Initial Spiritual Screening and Assessment: Five Things to Remember

  • To deliver holistic and person-centered palliative care (PC), the spiritual dimension must also be assessed. However, many nurses do not screen for or assess patient spirituality. This article presents five things that PC nurses can consider in order to improve their spiritual screening and assessment practices. These points are as follows: (1) Understand that spirituality is manifest in a myriad of ways and is not the same thing as religiosity. (2) Screen for spiritual distress, and then later conduct a spiritual history or assessment. (3) Remember that spirituality is not just something to assess upon admission. (4) Know that there are many ways to assess spirituality (it is not merely how a patient verbally responds to a question about spirituality or religiosity). (5) Remember that assessment can also be therapeutic.

A Study of Radioactive Contamination in Hospital Toilet (병원 화장실의 방사성 오염에 관한 연구)

  • Generally a patient who was injected radiopharmaceuticals for nuclear medicine examination is not an object of isolation. Therefore, when the patient uses toilet, we expect surface contamination of a toilet by radioisotope. The measured value is $25.69Bq/cm^2$(a restroom near admission and administration), $19.39Bq/cm^2$(a toilet near department of radiology). The study shows that 9 of 24 locations in controlled area exceed over surface contamination limit. From now on, we should find source of contamination through measurement radioactive nuclide to apply radiation safety management.

  • Objective : To report one case where Subdural Hematoma(SDH) in operation indication was treated by Scalp Acupuncture therapy. Methods : The changes in clinical symptoms of headache, dizziness, nausea, vommitting were described when this patient was treated with scalp acupuncture therapy and with herb medication. Results : Symptoms (headache, dizziness, nausea, vomiting. etc) at admission decayed gradually with Scalp Acupuncture therapy. The patients could walk alone at discharge. Conclusion : Though she had been in op. indication, the patient's health improved by herbmedications of Dodamcheseup($d{\breve{a}}ot{\acute{a}}nchu{\acute{u}}shi$)-Tang etc. and by Scalp Acupuncture therapy.

  • Objectives: The purpose of this study is to report the effect of Korean medical treatment using Yin-yang Balancing Therapy of TMJ (YBT) on correcting posture. Methods: During the admission period, the patient was treated by Korean medical treatment (YBT, acupuncture, herb medicine, etc) every each day. The standard intraoral balancing appliance was used in patient while sleeping and exercising. Results: After treatment for 35th, the angle changed from 23 degrees to 0 degree. The posture of the neck and shoulder became straight. The distorted facial midline was aligned straight. Conclusions: These results showed that YBT have an effect on correcting posture.

Management of Gastrointestinal Side Effect During Chemotherapy (항암화학요법에서 발생하는 소화기 부작용의 관리)

  • Gastrointestinal side effects including nausea and vomiting, diarrhea, constipation, oral mucositis and colitis, and hepatotoxicity are common occurrence during chemotherapy. Often they result in unplanned admission and interruption of scheduled therapy. Additionally they have a negative influence on patient's therapeutic outcome and quality of life. The assessment of gastrointestinal side effects is dependent on clinician assignment of a grade established by the National Cancer Institute Common Terminology Criteria for Adverse Events. Continued interest of gastrointestinal side effects has allowed identifying patients at higher risk and providing effective treatments to relieve painful symptom. Finally, proper prevention and management of chemotherapy-induced gastrointestinal side effects will be needed to improve patient's survival and quality of life.

A Case of Soyangin Daylong Generalized Primary Hyperhidrosis (종일 지속되는 소양인 원발성 전신 다한증의 치험 1례)

  • Objectives This case study aimed to report the improvement of stress-induced generalized primary hyperhidrosis of Soyangin patient. Methods Soyangin patient suffering from daylong generalized primary hyperhidrosis was treated with acupuncture, psychological counseling, and herbal medicine. The severity of hyperhidrosis was measured using Visual Analog Scale (VAS). Results & Conclusions After treatment with Tae-Geuk acupuncture, Dojeokganggi-tang, and psychological counseling, the VAS score of hyperhidrosis was reduced from 10 to 2 during three days of admission. Herbal medicine was prescribed for 30 more days after discharge. The relieved state was maintained when monitored after two months. This case report proposes the potential of Sasang constitutional medicine for stress-induced generalized primary hyperhidrosis.

Nutrition Intervention in a Gastric Cancer Patient With Gastrectomy for 12 Months: A Case Report

  • Gastrectomy is the most effective method of treating gastric cancer, but it is commonly associated with weight loss, nutritional deficiencies, and the increased risk of malnutrition due to post-surgery complications, including gastric stasis, dumping syndrome, malabsorption, and maldigestion. Malnutrition is a risk factor for postoperative complications and poor prognosis. To prevent it and guarantee a quick recovery after surgery, continuous and individualized nutrition intervention should be performed both before surgery and postoperatively. The Department of Dietetics at Samsung Medical Center (SMC) performed nutritional status assessment before gastrectomy, initial nutritional assessment within 24 hours of admission, description of therapeutic diet after surgery, nutrition counselling before discharge, and nutritional status assessment and individual nutrition counselling after 1, 3, 6, and 12 months from surgery. This is a case report of a patient who underwent gastrectomy as well as intensive nutrition intervention in SMC.

The Clinical Study on Hua-Tuo-Jia-Ji-Xue(華佗夾脊穴) Acupuncture Treatment of Patient with Thoracolumbar Compression Fracture (흉(胸)·요추압박골절(腰椎壓迫骨折) 환자(患者)의 화타협척혈 침치료(鍼治療)에 대한 임상적 고찰)

  • Objective : This study was performed to evaluate result of thoracolumbar compression fracture patient treated with Hua-Tuo-Jia-Ji-Xue(華佗夾脊穴) acupuncture. Methods : This clinical study was carried out 50 cases with thoracolumbar compression fracture, who had been admitted from January, 2000 to February, 2001, in the department of acupuncture and moxibustion, Hanseo University Oriental Medical Hospital. We divided two groups; A group, without using Hua-Tuo-Jia-Ji-Xue(華佗夾脊穴) acupuncture and the other group is B group, by using Hua-Tuo-Jia-Ji-Xue(華佗夾脊穴) acupuncture. Results : 1. The distribution of sex and age, 78% were female, and 84% were most above there sixties. 2. In the duration of symptom, the largest group was over 4 weeks. 3. In the distribution of cause, the largest group was "lifting heavy object"(32%) and the next was "slip down"(26%). 4. In the distribution of injured level was L1 body the most(19.6%) and T12 body was the next. 5. In the distribution of clinical symptoms on the admission, the largest group was lumbosacral pain(18%). 6. The result of treatment due to the duration of admission, 17case(34%) was the most above "Fair" during the 2~3 weeks. 7. The result of treatment, Good and Fair were the same in 24case(48%). 8. The duration of treatment was longer in high grade of clinical symptom. 9. B group discharged above good state 60% higher than A group.

Patient characteristics associated with length of stay in emergency departments (응급실 재원시간과 관련된 환자의 특성)

  • The length of stay in emergency departments has been used as a quality indicator to reflect the overall efficiency of emergency care. Identifying characteristics associated with length of stay is critical to monitor overcrowding and improve efficient throughput function of emergency departments. This study examined the level of waiting time for initial assessment by physician and length of stay in emergency departments. Furthermore, we investigated the characteristics of patients' attendance associated with length of stay. An observational study was performed for a sample of 1,526 patients visiting ten nation-wide emergency departments. A structured form was designed to collect information about patients' demographics, route of admission, time and mode of arrival, triage level, cause of attendance, initial assessment time by physician, departure time, and disposition. Multiple regression analysis was performed to determine factors associated with length of stay. The average length of stay was 209.4 minutes (95% confidence interval [CI]=197.1-221.7), with a mean waiting time for initial assessment of 5.9 minutes (95% CI=5.1-6.7). After controlling for emergency department characteristics, increasing age, longer waiting times, attendance due to diseases, higher acuity, multiple diagnoses($\geq$2) and requiring admission or transfer to other health care facilities were positively associated with length of stay in emergency departments. The findings suggest that both patients' characteristics and the flow between emergency departments and parent hospitals should be taken into account in predicting length of stay in emergency departments.

A clinical analysis of Admission Patients in Korean Medicine Hospital due to Traffic Accident (교통사고로 한방병원에 입원한 환자에 대한 후향적 분석 - 일개 한방병원을 중심으로)

  • Objectives : The purpose of this study was to investigate the characteristics of inpatients who were admitted to Korean medicine hospital due to traffic accident. Methods : We analyzed statistical study in 137 patients, who had admitted to M korean medicine hospital, in Gimpo city, Gyeonggi Province from January 1st, 2017 to June 31th, 2017 according to medical charts. Results : 1. In distribution according to age and sex, people in their 50s numbered the most. The 137 inpatients comprised of male (46.0%) and female (54.0%). 2. In distribution according to types of accident, Rear-end collision accident numbered the most (34.3%), followed by Frontal collision (21.2%) and Lateral collision (14.6%). 3. In distribution according to duration of treatment, most (54.7%) patients discharged within 1 week, followed by 1 week to 2 weeks(37.2%) 4. In distribution according to mean elevation of symptoms, Neck pain was the most (75.9%) symptom, followed by Low back pain (67.2%), Shoulder pain (46.0%) and Headache (37.2%). 5. In distribution according to treatment results, symptom improvement was the most (62.8%), followed by excellent (19.7%), mild improvement (14.6%). 6. In distribution according to duration of treatment, Most frequently prescribed herbal decoction was Tongdosan (45.7%), followed by Dangguijakyaksangagam (29.6%), and most frequently prescribed extract powder was Yunkyopaedocksan and Ojeoksan (12.7%). Conclusions : This study shows that aftereffects from the traffic accident tend to occur with various symptoms in various age groups. Korean medicine treatments are effective in patient who were admitted to Korean medicine hospital due to traffic accident.
