The Clinical Study on the Ordinary and Pathological Symptoms according to Sasang Constitution (사상체질에 따른 소증 및 병증지표의 임상적 연구 : 다기관 약진 대상자를 중심으로)

  • 1. Objectives The purpose of this study was to find the difference of ordinary and pathological symptoms according to Sasang Constitution. 2. Methods We recruited 418 subjects in the five oriental hospitals and researched normal symptoms and illness ones through a paper, who were diagnosed by the Sasang Constitution specialist in oriental hospital, who had remarkable improvement in their chief complain after Sasang Constitution medication. 3. Results 1) In ordinary symptoms, amount, speed and appetite items of Digestion are significantly different according to Sasang Constitution statistically(p<0.05). Feeling after evacuation of Feces, feeling after urination and yellow color of Urine, amount, feeling after sweat, location in hand/feet of Sweat, and in the others, Tireness in head, Drinking water, sensitivity, coldness in hand/feet, warmness in belly of Coldness/Warmness are also significantly different(p<0.05). 2) In pathological symptoms, palpitation, swelling and lack of appetite items are significantly different according to Sasang Constitution statistically(p<0.05). 3) In diseases, Hypertension and DM items are significantly different according to Sasang Constitution statistically(p<0.05). 4. Conclusions There were significantly difference items in ordinary, pathological symptoms and diseases according to Sasang Constitution.

A Study on the Method of Health Promotion in Korean Medicine: Correlation between Night-shift Work and Yin-deficiency (건강증진을 위한 한의학적 방법론 연구 -야간근로와 음허증의 상관관계를 중심으로-)

  • This study regarded irregular life styles such as nigh-shifts as contrary to the norm advised in the rules of Yang-saeng to analyze the problems brought about in relation to the concept of 'Yin-deficiency syndrome'. Yin-deficiency survey was given to sales workers on a big shopping mall in Seoul to compare the measurements of daytime workers to those of nigh-shift workers. The measurement of complaining symptoms related to Yin-deficiency of daytime workers and night-shift workers were compared, In comparison of the daytime workers and the night-shift workers, night-shift workers showed higher measurements than the daytime workers in the item of irritable fever on the five Hearts, flushing of the zygomatic region in the afternoon, tidal fever, dizziness, insomnia, yellow and scanty urination, and constipation. Especially tidal fever, insomnia, and constipation showed statistically significant difference. The total of ten items consisting of Yin-deficiency-related symptoms showed statistically significant high score in night-shift workers than the daytime workers. 'Factors for deficiency-type Heat' consisting of irritable fever on the five Hearts, flushing of the zygomatic region in the afternoon, tidal fever, and dried mouth and throat showed statistically significant high score in night-shift workers than the daytime workers. 'Accompanying factors' consisting of night sweats, emaciation, dizziness, insomnia, yellowish and scanty urination, and constipation also showed statistically and significantly high score in night-shift workers than the daytime workers. From the above results that night-shift workers show high degree of Yin-deficiency than daytime workers, and those overworking irregularly also show high degree of Yin-deficiency than those who work for adequate amount of time regularly implies that sitting up at night for work and sleeping at daytime, excessive work, and irregular life styles all function as high-risk factor for Yin-deficiency.

  • The proportion of cat cats among companion animals has been increasing at an average annual rate of 25.4% since 2012. Cats have strong wildness compared to dogs, so they have a characteristic of hiding diseases well. Therefore, when the guardian finds out that the cat has a disease, the disease may have already worsened. Symptoms such as anorexia (eating avoidance), vomiting, diarrhea, polydipsia, and polyuria in cats are some of the symptoms that appear in cat diseases such as diabetes, hyperthyroidism, renal failure, and panleukopenia. It will be of great help in treating the cat's disease if the owner can recognize the cat's polydipsia (drinking a lot of water), polyuria (a large amount of urine), and frequent urination (urinating frequently) more quickly. In this paper, 1) Efficient version of DeepLabCut for posture prediction running on an artificial intelligence server, 2) yolov4 for object detection, and 3) LSTM are used for behavior prediction. Using artificial intelligence technology, it predicts the cat's next, polyuria and frequency of urination through the analysis of the cat's behavior pattern from the home CCTV video and the weight sensor of the water bowl. And, through analysis of cat behavior patterns, we propose an application that reports disease prediction and abnormal behavior to the guardian and delivers it to the guardian's mobile and the main server system.

Changes in External Radiation Dose Rate for PET-CT Test Patients (PET-CT 검사 환자의 외부 방사선량률 변화)

  • This paper analyzes changes in the external radiation dose rate of PET-CT test patients as a part of providing basic materials for reduction of radiation exposure to PET-CT test patients. In theory the measurement of external radiation dose rate of PET-CT test patients shows that the further the distance from the patient injected with radioactive pharmaceutical and a longer time elapsement from the injection leads to a smaller amount of radiation. Particularly, the amount of radiation marked the highest in the chest was at 4.17 minutes immediately after the intravenous injection and in the head after 77.47 minutes after urination in advance to the PET-CT test. As in the generalized information, it is desired to keep distance between the patient and caretakers or professionals to reduce the amount of radiation exposure from PET-CT test patients and to resume contact the patient after the time when the radiation has reduced. If contact is unavoidable, it is desired to keep at least 200cm from the patient. In addition, the amount of radiation reached the highest in the chest at first and then in the head from 77 minutes after injection. Accordingly, it would be helpful in achieving the optimization if contact is made based on the patient's physical characteristics. This study is significant as it measures changes in radiation the dose rate by; distance from the PET-CT test patient, time elapsed, and specific parts of body. Further studies based on the findings in this paper are required to analyze changes in radiation dose rate in accordance with individual characteristics unique to PET-CT patients and to utilize the results to reduce the amount of radiation patient, caretakers and professions are exposed.

Analysis on Intake of Energy Drinks of High School Students in Gyeoungbuk Region (경북 지역 고등학생의 에너지 음료 섭취 실태 분석)

  • This study investigated intake of energy drinks, side effects and willingness to stop intake of 255 high school students in Gyeoungbuk region. The data were collected by the self-administered questionnaire. Frequency, t test, one-way ANOVA, Duncan's multiple range test, and ${\chi}^2$ test were conducted by SPSS Window V.21.0. About 78% of the respondents had experience of intake of energy drinks. The respondents were aware of energy drinks from 'advertisements on mass media (44.3%)', 'friends (32.2%)', and 'looking in stores (16.9%)'. The mean of the amount of energy drink intake per day was 30.62 mg. Forty-four percent of the respondents had energy drinks during an examination period, and 37% took it at home. The main reasons for intake of energy drinks were 'to fight off sleepiness', 'to recover from fatigue', and 'good taste' in order. Many respondents answered that energy drinks did not much help to increase concentration or learning ability. About 72% of the respondents experienced 'not feel sleepy' after having energy drinks. Half of the respondents experienced side effects such as palpitation, insomnia, and increase of urination. Most respondents had willingness to stop having energy drinks if it had bad effect on health.

Is It Useful and Safe to Maintain the Sitting Position During Only One Minute before Position Change to the Jack-knife Position?

  • Background: Conventional spinal saddle block is performed with the patient in a sitting position, keeping the patient sitting for between 3 to 10 min after injection of a drug. This amount of time, however, is long enough to cause prolonged postoperative urinary retention. The trend in this block is to lower the dose of local anesthetics, providing a selective segmental block; however, an optimal dose and method are needed for adequate anesthesia in variable situations. Therefore, in this study, we evaluated the question of whether only 1 min of sitting after drug injection would be sufficient and safe for minor anorectal surgery. Methods: Two hundred and sixteen patients undergoing minor anorectal surgery under spinal anesthesia remained sitting for 1 min after completion of subarachnoid administration of 1 ml of a 0.5% hyperbaric bupivacaine solution (5 mg). They were then placed in the jack-knife position. After surgery, analgesia levels were assessed using loss of cold sensation in the supine position. The next day, urination and 11-point numeric rating scale (NRS) for postoperative pain were assessed. Results: None of the patients required additional analgesics during surgical manipulation. Postoperative sensory levels were T10 [T8-T12] in patients, and no significant differences were observed between sex (P = 0.857), height (P = 0.065), obesity (P = 0.873), or age (P = 0.138). Urinary retention developed in only 7 patients (3.2%). In this group, NRS was $5.0{\pm}2.4$ (P = 0.014). Conclusions: The one-minute sitting position for spinal saddle block before the jack-knife position is a safe method for use with minor anorectal surgery and can reduce development of postoperative urinary retention.

Study on the Weight of Health Evaluation Indexes according to Sasang Constitution (사상체질별 건강 평가 지표의 중요도 조사 연구)

  • The purpose of this study is to survey the importance of items for the physiological and pathological symptoms for estimating a health condition according to Sasang constitution to establish the SHI(Sasangin Health Index) which will reflect the concept of inherent vitality. We surveyed expert opinion with questionnaires from 20 Sasang constitution specialists. The questionnaire was composed of 57 items on physiological and pathological symptoms and specialist opinions. Each item was marked from A to E according to importance in evaluating health state in each constitution, and if the mean score of an item was over 3.0, the item was regarded as important. Important indexes among physiological symptoms were diet, digestion condition, perspiration condition, frequency of defecation, heat and cold response, and temperature of drinking water in Soeumin, pathologic perspiration and defecation condition in Soyangin, repast, perspiration condition, and the amount of drinking water in Taeeumin, and urination frequency in Taeyangin. Important indexes among pathological symptoms were sighing, indigestion, and abdominal pain in Soeumin, oral condition, chest distress, brash, and amnesia in Soyangin, eye condition, palpitation, and edema in Taeeumin, and vomiting and incapacity of the lower limbs in Taeyangin. There are different health evaluating index and priority order in it according to Sasang constitution.

Effects of a Self-Efficacy Promotion Program for Management of Urinary Incontinence on the Degrees of Incontinence, Self-Efficacy and Quality of Life (요실금 관리 자기효능증진프로그램이 요실금 정도와 자기효능감 및 삶의 질에 미치는 효과)

  • Purpose: This study was conducted to develop a self-efficacy promotion program and to identify the effects of the program on degree of urinary incontinence, self-efficacy and quality of life in women with urinary incontinence. Method: The study used a pre-experimental design. Twenty participants were recruited from a nursing home located in Gwangju City and included residents, employees, and volunteer workers with urinary incontinence. Participants were surveyed from September 21 to November 2, 2005. The program treatment was provided to participants once a week for 6 weeks. Results: Frequency, amount, and situation scores for urinary incontinence, maximum and mean vaginal constriction pressure, vaginal constriction time, and nocturnal frequency were significantly improved after participating in the program (p<0.05). However, there was no significant pre- and posttest difference in frequency of daily urination (p=0.097). The degree of self-efficacy and quality of life for participants in the self-efficacy promotion program at posttest were significantly higher compared to the pretest scores (p<0.05). Conclusions: The results of the study indicate that a self-efficacy promotion program can be implemented for women with urinary incontinence in community health centers or nursing homes in order to reduce their incontinence symptoms, and increase self-efficacy and quality of life.

Effects of Acupuncture on Urinary Incontinence in Premenopausal Women : Preliminary Study (폐경 전 여성 요실금에 대한 침치료 효과 : 예비연구)

  • Objectives : The purposes of this preliminary study was to examine the feasibility of recruiting women into a clinical trial designed to examine the effects of acupuncture in treating urinary incontinence (UI) in premenopausal women with delivery history and the feasibility of performing the study procedures. We also tried to determine if there was preliminary evidence to suggest that acupuncture may be effective in reducing UI and improving disease-specific quality of life. Methods : This study was a pragmatic randomized clinical trial. Subjects between the ages of 20 and 49 years were randomly assigned to a treatment(n=11) or control group(n=11) and analyzed. Both groups were supposed to do Kegel exercise at home during 4 weeks and acupuncture was applied to the treatment group twice a week(8 sessions) additionally. Subjects performed 1 hour pad test and completed a 3-day urination diary, international consultation on incontinence modular questionnaire(ICIQ), and incontinence - quality of life(I-QOL) at base line and 5 weeks. Results : Both group showed improvement in 1 hour pad test and the reducing amount of UI was significantly larger in treatment group(p=0.0182). The significant improvements in ICIQ and I-QOL were also observed in treatment group and sustained until the follow-up measurement at 16 weeks. Any adverse reaction related to acupuncture did not happen. Conclusions : It was feasible to recruit subjects and perform the study procedures. The positive results of this study support the requirement for additional research investigating the efficacy of acupuncture in the treatment of UI in women.

A Study on Effectiveness of the Hospital-based Home Nursing Care of the Early Discharged Surgical Patients and its Cost Analysis (조기퇴원 수술환자의 병원중심 가정간호 효과 및 비용분석에 관한 연구)

  • Medical insurance and health care delivery system enabled Korean people to get the necessary medical service, but it caused increased needs for medical service, and resulted in the occurence of some problems such as a lack of manpower and medical facilities. In order to solve these problems, many countries, which already had medical insurance system had developed home care system and it has been regarded effective both in reducing costs and in increasing the rates of turnover of bed. Recently, Korea has included home nursing care in its health care delivery system, and some models of the hospital based home nursing care had been tried and its effects had been evaluated. So, author tried to run a home nursing care for the Cesarean section mothers and evaluate Its effects both in the mother's health and costs. This study was designed as a Quasi-experimental study. Subjects were thirty mothers who got Cesarean section operation in hospital in Seoul. Experimental group consisted of 15 volunteers, and control group were selected by means of matching technique. Data were gathered from February 1st to March 26th by two assistants who were trained by author. Experimental group were discharged on the 4th day after their operation, and got nursing care and assessment about their home three times on the 5th, 6th, and 7th day. Control group stayed in the hospital until 7th day as usual and were checked on the same day as above mentioned To evaluate the state of physiological recovery, vital signs, H.O.F, presence of edema in the legs, bathing, appetite, sleep, presence of pain or discomfort in the breasts, amount of lochia, color of lochia, defecation urination. To compare incidence of complication in experimental group with that in control group, specific assessment was done such variables as smell of lochia, presence of inflammation of operation wound, dizziness, and presence of immobilization in the extremities. The activities of daily living were checked Satisfaction of nursing were checked To calculate costs, author asked subjects to specify expenditure including hospital charge, traffic enpenses, and food expenses. The results were as fellows. 1. On effectiveness of home nursing careThere were n significant differences between experimental and control group in incidence of abnormal symptoms and any complication. The number of taking a bath [POD #5 P=0.001, #6 P=0.0003, #7 P=0.001] and the degree of appetite [POD #5 P=0.03, #6 P=0.02, #7 P=0.013] were significantly higher in experimental group than in control group. Contrary to author's expectation, the degree of the activities of daily living in experimental group was not higher than that of control group. All of the experimental group said they were satisfied with the home nursing care. 2. Cost analysis 1) Hospital charge of experimental group was lower than that of control group. [P=0.009] By taking home nursing care, average period of hospitalization was shortened to 3.1 days, and family members could save 22.8 hours. Total amount of money saved by early discharge was 3,443,093 Won. It is estimated that total amount of money saved by early discharge in a year will be 40,398,956 Won. 2) Home nursing care charge of 15 mothers was 1,781,633 Won. It is estimated that total amount of money Saved by it in a year Will be 20,904,493 Won. It was lower altogether than hospital charge of the three days which is 5th, 6th, 7th day of operation. The average cost of single home visit was calculated 10,940 Won. It took 87 minutes per round and it costed 1,017.3 Won. The average hour of home care was 39.0 minutes. 3) It is expected that early discharge can bring forth the increase of hospital income. On the condition that the rate of running bed is 100%, the expected increase of hospital income will be 202,374, 026 Won in a year. Suggestions for further study and nursing practice are as follows : 1. For the welfare of patients and the increased rates of running bed, home nursing care system should be included in the hospital nursing care system. 2. Studies to test effect of home nursing care on the patients with other diseases are needed. 3. Establishment of law on the practice of home nursing care is strongly recommended.
