The Effect of Sensory Diet Home Program for Children with Autism Spectrum Disorder Who Have Sensory Defensiveness: Case Report (감각방어를 가진 자폐 범주성 장애 아동을 대상으로 한 감각식이 가정프로그램이 감각조절과 사회적 능력에 미치는 영향: 사례보고)

  • Objective : The purpose of this study was to investigate the effects of sensory diet home program for children with autism spectrum disorder who have sensory defensiveness and explore how this intervention affect to sensory modulation and social interaction ability. Methods : The subject of this case study was a 12-year-old girl with autism spectrum disorder who was struggling with her sensory modulation problem. Parent interview and evaluations were conducted and subject-tailored sensory diet home program was made by the therapist. Parent education for the home program was provided to parents and everyday, 3 times, 30minutes of home program activities were conducted to the child for 23 days. Adolescent/Adult Sensory Profile and Social Maturity Scale were measured at pre and post intervention and parent wrote daily observation record about child during the intervention. Results : After the intervention, Low Registration, Sensory Sensitivity, Sensation Avoiding area of Adolescent/Adult Sensory Profile included to the normal range and the scores of the Social Maturity Scale improved. Conclusions : Child's sensory defensiveness was relieved and sensory modulation and controling emotion ability were improved. While there was significant improvement for sensory modulation and social interaction ability, this was a case study and there was only one subject. So we need to study with larger sample size to generalize this intervention's effectiveness.

Effect of Vitamin C Supplementation on Iron Status of Adolescent Girls with Low Hemoglobin Levels

  • This study investigated the effects of vitamin C supplementation on the nutritional iron status of 31 adolescent girls, aged 12-15 years, with low hemoglobin levels. They were randomly divided into four groups, and for two groups daily150mg or 900mg of L-ascorbic acid(AsA) was given in three equal doses at three meals during 9 weeks. To another group daily 60mg iron as ferrous sulfate was given in the same way as AsA. The control group was given sugar placebo. Body iron status was monitored through the determination of Hb, Hct, MCHC, and serum ferritin concentrations. Dietary AsA and iron intakes were measured from food consumption surveys performed by 3-day 24-hour recalls. The amount of absorbed iron was estimated from the model of Monsen et al. The average amounts of food iron for four groups were 12.3- 15.0mg and 11.1 - 18.9mg at initial and at final period of the supplementation trial, respectively. The tentatively estimated amount of absorbed iron was significantly increased in the 900mg AsA and iron supplementing groups, but not in the 150mg AsA and placebo groups. Both Hb and MCHC were improved to above normal levels in all groups except the placebo group. Hct was elevated only in the AsA 900mg group whose Hct was relatively lower than the other groups. Serum ferritin concentrations of the four groups, which were as low as 8.50 - 14.39ng/mL on average at the intial periods, augmented significantly to 20.18ng/mL and 26.63ng/mL in the 900mg AsA and iron groups, respectively. Serum ferritin was not elevated in either the AsA 300mg group or the placebo group. The above data indicated that the daily supplementaion of 150mg AsA to the meals containing 12-15mg iron per day promoted Hb levels of adolescent girls with low Hb, and the 900mg AsA supplementing improved not only Hb level but also body iron store. A supplementation of 60mg iron per day appeared to be slightly more effective in improving the iron status in comparison to the 900mg AsA supplement. Current State of Dietary Life Education through Rural Activities for Adolescent (청소년의 농촌체험활동을 통한 식생활교육 현황)

  • This study intends to suggest basic data for the activation of rural activities in dietary life education, through a survey of adolescents on rural activities in dietary life education. It was found that 63.2% of subject students had the experience of rural activities; and that as many as 35.2% among the students who experienced rural activities participated in the activities 5 times or more. And the survey showed that rural activities were led most frequently by a teacher in charge (37.9%), and rather rarely by a nutrition teacher (15.4%) or an expert in dietary life education (7.7%). As for food ingredients that adolescents experienced during rural activities, they were composed of easily available good cookers that are common in the Korean diet, and, among them, vegetables were experienced most, followed by cereals and fruits. On the other hand, as for food ingredients preferred by adolescents, it was found that boy students preferred meat, while girl students preferred fruits. And as for improvements in dietary life, it was found that making efforts to have good dietary habits had the highest score, which shows that rural activities contributed to the improvement in adolescents' dietary life. Therefore, given the fact that rural activities go some way towards improving adolescents' dietary life, it is advisable that more opportunities for experiencing rural activities be provided to adolescents in the future so as to help them have good dietary habits.

Effects of Behavior Modification and Aerobic Exercise on the Degree of Obesity, Eating Behavior, Depression and Self Esteem in Obese Adolescent Girls (행동수정프로그램과 에어로빅운동이 비만여중생의 비만도, 식이행동, 우울 및 자아존중감에 미치는 효과)

  • This study was designed to investigate the effects of behavior medication and aerobic exercise on the degree of obesity, eating behavior, depression and self esteem among adolescent girls who were overweight and obese. Forty seven subjects were selected from one girl's middle school located in Seoul. Seventeen girls received the behavior modification program by joining 60-90 minute group session weekly. Behavior modification, in this study, consisted of self-monitoring, changing patterns of life style and attitudes, teaching nutrition and physical activities, and asking personal help. The aerobic exercise program was given to 16 girls 3 times per week with 60 minutes each time. Forteen girls received both behavior modification and aerobic exercise. These interventions continued for 8 weeks. Changes after intervention were examined twice after 4 weeks and 8 weeks for degree of obesity and once after 8 weeks only for eating behavior, depression and self esteem. The results were as follows ; 1. Degree of obesity was significantly decreased after 4 weeks and 8 weeks in 3 groups. Degree of obesity after 4 weeks and 8 weeks were not significantly different among 3 groups. 2. The mean score of eating behavior related to obesity decresed significantly after intervention in the behavior modification group only. There was significant difference in eating behavior among 3 groups after intervention. 3. The behavior modification group showed no significant changes in depression and self esteem after intervention. In the aerobic exercise group and group who participated in both behavior modification and aerobic exercise, the level of depression decreased significantly and the level of self esteem increased significantly. There were no significant differences in the level of depression and self esteem among 3 groups after intervention. These findings indicate that behavior modification might be effective in decreasing degree of obesity and eating behavior related to obesity, aerobic exercise be effective in decreasing degree of obesity and the level of depression, and also in increasing the level of self esteem.

  • When children exhibit unusually intense or persistent sexual interests or behavior, this is an indication that they have been exposed to more than the expectable amount of sexual experience, Although sexualized behavior and/or statement is a symptom of sexual abuse, it is not necessarily an indication of sexual abuse. The possibility of false allegation needs to be considered, particularly if allegations are coming from the parent rather than a child, if parents are engaged in dispute over custody or visitation, and/or if the child is a preschooler. False allegations may arise in other situations as well, such as the misinterpretation of a child's statement or behavior by relative or caretakers. And children may make false statement in psychiatric evaluations. A case of false allegation of sexual abuse made by mentally retarded 10 year-old girl who has been located in an institute is presented. During her evaluation process, it was clinician's diagnostic impression that she had no been sexually abused but sexually stimulated in some way, and most of what she said was a fantasy lie. Its pathological mechanism is discussed in terms of pseudologia fantastica along with the review of literatures.

The Relation between Health Behavior and Asthma : Comparison of Male and Female Adolescents (청소년의 건강행태와 천식 진단과의 융합적 관련성: 성별 비교)

  • The purpose of this study was to investigate the relation between health behaviors and asthma among male and female adolescents. The data were derived from the 12th Korea youth risk behavior web-based survey 2016 in Korea. The subjects were 65,528 Korean adolescents. The diagnosis rate of asthma in male adolescents was 10.3% and the diagnosis rate of asthma in female adolescents was 7.8%. In male adolescent, health behaviors that related to diagnosis of asthma were drinking (OR=1.087), second-hand smoke exposure (OR=1.087), non-vigorous physical activity (OR=1.084), obesity (OR=1.291), insufficient sleep time (OR=1.113), and stress (OR=1.257). In female adolescents, health behaviors related to asthma were smoking (OR=1.242), second-hand smoke exposure (OR=1.111), vigorous physical activity (OR=0.899), obesity (OR=1.178), and stress (OR=1.251). The results of this study will be used as basic data to develop a health promotion program of asthma youth according to gender.

Psychotic Symptoms of Hashimoto's Encephalopathy: A Diagnostic Challenge

  • Hashimoto's encephalopathy (HE) is a rare and underdiagnosed neuropsychiatric illness. We present the case of a 17-year-old girl who was admitted to a tertiary-care psychiatric center with acute onset psychosis and fever. Her psychotic symptoms were characterized by persecutory and referential delusions, as well as tactile and visual hallucinations. Her acute behavioral disturbance warranted admission and treatment in a psychiatric setting (risperidone tablets, 3 mg/day). She had experienced an episode of fever with a unilateral visual acuity defect approximately 3 years before admission, which was resolved with treatment. Focused clinical examination revealed an enlarged thyroid, and baseline blood investigations, including thyroid function test results were normal. Abnormal laboratory investigations revealed elevated anti-thyroid peroxidase (anti-TPO) and anti-thyroglobulin (anti-TG) levels (anti-TPO of 480 IU/mL; anti-TG of 287 IU/mL). Results of other investigations for infection, including cerebrospinal fluid examination, electroencephalography, and brain magnetic resonance imaging were normal. She was diagnosed with HE and was treated with intravenous corticosteroids (methylprednisolone up to 1 g/day; tapered and discontinued after a month). The patient achieved complete remission of psychotic symptoms and normalization of the anti-thyroid antibody titers. Currently, at the seventh month of follow-up, the patient is doing well. This case highlights the fact that in the absence of well-defined clinical diagnostic criteria, a high index of suspicion is required for early diagnosis of HE. Psychiatrists need to explore for organic etiologies when dealing with acute psychiatric symptoms in a younger age group.

Cryptococcosis with Mediastinal Lymph Node and Lung Involvement in an Immunocompetent Adolescent: A Case Report (정상 면역체계를 가진 청소년에서 발생한 종격동 림프절과 폐 크립토코쿠스증 1예)

  • Cryptococcus neoformans is a fungus that primarily causes opportunistic infections in immunocompromised hosts. It can also cause various infections in immunocompetent patients. Pulmonary cryptococcosis involving the lymph node is relatively rare in immunocompetent patients. In this report, a previously healthy 17-year-old girl presented with high-grade fever and persistent cough. Chest X-ray and computed tomography (CT) scan revealed an anterior mediastinal conglomerate mass that expanded to the right supraclavicular area. Ultrasound-guided gun biopsy revealed histological evidence of cryptococcosis. Immunological screening tests did not identify immunodeficiency. She recovered completely with a combination therapy of amphotericin B and flucytosine for 2 weeks, followed by fluconazole for 8 months. The characteristics of cryptococcosis involving the mediastinal lymph node and the lung should be understood, and the possibility of cryptococcosis even in immunocompetent hosts should be considered.

Carpal Tunnel Syndrome Caused by Persistent Median Artery and Bifid Median Nerve in an Adolescent (청소년에서 정중동맥과 이분정중신경에 의해 발생한 수근관 증후군)

  • Carpal tunnel syndrome is rare in children. When it does occur in children, the most common causes reported are mucopolysaccharidosis and mucolipidosis. The median artery is a transitory vessel that develops from the axillary artery in early embryonic life and does not normally survive until postfetal life. In a small percentage of individuals, however, it persists into adulthood and is frequently accompanied by a bifid median nerve. A persistent median artery can be a cause of carpal tunnel syndrome in adults, but it is extremely rare in children and adolescents. This paper reports a case of a carpal tunnel syndrome caused by a persistent median artery and bifid median nerve in a 13-year-old girl.

Septic Arthritis of the Manubriosternal Joint in an Adolescent: A Case Report

  • Septic arthritis of the manubriosternal joint is a rare condition, especially in adolescents, who often present with nonspecific symptoms that can mimic more common conditions such as musculoskeletal chest pain, costochondritis. Here, we report a case of septic arthritis in a 17-year-old girl and highlight the challenges in diagnosing and managing this condition in adolescents. Initially presenting with acute chest pain diagnosed as transient nonspecific chest pain, the patient's subsequent visits to the emergency department unveiled escalating symptoms, including high fever, prompting advanced imaging. Ultimately, the diagnosis of septic arthritis of the manubriosternal joint was confirmed, with blood culture growth revealing Methicillin-sensitive Staphylococcus aureus. Diagnostic delays have been attributed to the absence of typical symptoms and patient reluctance to be hospitalized. Our case emphasizes the importance of considering rare infectious etiologies in adolescents with chest pain and emphasizes the need for heightened suspicion in unusual anatomical sites. Further research is required to elucidate the pathogenesis and risk factors associated with this condition to aid in prompt diagnosis and treatment.
