A Study on the Development of Evaluation Index for Safety Management Level of Shipping Company(2) : Development of Evaluation Items for Safety Management Index (해운선사 안전관리 평가지표 개발에 관한 연구(2) : 안전경영 지표 평가항목 개발)

  • This paper is a follow-up research of 'A study on the development of Safety Management Evaluation Index for Shipping Company'. It aims to develop the evaluation items of Safety Management Index(hereafter called as 'SMI') applicable to 'Safety Advantage Point', which is suggested as the sub-index of 'Safety Management Evaluation Index' function in the preceding research and also verify their adequacy. For that purpose, firstly, the 11 evaluation items were developed after collecting the practical opinions from safety management personnels in Korean shipping companies. Secondly, the survey was carried out for the verification of the adequacy of the 11 evaluation items. And lastly, its results was analysed, whether it has statistical meanings of differences in adequacy awareness amongst safety managers grouped depending on business type(coastal, ocean-going) and business capacity(number of owned-operating ships). As a result, there was no relevance between their adequacy awareness and business type of their companies. However, a difference in their adequacy awareness existed depending on their companies' business capacity(small, medium and large sized). In conclusion, when the safety management level of shipping companies is evaluated by SMI, the same indices can be applicable to both coastal and ocean-going shipping companies by a grouping of companies' business capacity.

An Action Convergence Research to Improve Evaluation Index of Pneumonia Adequacy in Regional Public Hospital (지역거점 공공병원의 폐렴 적정성 평가 지표 개선을 위한 융복합 실행연구)

  • This is an implementation study towards the development of a strategy to improve the pneumonia adequacy evaluation index of regional public hospital and to confirm its effectiveness. One physician, eight nurses, one computer room staff, and one nursing professor working at a regional base public hospital located in N city participated in this study from March to October 2021. An implementation study was conducted based on the conceptual model of Zuber-Skerritt & Fletcher. The stages of planning, implementation, evaluation, and reflection were operated in two cycles, and the effectiveness was evaluated in the second and third quarters. Compared to the pneumonia adequacy evaluation index in 2019, all indicators reached 100% of the standard target in 2021. The results of this study therefore infer that to provide higher quality medical services in the future, it is necessary to apply medical quality improvement activities through practical research under multidisciplinary participation and cooperation and to review the applicability of clinical sites.

Assessment of the Dietary Adequacy of Non-Insulin Dependent Diabetes Mellitus in Daegu Area (대구지역 인슐린 비 의존형 당뇨병 환자의 영양섭취 적정도 평가)

  • To assess the dietary therapy compliance of non-insulin-dependent diabetes mellitus (NIDDM) patients living in Daegu, we evaluated diet adequacy levels by index of nutritional quality (INQ), nutrient adequacy ratio (NAR), mean adequacy ratio (MAR), and dietary variety score (DVS) with food exchange system and Korean recommended dietary allowances (KRDA). One day dietary intake was measured by 24 hour recall method for 229 subjects over 20 years of age. Average daily energy intake was 1444 kcal. The relative ratio of carbohydrate, protein and fat in terms of energy intake was 69.4:14.5:16.0. The nutrient adequacy ratio (NAR) for Vitamin A, B$_2$, calcium were lower than 0.5 and MAR was 0.65. It appeared that the consumption of each food group as compared to prescribed food exchange unit was insufficient, in the order of dairy group (10.2 $\pm$ 25.2%), fruits group (58.2 $\pm$ 71.2%) and oils & fats group (42.9 $\pm$ 42.0%). The dietary variety score (DVS) was evaluated as an useful tool for diabetes' meal management when we accept minimum intake as 0.3 of food exchange unit. We found significant correlation between food groups and anthropometric indices: cereals group and % total body fat (r = 0.251, p < 0.01), meat group and waist circumference (r = 0.241, p < 0.01), vegetables group and WHR(r = 0.139, p < 0.05), and oils poop and WHR(r = 0.165, p < 0.05). from these results, we concluded that overall status of dietary intake of NIDDM patients in Daegu area was insufficient not only in quality but also in quantity. It is suggested that nutrition education for NIDDM patients should stress on balanced food consumption to meet proscribed amount by six food group to improve the inadequacy of dietary status.

A Study on Nutritional Intakes in Elderly People in Wando Area (완도지역 성인 및 노인의 영양소 섭취 실태조사)

  • The purpose of this study was to investigate nutrients intakes and nutritional quality of Adults and Elderly People in an island area (Wando). A three-day dietary intakes survey, using a 24 hour recall method was obtained from 187 subjects aged 46 to 84 (mean age 65.3) living in an island area (Wando). Nutient intakes were analyzed using CAN-Pro soft program and compared to Korean RDA. The quality of nutrients was assessed by analyzing nutrient adequacy ratio (NAR), mean adequacy ratio (MAR), and index of nutritional quality (INQ). The average daily mean energy intakes were 1869.0 kcal for males and 1943.9 kcal for females, respectively. Daily intakes of protein for males and females were 28.0 and 30.4 g and those of fat were 31.5 and 28.51 g, respectively. Nutrient consumed below $75\%$ of Korean RDA was protein, vitamin A, Ca and Zn in both males and females. Average CPF ratio of males and females were 78.8 : 6.0 : 15.1 and 80.0 : 6.4 13.4, respectively. Energy intake ratio from protein was significantly higher in over 60 years males. Carbohydrate dependency decreased with age. Protein dependency increased with age. Nutrient adequacy ratio (NAR) of energy, protein vitamin A and vitamin E were increased with age in males. The mean adequacy ratio (MAR), an index of overall dietary quality, were not significantly different by age group. Average MAR for males and females was 0.77 and 0.78, respectively. The index of nutritional quality (INQ) showed the tendency to decrease with age. Especially, there were significantly decreases in INQ of all nutrients, except protein, with age. Based on these results, it is evident that people in the island area did not consume enough nutrients. Specially, dietary intake of protein, vitamin A, vitamin E, and Ca were not adequate. Evaluation of Water Supply Adequacy using Real-time Water Level Monitoring System in Paddy Irrigation Canals (실시간 관개수로 수위 모니터링을 활용한 논 관개용수 공급적정성 평가)

  • Appropriate amount of water supply to paddy fields in proper time is important to achieve efficient agricultural water management. The purpose of this study is to evaluate the irrigation water supply adequacy for paddy fields using water level data in irrigation canals. For the evaluation, the real-time water level data were collected from main canals in the Dongjin irrigation district for 2 years. Using the water level data, delivered irrigation water amounts at the distribution points of each canal were calculated. The water balance model for paddy field was designed considering intermittent irrigation and the irrigation water requirement was estimated. Irrigation water supply adequacy was analyzed from main canals to the irrigation blocks based on the comparison between estimated requirement and delivered irrigation water amounts. From the adequacy analysis, irrigation water supply showed poor management condition in 2012 with low efficiency except the Daepyong canal section, and the adequacy in 2013 was good or fair except the Yongsung canal section. When irrigation water for paddy fields was insufficient, water supply adequacy was affected by irrigation area, but when irrigation water was enough to supply, adequacy was affected by distance from main canal to distribution points. These results of the spatial and temporal dimensions of the irrigation adequacy could be utilized for efficient irrigation water management to improve the temporal uniformity and equity in the water distribution for paddy fields.

Evaluation of Nutrient Intakes and Diet Quality of Rural Middle School Students (농촌지역 급식교 및 비급식교 중학생의 영양소 섭취 적절성에 관한 연구)

  • This study was performed to assess the nutrient intakes and diet quality of rural middle school students provided with and without the national school lunch program(NSLP). The data on dietary intakes were obtained by using food record method questionnaires which were collected from 340 rural middle school students provided with(n=177) and without (n=163) NSLP. Diet quality was accessed by NAR(nutrient adequacy ration), MAR(mean adequacy ration), and INQ( index of nutritional quality). Total energy intakes of the boys were 2123 kcal with NSLP and 1857 kcal without NSLP. Total energy intakes of the girls were 1913 kcal with NSLP and 1814 kcal without NSLP. Phosphorus, vitamin A, vitamin E, ascorbic acid, thiamin, riboflavin, and niacin intakes were above the RDA in the NSLP group. For those without NLSP, phosphorus, vitamin E, thiamin intakes were above RDA, however, calcium, iron vitamin A, vitamin $B_6$ were less than the RDA. The mean adequacy ration(MAR), an index of dietary quality was 0.69(with) 0.62(without) for males and 0.62(without) for females The indexes of nutritional quality(INQ) were over 1 for most nutrients except calcium, vitamin A, vitamin B$_2$ and zinc. The study showed that total daily energy and nutrient intakes were significantly higher in students provided with than those without school-lunches. Thus, the school-lunch program is recommenced and necessary to improve the nutritional status of middle school students.

A Study on the Adequacy Analysis of Distribution Line Drop for Improvement of Consumer Voltage Fluctuation (수용가 전사변동의 개선을 위한 배전선로의 적정성해석에 관한 연구)

  • In this paper, a reliable and convenient evaluation method for the adequacy of any radial distribution line's voltage drop is proposed for improvement of the voltage regulation of the distribution line under operation. Using the voltage measurement of the first load branch point and the last point on the given distribution line its average line voltage drop and the line voltage drop variance by pure load variation are computed. And by these values and allowable or required line drop from proper voltage regulation view point, present adequency index and operation improvement index of the line drop are newly introduced for the evaluation of the given distribution line's voltage regulation. As a result of application to the 4 cases of study, the proposed method is proved to be reliable and convenient.

Assessment of pain and adequacy of pain management in hospitalized cancer patients

  • The author investigated pain experiences of 90 cancer patients and the adequacy of pain treatment they have received during their stay at a large medical center in T city between October 1994 and August 1995. Pain was assessed by the Shortened BPQ and results are summarized as follows: As for ratings of “worst pain” during the 24 hour period, 70% of the patients reported they had “severe” pain. As for ratings on “pain now,” 43% of the cancer patients reported “moderate to severe” pain. Over 46% of the patients reported a pain relief score of 0(not at all) or 1(somewhat) even after receiving pain medication. Adequacy of analgesic treatment was evaluated by comparing the patient's reported level of pain and the analgesic use, namely, the pain management index(PMI). The PMI indicated that 58% of the patients were undertreated for the pain control. In review of nurse's notes. systematic pain assessment was scarcely recorded, although pain documentation appeared in 70% of the notes; and the contents were mostly simple description. In conclusion, the results of patient's pain ratings, the PMI and poor pain documentation in the nurse's notes implied poor pain assessment and management.

A Study on Nutrient Intake of College Student in Seoul and Yanbian (연변 조선족과 한.중 대학생의 영양소 섭취상태 비교)

  • The purpose of this study was to compare nutritional intakes of college students in Seoul and Yanbian by ethnic : Korean, Korean-Chinese, Chinese. The questionnaire were applied to individual interviewing method. Statisticai data analysis was completed by SAS 8.0 package program for t-test, ANOVA and multiple comparison. The results of this study were as follows. Average energy intake per day was high in Korean-Chinese. Nutrient Adequacy Ratio(NAR) was highest in iron and lowest in zinc. Mean Adequacy Ratio(MAR) score was $0.68{\sim}0.85$ for each group. The indexes of nutritional quality(INQ) were over 1.0 for most nutrients except 0.41 of zinc and 0.76 of calcium. Average energy intake ranked high in Korean-Chinese, Korean and Chinese.

Estimation and adequacy of dentist supply through the dental care productivity (생산성을 고려한 치과의사의 공급 추계와 적정성)

  • Running title: Adequacy of dentist supply Objectives: The proper provision of dentists is very important for delivering the dental care that people need. The purpose of this study is to evaluate the adequacy of dentist supply and to predict the proper supply of dentists in Korea for the next 10 years. Methods and materials: Using the data from 2004 to 2017 in the Statictics Korea, the dental needs were measured as days of admission and the annual total dental care hours in 2020, 2023, 2026 and 2030 were predticted using regression analysis. The dental care productivity (average in-office hours) of dentists was analyzed using regression analysis including age, gender, education level and region as a confounder. The annual care hours were predicted according to seven scenarios based on the percentage of women among dentists, post-dentist education levels, the percentage of dentists in the metropolitan area, aging and retirement age. Results: The needed dentist based on the data in the Statictics Korea will be 27, 288 ~ 27, 311 in 2020, 28,104 ~ 28,1785 in 2023, 28,977 ~ 29,124 in 2026, and 30,174 ~ 30,413 in 2030. On the other hand, the number of dentists in Korea will be 26,945 in 2020, 28,813 in 2023, 30,682 in 2026 and 33,173 in 2030. The adequacy index of dentist supply will be -0.01 in 2020 to +0.10 in 2030. Conclusions: Our data showed that dentists will be adequately supplied in Korea for the next 10 years. Overall, the ratio between dentist versus 1,000 Koreans will be increased from 0.5 in 2019 to 0.7 in 2020, which will meet the mean of OECD countries.
