The Analysis of Self-Mutilation in Adolescence Based on the Theory of Mentalization: From Sukhvinder in the Novel 'Casual Vacancy'

  • Objectives: Adolescence involves a number of developmental processes, as well as unique psychological characteristics and behaviors. An increased rate of internet and game addictions, school violence, and suicide may either represent aspects of adolescence or a psychopathological phenomenon. There is an urgent need to develop software programs that can prevent and resolve adolescent behavioral problems. We applied the mentalization theory to interpret and find solutions for problems faced by adolescent characters in literature. Methods: In Joan Rowling's novel "Casual Vacancy," Sukhvinder is a girl with problems representative of those encountered by modern adolescents; she is a victim of bullying and engages in self-mutilation. We targeted her problematic behaviors as representative of a prementalized state. Results: Born into an upper-class English family with Pakistani origins, Sukhvinder, unlike her siblings, fails her parents' expectations. Whenever she faces a psychological crisis, she regresses into the teleological mode (the most primitive pre-mentalization stage) and regains her sense of self by cutting herself. After her friend's suicide, however, she begins to communicate with her parents and moves toward mentalization. Conclusion: By analyzing Sukhvinder's behavior, we assessed patterns of attachment, empathy, and mentalization, and identified corrective approaches for problematic behaviors. We believe that the presented interpretation may serve as a foundation for the development of models for understanding adolescent deviant behaviors.

  • 애착이 무엇이고 어떤 역사적 배경을 가지고 있으며 인생초기에 어떻게 발달하는가와 함께 그것이 후속적 발달에 주는 영향을 선행연구에 기초하여 개관하였다. 이와 함께 애착이 갖는 임상적 시사점과 임상적 적용을 위해 우선적으로 해결되어야할 문제점을 논의하였다. 애착은 인생초기에 형성되고 그것의 질적 특성은 내적 작동모델의 형태로 일생동안 지속되기 때문에 개인의 정상발달은 물론 병리적 발달에 영향을 미치지 않을 수 없다. 그러므로 이 논문은 애착의 중요성은 물론 병리적 애착의 예방과 치료를 위해 어떤 노력을 해야할 것인가에 관한 통찰을 제공할 것이다.

발달장애아동을 대상으로 낮병동 치료 프로그램의 효과 (Efficacy of a Day-Center Treatment Program for Children with Developmental Disorders)

  • Objectives:This study was conducted in order to evaluate the effectiveness of a day-center treatment program to promote development of children with pervasive development disorder (PDD) and pervasive development disorder/mental retardation (PDD/MR). Methods:Twenty five children (14 in the PDD group and 11 in the PDD/MR group) participated in a day-center treatment program. They had been enrolled in the whole program for 2-3 years. Their performance was evaluated according to the Preschool Language Scale (PRES), Social Maturity Scale (SMS), and Korean version of the Childhood Autism Rating Scale (CARS). They were grouped by diagnosis at the beginning of the program and the treatment effect was compared. Results:Children who participated in the day-center treatment program showed a significant increase in their PRES and SMS scores and a decrease in their CARS scores. Conclusions:A day-center treatment program is effective for development of children with PDD and PDD/MR.

음악치료를 통한 아동 및 청소년 집단 프로그램 효과에 대한 메타분석 (A Meta-Analysis of the Effects of Children and Adolescent Group Music Therapy)

  • This study used meta-analysis to compare analyze data regarding the effects of music therapy on children and adolescent. The study examined 96 ESs using means and standard deviations from 77 Korean degree dissertations conducted between2000 and 2010. Results were as follows: The overall effect size (ES) of the therapy program was found to be 0.14, which, according to Cohen's ES interpretation criteria, is small. The mean ES was influenced by the subjects (children/ adolescents), ES of children(0.50) was bigger than ES of adolescents(-0.23). When results were sorted by the variables of therapy part: social, emotional and behavior development), the ES of social development was the biggest (1.11), followed by behavior development (-1.07) and emotional development (-0.01). However, other variables (total number of procedures, number of treatments per week, the average treatment time, therapist characteristics) made no statistically significant differences to the efficacy of the therapy. Finally, limitations of this study and potential future research are discussed.

중년기 남성의 개인내적 변인, 가족 변인 및 직무만족이 심리적 안녕감에 미치는 영향 (The Effects of Personal Psychological Variables, Family Variables, and Job Satisfaction on the Psychological Well-being of Middle-aged Men)

  • The purpose of this study is to examine the effect of personal psychological variables(cognitive flexibility, emotion dysregulation, subjective health condition), family variables(marital satisfaction, father-adolescent communication) and job satisfaction on the psychological well-being of middle-aged men. For this research, questionnaires were administered to 550 middle-aged men residing in Seoul, Gyeonggi and Incheon. Among 352 questionnaires collected, 327 were selected and statistically analyzed by average, standard deviation, frequency analysis, t-test, one-way ANOVA, and hierarchical regression analysis using SPSS Win 21.0. The main results of the study are as follows: First, according to middle-aged men's social-demographic variables, the level of their psychological well-being differed significantly. Second, cognitive flexibility was the most significant predictor of their psychological well-being. In addition, emotion dysregulation, job satisfaction, father-adolescent communication, and subjective health condition were significant predictors. These results suggest that cognitive reconstruction training or problem-solving intervention could be useful for the psychological well-being of middle-aged men.

한국판 청소년용 도덕발달수준 평가도구의 개발 (DEVELOPMENT OF THE KOREAN FORM OF MORAL DEVELOPMENT INVENTORY FOR ADOLESCENTS)

  • 본 연구는 일반 청소년은 물론 문제 행동을 나타낼 가능성이 보다 높은 소위 비행청소년을 대상으로, 도덕발달수준을 평가할 수 있는, 신뢰도와 타당도가 갖추어진 평가도구를 개발함에 그 목적이 있다. 연구대상은 개발하고자 하는 평가도구이며, 중학교 1학년에서 고등학교 3학년까지 남녀 한 학급씩을 무작위 선정한 507명을 포함하여 품행장애군과 정신과 환자대조군 각 28명씩을 검사대상으로 하였다. 연구방법으로는 우선 평가도구의 골격이 되는 Rest의 원문에 관하여 그 검사도구의 제작 및 신뢰도와 타당도 검사 과정과 결과를 숙지하였다. 그 다음으로 우리의 문화와 정서에 맞는 이야기 구성을 하였는데, 이 과정에서는 정신과 전문의 두 명과 임상심리학 전문가 한 명과의 의견교환 및 자문으로 작성하였다. 일차적으로 만들어진 평가도구는 중학생 여섯 명을 대상으로 사전검사를 실시하였으며 사전검사에서 나타난 보다 실제적 측면에서 평가도구의 문제점을 검토하여 최종 평가도구를 구성하였다. 최종완성된 평가도구의 타당도 평가에는 세 가지 방법이 사용되었는데, 첫째로 구성타당도를 살펴본 결과, 발달에 따른 구성 요인의 변화를 보기 위하여 학년의 증가에 따른 도덕발달수준의 증가 여부를 살펴보았는데, 학년의 증가라는 발달에 따라 본 연구의 구성 요인이라고 할 수 있는 도덕적 판단능력의 분명한 증가를 확인할 수 있었다. 둘째로 기존의 간이형 설문도구와 비교하여 보는 공존타당도를 보았는데, 두 도구간의 상관계수는 0.87이었다. 세째로 변별타당도는 품행장애군, 정신과 환자대조군, 정상대조군 각 28명씩을 대상으로 시행한 성적을 비교분석으로 평가하였는바, 품행장애군과 다른 두 군간의 유의한 차이를 확인할 수 있었다. 신뢰도 평가를 위하여 정상 청소년 42명과 품행장애군 28명을 대상으로 3주 간격으로 검사-재검사를 시행하여 평가분석하였는데, 그 상관계수는 정상군에서는 0.88, 품행장애군에서는 0.63이었다. 각 이야기와 전체간의 상관 정도를 측정하여 내적일치도를 구한 결과 Cronbach’s alpha가 0.71이었다.

자폐스펙트럼장애의 조기선별에 대한 관련 분야 종사자의 인식 조사 (A Study on Practitioner's Perceptions on Early Screening of Autism Spectrum Disorder)

  • Objectives: The purpose of this study is to investigate the professional knowledge and perceptions of the early screening of Autism Spectrum Disorder (ASD) in practitioners who have contact with patients with ASD. Methods: A survey was carried out among 674 practitioners in total, where practitioners are defined as those who work at primary medical centers, public institutions, educational institutions and treatment institutions. The survey was carried out both online and offline, and it mainly focused on 1) knowledge about ASD symptoms, 2) knowledge about the early screening of ASD, 3) measures taken after ASD detection, 4) thoughts on the development of early screening tools for ASD, and 5) the current status of ASD treatment. The data collected were analyzed through descriptive statistics, analysis of frequency and cross tabulation analysis using SPSS WIN 22.0. Results: The results of this study suggest that the practitioners were not aware of the exact symptoms of ASD and their professional knowledge and the environment for early screening were insufficient. Furthermore, very few and inappropriate measures were taken after the detection of ASD. In addition, there was a high demand for early ASD screening tools to be used on site and, regarding treatment, the significance of the implementation of evidence based treatments as well as the continuity of relevant research came to the fore. Conclusion: It seems that there is a lack of knowledge and perception of the early screening of ASD and that education and training among practitioners is urgently required. This issue is discussed in more detail in the paper.

발달증진프로그램이 발달지연 아동 어머니의 양육 스트레스에 미치는 영향 (The Effect of the Mother-Child Development Promotion Program for the Child with Developmental Delay on Mother's Depressive Mood and Parenting Stress)

  • Objectives : We investigated the parenting stress and depressive mood of mothers with developmentally-delayed children and the effects of participation in a mother-child development promotion program on same. Methods : Subjects were the mothers of 20 developmentally-delayed children. The severity of the mothers' parenting stress was evaluated via the Korean version of the Parenting Stress Index, Short Form (K-PSI-SF) and the severity of their depressive symptoms were evalueted by the Korean version of the Beck Depression Inventory (K-BDI). Results : The mean K-PSI-SF score and mean K-BDI score for these mothers were 93.35 (SD=10.47) and 23.25 (SD=10.42), respectively. These scores correspond to high parenting stress and moderate depression. The mothers who participated in a mother-child attachment- promotion program showed significant decreases in their K-PSI-SF and KBDI scores. Conclusion : Our data suggest that a mother-child attachment promotion program with emphasis on child development is effective in reducing parenting stress and depressive mood in mothers of developmentally-delayed children.

  • Objectives : The purpose of this study was to investigate the effectiveness of a day-hospital treatment program designed to help development of children with pervasive developmental disorder (PDD) and mental retardation (MR). Methods : Retrospective review of the charts of 32 children (28 with PDD, 4 with MR), who participated in a day-hospital treatment program of Seoul Metropolitan Eunpyeong Hospital, from October 2008 to February 2012, was conducted. Development level of each patient was evaluated according to the Psycho-Educational Profile-Revised (PEP-R), Social Maturity Scale (SMS), and Childhood Autism Rating Scale (CARS). Evaluation was done at two points, before participation and after 1 year participation. Results : Children who participated in the day-hospital treatment program showed significant improvement in all categories of PEP-R, SMS, and CARS. Conclusion : Day-hospital treatment program is effective for helping development of children with PDD and MR.

청소년 ADHD 평가 도구 개발을 위한 예비연구-CASS(S)의 신뢰도 및 타당도 연구 (A PRELIMINARY STUDY FOR THE DEVELOPMENT OF THE ASSESSMENT SCALE FOR ADHD IN ADOLESCENTS:RELIABILITY AND VALIDITY FOR CASS(S))

  • 주의력결핍과잉운동장애(ADHD)는 소아질환으로 생각되었으나, 청소년과 성인에서도 문제가 되는 것으로 보고되고 있다. 질환의 유병율이 2.0~14.3%로 다양하게 보고되고 있는데, 그 이유로는 질병 정의 기준의 차이, 평가방법의 차이 외에 연구대상자의 연령 차이 때문 등을 생각할 수 있다. 따라서 소아, 청소년, 성인을 대상으로 각각 적용가능한 ADHD 평가도구가 필요하다. 본 연구는 청소년에게 사용가능한 ADHD 유병율 평가도구의 개발을 위하여 Conners-Wells' Adolescent Self-Report Scale(Short Form)[CASS(S)]을 번안하여 신뢰도와 타당도를 확인하고자 하였다. CASS(S)는 세 명의 정신과 전문의와 임상심리전문가 한 명이 번안, 수정하였다. 공존타당도 및 검사-재검사 신뢰도를 보기 위하여 K-YSR 중 주의력 관련 9문항과 번안된 CASS(S)를 서울 시내 중학교에서 학년당 한 학급씩 선택하여 삼 개 학년에게 2주 간격으로 실시하였다(총인원 115명:남학생 58명, 여학생 57명). 신뢰도 평가를 위하여 내적일관성을 나타내는 Cronbach's alpha를 구하였다. 구성타당도를 알아보기 위하여 요인분석을 실시하였다. 요인분석을 위한 검사대상은 전국의 중고교생 4,354명으로 하였다. 연구결과 공존타당도를 알아보는 상관계수는 .65(p<.01), 검사-재검사 신뢰도를 알아보는 Pearson 상관계수는 .83으로 만족할 만한 수준이었다. 내적 일관성은 Cronbach's alpha .88로 만족할만한 수준이었다. 요인분석 결과 품행문제, 과잉행동문제, 인지문제의 세 가지 요인으로 나타났다.
