THE EFFECT OF MULTIPLE APPLICATION ON MICROTENSILE BOND STRENGTH OF ALL-IN-ONE DENTIN ADHESIVE SYSTEMS (All-in-one adhesive의 다층적용이 미세인장결합강도에 미치는 영향)

  • The purpose of this study was to evaluate the effect of multiple application of all-in-one dentin adhesive system on microtensile bond strength using confocal laser scanning microscope and microtensile bond strength test. Flat occlusal dentin surfaces were prepared using low-speed diamond saw. In group I, Scotchbond Multipurpose (SM) was applied by manufacturer's recommendation. In group II, after Adper Prompt L-Pop was applied for 15s and light cured for 10s. the second coat was re-applied and light-cured. In group III, after light-curing the second layer. the third coat was re-applied and light-cured. Specimens bonded with a resin-composite were sectioned into resin-dentin stick for measuring the adhesive layer thickness by confocal laser scanning microscope and evaluating micro-tensile bond strength. The adhesive layers of three-step dentin adhesive system. 3 coats of Adper Prompt L-Pop had significantly thicker than SM. 2 coats of Adper Prompt L-Pop (p < 0.05). However. there was no significant differences in bond strengths between SM and 3 coats of Adper Prompt L-Pop (p > 0.05). And SM. 3 coats of Adper Prompt L-Pop had significantly higher than 2 coats of Adper Prompt L-Pop in bond strengths (p < 0.05).

Clinical experience of open heart surgery -113 cases- (개심술 113예에 대한 임상적 고찰)

  • One hundred thirteen cases of open heart surgery were performed in the department of thoracic and cardiovascular surgery of Inha General Hospital from April 1986 to April 1987. There were 73 cases of congenital heart disease and 40 cases of acquired valvular heart disease, including one redo case respectively. The technique of deep hypothermia with circulatory arrest was used widespreadly for infants and small children, and early extubation was performed as possible in the great number of all cases. There were three operative deaths [2.7%], all in congenital cases, and three follow-up deaths, all in acquired cases.

Ruptured Aneurysm of Aortic sinus of Valsalva -A Report of 6 Cases- (대동맥동 동맥루 파열 -수술치험 6예-)

  • The purpose of this paper is to present author's experience with 6 cases of ruptured aneurysm of sinus of Valsalva which were treated surgically during last 10 years. Among the 6 cases, 5 were male and one was female. All of them originated from the right coronary sinus and 5 cases were ruptured into the RV while remained one into RA. The diagnosis was obtained in 4 cases by cineangiocardiogram. Clinically, we had difficulties in differential diagnosis with combined cases of VSD with A.I. and had special experience in its differentiation during cardiac catheterization. By simultaneous trans-venous and trans-arterial catheterization, identified two catheter tips in the RV, and pull back tracing obtained aortic pressure directly from RV, and RA from RV pressure which were benefit in confirm ruptured aneurysm of the aortic sinus. Surgical correction was performed by means of direct suture closure or combined Teflon pledget Of patch enforcement graft after aneurysm resection by trans-RA or trans-RV approach. All patients had no history of bacterial endocarditis, syphilis, or tuberculosis and operative findings revealed intact coronary sinus except involved one moreover 3 cases combined with high VSD which uggested congenital in origin although pathologic reports revealed only fibrosis. Post-operative course were uneventful in all cases but one who had bleeding and 2 months to 9 years follow up results were good and spend their usual life in all cases.

A Study on Safety and Performance Evaluation of Smart All-in-one Cardiopulmonary Assist Device (스마트올인원 심폐순환보조장치의 안전성 및 성능평가에 관한 연구)

  • The existing Extracorporeal membrane oxygenation(ECMO) and Cardiopulmonary bypass system(CPB) have been developed and applied to various devices according to their respective indications. However, due to the complicated configuration and difficult usage method, it causes inconvenience to users and there is a risk of an accident. Therefore, smart all-in-one cardiopulmonary circulation device is being developed recently. The smart all-in-one cardiopulmonary assist device consists of a blood pump for cardiopulmonary bypass, a blood oxidizer for cardiopulmonary bypass, a blood circuit for cardiopulmonary bypass, and an artificial cardiopulmonary device. It is an integrated cardiopulmonary bypass device that can be used for a variety of purposes such as emergency, intraoperative, post-operative intensive care, and long-term cardiopulmonary assist, combined with CPB used in open heart surgery and ECMO used when patient's cardiopulmonary function does not work normally. The smart all-in-one cardiopulmonary assist device does not exist as a standard and international standard applicable to advanced medical devices. Therefore, in this study, we will refer to the International Standard for Blood Components, the International Standard for Blood, the Guideline for Blood Products, and prepare applicable performance and safety guidelines to help quality control of medical devices, and contribute to the improvement of the health of people. The guideline, which is the result of conducted a survey of the method of safety and performance test, is based on the principle of all-in-one cardiopulmonary aiding device, related domestic foreign standards, the status of domestic and foreign patents, related literature, blood pump(ISO 18242), blood oxygenator (ISO 7199), and blood circuit (ISO 15676) for cardiopulmonary bypass.The items on blood safety are as follows: American Society for Testing and Materials ASTM F1841-97R17), and in the 2010 Food and Drug Administration's Safety Assessment Guidelines for Medical Assisted Circulatory Devices. In addition, after reviewing the guidelines drawn up through expert consultation bodies including manufacturers / importers, testing inspectors, academia, etc. the final guideline was established through revision and supplementation process. Therefore, we propose guidelines for evaluating the safety and performance of smart all-in-one cardiopulmonary assist devices in line with growing technology.

  • In this study, we focused on development of the all-in-one case insulation for end-burning solid rocket motors. Material of insulation used unvulcanized rubber based on EPDM/kevlar. In case of boots insulation, preforms was made by using hot press molding, and then the tape was inserted between two preforms for all-in-one curing in the case. Finally bladder method was applied for curing of insulation.

EFFECT OF BENZALKONIUM CHLORIDE ON DENTIN BONDING WITH BPDM/HEMA SYSTEM (Benzalkonium Chloride가 BPDM/HEMA계 접착제의 상아질 접착에 미치는 영향)

  • The purpose of this study was to elucidate the effect of benzalkonium chloride on tensile bond strength of BPDM/HEMA dentin bonding. One hundred sixty dentin specimens from freshly extracted bovine mandibular incisors were used, and 0, 0.02, 0.1 or 0.5% benzalkonium chloride solution was applied to the dentin specimen with/after phosphoric acid. 32% phosphoric acid was used when the specimens were bonded with One-Step$^{TM}$, a BPDM/HEMA system and 10% was used when bonded with All-Bond$^{(R)}$ 2, a NTG-GMA/BPDM system. Aelitefil$^{TM}$ composite resin was bonded to the pretreated dentin specimen with the use of All-Bond$^{(R)}$ 2 or One-Step$^{TM}$ dentin bonding agent. After the bonded specimens were stored in $37^{\circ}C$ distilled water for 24 hours, tensile bond strength was measured. The fractured dentin specimens were examined under the scanning electron microscope. The results were as follows : Benzalkonium chloride application after acid-treatment resulted in decrease of dentin bond strength of One-Step$^{TM}$, a BPDM/HEMA system (p>0.05). Benzalkonium chloride application did not exert any influence on dentin bond strength of All-Bond$^{(R)}$ 2, a NTG-GMA/BPDM system (p>0.05). There was no relationship between the concentration or application method of benzalkonium chloride and the dentin bond strength of One-Step$^{TM}$ or All-Bon$^{(R)}$ 2 (p<0.05). On SEM examination of the fractured dentin-resin interface, while mixed failure was prominent in dentin bonding with One-Step$^{TM}$, adhesive and mixed failures were seen together in dentin bonding with All-Bond$^{(R)}$ 2 regardless of the concentration and application method of benzalkonium chloride.

Effect of additives and filling methods on whole plant corn silage quality, fermentation characteristics and in situ digestibility

  • Objective: This project aimed to evaluate the effects of both different additives and filling methods on nutritive quality, fermentation profile, and in situ digestibility of whole plant corn silage. Methods: Whole plant corn forage harvested at 26.72% dry matter (DM) was chopped and treated with two filling methods, i) fill silos at one time (F1), ii) fill silos at three times (F3), packing samples into one/three silo capacity at the first day, another one/three capacity at the second day, then one/three at the third day, three replicates. For each replicate, samples were treated with three additives, i) control (CTRL, no additive), ii) Sila-Max (MAX, Ralco Nutrition Inc., Marshall, MN, USA), and iii) Sila-Mix (MIX, Ralco Nutrition Inc., USA). With three replicates of each secondary treatment, there were nine silos, 54 silos in total. Each silo had a packing density of 137.61 kg of DM/m3. All silos were weighed and stored in lab at ambient temperature. Results: After 60 d of ensiling, all items showed good silage fermentation under MAX filled one time or three times (p<0.01). Higher silage quality for all additives was obtained at filling one time than that filled three times (p<0.01). The highest DM and lowest DM loss rate (DMLR) occurred to MAX treatment at two filling methods (p<0.01); Digestibility of acid detergent fiber, neutral detergent fiber (NDF), and curde protein had the same results as silage quality (p<0.01). Yield of digestible DM and digestible NDF also showed higher value under MAX especially for filling one time (p<0.05). Conclusion: All corn silages showed good fermentation attributes (pH<4.0). The forage filled one time had higher silage quality than that filled three times (p<0.01). MAX with homofermentative lactic acid bacteria enhanced the lactic acid fermentation, silage quality and nutrient digestibility, and so improved the digestible nutrient yield.

  • This paper presents new architectures based on the linear feedback shia resister architecture over GF(2m). First we design a modular multiplier and a modular squarer, then propose an architecture by combing the multiplier and the squarer. All architectures use an irreducible AOP (All One Polynomial) as a modulus, which has the properties of all coefficients with '1'. The proposed architectures have lower hardware complexity than previous architectures. They could be. Therefore it is useful for implementing the exponentiation architecture, which is the con operation in public-key cryptosystems.

Cardiac Tumors (심장종양 6례 보고)

  • Primary cardiac tumors are uncommon in all age group. In contrast, tumors metastatic to the heart are significantly more common. On rare occasions, tumor may extend into the heart chamber via inferior vena cava from other parts of the body, such as liver, kidney, and uterus cava. With recent advancement in diagnostic imaging modalities and surgical techniques, cardiac tumors are now potentially curably form of heart disease. The most important factor in diagnosing the tumor is a high index of clinical suspicion. Six patients underwent surgical removal of intracardiac tumor during a 5-year period. The mean age of the 4 women and two men was 40 years [range 23 to 60]. All patients were operated on in the last five years of the studied period. All patients had symptoms varying in duration from 1 month to 4 years [average 13 months]. 2-Dimensional echocardiography contributed most to preoperative diagnosis, confirming presence of an intracardiac tumor in all examined patients. Of the six intracardiac tumor, 5 were myxomas [4 left atrial and 1 right ventricular] and one right atrial metastasis from hepatocellular carcinoma of the liver. In all cases, tumor masses were successfully excised. One patient expired after the operation on account of low cardiac out-put syndrome. Remained one patient among six, tumor mass extended into RA and RV with a stalk via IVC. On later follow-up study showed cold area on liver scan [hepatocellular ca.], so she was transferred to internal medicine, department for chemotherapy. Follow up results showed no signs of tumor recurrence in 4 myxoma cases.
