골반바닥근 운동방법이 골반바닥근과 몸통근육의 근두께에 미치는 영향에 대한 융합적 연구 (The Convergence Study on the Effects of Three Pelvic Floor Muscle Excercise on Thickness of Pelvic Floor Muscle and Abdominal Muscles)

  • 본 연구에서는 실시간 초음파 영상 분석을 통해 세 가지 골반바닥근 수축 운동이 골반바닥근과 배가로근, 배속빗근, 그리고 배바깥빗근의 두께에 미치는 영향을 분석하였다. 골반바닥근과 몸통 근육들의 근두께는 초음파 영상 장비를 이용하여 안정시, 전통적 골반바닥근 수축[운동 A]시, 엉덩 모음근과 골반바닥근 동시 수축[운동 B]시, 실시간 초음파 영상장비를 이용한 골반바닥근 수축 유도[운동 C])시 측정하였다. 측정 결과, 다른 운동들에 비해 운동 C 수행 시 안정 시 보다 가장 크게 골반바닥근의 두께가 감소하였고(p<.05), 배가로근 역시 다른 운동들에 비해 운동 C 수행 시 안정 시 보다 근두께가 가장 크게 증가하였다(p<.05). 이와 같은 결과를 바탕으로, 본 연구에서 수행한 세 가지 골반바닥근 수축 방법들 중에서 실시간 초음파 영상 장비를 이용하여 골반바닥근 수축을 유도하는 운동 C 방법이 골반바닥근 수축을 통한 여성들의 오줌새기를 개선시킬 수 있는 방법으로 추천될 수 있을 것으로 사료된다.

내전형 연축성 발성장애 감별진단 문항 개발과 임상적 유용성 평가 (Development of Differential Diagnosis Scale Items for Adductor Spasmodic Dysphonia and Evaluation of Clinical Availability)

  • Background and Objectives The purpose of this study was to develop the differential diagnosis scale containing items from adductor spasmodic dysphonia (ADSD) to muscle tension dysphonia (MTD) and the determine clinical utility of newly developed items. Materials and Method The four parts of pitch, redirected phonation, automatic speech and voiced sound were selected for analyzing the characteristics of ADSD in the literature. One part of tense voiceless sound was developed according to the Korean manner of articulation. The content validity was evaluated based on 5 scales (1-5 point) analysis from 30 experts. One hundred patients (50 ADSD and 50 MTD) were recorded in reading a sentence and sustained phonation. The two speech language pathologist evaluated recorded voices through a blind test using 4 scales (0-3 point) for newly developed items. Results As a result of verifying the content validity of items with experts, it was identified that the differentiated items were valid with 4.2 out of 5. Through the differential diagnosis between two groups according to the items, the correlation between sub-domains and total scores was shown as higher than 0.710. The result of analyzing the reliability on each diagnosis domain was 0.840-0.893, which showed the internal consistency of items was great. Newly developed five parts of ADSD were significantly higher than those of MTD with strong correlation (p<0.01). The reliability among the evaluators was analyzed as high with 0.892. Conclusion In this study, the differential diagnosis scale of ADSD was revealed as having validity and reliability. It is considered that it will be useful for differentiating ADSD and MTD in the clinical field.

교각 운동 시 대퇴의 내-외전과 경골의 내-외회전에 따른 하지의 근육활성 비교 (Comparison of Muscle Activity of Lower Limbs in Bridging Exercise according to Thigh Adduction-Abduction and Tibia Internal-External Rotation)

  • Background: This study tried to identify the bridge exercise posture for efficient exercise application by comparing muscle activity of buttocks and thighs according to internal-external rotation and pronation & supination in bridge exercise. Method: Nine males in their 20s living in D city were randomly selected as subjects. Muscles such as vastus medialis oblique (VMO), vastus lateralis (VL), semitendinosus (ST), biceps femoris (BF), gluteus maximus (GMAX), gluteus medius (GMED), tensor fasciae latae(TFL), and adductor longus (ADL) were measured using eight channel surface electromyogram (MyoSystem 1400A, Noraxon, USA) to measure muscle activity. Statistics process was performed through paired t test. Results: In the changes in electromyogram signals according to internal-external rotations according to internal-external rotation of shinbones, in most cases muscle activity was higher in external rotation than in internal rotation, but there was no statistical significance (p>.05). In particular, it was lower in TFL and ADL. There was no statistical significance in the comparison between two groups (p>.05). In the changes in electromyogram signals according to internal-external rotations according to pronation & supination of thighs, GMED showed significantly higher value in supination than in pronation (p>.05), and in ADL pronation is significantly higher than supination (p<.05). Conclusion: In internal-external rotation of shinbone and pronation & supination of thighs in bridge exercise, changes in muscle length can make effects on muscle activity of buttocks and thighs. Therefore, muscle strength enforcement program on buttocks and thighs through bridge exercise can make effects on patients with lower limb functional damages in clinical situations.

신경성 발성장애와 기능성 발성장애의 감별 진단 (Differential Diagnosis between Neurogenic and Functional Dysphonia)

  • Voice disorder is classified into three categories, structural, neurogenic and functional dysphonia. Neurogenic dysphonia refers to a disruption in the nerves controlling the larynx. Common examples of this include complete or partial vocal cord paralysis, spasmodic dysphonia. Also it occurs as part of an underlying neurologic condition such as Parkinson's disease, myasthenia gravis, Lou Gehrig's disease or disorder of the central nervous system that causes involuntary movement of the vocal folds during voice production. Functional dysphonia is a voice disorder in the absence of structual or neurogenic laryngeal characteristics. A near consensus exist that Muscle tension dysphonia (MTD) is functional voice disorder wherein hyperfunctional laryngeal muscle activity whereas Spasmodic dysphonia (SD) is neurogenic, action-induced focal laryngeal dystonia including several subtype. Both Adductor type spasmodic dysphonia (AdSD) and MTD may be associated with excessive supraglottic contraction and compensation, resulting in a strained voice quality with spastic voice breaks. It makes these two disorders extremely difficult to differentiate based on clinical interpretation alone. Because treatment for AdSD and MTD are quite different, correct diagnosis is important. Clinician should be aware of the specific vocal characteristics of each disease to improve therapeutic outcome.

복부 동시수축 형태에 따른 하지 근활성도 비교 (Comparison of Lower-Limbs Muscle Activity according to the Abdominal Co-contractive Activation)

  • 본 연구는 복부 동시수축 형태에 따라 하지의 근활성도에 어떤 영향을 미치는지에 대해 알아보고자 하였다. 연구대상자는 허리와 하지에 통증이 없고, 근골격계에 이상이 없는 건강한 20대 여성 30명을 대상으로 진행하였다. 복부 이완(Rest), 복부 당기기 기법(ADIM), 복부 브레이싱 기법(ABM)을 유지하는 각 자세에서 무릎의 굽힘없이 발을 능동적으로 바닥으로부터 20cm 올린 상태(ASLR)로 약 10초간 유지하도록 하여 하지의 5개 근육(중간볼기근, 엉덩긴모음근, 넙다리곧은근, 넙다리두갈래근, 앞정강근)의 근활성도를 측정하였다. 연구결과, Rest에서 ASLR을 유지하였을 때보다 ADIM 수축형태로 ASLR 유지할 때 하지의 모든 근육에서 근활성도가 높게 나타났다. 또한 Rest에서 ASLR을 유지하였을 때보다 ABM 수축형태로 ASLR을 유지하였을 때 중간볼기근을 제외한 나머지 근육에서 모두 근활성도가 높게 나타났다. ADIM과 ABM을 비교한 결과 중간볼기근과 앞정강근은 ADIM 수축형태에서, 엉덩긴모음근, 넙다리곧은근, 넙다리두갈래근은 ABM 수축형태에서 통계적으로 유의하게 높은 근활성도가 나타났다. 본 연구의 결과로 체간중심 안정화 운동이 필요한 요통 환자의 선제적 복부 수축형태로서 뿐만 아니라 하지의 선택적 근력강화가 필요한 환자들에게 개별화된 방법으로써 근거를 제시할 수 있을 것이다.

무지근육에서 상상의 운동과 연관된 촉진과 억제에 관한 운동 유발 전위 검사 (Motor Evoked Potentials Study of the Facilitation and Reciprocal Inhibition Induced by Motor Imagination in the Thumb Muscles)

  • Purpose: MEPs elicited by transmagnetic stimulations of the motor cortex are facilitated by voluntary muscle contraction. We evaluated the effects of the imagination of the movements on latencies of MEPs and reciprocal inhibition by using transmagnetic stimulations. Methods: Twenty two healthy volunteers(eight men and fourteen women) were studied. TMSs were delivered at rest and during imagining abducting or adducting right thumb. A stimulator with a round coil and a fixed intensity of 80% of maximum was used to evoke MEPs. MEPs were evoked by magnetic stimulations over the scalp and cervical spine(C7-T1), and central motor conduction times(CMCT) were calculated by subtracting the latency of compound muscle action potentials(CMAPs) obtained by stimulating over the cervical spine from that obtained by stimulating over the scalp. The motor evoked potentials were recorded from right abductor pollicis brevis muscle(APB) and adductor pollicis muscle(AP) simultaneously. Results: Imagination of abduction resulted in a shortened latency of the CMAPs in APB, and a prolonged latency in AP. Imagination of adduction resulted in a shortened latency in AP, and a prolonged latency in APB. But the imagination caused no significant change in the latency of CMAPs elicited by stimulation over cervical spine. Therefore, the changes of the CMCTs account for these latency changes with imagination of movement. With the imagination of abduction, there are significant reduction of the CMCT's in APB(10.8%) and prolongation in AP(5.8%). On the other hand, with the imagination of adduction, prolongation of the CMCT's in APB(7.3%) and reduction in AP(5.9%) were observed. Conclusion: These findings indicate that imagination of muscle contraction increases the excitability of the human corticospinal system. Reciprocal inhibition may be accountable for the prolonged latency in the antagonist muscle.

등척성 엉덩관절 모음 및 벌림 시 골반 벨트가 엉덩관절 근육의 근력과 복부 근육 근 활성도에 미치는 영향 (Effects of a Pelvic Belt on Hip Muscle Forces and Abdominal Muscle Activities During Isometric Hip Adduction and Abduction)

  • Background: To improve lumbo-pelvic stability, passive support devices (i.e., a pelvic belt) are recommended clinically. Nevertheless, to understand the influence of passive support on lumbo-pelvic stability, it is necessary to examine the influence of a pelvic belt on the abdominal and hip abductor muscles. Objects: To examine the effects of a pelvic belt on the forces of the hip adductor and abductor muscles and activity of the abdominal muscles during isometric hip adduction and abduction. Methods: This study recruited 14 healthy men. All subjects performed isometric hip adduction and abduction with and without a pelvic belt in a neutral hip position. Load cells, wrapped with a non-elastic belt, were placed above the medial and lateral malleoli of the dominant leg to measure the muscle forces of the hip adductors and abductors, respectively. The forces of the hip adductors and abductors were measured using a load cell during isometric hip adduction and abduction, while the electromyographic activities of the bilateral rectus abdominis, internal oblique, and external oblique muscles were measured. Results: The forces generated by the hip adductors and abductors were significantly greater with the pelvic belt than without (p<.05). No significant differences in abdominal muscle activities between the two conditions were found (p>.05). Conclusion: These findings suggest that use of a pelvic belt could lead to effective strengthening exercise of hip muscles in individuals with sacroiliac joint pain.

Sustantial Observation on Foot Taeyang Meridian Muscle in Human Lower Limb from a Anatomical Viewpoint

  • Objective : This study was carried to identify the anatomical component of FTMM(Foot Taeyang Meridian Muscle) in human lower limb, and further to help the accurate application to real acupuncture. Methods : FTM at the surface of the lower limb was labelled with latex. And cadaver was stripped off to demonstrate muscles, nerves and the others and to display the internal structures of FTMM, being divided into outer, middle, and inner layer. Results : FTMM in human lower limb is composed of muscles, nerves, ligaments etc. The internal composition of the FTMM in human lower limb are as follows : 1) Muscle : Gluteus maximus. biceps femoris, semitendinosus, gastrocnemius, triceps calf, fibularis brevis tendon, superior peroneal retinacula, calcaneofibular ligament, inferior extensor retinaculum, abductor digiti minimi, sheath of flexor tendon at outer layer, biceps femoris, semimembranosus, plantaris, soleus, posterior tibialis, fibularis brevis, extensor digitorum brevis, flexor digiti minimi at middle layer, and for the last time semimembranosus, adductor magnus, plantaris, popliteus, posterior tibialis, flexor hallucis longus, dorsal calcaneocuboidal ligament at inner layer. 2) Nerve : Inferior cluneal nerve, posterior femoral cutaneous n., sural cutaneous n., proper plantar branch of lateral plantar n. at outer layer, sciatic nerve, common peroneal n., medial sural cutaneous n., tibial n. at middle layer, and for the last time tibial nerve, flexor hallucis longus branch of tibial n. at inner layer. Conclusions : This study proves comparative differences from already established studies from the viewpoint of constituent elements of FTMM in the lower limb, and also in the aspect of substantial assay method. We can guess that there are conceptional differences between terms (that is, nerves which control muscles of FTMM and those which pass near by FTMM) in human anatomy.

The Effect on the Hip Muscle Activation of the Fall Direction and Knee Position During a Fall

  • Background: A hip fracture may occur spontaneously prior to the hip impact, due to the muscle pulling force exceeding the strength of the femur. Objects: We conducted falling experiments with humans to measure the activity of the hip muscles, and to examine how this was affected by the fall type. Methods: Eighteen individuals fell and landed sideways on a mat, by mimicking video-captured real-life older adults' falls. Falling trials were acquired with three fall directions: forward, backward, or sideways, and with three knee positions at the time of hip impact, where the landing side knee was free of constraint, or contacted the mat or the contralateral knee. During falls, the activities of the iliopsoas (Ilio), gluteus medius (Gmed), gluteus maximus (Gmax) and adductor longus (ADDL) muscles were recorded. Outcome variables included the time to onset, activity at the time of hip impact, and timing of the peak activity with respect to the time of hip impact. Results: For Ilio, Gmed, Gmax, and ADDL, respectively, EMG onset averaged 292, 304, 350, and 248 ms after fall initiation. Timing of the peak activity averaged 106, 96, 84, and 180 ms prior to the hip impact, and activity at the time of hip impact averaged 72.3, 45.2, 64.3, and 63.4% of the peak activity. Furthermore, the outcome variables were associated with fall direction and/or knee position in all but the iliopsoas muscle. Conclusion: Our results provide insights on the hip muscle activation during a fall, which may help to understand the potential injury mechanism of the spontaneous hip fracture.

The Effects of Dietary Lysine Deficiency on Muscle Protein Turnover in Postweanling Pigs

  • The main purpose of this study is to investigate the effects of dietary lysine deficiency on protein turnover of porcine muscles. There were 18 LYD three-breed-crossing postweanling barrows from six litters cannulated with gastric tubes through the esophagus at approximate 10 kg of body weight and allocated into three treatment groups. When their body weights reached over 12 kg, one group was sacrificed for determining the initial protein masses of m. masseter, m. longissimus dorsi, m. adductor and m. biceps femoris from the right body side. The others received a diet containing 100% or 61.4% (calculated values) of the lysine requirement (NRC, 1998) multiplied by 1.103 for a period of 17 days. Daily feed provision was computed for each pig according to body weight at the same day. All pigs were infused a flooding dose of $^2$H$_5$-phenylalanine to determine the fractional protein synthesis rates (FSR) of the aforementioned muscles in the end. Their four muscles from the right body side were also dissected for measuring the fractional rates of protein accretion (FAR). As for protein degradation, fractional rates (FDR) were calculated by differences between synthesis and accretion. Results showed that the lysine deficiency resulted in, significantly (p<0.05), lighter body weights, smaller muscles and a slower growth rate. The protein mass, accreted by the muscles, of the deficient group was only 54% averaged of the pigs fed adequately (p<0.05). The FAR of these muscles in the deficient group was significantly lower (p<0.05) and only achieved 61.1% averaged of the control; there was no significant difference (p>0.05), nevertheless, in the amino-acid composition of muscles between two groups. The lysine deficiency reduced significantly (p<0.05) the FSR of m. longissimus dorsi but did not influence its FDR. The m. biceps femoris also presented an inhibited FSR while its FDR reduced only exhibited a very high tendency (p = 0.055) compared to the adequately-fed pigs. As for the m. masseter and m. adductor, both of the FSR and FDR were depressed significantly (p<0.05) by the lysine deficiency, and changes in the FSR were severer than those in the FDR, so that their FAR were significantly slower (p<0.05) in comparison with the control group. The lysine deficiency also inhibited the RNA translation activity of the muscles while the effects on RNA capacity were not significant (p>0.05). In conclusion, the FAR of muscle protein was changed by the current lysine deficiency through the alterations in the FSR and/or FDR.
