건강한 성인에서 감염성 질환 후 발생한 구개누공 치험례 (Palatal Fistula of a Healthy Adult after an Infectious Disease: A Case Report)

  • Purpose: In rare cases, infectious disease can become the cause of palatal fistula as it usually occurs in immunocompromised patients. We are reporting this case because we have successfully reconstructed palatal fistula occurring due to infectious disease in a healthy adult. Methods: A 62-year-old female patient had visited our hospital with a palatal hole that had occurred after suffering from a severe febrile disease. An oro-nasal fistula measuring $1.5{\times}1.3cm$ has been observed in the junction of the soft palate and hard palate, and is presumed to be caused by an infectious disease according to the patient's history. It was reconstructed using a hinge flap and a mucoperiosteal flap. Results: For 6 months, no recurrence or complications had been observed. The post operative patient was satisfied with the improvement in nasal speech and fluid regurgitation. Conclusion: We reported this case since we had obtained a functionally satisfactory result by reconstructing the palatal fistula due to infectious disease in a healthy adult.

  • Adult-onset Still's disease (AOSD) is an inflammatory disorder that's characterized by daily, spiking high fever, arthritis and an evanescent, salmon-pink rash. AOSD is diagnosed purely on the basis of the typical clinical features of the illness. The symptoms commonly include swelling of the lymph nodes, enlargement of the spleen and liver, and a sore throat. AOSD is difficult to differentiate from systemic lupus erythematosus (SLE) due to the similar clinical manifestations. We report here on a case of a 16-year-old female patient with autism and epilepsy and who complained of daily spiking fever for 20 days. The patient had maculopapular skin rashes on the face and whole body and lymphadenopathy. The liver function tests were elevated mildly. The initial rheumatoid factor (RF) and antinuclear antibody (ANA) tests were negative. We diagnosed her as having adult-onset Still's disease according to the criteria of Yamaguchi. We successfully treated her with oral prednisolone. But her antinuclear antibody test was changed to positive after discharge. So we finally diagnosed her as having SLE.

성인 척추질환자의 발균형 및 보행형태에 대한 운동역학적 분석 (Kinetic Analysis of Foot Balance and Gait Patterns in Patients with Adult Spinal Disease)

  • Objective: The aim of this study was to provide kinematic data on the characteristics of spinal disease patients by comparing and analyzing kinematic variables related to foot balance and gait pattern of spinal disease. Method: The subjects of the study included 40 adult men and 60 adult women who visited the hospital in Busan. Patients who were diagnosed with spinal disease by a physician through X-ray examination were selected as subjects for the diagnosis of vertebral disc herniation, spinal stenosis, spinal disease diagnosed with spinal disease and the general public. Left and right foot pressure and contact area were checked by Gaitview pro meter. X-ray photographs were taken with a Zen-2090 mobile fluoroscopy under physicians' direct participation. One-way ANOVA was performed to compare the differences between the kinematic variables and post-hoc was performed by the Duncan method. Results: The difference in contact area between the left foot and the right foot was $115.30{\pm}14.15cm^2$ in the left side and $124.25{\pm}13.65cm^2$ in the left side in the spinal disease patients. The difference in pressure between the left and right side of the spinal disease patients was wider than that of the general people. Especially, the right side of the spinal disease patients showed a larger area of left foot contact than the general population. Conclusion: Spinal disease patients have wider contact area of the left foot than those of the general population. In the case of right spinal disease, the left foot support area is widened due to pain. In the gait, women showed slightly more posterior body center than men, and the upper body muscle imbalance and immobilization due to the spinal disease caused imbalance of the muscles moving to the lower limb, It was analyzed to inhibit movement.

황색육아종과 동반된 면역글로불린G4관련안질환 1예 (IgG4-related Ophthalmic Disease Associated with Adult Xanthogranulomatous Disease)

  • 목적: 기존 안와 황색육아종이 진단되었던 환자에서 재발된 피부 황색육아종을 통하여 면역글로불린G4관련안질환이 동반 진단된 증례를 보고하고자 한다. 증례요약: 양안의 눈꺼풀 부종을 주소로 38세 남성이 내원하였다. 전안부 및 후극부에서 특이 소견이 관찰되지 않았다. 조직검사상 포말조직구 및 토우튼 거대세포가 관찰되며 S100 및 CD1 염색에 음성 소견을 보여 성인황색육아종 진단하 경구 아자씨오프린과 프레드리솔론 치료를 받았다. 4년 뒤 양안의 노란색 눈꺼풀 종괴를 주소로 재내원하였다. 자기공명영상에서 양안 안와 상외측의 대칭적인 연부조직 부종이 관찰되었고, 혈액검사상 immunoglobulin G (IgG) 및 IgG4 수치가 상승되었다. 조직검사 후 면역화학염색에서 IgG4+/IgG+ 10-20%, IgG4+ 22/고배율시야가 확인되었으며 과거 조직을 재염색하여 IgG4+/IgG+ 40-50%, IgG4+ 59/고배율시야를 확인하여 전형적인 면역글로불린G4관련안질환으로 새롭게 진단하였다. 결론: 비특이적인 눈꺼풀 부종환자에서 면역글로불린G4관련안질환을 고려해야 하며, 기존 황색육아종 환자에서도 면역글로불린G4관련안질환이 동반될 수 있다.

Association between Thioridazine Use and Cancer Risk in Adult Patients with Schizophrenia-A Population-Based Study

  • Objective Several cell line studies have demonstrated thioridazine's anticancer, multidrug resistance-reversing and apoptosis-inducing properties in various tumors. We conducted this nationwide population-based study to investigate the association between thioridazine use and cancer risk among adult patients with schizophrenia. Methods Based on the Psychiatric Inpatient Medical Claim of the National Health Insurance Research Database of Taiwan, a total of 185,689 insured psychiatric patients during 2000 to 2005 were identified. After excluding patients with prior history of schizophrenia, only 42,273 newly diagnosed patients were included. Among them, 1,631 patients ever receiving thioridazine for more than 30 days within 6 months were selected and paired with 6,256 randomly selected non-thioridazine controls. These patients were traced till 2012/12/31 to see if they have any malignancy. Results The incidence rates of hypertension and cerebrovascular disease were higher among cases than among matched controls. The incidence of hyperlipidemia, coronary artery disease and chronic pulmonary disease did not differ between the two groups. By using Cox proportional hazard model for cancer incidence, the crude hazard ratio was significantly higher in age, hypertension, hyperlipidemia, cerebrovascular disease, coronary artery disease and chronic pulmornary disease. However, after adjusting for other covariates, only age and hypertension remained significant. Thioridazine use in adult patients with schizophrenia had no significant association with cancer. Conclusion Despite our finding that thioridazine use had no prevention in cancer in adult patients with schizophrenia. Based on the biological activity, thioridazine is a potential anticancer drug and further investigation in human with cancer is warranted.

만성 심혈관질환자의 삶의 질 의미 (Quality of Life in Patients with Chronic Cardiovascular Disease)

  • Purpose: The purpose of the present study was to explore the meaning of quality of life in patients with chronic cardiovascular disease. Methods: A grounded theory method guided data collection and analysis. A total of 16 adult outpatients with chronic cardiovascular disease was participated. Data were collected through individual in-depth interviews. All interviews were audio taped and transcribed verbatim. Coding was used to establish different concepts and categories. A theoretical sampling technique was used to obtain diverse data from many relevant categories. Results: Seven categories were extracted, and they divided into constructing and intervening factors of quality of life. Constructing factors were uncertainty, recovery in the sense of control and maintaining social life. Intervening factors were symptom experiences, social support, taking care of themselves, and reflecting life. 'Keeping restrictive conditions under control' was emerged as a core category. The meaning of quality of life in patients with cardiovascular was explained according to the levels of keeping restrictive conditions under control. Conclusion: The result of this study may contribute for health professionals to understand the quality of life in patients with cardiovascular disease.

A Long Way from Transfer to Transition: Challenges for Pediatric and Adult Nephrologists

  • Significant advances in the diagnosis and medical care of children with chronic kidney disease (CKD) are major reasons for the better survival rates of children and adolescents with CKD than the survival rates reported in previous decades. These patients are reaching adulthood, and therefore require a transition to adult medical care. This transition phase is well-recognized to be associated with considerably increased morbidities and medical problems, such as non-adherence, graft loss after transplantation, and loss to follow-up. Low adherence increases morbidity and medical complications and contributes to poorer qualities of life and an overuse of the health care system. However, these tragic outcomes may be avoidable through a structured and well-defined transition program. In the last decade, there has been increasing interest to resolve these medical and psychological problems that occur during the transfer of young adult patients from pediatric to adult renal units. The aims of a successful transition from pediatric to adult medical care include enhancing the individual development of better health-competence and stabilizing, or even improving, the state of health. This review will focus on various aspects of the transition phase of adolescents who have CKD or who underwent kidney transplantation from pediatric to adult nephrology care.

파킨슨 환자의 보행에 관한 연구 (The Research of Gait on Parkinson's Disease)

  • Purpose:To investigate of gait component in Parkinson's Disease patient. Methods:participated Parkinson's Disease patient(n=12) and Normal adult(n=13). gait measure used by GaitRite. Results:SPSS for win version 12 was used for statistic analysis and independent t-test used to find between two groups. In the comparison of temporal parameter of gait between groups, the swing phase was significant decreased in Parkinson's groups, in the stance phase was significant increased in Normal groups, in the single support was significant decreased in Parkinson's groups and in the double support was significant increased in Parkinson's groups(p<.05). In the asymmetrical ratio of singele support was significant increased in Parkinson's groups(p<.05), and the swing phase and stance phase was significant increased in Parkinson's groups(p<.05). Conclusion:In the Parkinson's Disease patient gait showed temporal and spatial component variable changes comparison normal adult. therefore, it was seems to very important considerable at gait tranning in clinical intervention.

  • This thesis is a study to develop the tentative objectives and present the professional and courses for the nursing college. The conclusions conducted by the purpose of this study are as follows. First, on the basis of the concepts of the nursing and the view-points of nursing education, the tentative goals for the nursing education are set up as follows. 1. To understand human being's life, dignity and their physical, mental, and social aspects. 2.
