일축인장시험을 통한 투명교정장치용 고분자 소재의 역학적 특성 분석 (Analysis of Mechanical Properties of Polymer Material for Clear Aligner using Uniaxial Tensile Test)

  • Clear aligners are popular in the field of dental orthodontic treatment because they offer a discreet alternative to braces due to their use of transparent materials. They are formed from flat transparent polymer materials by hot pressed molding. It is necessary to know the mechanical properties of the polymer materials to be able to form the exact shapes of the clear aligners. However, this information is not publicly available. In this study, we present a method to reliably measure the mechanical properties of clear aligner polymer materials and analyze the factors effecting these mechanical properties. First, we surveyed standards related to the mechanical properties of polymer materials to obtain reliable data. Consequently, ISO 527 was selected for use in this study because of the size and thickness of the flat transparent polymer material. The uniaxial tensile tester was constructed and it was verified whether displacement of a crosshead could be regarded as a displacement of gauge-length by optical analysis. Uniaxial tensile tests of three thicknesses from three different companies were performed and each engineering stress-strain curve was measured. Tensile strengths and elastic moduli were obtained by analysis of the stress-strain curves. The tensile strength and elastic modulus of ISO 527 was found to be approximately 50MPa and 2.3GPa, respectively. Both values showed material and thickness dependency.

Treatment of anterior open bites using non-extraction clear aligner therapy in adult patients

  • Objective: The purpose of this study was to examine the effectiveness and mechanism of clear aligner therapy for the correction of anterior open bite in adult nonextraction cases. Methods: Sixty-nine adult patients with anterior open bite were enrolled and classified into Angle's Class I, II, and III groups. Fifty patients presented with skeletal open bite (mandibular plane angle [MPA] ≥ 38°), whereas 19 presented with dental open bite. Fifteen cephalometric landmarks were identified before (T1) and after (T2) treatment. The magnitudes of planned and actual movements of the incisors and molars were calculated. Results: Positive overbite was achieved in 94% patients, with a mean final overbite of 1.1 ± 0.8 mm. The mean change in overbite was 3.3 ± 1.4 mm. With clear aligners alone, 0.36 ± 0.58 mm of maxillary molar intrusion was achieved. Compared with the Class I group, the Class II group showed greater maxillary molar intrusion and MPA reduction. The Class III group showed greater mandibular incisor extrusion with no significant vertical skeletal changes. Conclusions: Clear aligners can be effective in controlling the vertical dimension and correcting mild to moderate anterior open bite in adult nonextraction cases. The treatment mechanism for Class III patients significantly differed from that for Class I and Class II patients. Maxillary incisor extrusion in patients with dental open bite and MPA reduction with mandibular incisor extrusion in patients with skeletal open bite are the most significant contributing factors for open bite closure.

Periodontal health status, oral microbiome, white-spot lesions and oral health related to quality of life-clear aligners versus fixed appliances: A systematic review, meta-analysis and meta-regression

  • Objective: Assess and evaluate the different indicators of oral health-related quality of life (OHRQoL) among patients treated with clear aligners (CAs) versus those treated with conventional fixed orthodontics (FAs). Methods: An electronic search was performed on the database is Web of Science, Scopus, and Embase databases. Randomized and non-randomized control trials, cross-sectional, prospective cohort and retrospective trials were included. Quality was assessed with risk of bias tool and risk of bias in non-randomised studies. Meta-analyses were performed with random effects models, estimating the standardized and non-standardized mean differences, odds ratio and risk ratio as the measure of effect. The effect on time was determined using a meta-regression model. Results: Thirty one articles were included in the qualitative synthesis and 17 in the meta-analysis. CAs had a significantly lower negative impact on QoL, with an "important" effect size, while the influence of time was not significant. Periodontal indicators plaque index (PI), gingival index (GI), probing depth (PD), and bleeding on probing show significantly better values in patients treated with CAs, with moderate to large effect sizes. PI and GI have a significant tendency to improve over time. In microbiological indicators, CAs present a lower biofilm mass without differences in the percentage of patients with high counts of Streptococcus mutans and Lactobacilli bacteria. The risk of white spot lesion onset is ten times lower in carriers of CAs. Conclusions: Patients wearing CAs show better periodontal indicators, less risk of white spot development, less biofilm mass and a better QoL than patients with FAs.

Clinical expression of programmed maxillary buccal expansion and buccolingual crown inclination with Invisalign EX30 and SmartTrack aligners and the effect of 1-week vs. 2-week aligner change regimes: A retrospective cohort study

  • Objective: This retrospective cohort study aimed to assess and compare the accuracy of 3 different Invisalign® treatment regimens in terms of variations of aligner change frequency and type of aligner material in achieving maxillary dental buccal expansion. Methods: Altogether, 120 adult patients whose treatment involved maxillary dental expansion with Invisalign® were included. The patients were divided into 3 groups, with each group comprising 40 patients as follows: SmartTrack® 1-week changes (ST1), SmartTrack® 2-week changes (ST2), and EX30® 2-week changes (EX2). The groups were assessed by comparing actual changes achieved with those prescribed by ClinCheck®. The rates of clinically significant inaccuracies (CSI) observed for buccal expansion (≥ 0.5 mm) and buccolingual inclination (≥ 2°) during expansion were then determined. Results: In terms of expansion, the ST1 group demonstrated the highest CSI rate at all tooth levels, whereas the ST2 group had the lowest rate of CSI and the lowest mean inaccuracy for each tooth level. In terms of buccolingual inclination, the ST1 group had the highest CSI rate across all tooth levels, whereas the EX2 group had the lowest CSI rate at all tooth levels except for the canine level where the ST2 group had the lowest CSI rate. A tendency toward overexpression of buccal crown inclination, and underexpression of buccal expansion was observed at all tooth levels. Conclusions: Two-week aligner change regimens offer improved accuracy compared with 1-week aligner changes. SmartTrack® 2-week changes were the most accurate for buccal expansion, whereas EX30® 2-week changes were the most accurate for buccolingual inclination.

  • Nano-stages are used in many ultra-precision systems, such as scanning probe microscope(SPM), optical fiber aligners, ultra-precision cutting, measuring systems, and optical systems. It is difficult to find the solutions because the performances and characteristics of nano-scale motion stage are determined by various factors. To understand effects of nano-scale motion stage, three types of hinge structures were designed and manufactured. Each hinge structures were designed following with the results of simulation. And from the result of experiments, hysteresis, displacement, and accuracy were compared with each hinge structures.

투명 장치의 열가소성 재료의 올바른 이해 (Physical properties of thermoplastic material for clear aligners)

  • Recent technological advance have greatly expanded the application of invisible orthodontic treatment using clear thermoplastic materials. However, the final outcomes using clear aligner system do not achieve the level of final goal frequently, which results in case refinement, midcourse correction, or fixed orthodontic treatment. Therefore, mechanical properties of thermoplastic materials should be considered to improve the quality of outcomes. The purposes of this special article were to evaluate the force and stress depending on the materials, deflection and thickness of thermoplastic materials and to evaluate the mechanical properties of thermoplastic materials after repeated loading. Thickness and amount of deflection rather than products and materials showed the largest effect on force and stress. In all products, at least 159 gf of force was required for more than 1.0 mm deflection or when materials with 1.0 mm thickness were deflected. Orthodontic forces delivered by thermoplastic materials depend on the materials, thickness, amount of activation, and intra-oral condition. Proper thickness of thermoplastic materials and deflection level of tooth movement should be decided for the efficient and physiologic tooth movement.

  • 현재 치아 교정 분야에서 투명교정의 중요성이 점점 더 부각되고 치아 교정 시장에서의 점유율도 점점 높아지고 있다. 현재 세계 1등 기업이 전 세계 시장의 대부분을 차지하고 있고 수많은 후발 주자들이 제품 개발에 많은 공을 들이고 있다. 국내에서도 많은 회사에서 투명 교정 시스템을 개발하고 있고 그 중에 세라핀 투명 교정 시스템은 세계 1등 기업의 제품과 마찬가지로 단순한 부분 교정이 아닌 포괄적 교정 치료를 지향하고 있다. 본 증례 보고에서 세라핀 투명 교정 시스템을 이용한 두 증례를 보고하고자 한다.

  • 경미한 I급 부정교합의 경우, 회전 및 경사 조절은 먼저 NiTi 호선을 사용하여 달성한 후, 순설 이동 및 약간의 수직 조절은 투명교정장치로 이루는 방법을 선택할 수 있다. 이때 tooth capture가 잘 안 되는 부위에서는 slingshot 또는 수직 고무줄을 사용하여 보완할 수 있다.

A systematic review of the accuracy and efficiency of dental movements with Invisalign®

  • We are currently living in an era where the use of computer-aided design/computer-aided manufacturing has allowed individualized orthodontic treatments, but has also incorporated enhanced digitalized technology that does not permit improvisation. The purpose of this systematic review was to analyze publications that assessed the accuracy and efficiency of the $Invisalign^{(R)}$ system. A systematic review was performed using a search strategy to identify articles that referenced $Invisalign^{(R)}$, which were published between August 2007 and August 2017, and listed in the following databases: MEDLINE, Embase, Cochrane Library, Web of Knowledge, Google Scholar, and LILACS. Additionally, a manual search of clinical trials was performed in scientific journals and other databases. To rate the methodological quality of the articles, a grading system described by the Swedish Council on Technology Assessment in Health Care was used, in combination with the Cochrane tool for risk of bias assessment. We selected 20 articles that met the inclusion criteria and excluded 5 due to excess biases. The level of evidence was high. Although it is possible to treat malocclusions with plastic systems, the results are not as accurate as those achieved by treatment with fixed appliances.

Evaluating anchorage loss in upper incisors during distalization of maxillary posterior teeth using clear aligners in adult patients: A prospective randomized study

  • Objective: To evaluate the effect of clear aligner treatment and differential sequence distalization of maxillary posterior teeth on anchorage loss in the upper incisors (U1s). Methods: This study used lateral cephalometries and digital models of 12 patients treated with 33% sequential distalization (group 1, mean age: 22.9 ± 0.7 years, five males, seven females) and 12 treated with 50% sequential distalization (group 2, mean age: 25.83 ± 0.5 years, three males, nine females) acquired before and after distalization of upper second premolars (U5) and upper first molars (U6) and upper second molars (U7). The amount of distalization was determined as 2.5 mm in both the groups. Independent Samples t test was used to compare normally distributed parameters. Mann-Whitney U and Wilcoxon tests were used to compare parameters that were not normally distributed. Results: In both groups, the posterior teeth significantly moved by tipping distally and the U1s were displaced anteriorly. Increase in maxillary posterior transverse width (P < 0.001) and distopalatal rotation were observed in U5, U6, and U7 after distalization. It was also observed that U1 was significantly more proclined (1.82°; P < 0.001) and protruded (0.62 mm; P < 0.001), and the overjet (0.45 mm; P < 0.001) increased more in group 1 than in group 2. Conclusions: After sequential distalization of maxillary posterior teeth, more anchorage loss was observed in the anterior region in group 1 than in group 2.
