The Relations among Self-care, Symptom Experiences, and Disease Status of the Patient with Liver Cirrhosis (간경변증 환자의 자가간호수행과 증상경험 및 질병상태와의 관계)

  • Purpose: The purpose of this study was to examine the degree of self-care, symptom experience and disease state among the patients with liver cirrhosis and to identify its correlation. Methods: The subjects were 220 patients with liver cirrhosis who were treated in D-university hospital in Busan. The instruments used for study were the self-care inventory, the symptom experience inventory and modified childpugh classification scale. Results: The average points of the patient's self-care performance, symptom experience, and disease status were 3.67, 29.10 and 6.12 respectively. Their self care was negatively correlated to symptom experience, and symptom experience was positively correlated to disease state. Conclusion: This research findings suggested that active self-care by patients with liver cirrhosis might contribute to reduce various symptoms they can undergo and to prevent aggravation of disease state. Therefore, patients with liver cirrhosis have to carry out self care actively in daily life and discreet nursing intervention should be offered to facilitate their self-care.

The Effect of Supportive Nursing Care on the Knowledge Level and Compliance of Sick-Role Behavior in Patients with Coronary Artery Disease after Coronary Angiography (지지간호가 관상동맥조영술을 받은 관상동맥질환자의 질병관련 지식과 환자역할행위 이행에 미치는 효과)

  • Purpose: The purpose of this study was to determine the effect of supportive nursing care on the level of knowledge and compliance with sick-role behavior in patients with coronary artery disease after coronary angiography (CAG). Methods: A quasi-experimental research was performed with 81 subjects with coronary artery disease who were admitted for CAG to a cardiovascular department. Among the selected subjects, 40 of them were assigned to experimental group and the rest of them were assigned to control group by convenience. Supportive nursing care was implemented twice by a researcher. Results: 1. The mean score of knowledge related to coronary artery disease was higher in the group who received supportive nursing care than that of the control group (t=2.259, p=.027). 2. The mean score of compliance of sick role behavior was higher in the group who received supportive nursing care than that of the control group (t=4.580, p=.001). Conclusion: The supportive nursing care after CAG was effective in increasing the knowledge level and compliance of sick-role behavior in patients with coronary artery disease. Further studies would be recommended to identify long-term effectiveness of supportive care on patients' outcomes.

  • The purpose of this study was to propose the Guidelines for the Program and Space of Adult Day Care Center. Recently the Adult Day Care Center is extremely on demand caused by the increasing population of the elderly. In order to provide the affordable and efficient rehabilitation program the architectural setting should be turned to that purpose. In particular, the adult day center program must have the programs for three kinds of the elderly in terms of health status, for example, Altzheimer's disease, paralysis, and sickly elderly. Therefore this study propose the way of planning the program and spaces to give the appropriate care for Adult Day Care Center.

The Occurrance of Velogenic Viscerotropic Newcastle Disease Virus in an Adult Peacock (성숙 공작(Pavo cristatus)에서 발생한 내장 친화형 뉴캣슬병 바이러스 강독주)

  • A two-year-old male peacock (Pavo cristatus) showed acute watery green diarrhea, followed by neurological signs including torticollis and muscular tremor. By the hemagglutination inhibition test for detecting the antibody against the Newcastle disease virus (NDV), the peacock serum inhibited the agglutination of chicken red blood cells. Grossly distinctive hemorrhagic lesions were found in the mucosa of proventiculus and intestine and lung. The spleen revealed multiple variable sized necrotic foci. Histologically, the mucosa of gastrointestinal track had hemorrhagic lesions and some of them underwent ulceration. The spleen exhibited multiple variable sized necrotic foci in which fibrin exudation was marked. Central nervous system had mild non-suppulative menin-goencephalitis consisting of vasculitis, perivascular hemorrhage, gliosis and meningitis. The cells particularly in the cerebellum were degenerative to necrotic. Some of these nerve cells revealed characteristic peripheral chromatolysis. From the present serological and pathological findings, it is suggested that NDV causing death of peacock was velogenic viscerotropic strain. This is the first report of the occurrence of velogenic viscerotropic NDV in an adult peacock in Korea.

Rapid Progression of Cerebral Infarction after Intraventricular Hemorrhage in Adult Moyamoya Disease

  • The authors present a rare case of adult moyamoya disease in which a patient experienced rapid progression of cerebral infarction after intraventricular hemorrhage (IVH). A healthy 39-year-old woman was admitted to our hospital with sudden headache, a decreased level of consciousness and mild tetraparesis. Initial magnetic resonance imaging revealed small cerebral infarction and IVH. Although the patient underwent conservative therapy including hypervolemia, hemodilution, keeping moderate hypertension and administration of a free radical scavenger, she showed a fulminant clinical course of cerebral infarction. The authors discuss the possible pathophysiology and suggest the treatment for such cases.

Exterior Environments for the Elderly with Dementia in the U.S.A. (치매노인을 위한 시설의 옥외공간에 관한 연구 -미국의 사례를 중심으로-)

  • The purpose of this study is to understand the importance of exterior environments and to identify the design guidelines for the therapeutic garden for cognitively impaired seniors with Alzheimer's disease and related dementias. The data was collected from 4 Adult Day Care Centers, 2 Nursing Homes, & 1 Assisted living in the U.S.A. from 2001 to 2002 by interview and the floor plan about the outdoor space. Although some may believe that patients with Alzheimer's disease and related disorders are unresponsive to environment, evidence shows that environments especially designed for cognitively impaired seniors can maintain or increase their level of functioning. The use of specially designed exterior environments nay have in reducing incidents of aggressive behavior, and contributing significantly to a wanderer management program. The checklist includes 6 dimensions: safety environment, various sensual stimulations, social interactions, wandering, privacy, and support orientation and reality. The results indicated that the exterior environment have to support wandering behavior, orientation and reality.

  • God created the havens and earth and god gave man every seed-bearing plant and every tree that has fruit with seed in it. These food Is all vegetable food that don't take diseases of adult people. But God gave Noah the green plants. Everything that lives and moves will be food for Noah. Just as God gave man the green plants. And then man must not eat meat (animal protein food) that has its lifeblood still in it and God must not eat fat in it. The fat contains much fat (saturated fatty acids) and cholesterol that have susceptibility to disease of coronary heart, hypertension and atheriosclerosis etc. God must not eat these fat before we don't know that It have susceptibility to disease of adult people.

Status and Need of Health Education of Residents in Rural Area (농촌지역 주민의 보건교육에 관한 실태)

  • The objectives of this study were to find the status of health education of residents. and to analyze the status of knowledge. attitude. and practice about adult disease. especially about hypertension. diabetes mullitus, cancer and health care system etc. and to find the relation between the factors and knowledge status. attitude and practice. The survey was carried out for half month from July 1 to 15. 1997. The subjects were 283 persons in the rural areas that were chosen from one country. Muan Gun near Mokpo City. To interview the rural residents, 5 volunteer interviewers were recruited from university students whose major is nursing, and they were trained about the questionnaire. The percentage of the acceptance of health education are $1.1\%-9.2\%$ of the subjects that the degree of the percentage of the health education was highest in diabetes mellitus. and the next were hypertension. cancer. joint disease. CVA. Therefore the need of the health education was very high. Of the subjects. $13.4\%-60.8\%$ wanted to receive the health education about each disease. Highest proportion of the subjects indicated hypertension and joint disease. Of the subjects $42.1\%-6.7\%$ knew each disease. The degree of knowledge was highest in cancer. followed by T.B., D.M., and hypertension. Of the subjects. $58.5\%$ practiced after receiving health education. and $47.3\%$ were getting periodic health examination during the last one year. Of the subjects. $76.1\%-94.4\%$ did not practice preventive methods such as low-salt diet. periodic health examination and exercise etc.. The knowledge and attitude. and practice of the health were more active in male persons and in highly educated persons significantly than their counterparts. As the adult diseases now are more prevalent than before. more attention should be put on health education to prevent adult diseases in the dimension of developed practice method.

Risk Factors for the Prevalence of Periodontal Diseases among Adult Workers (성인 근로자의 치주질환 유병 관련 위험요인)

  • This study examined the demographic and oral behavioral risk factors for periodontal disease in adult workers. The research subjects and method were conducted targeting 1,650 Korean adult workers aged 20-64 utilizing the data from the fifth Korea National Health and Nutrition Examination Survey (KNHANES V-3). As a result of this study, the periodontal tissue patients showed the risk factors for gender, age, income level, educational level, systemic disease, sleep hours, oral health level, biting trouble, and chewing difficulty. In terms of the greatest influence, the workers with problems with biting and chewing had a more than 3.5 times higher likelihood of being affected by periodontal disease. The findings of this study show that there are a variety of risk factors for periodontal diseases in Korean workers. To reduce the prevalence rate of periodontal disease, regular checkups including dental checkups and sufficient sleeping will be necessary, and the level of stress should be decreased. In addition, the government should take the required measures to remove the socioeconomic inequality, such as income gap or educational divide.

Radiographic diagnosis of canine heartworm disease without clinical signs (임상증세를 보이지 않는 개심장사상충증의 방사선학적 소견)

  • Dirofilaria immitis, the causative agent of canine heartworm disease, inhabits mainly in the pulmonary arteries and the heart of dogs and cats, causing circulatory and respiratory disorders. Although diagnosis of the disease is based on the presence of microfilaria in the peripheral blood or of specific antigens released from the adult worms into the peripheral blood, the severity of the infection and the assessment of disease progression are based on the clinical signs and radiographic image analysis. We analysed 12 mixed-bred Jindo dogs naturally infected with Dirofilaria immitis without any clinical signs and compared the radiographic images of the heart and the lung with the number of adult worms at necropsy. The dorsoventral radiographs of 12 infected dogs revealed that the right caudal lobar pulmonary arteries (RCaLPA) were dialated in 66.7% of dogs, whereas the main pulmonary artery segment was enlarged in 50.3% of dogs. The cranial lobar pulmonary arteries (RCrLPA) were dilated or pruned in 33.3% of dogs. All dogs displayed the interstitial lung pattern, while the vertebral heart size (VHS) was of normal range. Although a range of 9 to 166 adult worms (av. 45.6) was found in the pulmonary arteries, in the heart or in the vena cava, no correlation with the radiographic findings and/or with the clinical signs was observed.
