Purpose: This study aimed to propose a fully automatic landmark identification model based on a deep learning algorithm using real clinical data and to verify its accuracy considering inter-examiner variability. Materials and Methods: In total, 950 lateral cephalometric images from Yonsei Dental Hospital were used. Two calibrated examiners manually identified the 13 most important landmarks to set as references. The proposed deep learning model has a 2-step structure-a region of interest machine and a detection machine-each consisting of 8 convolution layers, 5 pooling layers, and 2 fully connected layers. The distance errors of detection between 2 examiners were used as a clinically acceptable range for performance evaluation. Results: The 13 landmarks were automatically detected using the proposed model. Inter-examiner agreement for all landmarks indicated excellent reliability based on the 95% confidence interval. The average clinically acceptable range for all 13 landmarks was 1.24 mm. The mean radial error between the reference values assigned by 1 expert and the proposed model was 1.84 mm, exhibiting a successful detection rate of 36.1%. The A-point, the incisal tip of the maxillary and mandibular incisors, and ANS showed lower mean radial error than the calibrated expert variability. Conclusion: This experiment demonstrated that the proposed deep learning model can perform fully automatic identification of cephalometric landmarks and achieve better results than examiners for some landmarks. It is meaningful to consider between-examiner variability for clinical applicability when evaluating the performance of deep learning methods in cephalometric landmark identification.
Objective : The purpose of this study was to determine the anatomic relationships between neurovascular structures and the transverse carpal ligament so as to avoid complications during endoscopic carpal tunnel surgery. Methods : Twenty-eight patients (age range, 35-69 years) with carpal tunnel syndrome were entered into the study. We examined through wrist magnetic resonance imaging in three different positions (neutral, radial flexion, and ulnar flexion) and determined several anatomic landmark (distance from the hamate hook to the median nerve, ulnar nerve, and ulnar vessel) based on the lateral margin of the hook of the hamate. The median nerve and ulnar neurovascular structure were studied with the wrist in the neutral, ulnar, and radial flexion positions. Results : The ulnar neurovascular structures usually passed just over or ulnar to the hook of the hamate. However, in 12 hands, a looped ulnar artery coursed 0.6-3.3 mm radial to the hook of the hamate and continued to the superficial palmar arch. The looped ulnar artery migrates on the ulnar side of Guyon's canal (-5.2-1.8 mm radial to the hook of the hamate) with the wrist in radial flexion. During ulnar flexion of the wrist, the ulnar artery shifts more radially beyond the hook of the hamate (-2.5-5.7 mm). Conclusion : It is appropriate to transect the ligament greater than 4 mm apart from the lateral margin of the hook of the hamate without placing the edge of the scalpel toward the ulnar side. We would also recommend not transecting the transverse carpal ligament in the ulnar flexed wrist position to protect the ulnar neurovascular structure.

Statement of problem: It is very important to establish the suitable position for artificial maxillary anterior teeth and the orientation of occlusal plane in fabrication of the complete dentures. Incisive papilla has been considered the most useful anatomic landmark in the arranging of a maxillary anterior artificial teeth. Purpose: To determine correct position of upper anterior artificial teeth in complete denture patients, relationship of incisive papilla, intercanine line, occlusal plane were evaluated in Korean adults. Materials and Method: Maxillary casts were made in 60 Korean dentate subjects. Each cast was mounted in Hanau modular articulator using Hanau spring bow. Then, anatomic landmarks were determined in each cast. Distance from central incisor to incisive papilla, distance from incisive papilla to intercanine line and discrepancies between frankfurt plane and occlusal plane were measured and analysed. Results: 1. The mean distance between the posterior point of incisive papilla and the incisal edge of central incisor was 11.20 mm (in male 11.77 mm, in female 10.55 mm) and there was no significant difference in measured values between male and female 2. The mean distance between the posterior point of incisive papilla and intercanine line was 2.52 mm (Male 2.57 mm, Female 2.35 mm) and there were no significant differencies in measured values between male and female 3. The mean discripancy between the Frankfurt plane and the occlusal plane were 9. 75 degree (Male 9.81 degree, Female 9.55 degree), and there was no significant difference in measured values between left and right sides.
The purpose of this study was to obtain the guides of establishing vertical dimension by analyzing the facial and oral landmarks of Korean adults. The following conclusions were obtained from this study. 1. The ratio of bizygomatic width to dental arch width was 3.26 : 1 in male and 3.21 : 1 in female, and the ratio of vertical dimension to dental arch length was 2.49 : 1 in male and 2.39 : 1 in female. 2. It was obtained by analysis of multiple regression that the corelative formula, vertical dimension$=23.37+0.24{\times}bizygomatic$$width+0.29{\times}dental$ arch length. 3. There was no statistically significant difference between the results of vertical dimension acquired by means of Hayakawa's prediction($68.04{\pm}3.16mm$ in male, $64.38{\pm}3.00mm$ in female) and that of vertical dimension by this study.
The PET/CT scanner is an evolution in image technology. The two modalities are complementary with CT and PET images. The PET scan images are well known as low resolution anatomic landmak, but such problems may help with interpretation detailed anatomic framework such as that provided by CT scan. PET/CT offers some advantages-improved lesion localization and identification, more accurate tumor staging. etc. Conventional PET employs tranmission scan require around 4 min./bed position and 30 min. for whole body scan. But PET/CT scanner can reduced by 50% in whole body scan. Especially nowadays PET scanner LSO scintillator-based from BGO without septa and operate in 3-D acquisition mode with multidetectors CT. PET/CT scanner fusion problems solved through hardware rather than software. Such device provides with the capability to acquire accurately aligned anatomic and functional images from single scan. It is very important to effective detection from gamma ray source in PETdetector. And can be offer high quality diagnostic images. So we have study about detection processing of PET detector and high quality imaging process.
Purpose: The purpose of this study was to evaluate possible differences in the location of the inferior alveolar canal in male and female Egyptians. Materials and Methods: This cross-sectional retrospective study involved the evaluation of 210 CBCT scans of Egyptian individuals (18-70 years old). The inferior alveolar canal was localized by measuring 8 linear dimensions: 2 for the vertical localization of the mental foramen (superior and inferior to the mental foramen), 4 at the first molar bifurcation for the vertical and horizontal localization of the inferior alveolar canal (superior, inferior, buccal, and lingual to the inferior alveolar canal), and 2 for the horizontal localization of the mandibular foramen (anterior and posterior to the mandibular foramen). The measurements were statistically analyzed via comparative analysis, stepwise logistic regression, and receiver operating characteristic (ROC) curve analysis. Results: Six of the 8 measured distances differed to a statistically significant extent between the sexes. Regression analysis suggested a logistic function with a concordance index of 84%. The diagnostic accuracy capabilities of the linear measurements as sex predictors were calculated using ROC analysis, and the 6 best predictors for sex determination were selected and ranked from highest to lowest predictive power. Moreover, combining these 6 predictors increased the predictive power to 84%. Conclusion: The location of the inferior alveolar canal in the Egyptian population varies significantly by sex; accordingly, this anatomic landmark could be used as a reliable indicator of sexual dimorphism.

Purpose: For minimally invasive procedures, three-dimensional (3D) anatomical knowledge of the structures of the face is essential. This study aimed to describe the thickness of the skin and subcutaneous tissue and depths of the facial muscles located in the infraorbital region using a 3D scanner to provide critical clinical anatomical guidelines for improving minimally invasive cosmetic procedures. Materials and Methods: The 3D scanning images of 38 Korean cadavers (22 males and 16 females; age range: 51~94 years at the time of death) were analyzed. Eight facial landmarks (P1~P8) were marked on the cadaveric faces. The images were scanned in three steps-undissected face, hemiface after skinning, and revealing the facial muscles. Student's t-test was used to identify significant differences. Result: The skin and subcutaneous tissue tended to become thicker from the upper to lower and medial to lateral aspects, and the muscles followed the same pattern as that of the most superficial located muscle and the deepest located muscles. No significant sex-related differences were found in the skin at any landmark. However, the muscles tended to be deeper in the female participants. Conclusion: The study data can serve as a basis for creating or enhancing clinical anatomy-based guidelines or improving procedures in the infraorbital region.
Purpose: Recently, the incidence of early gastric cancer (EGC) patients is rapidly increased in Korea. However, they're often not perceptible by surgical palpation or inspection. The aim of this study is 1) to develope a software that can locate the tumor and measure the mucosal distance from an anatomic landmark to the tumor using CT gastrography and 2) to compare the distance measured by the developed software with the distance measured by the pathologic findings. Materials and Methods: Between January 2004 and September 2005, sixty patients (male=45, female=15, mean 57.8 years old) estimated for EGC with preoperative CT scans and undergone gastrectomies in Kyungpook National University Hospital were enrolled in this study. Preoperative CT scans were performed after insufflations of room air via 5 Fr NG tube. The scans included the following parameters: (slice thickness/reconstruction interval: 0.625 mm, kVp: 120, mAs: 200). 3D volume rendering and measurement of the surface distance from the pylorus to the EGC were performed using the developed software. Results: The average difference between the lesion to pylorus distances measured from pathologic specimens and CT gastrography was $5.3{\pm}2.9\;mm(range,\;0{\sim}23\;mm)$. The lesion to pylorus distance measured from CT gastrography was well correlated with that measured from the pathologic specimens (r=0.9843, P<0.001). Conclusion: These results suggest that the surface distance from an anatomic landmark to the EGC can be measured accurately by CT gastrography. This technique could be used for preoperative localization of early gastric carcinomas to determine the optimal extent of surgical resection.
Most of the diagnostic methods currently used for the detection of neoplastic masses provide indirect evidence. To obtain greater specificity in the interpretation of neoplasias by in vivo methods, the immunological approach appears to be most promising. Two problems that interfered with progress in this field were the lack of tumor specific antigen and the lack of well-defined and reproducible antibodies. To improve the sensitivity and specificity of radioimmunoscintigraphy as a technique for tumor localization, the use of monoclonal antibodies, fragments of antibodies and single photon emission computerized tomography (SPECT) are reasonable. The obvious advantages of monoclonal antibodies are their homogeneity, their specificity for the immunizing antigen and the reaction with a single determinant-thus no large immunecomplexes with antigen are formed. Monoclonal antibody technique has recently provided an opportunity to reevaluate the role of nuclear medicine for the diagnosis of malignant diseases by using the immunological approach. Out first results by means of radioimmunoscintigraphy of CEA and CA 19-9 producing tumors using a cocktail of fragments F $(ab')_2$, of mocolonal antibodies to CA 19-9 and CEA labeled with $^{131}I$ (IMACIS-1) are reported. The aims of this investigation was to evaluate the role of immunoscintigraphy in patients with colorectal and other cancers for diagnosis of local recurrences and metastasis. This report contains results of the first 8 colorectal and pancreas cancer patients with the elevation of the level of serum CEA and/or CA 19-9. IMACIS-1 was injected intravenously during 30 minutes in 100 ml saline solution after skin test. Planar scintigrams were recorded 3, 5 and 7 days after the injection of the IMACIS-1. Anterior, lateral and posterior views of the liver as well as anterior and posterior views of the pelvis were obtained in each patients as an $^{131}I-antibody$ image. We were able to localize exactly the malignant process with the double-nuclide double-compound $^{99m}Tc\;^{131}I$ (Tc+l) scintigrams. In Tc & I double-nuclide scintigraphy, computer subtraction display provided more clear localization of the tumor. We compared the results of radioimmunoscintigraphy with CT, ultrasonograms, conventional scintigrams. The results were as follows: 1) The sensitivity and specificity of radioimmunoscintigraphy using the fragments $F(ab')_2$ of the cocktails of CEA and CA 19-9 monoclonal antibodies were 80% and 100% respectively. 2) Tumor detection rate was not proportionated to the level of serum tumor markets. 3) Second tracer technique was essential for tumor localization as an anatomic landmark using double-nuclide scintigraphy. 4) A slow infusion of the antibodies was necessary to prevent the formation of large immune complexes. 5) Tumor/non-tumor radioactivity was most elevated at 7 days delayed imaging. 6) Using planar scintigraphic technique of $^{131}I$ labeled monoclonal antibodies are possible for imaging most of the tumors.
Purpose: The purpose of this study was to evaluate the validity of reference points for edentulous patient by examining the correlation of teeth and face, and intraoral anatomic landmarks. Materials and methods: We examined a facial outline, length, bizygomatic width, nasion - gnathion length, glabella - nasion distance in 270 men and 280 women satisfied with inclusion criteria from Seoul National School of Dentistry. The shape of maxillary central incisor, mesiodistal crown width and length of maxillary 6 incisors, distance from incisive papilla to labial surface of maxillary central incisor, and perpendicular distance from incisive papilla to intercanine line were measured in the stone model. We analyzed the ratio and relevant relation statistically. Results: The probability on having the same shape of face and the relative same shape maxillary incisor was 55.56% and 46.43% for men and women. The facial length proved to be a more valuable measurement in women in the tooth selection. The ratio of bizygomatic width to mesiodistal width of maxillary central incisor, and the ratio of bizygomatic width to width of maxillary 6 incisors were 16.8 : 1 and 3.0 : 1 and were positively correlated with each other. The distance of the canines from the maxillary incisal papilla was $1.33{\pm}1.28mm$. The distance between the center of the incisal papilla and the labial surface of their maxillary central incisor was $9.23{\pm}1.20mm$. Conclusion: It was showed that anatomical reference points in tooth selection and arrangement for edentulous patient are useful and have validity in our limited study.
