cortex ; 3) deep arterial supply ; 4) deep venous drainage ; 5) juxtaposition to the choroid plexus with which arterial supply and venous drainage are shared. A 16-year-old female student presented with intraventricular hemorrhage from a right parasplenial-subtrigonal AVM. The lesion, fed by posterior cerebral artery and drained into the vein of Galen, was successfully treated by the inter-hemispheric parietooccipital approach. To avoid visual field defect a small incision was made on precuneus anterior to the calcarine sulcus. In this report, the authors describe a surgical approach with special consideration on preservation of visual field.
Kimura's disease is a chronic Inflammatory disorder of unknown etiology, presenting usually as a painless subcutaneous swelling in the head and neck region or in the lymph nodes. We experienced a case of Kimura's lymphadenitis with characteristic Warthin-Finkeldey type polykaryocytes by fine needle aspiration cytology. The patient was a 10-year old male with two enlarged lymph nodes in the postauricular area. Fine needle aspiration cytology from the lymph nodes disclosed hypercellular smears with some scattered eosinophils and polykaryocytes in a polymorphous lymphoid background. There were also fragmented vessel wails and activated endothelial cell clusters in the slightly necrotic background. The Warthin-Finkeldey type polykaryocytes had three to thirty nuclei and prominent nucleoli with cytoplasmic borders. Their nuclei were arranged in grapevine or ring shaped clusters. As these polykaryocytes could also be found in lymph nodes and extranodal tissues of both reactive and neoplastic lymphoid disorders, polykaryocytes themselves are clinically nonspecific. However, the morphologic features of the Warthin-Finkeldey type giant cells are quite different from the foreign body type or Langhans' type giant cells. When the characteristic cytologic features of Kimura's disease such as significant number of eosinophils in a background of lymphoid cells asd proliferation of vessels and endothelial cells are also observed in the smear, it is possible to suggest this diagnosis in the appropriate clinical setting.
Background The purpose of this study was to compare the anatomical features of the internal mammary vessels (IMVs) at the second and third intercostal spaces (ICSs) with regard to their use as recipient vessels in deep inferior epigastric artery perforator (DIEP) flap breast reconstruction. Methods A total of 38 consecutive DIEP breast reconstructions in 36 patients were performed using IMVs as recipient vessels between March 2017 and August 2018. The intraoperative findings and postoperative complications were analyzed. Anatomical analyses were performed using intraoperative measurements and computed tomography (CT) angiographic images. Results CT angiographic analysis revealed the mean diameter of the deep inferior epigastric artery to be 2.42±0.27 mm, while that of the deep inferior epigastric vein was 2.91±0.30 mm. A larger mean vessel diameter was observed at the second than at the third ICS for both the internal mammary artery (2.26±0.32 mm vs. 1.99±0.33 mm, respectively; P=0.001) and the internal mammary vein (IMv) (2.52±0.46 mm vs. 2.05±0.42 mm, respectively; P<0.001). Similarly, the second ICS was wider than the third (18.08±3.72 mm vs. 12.32±2.96 mm, respectively; P<0.001) and the distance from the medial sternal border to the medial IMv was greater (9.49±2.28 mm vs. 7.18±2.13 mm, respectively; P<0.001). Bifurcations of the IMv were found in 18.4% of cases at the second ICS and in 63.2% of cases at the third ICS. Conclusions The IMVs at the second ICS had more favorable anatomic features for use as recipient vessels in DIEP flap breast reconstruction than those at the third ICS.
한국산 조팝나무속(Spiraea L.)에 속하는 3절 18분류군(sectts. Calospira: 복산방화서절 - 7분류군, Chamaedryon: 산방화서절 또는 산형화서절 - 10분류군, Spiraria: 원추화서절 - 1분류군)에 대해 엽병의 횡단면을 단부, 중앙부, 기부로 나누어서 비교하고 기재하였다. 엽병의 횡단면은 아원형, 렌즈형이거나 반달형이고, 두 개의 분리된 날개를 갖는 분류군과 날개를 갖지 않는 분류군으로 세 부위(단부, 중앙부, 기부)에서 차이를 보였다. 엽병의 해부학적 형질로는 횡단면의 모양 및 크기, 유관속의 모양 및 크기, 그리고 모용 및 결정체의 모양과 존재 여부들이 분류군 간에 비교 되었다. 본 연구 결과에서는 엽병의 해부학적 형질들이 한국산 조팝나무속내 분류체계에 적용시키기에는 미약하였지만, 종간 진단형질로는 유용함이 확인되었다. 엽병형질들을 바탕으로 한국산 조팝나무속 분류군에 대한 검색표를 제시하였다.
Anatomical characteristics on abnormal type of bisected compression woods and light colored wood intervened between these dark colored compression woods on under part and their respective opposite woods on upper parts of cross section in Pinus densiflora S. et Z. stem were compared through optical microscopic investigation. and the obtained results were summarized as follows: 1. Tracheid length increased linearly as the number of rings from the pith increases in all investigated parts, but increasing tendency in tracheid length was similar within bisected two compression woods and their opposite woods. 2. Tracheids were the shortest in bisected two compression woods and tracheid length in intervened wood between these two compression woods was longer than in compression woods but shorter than in the opposite woods. 3. Bisected two compression woods and intervened wood between these two compression woods revealed similar features in tracheid transition from springwood to summerwood, intercellular space, tracheid entanglement, and cross sectional view of tracheid. but spiral check was observed only in these two compression woods, therefore the intervened wood between these two compression woods disclosed anatomical characteristics similar to, not typical of, compression woods.
The effect of Roentgen rays on carcinoma of the cervix has long been of great interest to both radiologists and gynecologists. Since most cervical carcinomas are treated by irradiation, any additional knowledge either concerning the radiosensitivity of cervical tumors or their ultimate prognosis would be of value. The vaginal smear is considered to be one of convenient and rapid methods to study the effects of radiation on cervical malignancy. We observed morphologic changes in 297 cytologic preparations obtained from 60 patients who had underwent irradiation for cancer of the cervix. With the morphologic parameters such as cytoplasmic vacuolization, cytoplasmic basophilia, multinucleated giant cell formation, polymorphonuclear leucocytes (PMNL) sticking and postradiation dysplasia, we analyzed the findings in relation to the follow up time interval. The most common effect was the cytoplasmic vacuolization with basophilia of basaloid cells, which were noted in more than 90% of followed patients. The multinucleated giant ceil formation and PMNL stickering were noted in 38 cases(63%) and 48 cases(80% ) respectively. The differential diagnosis of postradiation dysplasia from recurrent or persistent carcinoma, reparative atypical cells, and regressing tumor cells was difficult and further study seems to be needed to clarify the more accurate morphologic features and biologic behavior.
인체의 해부학적 구조에 관해 다른 영상들보다 정확한 정보를 제공하는 MRI에 대해 많은 연구가 진행중이다. 본 논문에서는 대퇴골두 무혈성 괴사의 자동 진단에 필요한 대퇴골두 영역의 추출에 관한 효과적인 방법을 제안한다. 대퇴골두의 해부학적 특성과 Hough transform을 이용하여 대퇴골두 영역을 설정하였고, 대퇴골두 영역을 분할하는 방법으로서 영역 확장법과 히스토그램 기반 영역 분할 방법의 장점을 결합한 방법을 고안하였다. 본 논문에서 제시한 방법은 정상적인 대퇴골두와 무혈성 괴사의 초기 단계의 대퇴골두 뿐만 아니라 괴사가 심한 대퇴골두에 대해서도 좋은 결과를 얻을 수 있었다.
Purpose: Fossa navicularis is a bone defect in the clivus. Familiarity with this anatomical variant is important because it is close to vital anatomical structures in the base of the skull. The aim of this study was to determine the prevalence and morphometric properties of fossa navicularis within the clivus in a Turkish subpopulation using cone-beam computed tomography (CBCT). Materials and Methods: A total of 168 CBCT scans (female: 96, male: 71) were evaluated. High-quality CBCT images of patients without a syndromic condition or a history of neurological disease or surgery were included in the study. The prevalence, depth, length, and width of the fossa navicularis were performed. Results: The prevalence of fossa navicularis was 27.5% (n=46 patients). Sex was not associated with the depth, length, or width of the fossa navicularis (P>0.05). A significant positive correlation was found between age and length of the fossa navicularis(P>0.05). Conclusion: Fossa navicularis was found to be rare (27.5%). Anatomical variants of the skull base can also be clearly identified on CBCT images. The results of this study may be useful to radiologists, anatomists, and surgeons interested in the skull base.
The purpose of this study was to find anatomical variation of each small breed dog's dentition for designing more effective dental chews. Small breed dogs were volunteered for dental impressions with dental stone and alginate under tiletamine-zolazepam, tramadol, and medetomidine intravenous anesthesia. Twenty-two criteria were measured to compare dental impressions. Twenty-five dogs (9 Malteses, 8 Miniature Poodles, and 8 Shih-Tzus) were recruited. Statistically, Shih-Tzus had smaller teeth and shorter interdental spaces than those of Maltese and Miniature Poodles. Grossly, the distance between upper teeth and lower teeth was wider in Shih-Tzus. Shih-Tzu had different dental anatomy in comparative to Miniature Poodle and Maltese. It was recommended to design the dental chew considering these differences.
Construction of an obturator for rehabilitation of a patient who underwent a maxillectomy is vital. Routinely a constructed obturator includes denture portion. A patient who may present anatomical limitations due to surgical or radiotherapy complications often challenges the clinician. Purpose: This clinical report describes a patient with severe trismus after surgical resection and radiotherapy treatment of a tumor in the upper left maxilla. Conclusion: This report describes the concepts of using a rotational path insertion for an obturator and a separately constructed maxillary denture. The stability and retention of the obturator were obtained from anatomical features. Where as these were achieved through magnetic attachments and the remaining edentulous ridge to ensure esthetic and function of the prosthesis.
