Clinical Practice Guideline of Acute Respiratory Distress Syndrome

  • There is no well-stated practical guideline for mechanically ventilated patients with or without acute respiratory distress syndrome (ARDS). We generate strong (1) and weak (2) grade of recommendations based on high (A), moderate (B) and low (C) grade in the quality of evidence. In patients with ARDS, we recommend low tidal volume ventilation (1A) and prone position if it is not contraindicated (1B) to reduce their mortality. However, we did not support high-frequency oscillatory ventilation (1B) and inhaled nitric oxide (1A) as a standard treatment. We also suggest high positive end-expiratory pressure (2B), extracorporeal membrane oxygenation as a rescue therapy (2C), and neuromuscular blockage for 48 hours after starting mechanical ventilation (2B). The application of recruitment maneuver may reduce mortality (2B), however, the use of systemic steroids cannot reduce mortality (2B). In mechanically ventilated patients, we recommend light sedation (1B) and low tidal volume even without ARDS (1B) and suggest lung protective ventilation strategy during the operation to lower the incidence of lung complications including ARDS (2B). Early tracheostomy in mechanically ventilated patients can be performed only in limited patients (2A). In conclusion, of 12 recommendations, nine were in the management of ARDS, and three for mechanically ventilated patients.

A Study on the Non-market Economic Value of Marine ranches and Marine Forests Using Contingent Valuation Method (조건부가치측정법(CVM)을 이용한 바다목장과 바다숲의 비시장 경제가치 연구)

  • The Korean government has been carrying out the marine ranch development project since 1998 with the purpose of responding to the decrease in coastal fishery resources and fishery income, preparing a systematic management system for the sustainable use of fishery resources and realizing advanced fisheries power by expanding and upgrading fisheries resource development projects. In addition, the government established the Korea Fisheries Resources Agency and promoted projects for the protection and management of fishery resources by increasing basic productivity by artificially creating marine forests in areas where whitening events occur. Since the project of building marine ranches and marine forests requires immense government financial support, it is important to estimate the economic value and thoroughly evaluate the feasibility of the project. In this paper, the project of non-market economic value of the development of marine ranches and the development of marine forests was estimated. CVM (Contingent Valuation Method) was applied as a methodology for benefits estimation. Prior to the analysis, a one-on-one interview survey was conducted with participation of 512 residents and 514 residents respectively for the project of creating a marine ranch and developing a marine forest. A DBDC (Double-Bounded Dichotumous Choice) model was applied in the WTP (Willingness To Pay) analysis model and the socioeconomic variables of the surveyor, such as sex, age, education and income, were reflected in the model. The economic benefits from the two projects, namely, building of marine ranches and developing marine forests were estimated to be equal to 4,608 won and 7,772 won per household per year, respectively. According to the results of the survey, it seems that respondents think that marine forests are more valuable than marine ranches. This is as a result of ordinary citizens' thought that the marine ranches are more cost-effective than the marine forests. The benefits estimated through this study can be used for analysis of economic feasibility prior to carrying out the project of building marine ranches and developing marine forests, and are considered to be the valuable for policy-making purposes and finding social and economic consensus.

  • Thus this study looked into existing literature focusing on generalities, and after literature studies, hypothesis was set up to solve the problems in the study. According to literature examination, self employed consulting was found to be comprised of four areas: awareness, reliability, satisfaction, and utilization, while consulting could be divided into four customer perspectives: customer, financial, internal processing, and learning and growth. An empirical study was conducted to verify the causal relationship between these causes, and we describe the findings of the study on the business management performance pursuant to self employed business consulting as follows: In this study, we examine an overall business management performance measurement by adopting the four variables of self employed business consulting, and enhance the chance of success by having systematic access to business establishment. In conclusion, in order to increase the success rate of the small business start-up, it is important to choose such items that fit the founder's experience and the characteristics of the business zone, and a successful founding of a business will be accomplished only when sufficient funding is combined with successful running, therefore, most importantly, striking a balance between the factors should start with the founder as the center, and there must be professional business knowledge and technical assistance by the business start-up support agency.

  • Recently, accumulation of technology, transfer, absorption, and commercialization is being significantly recognized as a key factor for sustainable growth of the 21st century global economy. The government established "Technology Transfer and Commercialization Promotion Act" In order that the technology developed at public research institutions can be transferred to the private sector and commercialized; the technology developed in the private sector can be traded and commercialized. Also, the Article 14 of "Technology Transfer and Commercialization Promotion Act" is concerning technology transfer agents' registration, promotion, and support: it introduced the special status system of technology transfer agents and the government registers, manages, and supervises it. In most developed countries, the technology transfer agency is transferred to the private sector and it is referred as a technology transfer agent or technology broker. In the domestic market, despite the introduction of the above special status system and the building a various organization and transfer medium system for boosting the market's revitalization, some problems occurred; because the registration system of a technology transfer agent and legal basis and system about its following-up control fall short. For example, recently technology transfer-related performance exemption has brought the activation of technology transfer agent's registration, but there was the limit of selecting the expert above a certain level. Therefore, some countermeasures for this are urgent, In addition, through a compulsory training completion system before the technology trade agent registration is prepared, a short period of curriculum was not sufficient to provide applicants various specialized knowledge. In this research, it is considered about the reform of technology transfer agent through its comparative study in China and Korea. Some improvements are suggested for expanding the market of technology transfer commercialization, assuring the agents' service and strengthening the competitiveness.

Study on Effective Utilization Strategies of the Home Health Care Program in the Community Care Service of Welfare Policy (재가 복지 봉사 사업의 가정 간호 사업연계 필요 조사)

  • This study was designed out to develop a home health care service for nurses working in community care services. This study investigates actual conditions at welfare institutions related to health needs the demands of clients, and the state of home health care services we hope that this study will improve upon the current service system. In Korea home health care services are still developing and only new becoming a part of the health care supply system. The data was collected by recording the client home nursing assessments modified to the situation of UTMB home health agency. In this study 107 clients were selected for home care who needed care for physical and mental deficits. The study lasted from March to November of 1995 at one of the welfare service institutions in Chunchon city. The results show that those who most frequently needed care services were over 50 years old with a health deficit of 80.3%, followed by sex as women who needed care at 59.8%. 50.5% of the clients had very little education. 99.1% of the clients live with their family, and a medical diagnostic analysis reveals that 73.9% of the 5 year period of illnesses were the following : 38.8% - muscular -skeleton system disorder, 24.4% - hypertension and stroke, 25.7% sole disease of arthritis. For behavioral conditions 43.3% of the patients were without care services, 56.6% of the patients were taking treatment that 73.5% of those were taking medication. The most main complaint of patients were 22.4% of pain in the extremities, next were 16.8% of a limitation of body activities, 15.0% was lumbo-sacralgia. According to the investigator who was a senior student nurse, the following suggestions were made: 32.7% for curative medical services, 29.9% for physical exercise, 19.6% for emotional support. Consultation nursing services consisted of 67.2% for physical therapy, 11.2% for the maintaining healing, 9.4% for counseling. The patients at home, required assistance most frequently for muscular-skeleton problems under the category of physical systems (33.3%). But, on the other hand, 49.5% of the patients required care givers at home, 28.2% had a knowledge deficit, 21.0% had malnutrition, 18.4% had bad impaired communication. The character of health problems were devided into chronic disease(67.0%), accidents(I3.1%), and general disease(15.9%). 86% of the disabled client had an impairment of the physical system. Eating (86.9%) , Toileting(77.6%), and personal care showed much the same of ADL condition, the level and range of achievement of mobility, the most frequently self performed was 81.3% only in a room size area, and 40.2% were completely dependent when going out. Although there were a large number of home care services in th community at these welfare institutions, many clients needed a variety of curative services. As policy changes have gathered momentum, responsibility for the development of a more suitable program was demanded by the clients from the community.

Effects of the Number of Visits and Length of Stay in Urban Forests on Subjective Well-Being - A Case Study of Seoul - (도시림의 방문회수와 체류시간이 주관적 웰빙에 미치는 영향 - 서울시를 중심으로 -)

  • The purpose of this study is to investigate if subjective well-being could be improved by visiting urban forests near residential areas. Because visiting an urban forest is not an intense positive experience, this research is focused on frequency of affective experience rather than intensity. The independent variables are number of visits and length of stay. The dependent variables are positive affect, negative affect, and life satisfaction. A polling agency was employed to select 600 respondents by quota sampling, and data was collected by online survey. The results of ANOVA showed that there was no interaction between the number of visits and length of stay. Regardless of the number of visits, the subjective well-being of visitors of urban forests was enhanced: (a) positive affect of respondents who had visited in the past 2 weeks was increased while negative affect was decreased, and (b) life satisfaction for those who had visited at least 1 time per month was enhanced among usual visitors. The stay of length, however, had little effect on the increase or decrease of these three variables. The results of this study support the existing theory that one could reset their genetically determined happiness set point to a higher level by participating in intentional activities such as visiting urban forests that offer ways to achieve long-lasting changes in well-being. This means that it would be a valuable government investment to construct and maintain urban forests for improving citizens' welfare. A few comments were suggested regarding data collection and inclusion of influencing variables to make future subjective well-being studies more reliable.

  • With the liberalization of distribution services industry since 1996 as well as changes in customers' appetite to spend and consumer behaviors, new forms of distribution services have mushroomed such as superstores and SSM(Super Supermarket), online shopping, TV shopping channels. On the contrary, traditional markets have sunk into a swamp of stagnation. As this stage of stagnation of markets negatively affects tradespeople's employment, livings, and local economy, the Korean government has conducted policies to support improvement of market facilities and management since 2004. However, the government has separated facility improvement from management improvement. As of June 2008, there are 1,550 traditional markets in Korea and among these there are 388 public markets which local governments have established and managed. Public markets have possibilities to be developed as successful models of market revitalization since they can get all-expenses-paid supports for facility improvement, control of merchandise assortment, educational program for merchants. Such successful (strategic hub) markets could become the benchmark for other neighboring markets and tradespeople which could lead other successful cases. In order to produce such effects, the market should have optimized facilities, merchandise and services, co-marketing strategies with nearby markets, group purchase strategies. The hub market could play a critical role in distribution of local goods and developing high value products.

  • During the 1990s the workplace has grown more complex and business competition has increased world-wide. All organizations, whether for-profit or non-profit have been forced to respond to market changes. More advanced information and technology, greater product diversity, shorter product life cycles, increased quality requirements, more regulation oversight, decreasing productivity, more competitors, and increasing overhead costs have motivated organizations to focus on ways to deliver products cheaper, better, and faster. Many organizations are searching for ways to reduce costs through downsizing, reengineering business processes, implementing quality management, outsourcing, and improving cost management. Support departments that provide services internal to an organization such as human resources, legal, and environmental, safety, and health (ES&H) are often the first organization targeted for cost reduction and cost control initiatives because these functions are part of a rapidly increasing overhead cost. Recently, ES&H functions are incresingly being integrated into the business of business to contribute value to organization beyond mere compliance with ES&H regulations. The discussions and development of the ISO compatible Environmental Management Standards or Occupational Safety and Health Management Standards is another impetus to integrate ES&H function into the business of business. Thus, ES&H professional need new skills to analyze the cost of their function and communicate the value of the products and services they provide. In recent years, the need for and the importance developing cost management and business skills by ES&H professionals have been emphasized in the literature. Communicating with decision makers in terms of cost and value to the organization, and by using business language and business arguments is the first step toward effectively integrating ES&H activities into the business of business. Activity-based costing (ABC) is a cost management method that measures the cost of a product or service based on the actual use of resources by activities, and based on the actual amount of activities used to produce a product or service. ABC is recommended as a tool for managers of ES&H organizations to determine the cost of developing and providing ES&H products within a for-profit firm or non-profit agency. This paper discusses the trend of integration of ES&H functions into the mainstream of business activities within an organization. The general principles of treditional cost accounting are presented as a bases for understandging why and how ABC will provide more accurate estimates of cost. The principles and concepts of ABS are presented as a tool for determining more accurately the true cost of ES&H products and services.

Spatio-temporal Analysis of Freeway Emissions for Establishing Public Health Policies Based on Transportation (교통기반 공공보건 정책 수립을 위한 고속도로 차량배출가스 시공간 패턴분석)

  • Vehicle emissions have been known as a critical factor to give a negative impact on the public health. In particular, particulate matters(PM) and NOx are highly related with respiratory diseases such as asthma. This study aimed at analyzing spatio-temporal patterns of PM and NOx generated from urban freeway traffic. MOVES, which is a well-known emission analysis tool presented by US Environmental Protection Agency(EPA), was applied to estimate PM and NOx based on traffic volume and speed data obtained from Seoul Outer Ring Expressway during January~June, 2012. K-means clustering analysis was used for categorizing the Level of Vehicle Emissions(LOVE) to support more systematical identification of the significance of emissions. Then, spatio-temporal analyses of estimated emissions were conducted by LOVE. Finally, this study proposed a set of strategies to reduce both PM and NOx to enhance public health based on analysis results.

A study on Hepatitis B and knowledge about AIDS of dental hygiene student in Korea (전국 치위생과 학생들의 B형간염과 AIDS에 관한 지식도 조사)

  • The purpose of this was to evaluate the level of knowledge about Hepatitis B and AIDS among dental hygiene students at six dental hygiene educational programs in nationwide, compare the findings to those of Song's study. 1703 dental hygiene students surveyed by mail and answered a questionnaire, using the questionnaire used developed by DiClemente el al., on Hepatitis B vaccination, antibody production, experience of unintentional needle stick injuries, experience of surgical operation and blood transfusion, personal risk factors for HBV and HIV, management of HBV and HIV postexposure management et al. The data indicated that dental hygiene students (79.0%) in this study had more HIV vaccination than dental hygiene(74.3%) in Song's study. As for recognition of antibody production after Hepatitis B vaccination, only 34.6% of respondents answered they knew having antibody production and 95% of respondents knew not having antibody production. Over one-half of the respondents (55%) answered they didn't even recognize having antibody production. 52.3 percent of dental hygiene students experienced unintentional needle stick injuries. The findings on the knowledge about Hepatitis B and AIDS were that dental hygienists (76.5 points) received higher mean score than those of dental hygiene students (71.0 points) and that the distribution of right answer rate also showed wider range in dental hygiene students (27.7~97.5%) than those of dental hygienist (41.2~99.5%). There were no statistical differences among dental hygiene education programs. Finding of this study support that the curriculum of dental hygiene program should include instruction on sources and methods of transmission of infectious diseases, risk of virus transmission in the workplace and principles of infection control. Furthermore, infection-control practices should be utilized routinely and their application to dental personnel, management of HBV and HIV postexposure management. Furthermore, for preventing the further spread of infectious diseases (HBV, HIV and AIDS${\cdots}$) caused by bloodborne viruses, imposing an obligation rather than recommendation on Hepatitis B vaccination to all dental personnel and routinely utilizing infection-control guidelines for all dental patients in dental practices (include dental educational programs) should be accomplished by coordination the government agency.
