  • Objectives: This study examined differences in health care spending and characteristics among older adults in Korea by high-cost status (persistently, transiently, and never high-cost). Methods: We identified 1 364 119 older adults using data from the Korean National Insurance Claims Database for 2017-2019. Outcomes included average annual total health care spending and high-cost status for 2017-2019. Linear regression was used to estimate differences in the outcomes while adjusting for individual-level characteristics. Results: Persistently and transiently high-cost older adults had higher health care spending than never high-cost older adults, but the difference in health care spending was greater among persistently high-cost older adults than among transiently high-cost older adults (US$20 437 vs. 5486). Despite demographic and socioeconomic differences between transiently high-cost and never high-cost older adults, the presence of comorbid conditions remained the most significant factor. However, there were no or small differences in the prevalence of comorbid conditions between persistently high-cost and transiently high-cost older adults. Rather, notable differences were observed in socioeconomic status, including disability and receipt of Medical Aid. Conclusions: Medical risk factors contribute to high health care spending to some extent, but social risk factors may be a source of persistent high-cost status among older adults in Korea.

  • Purpose: The purpose of this study was to explore how older adults kept their health good at a doctorless farm village. Method: Data was collected through in-depth interviews with 32 participants who were over 65 years old and analyzed in terms of Strauss and Corbin's (1990) grounded theory methodology. Result: The Core Category of health care of older adults was identified as 'enduring physical changes by themselves'. The process of this could be divided into 4 stages: the stage of bearing, the stage of managing daily living activities, the stage of passively collecting information and the stage of minimally utilizing health care services. Older adults accepted the aging process positively but health sources limitation passively, so they managed daily living activities and used natural food for health. In addition, they collected information related to health care and used health care services minimally. Conclusion: We found that participants managed their health passively because of negative attitudes toward active health behaviors of older adults by themselves and the difficulty of access to health care services. Therefore, various community health services for older adults need to be developed to empower older adults in the community.

  • Purpose: This descriptive survey-based study aimed to identify the factors associated with obese adults' quality of life (QOL) using a secondary analysis of data collected through the 8th Korea National Health and Nutrition Examination Survey (2019). Methods: In this study, factors affecting the health-related quality of life of obese adults were analyzed by categorizing them into three independent variables: general characteristics, health behaviors, and mental health characteristics. Using t-test and analysis of variance (ANOVA), an analysis was performed to determine the differences in obese adults' QOL depending on their general characteristics, health behaviors, and mental health conditions. A hierarchical regression analysis was performed to identify the factors that affect obese adults' QOL. Results: In obese adults' general characteristics, the risk factors for QOL were older age, male, lower-income level, presence of spouse, and occupation. With regard to health behaviors, the risk factors were lack of exercise. In terms of mental health conditions, the risk factors were stress and suicidal ideation. Conclusion: Obese adults' QOL was affected by age, sex, income level, absence/presence of a spouse, occupation, health behaviors (e.g., exercise), and mental health conditions (e.g., stress and suicidal ideation).

  • This study was done to suggest directions for research and interventions of health promoting behaviors in Korean older adults in the future. Thirty seven articles for health promoting behaviors in Korean older adults were reviewed and analyzed. Findings are summarized as follows: 1) The total scores of the HPLP in Korean older adults were 2.30-2.44 out of 4 points. In the subscale, the highest degree of performance is nutrition, following interpersonal support, self actualization, stress management, health responsibility and the lowest degree of performance was exercise. 2) The total scores of the Health Behavior Assessment Tool of the Korean Elders were 2.87-3.2 out of 4 points. 3) Among the characteristics of older adults, monthly pocket money, previous job had consistently significant relationships with health promoting behaviors. Sex, job and presence of disease were consistently insignificant relationships with health promoting behaviors. 4) Perceived health status, self efficacy, self esteem, family support and social support had consistently significant correlations with health promoting behaviors. 5) In regression analysis, self efficacy, family support, depression, self esteem were the most powerful predictors of health promoting behavior in more than two articles. Predictors accounted for 14.2-65.2 % of the variance in health promoting behaviors of Korean older adults. On the basis of above findings, It is necessary to develop the interventions for more regular practice of the health promoting behaviors in Korean older adults. The interventions are recommended to focus increasing the exercise & health responsibility and to use the strategies to increase self esteem, self efficacy, social support including family support.

  • Purpose: The purpose of this study is to identify personal factors, social factors, and environmental factors related to physical activity in older adults in urban and non-urban areas. Methods: We used source data from the 2017 Community Health Survey. The subjects of this study included some older adults aged 65 and over, and analyzed the data of 23,043 older adults living in the urban and 34,063 older adults living in the non-urban area. Results: The common factors influencing physical activity in older adults by region include current smoking and drinking, BMI, sleep duration, and subjective health status, help with neighbors, frequency of meeting with neighbors and friends, participation in social and leisure activities, and falls experience (p<.001). However, the living environment, public transport satisfaction, and medical service use significantly associated with physical activity for only older adults living in the urban area (p<.001). Conclusion: In order to improve physical activity in older adults in the community, it is necessary to consider not only the improvement of individual factors that practice health behaviors but also health promotion strategies that take into account social and environmental factors because there are environmental differences among regions.
