정맥내 통증 자가조절법에 의한 술후통증관리 1,590예에 대한 분석 (Analysis of 1,590 Patients of IV-PCA for Postoperative Pain Management)

  • Background: We started postoperative pain management service using an intravenous patient-controlled analgesia (IV-PCA, PCA), which is known as convenient and effective analgesic method. In this report, we describe the efficacy and safety of PCA and the experience of developing an acute pain service to treat postoperative pain using a PCA. Methods: Practices of an acute pain service were started at a ward for general surgery after preparation of the standardized protocols for PCA. In each patient, PCA was connected following administration of initial loading doses of analgesics at recovery room after operation. All patients were checked by acute pain service team once or twice daily. The scope of acute pain service was gradually spread to other departments such as orthopedic, thoracic, obstetric and gynecologic departments by requests of patients or surgeons. We managed 1,590 patients during first 22 months. among them, nine hundred seventy two cases were prospectively evaluated for their analgesis efficacy and side effects of PCA. Results: The number of patients was increased day by day. the most common type of operation was gastrectomy (21.6%). Commonly used analgesics were nalbuphine (59%) and morphine (37%). The mean duration of PCA attachment was 3.3 days. The degree of analgesia on operation day was good in 44.8% and tolerable in 52.6% of patients. Only 3.9% of patients complained severe pain during their postoperative periods. One elderly patient experienced respiratory depression (0.06%) owing to accidental misuse of PCA by his relatives. Overall patient's satisfaction was over 93%. Conclusion: According to our experiences, we conclude that PCA is an effective, relatively safe and highly satisfactory method for postoperative pain management. Because of these advantages of PCA, the creation of our acute pain service using a PCA was successful and expanded rapidly.

Comparison of patient-controlled epidural analgesia with patient-controlled intravenous analgesia for laparoscopic radical prostatectomy

  • Background: Patient-controlled epidural analgesia (PCEA) is known to provide good postoperative analgesia in many types of surgery including laparoscopic surgery. However, no study has compared PCEA with patient-controlled intravascular analgesia (PCIA) in laparoscopic radical prostatectomy (LARP). In this study, the efficacy and side effects of PCEA and PCIA after LARP were compared. Methods: Forty patients undergoing LARP were randomly divided into two groups: 1) a PCEA group, treated with 0.2% ropivacaine 3 ml and 0.1 mg morphine in the bolus; and 2) a PCIA group, treated with oxycodone 1 mg and nefopam 1 mg in the bolus. After the operation, a blinded observer assessed estimated blood loss (EBL), added a dose of rocuronium, performed transfusion, and added analgesics. The numeric rating scale (NRS), infused PCA dose, and side effects were assessed at 1, 6, 24, and 48 h. Results: EBL, added rocuronium, and added analgesics in the PCEA group were less than those in the PCIA group. There were no significant differences in side-effects after the operation between the two groups. Patients were more satisfied with PCEA than with PCIA. The NRS and accumulated PCA count were lower in PCEA group. Conclusions: Combined thoracic epidural anesthesia could induce less blood loss during operations. PCEA showed better postoperative analgesia and greater patient satisfaction than PCIA. Thus, PCEA may be a more useful analgesic method than PICA after LARP.

소아의 편도절제술시 Nalbuphine정주가 술후 진통에 미치는 영향 (The Effects of Intravenous Nalbuphine on the Posttonsillectomy Pain in Children)

  • Background: Tonsillectomy is one of the most common operation in children. Postoperative pain and its sequelae are universal complaints of the patients. The purpose of this study was to evaluate the effects of nalbuphine on the posttonsillectomy pain in children. Methods: Fifty-four pediatric patients undergoing tonsillectomy under general anesthesia were randomly allocated to one of the 3 groups 1) control group who received no analgesics, 2) received IV nalbuphine before induction of anesthesia and 3) received IV nalbuphine after both tonsillectomy. In postanesthetic recovery room, comfort level in all patients was assessed using the objective pain scale (OPS). Systolic blood pressure, diastolic blood pressure and heart rate were measured at just before and immediately after extubation and postanesthetic recovery room. Results: The pain scale score in group 2 was significantly lower than group 1, but no significantly different with group 3. There were no significant differences in blood pressure among three groups. The heart rate in group 2 and 3 was significantly lower than group 1 only at immediately after arriving recovery room. Conclusions: Administration of nalbuphine before induction is more effective on postoperative pain control after tonsillectomy in children.

Intravenous Nefopam Reduces Postherpetic Neuralgia during the Titration of Oral Medications

  • Background: The recently known analgesic action mechanisms of nefopam (NFP) are similar to those of anticonvulsants and antidepressants in neuropathic pain treatment. It is difficult to prescribe high doses of oral neuropathic drugs without titration due to adverse effects. Unfortunately, there are few available intravenous analgesics for the immediate management of acute flare-ups of the chronic neuropathic pain. The aim of this study was to determine the additional analgesic effects for neuropathic pain of NFP and its adverse effects during the titration of oral medications for neuropathic pain among inpatients with postherpetic neuralgia (PHN). Methods: Eighty inpatients with PHN were randomly divided into either the NFP or normal saline (NS) groups. Each patient received a 3-day intravenous continuous infusion of either NFP with a consecutive dose reduction of 60, 40, and 20 mg/d, or NS simultaneously while dose titrations of oral medications for neuropathic pain gradually increased every 3 days. The efficacy of additional NFP was evaluated by using the neuropathic pain symptom inventory (NPSI) score for 12 days. Adverse effects were also recorded. Results: The median NPSI score was significantly lower in the NFP group from days 1 to 6 of hospitalization. The representative alleviating symptoms of pain after using NFP were both spontaneous and evoked neuropathic pain. Reported common adverse effects were nausea, dizziness, and somnolence, in order of frequency. Conclusions: An intravenous continuous infusion of NFP reduces spontaneous and evoked neuropathic pain with tolerable adverse effects during the titration of oral medications in inpatients with PHN.

전자궁적출술후 시행한 정맥내 통증자가조절법의 임상 경험 (Clinical Experience on Intravenous Patient Controlled Analgesia after Total Abdominal Hysterectomy)

  • Background: We studied 150 patients who received intravenous patient controlled analgesia (PCA) after total abdominal hysterectomy to evaluate pain relief, analgesic consumption, patient's satisfaction and side effects. Methods: We made total 40 ml of analgesic mixture with morphine 40 mg, ketorolac 120 mg, droperidol 3 mg and normal saline. Loading/bolus/basal infusion dose and lockout interval was 2 ml, 1.5 ml, 0.5 ml/hr and 10 min, respectively. Numerical rating scale (NRS) pain score, cumulative analgesic consumption, degree of satisfaction, and incidence of side effects were evaluated. Also, correlation of age and edu ion with analgesic consumption were evaluated. Results: The average pain scores using NRS were $3.1{\pm}1.7$ (6 h), $2.1{\pm}1.5$ (24 h), $1.7{\pm}1.5$ (48 h). The average cumulative analgesic consumption were $11.7{\pm}5.0$ ml (6 h), $23.0{\pm}6.7$ ml (24 h), $32.1{\pm}3.7$ ml (48 h). The degree of satisfaction in postoperative pain control was good in 94% of patients. There was no correlation between degree of satisfaction and analgesic consumption. Also age and level of edu ion did not correlated with analgesic consumption. Conclusions: Intravenous PCA with morphine, ketorolac, and droperidol is an effective method of postoperative pain control because it provides adequate pain relief and a few side effects with high patient's satisfaction. However, age and level of education did not correlated with analgesic consumption.

말기 암 환자의 마지막 24시간 동안 진통제 사용의 분석 : 내과의사와 외과의사의 비교 (The Use of Analgesics in the Last 24 hours of Life of Patients with Advanced Cancer : A Comparison of Medical Physicians and Surgeons)

  • 목적 : 암은 우리나라 사망원인의 첫째이며 말기암환자에서 보이는 신체증상 중 통증은 의료인이 반드시 조절을 해주어야 한다. 암환자의 90%이상에서 통증조절이 가능하며, 말기 암환자의 임종 24시간 전 $33{\sim}60%$에서는 의식이 명료하여 진통제의 경고, 투여가 가능하다. 마지막 24시간 동안의 진통제 사용실태와 내과계와 외과계 의사에따른 통증조절 방법의 차이를 조사함으로서 보다 계획적이고 적극적인 통증 조절 중요성의 중요성을 강조하고자 본 연구를 시행하였다. 방법 : 1994년 7월 1일부터 1995년 6월 30일까지 모대학병원에 입원하여 암으로 사망한 환자 160명을 대상으로 의무기록지, 간호기록지를 조사하는 후향적 코호트 방법을 시행하여 인구통계학적 자료와 사망 24시간전의 의식상태, 통증여부, 암의 종류 등을 조사하였다. 대상자를 내과의사와 외과의사에 의해 치료받은 군으로 나누어 임종전 24시간 동안 투여한 진통제의 용량, 투여방법, 투여종류, 필요시 마타 투여된 진통제의 평균 횟수를 비교 분석하였다. 진통제의 용량은 경구몰핀 등가량으로 환산해서 계산하였다. 결과 : 1) 전체 160명 중 남자가 102명(63.8%)명, 여자는 58명(36.2%) 이었고 평균연령은 56.4세였다. 2) 평균입원 27.8일이었으며 원발부위에 따른 암의 종류는 위암 42명(26.3%), 폐암 29명(18.1%), 간암 29명(18.1%), 췌장암 10명(6.2%), 대장암 9명(5.6%), 자궁경부암 6명(3.7%), 유방암 5명(3.1%)의 순이었다. 3) 160명 중 125명(78.13%)이 통증을 호소했고 진통제를 투여받은 환자는 내과계 103명중 66명(64.08%), 외과계 57명 중 31명(54.39%)이었다. 97명 중 50명(51.55%)에서 경고, 투여가 가능했다. 4) 임종 24시간 전에 86명(53.75%)에서 의식이 명료했다. 5) 규칙적으로 투여된 진통제의 종류는 경구용 모핀이 34명(내과계 24명, 외과계 10명)으로 제일 많았고, 주사용 모르핀이 26명(내과계 20명, 외과계 6명), 경구용 코데인이 8명 (내과 5명, 외과 3명)의 순이었다. 비정기적으로 통증조절을 위해 투여된 약물로는 주사용 모르핀이 16회, 데메롤 근주가 12회, nubain 근주가 9회의 순이었다. 6) 규칙적으로 투여된 진통제의 평균량은 내과계에서 115.41 OME에서 외과계 52.7 OME 보다 많았으나(P<0.05), 환자 요구시마다 투여된(p.r.n.) 진통제의 양은 외과계가 66.64 OME 로서 내과계 23.49 OME 보다 유의하게 많았다(P<0.01). 7) 통증호소시에 투여한 진통제의 횟수에 있어서도 내과계 평균은 0.62회, 외과계는 1.88로 유의한 차이가 있었다(P<0.001). 8) 보조진통제의 사용은 내과계에서 57명(55.34%), 외과계에서 25명(43.86%)이 사용했다. 결론 : 임종 24시간 전에 과반수 이상이 의식이 명료했으며 내과의사가 외과의사보다 적극적으로 통증조절을 하고 있으나 많은 의사들이 규칙적인 통증조절보다는 비정기적인 투약과 진통제의 비경구투여를 선호하였다.

케타민의 소량 분할 정주에 의한 척수손상 환자의 통증 관리 -증례 보고- (Management of Spinal Cord Injury Pain with Small Divided Doses of Intravenous Ketamine -Two case reports-)

  • Chronic pain is a frequent complication after spinal cord injury. Various medical and surgical approaches have been applied for management of spinal cord injury pain but none of them are definitive. The N-methyl-D-Aspartate (NMDA) receptor antagonist, ketamine has been reported to have a significant effect in the management of neuropathic pain. We used small divided doses of intravenous ketamine (30 mg divided by 6 equals 5 mg, 5 min interval) in spinal cord injury patients suffering from chronic pain, and accomplished significant pain relief without side effects.

설인신경통 환자에서 Phenytoin 정주의 효과 -증례 보고- (The Effect of IV Infusion of Phenytoin for Glossopharyngeal Neuralgia -A case report-)

  • Glossopharyngeal neuralgia is a rare syndrome that involves episodic bursts of pain in the sensory distributuion of the ninth cranial nerve. The nature of the pain is characterized by excruciating shock-like pain in the region of the tonsillar fossa or pharynx and can radiate to the ear or the angle of the jaw. Like trigeminal neuralgia, glossopharyngeal neuralgia typically responds to anticonvulsant agents such as carbamazepine. However, dose of carbamazepine needs to be increased gradually to avoid side effects. If the patient can not tolerate until effective carbamazepine level is reached, phenytoin can be administered intravenously at the same time that oral carbamazepine therapy is begun. We present fifty-three year old female patient suffering from glossopharyngeal neuralgia who did not respond to initial carbamazepine therapy, but responded to concomitant intravenous infusion of phenytoin.

Morphine 정주를 이용한 술후 통증 조절시 진토제 일회 정주의 지속적 항오심 및 항구토 효과 (Continuous Antiemetic Effects of Single Intravenous Injection of Antiemetics during Postoperative Pain Control with Morphine)

  • Background : This study was designed to evaluate the continuous effects of single intravenous injection of antiemetics on nausea and vomiting during continuous morphine injection for postoperative pain control. Methods : Prior to the study, we divided patients into two major groups according to the type of surgery performed intra-abdominal(Open: O) and non intra-abdominal(Close: C). When patients regained orientation after routine general anesthesia, enflurane-$O_2-N_2O$, we injected bolus dose of morphine and started continuous injection of morphine for postoperative pain control(Group I; Control). After bolus injection and just before continuous injection, we injected single dose of droperidol(Group II) or ondansetron(Group III). Mean arterial blood pressure, heart rate, pain score and symptom-therapy score were checked at 10 minutes, 4, 8, 16, 24, 36 hours after continuous morphine injection. Results : The pain score of group III was lower than group II(10 min.) and group I(24, 36 hours) in the open group. Symptom-therapy score of group III(10 min., 4, 24 hours) and group II(10 min.) were lower than group I in the open group. In the close group, symptom-therapy score of group III(8 hours) was lower than group I. Conclusions : Single intravenous injection of antiemetics have a tendency of lowering symptom-therapy score for 36 hours in spite of their relatively short elimination half-life.

Effect of Intravenous Intraoperative Esmolol on Pain Management Following Lower Limb Orthopedic Surgery

  • Background: Lack of proper control of acute postoperative pain often leads to lingering or chronic pain. Several studies have emphasized the role of beta-blockers in reducing postoperative pain. Esmolol is a selective short-acting beta-blocker that produces few side effects. The purpose of this study was to examine the effect of intravenous intraoperative esmolol on postoperative pain reduction following orthopedic leg fracture surgery. Methods: In a clinical trial, 82 patients between 20-65 years of age with tibia fractures and American Society of Anesthesiologists (ASA) physical status I & II who underwent surgery were divided into two groups. Group A received esmolol and group B received normal saline. Postoperative pain was measured at three time points: entering the recovery unit, and at 3 h and 6 h following surgery, using the Visual Analogue Scale (VAS). A P value of < 0.05 was considered significant. Results: Mean VAS scores at all three time points were significantly different between the two test groups (P = 0.02, P = 0.0001, and P = 0.0001, respectively). The consumption of pethidine was lower in group A than in group B (P = 0.004) and the duration of its effect was significantly longer in time (P = 0.026). Conclusions: Intravenous intraoperative esmolol is effective in the reduction of postoperative pain following leg fracture surgery. It reduced opioid consumption following surgery and delayed patient requests for analgesics.
