Psychiatric Evaluation of Maladaptive Male Conscripts in a Division of the Republic of Korean Army : Focus on Green Camp Participants (한국 육군 일개 사단에서 군 복무 부적응자들의 정신의학적 평가 : 그린캠프 참가자들을 중심으로)

  • Objectives : In this study, we aimed to elucidate the demographic and psychological characteristics of maladaptive soldiers in the Republic of Korean Army. Methods : Study participants included 110 male conscripts who had participated in the Green camp, which was a form of group psychosocial treatment program for maladaptive soldiers. All participants were interviewed and diagnosed by a psychiatrist, and classified into two groups according to whether they left or rejoined the army. We analyzed the differences between these two groups in terms of demographic features, psychiatric diagnosis and the Minnesota Multiphasic Personality Inventory (MMPI) subscores. Results : Compared with soldiers who rejoined their units, those who left military service exhibited significantly lower educational level (p=0.041) and a higher rate in psychiatric treatment prior to enlistment (p=0.011). Among the 106 subjects, 63 (59.4%) and 23 (21.7%) were diagnosed with personality disorder and adjustment disorder, respectively. Further, those who left military service were diagnosed more frequently with mood disorder (p=0.001) and schizophrenia (p=0.014) than those who rejoined their units. Additionally, the MMPI scores of soldiers who left military service were significantly higher on the psychasthenia (p=0.028) and schizophrenia (p=0.039) scales than those of soldiers who rejoined their units. Conclusion : Most of the maladaptive soldiers were diagnosed with mental disorders. The results of this study suggest that systematic, consistent psychiatric evaluation and intervention for maladaptive male conscripts is crucial in the Republic of Korean army.

Effect of Three Different Species of Hay on Dry-matter Intake and Serum Cortisol of Ewes

  • To determine whether the feeding of different species of hay affects the dry-matter intake and the serum cortisol level of sheep, 6 non-pregnant, non-lactating ewes were offered alfalfa (Medicago sativa) hay (Al), oats (Avena sativa) hay (Ot) and perennial ryegrass (Lolium perenne) hay (Pr) under 6 kinds of treatment including 1) change from Al to Ot, 2) Al to Pr, 3) Ot to Al, 4) Ot to Pr, 5) Pr to Al, and 6) Pr to Ot. The experimental design was a 6${\times}$6 Latin square with a 14 day period of which 9 days were a preliminary period and 5 days as a digestion trial period. The change in hay feeding was done abruptly on the first day of each period without an adaptation period. The blood from the jugular vein was collected on the first, third and the last day of each period through a sterile catheter for the analysis of cortisol. The dry-matter intake was recorded daily throughout the period. The mean daily intake of dry matter (DM, g/kg live weight) was significantly different among the 3 species of hay (Al>Ot>Pr; p<0.05). The digestibility of DM for Al and Ot was the same, but that for Pr was significantly lower than Al and Ot (p<0.05). The mean serum cortisol levels were significantly different among the hays (p<0.05). The level for Pr was the highest and that for Al was the lowest. The abrupt change of hay feeding of 6 treatments produced a significant change in the serum cortisol levels. The DM intake was inversely related to the change of the cortisol level. It is suggested that the animal' intake response to different species of hay may be partly motivated by the psychological feelings toward the hay offered.

Focus Group Study on Psychosocial Distress of Cancer Patients (치료 중인 암 환자들의 심리사회적 어려움에 관한 포커스 그룹 연구)

  • Purpose: The purpose of the study was to describe psychosocial difficulties experienced by cancer patients. Methods: Three focus group interviews were conducted to collect the data from 19 outpatients with cancer undergoing treatments during 2009. All interviews were audio-recorded and transcribed verbatim. The data were analyzed using constant comparative analysis of grounded theory. Results: As a result of constant comparative analysis, 'psychosocial distress' was identified as a core category, and seven subcategories were identified. 'Plunged into negative emotions' and 'damaged self-identity' were identified as major categories in the individual dimension. 'Difficulty in dealing with diagnosis' and 'hasty expectations and concerns' were identified in the dimension of family. 'Inefficient communication' and 'lack of necessary information' were identified in the dimension of health care setting. 'Lonely journey into a strange territory' was identified in the dimension of society. Conclusion: The results of the study indicate that cancer patients experience a wide range of psychosocial problems. Thus, in assessing and relieving psychosocial distress of cancer patients, it is necessary to focus not only on the level of the individual but also on the levels of family, health care setting, and society in general is necessary.

Nurses' Experience of Middle East Respiratory Syndrome Patients Care (메르스 환자 간호에 대한 간호사의 경험)

  • This study was conducted to investigate the practical experience of nurses in MERS patients care. Nine nurses in one general hospital in S city underwent in-depth interviews from August 2015 to March 2016. The collected data were analyzed according to the procedure of the phenomenological method of Colaizzi. The following five categories were derived: '1. Anxiety and burden due to the risk of transmission of new infectious diseases', '2. Social isolation because of being the nurse in charge of MERS patients', '3. Faced with problems due to an unprepared treatment environment', '4. Overcome the burdensome MERS patient care process', '5. Reflect on the steps for preparing against new infectious diseases'. These results suggest that it is important to draw positive experiences as well as the negative effects of nurses who are involved in treatment of various infectious diseases, including MERS. Based on this, it can be used as a basis for reducing negative psychological and social impacts and improving positive adaptation. Additionally, it is necessary for healthcare workers including nurses to establish the necessary workforces, departments and guidelines for nursing new infectious disease patients.

The Effects of a Comprehensive Education Program on Knowledge, Self-Efficacy, and Coping Style among Newly Diagnosed Patients with Breast Cancer (통합적 교육프로그램이 진단 직후 유방암 환자의 지식, 자기효능감 및 대응방식에 미치는 효과)

  • Purpose: The purpose of the study was to identify the effect of a comprehensive education program on knowledge, self-efficacy for self-management, and coping style among newly diagnosed patients with breast cancer. Methods: This study used a one group pretest-posttest design. Data were collected from April to September, 2010 at a cancer-specific hospital in Goyang, Korea. One hundred forty three women received a comprehensive education program consisting of 3 sessions composed of 8 themes was conducted by several experts (nurses, doctors, dietitians and clinical psychologists) for 3.5 hours a day for 3 days (1 day before and 2 days after the operation). Results: This study showed that knowledge and self-efficacy for self-management significantly increased after education (p<.001). In terms of coping style, however, only the subscale, 'anxious preoccupation', significantly improved (p=.001) among 5 sub-scales of coping style after education. Conclusion: A comprehensive education program for newly diagnosed patients with breast cancer was effective for increasing the knowledge and self-efficacy for self-management of breast cancer, in addition to decreasing the coping style of 'anxious preoccupation'. However, it did not improve other coping style. Therefore, other strategies for improving coping style are needed.

Estimation of Carbon Uptake for Urban Green Space: A Case of Seoul (도시 녹지 가치 평가를 위한 탄소 흡수량 추정 - 서울시를 대상으로 -)

  • Urban green space is often at the centre of the debate on urban substantiality because it provides functions of space, e.g. for wildlife, recreation, growing vegetables, psychological wellbeing, social interaction, etc. Traditionally, the various functions of urban green spaces clearly show that green spaces contain important values that contribute to the overall quality of urban life. After Kyoto protocol, it has becoming important to more accurately evaluate carbon uptake by urban green space. Many studies have analyzed the benefits, costs, and carbon storage capacity associated with urban green space. These studies have been limited by a lack of research on urban tree biomass and carbon uptake by soil, such that estimates of carbon storage in urban systems. This study calculate more accurately the amount of carbon uptake by urban green space. This study also complement the existing methods to estimate the urban green space carbon uptake. It has been studied how to evaluate carbon uptake function of urban green space. The surface area of urban green space increased 5% by complemented method and carbon uptake is also increased. Based on this result, the carbon uptake per capita was analysed and compared to the area of carbon uptake. And this study discussed the reasons for the differences between the new and earlier estimates, as well as implications for our understanding of the global carbon cycle. In conclusion, these results could contribute as preliminary data to policy makers when climate change adaptation strategy is established.

Penile Reconstruction Using a Radial Forearm Free Flap with Modified Biemer's Method (변형 Biemer 방법에 의한 요골부위 전박 유리피판을 이용한 음경재건)

  • As the defects of the penis caused by trauma, surgical amputation, or congenital abnormality give the patients both psychological trauma and functional impairment, reconstruction of the penis is mandatory. Radial forearm free flap is reliable one-stage procedure, which can reconstruct both the phallus and the urethra. Chang and Whang's adaptation of the "tube-in-a-tube" concept and its incorporation into a free flap design represented a major advance in microsurgical phallic construction. Biemer described a modification of the radial forearm flap design in which the neourethra was centered over the radial artery, but the phallic shaft was separated into two paraurethral swatches. The authors have performed one-stage penile reconstruction in two patients since 1998, using a radial forearm free flap. Our present design incorporates the original Biemer triple skin island and includes a fourth distal island for neoglans. One case was the amputation of the penis from felonious assault and the other case was the iatrogenic penile amputation from repetitive urologic surgery for congenital hypospadia. All patients showed aesthetically acceptable results and good tactile sensory recovery. Severe complications such as necrosis, fistula, or urethral stricture were not occurred. Biemer's method modified by the authors is reliable one-stage penile reconstruction providing good aesthetic and functional results.

Overcoming Experiences of Family Members Caring for Elderly Patients with Dementia at Home (재가 치매 노인환자를 돌보는 가족원의 극복 경험)

  • Purpose: The purpose of the study was to understand and describe the overcoming experiences of family members caring for elderly patients with dementia at home. Methods: Data came from autobiographies on the overcoming experiences of caregiving from 31 participants, who had submitted the autobiographies to a public contest held by the Seoul Metropolitan Center for Dementia in 2012. Data were analyzed using qualitative content analysis. Results: Four overcoming stages emerged from the analysis: confronting stage; challenging stage; integrating stage; and transcendental stage, representing transformation of experiences from frustration and suffering to happiness and new hope in life. The confronting stage illustrates severe negative feelings and exhaustion occurring after the diagnosis of dementia. The challenging stage signifies major driving forces in taking good care of their patients. It includes tender loving memories about the patients as well as family and social supports. The integrating stage shows genuine empathy for the patients' situation and the happiness of 'here and now', while the transcendental stage represents new hope in the future. Conclusion: Health professionals need to support caregivers to find true meaning of caring and happiness in everyday life, while providing specific information on dementia care and relieving various negative feelings.

An Exploratory Research on Individual Differences of Technostress: In Convergence Age (융복합 시대에서 테크노 스트레스의 개인별 차이에 대한 연구)

  • Technostress- a term coined by Craig Brod- refers to a modern disease of adaptation caused by an inability to cope with new technologies. Stress is a person's adaptive response to a stimulus that places excessive psychological or physical demands on that person. In a similar vein, technostress can affect a variety of side effect such as low work productivity and resistance against new technology. The purpose of this study is thus to investigate of differences of technostressors using demographic informations such as gender, age, computer use skill, and industrial types. To do this, we conducted independence t-test and analysis of variance. We found that demographic characteristics can affect the differences of technostressors. Conclusions and implications are discussed in the last section of this paper.

Blindness Experience of Family of Persons with Unilateral Acquired Blindness (일측 중도시각장애인과 가족의 체험연구)

  • Purpose: The purpose of this study is to discover the nature from the life experience of a person with unilateral acquired blindness and his/her family after losing the eyesight and adapting in the environment and to find the meaning of life and how to solve the problem in psychosocial aspect. Methods: This study uses one of the qualitative research methods which explains how families with the unilateral acquired blind perceive blindness after experiencing it and observes how they signify it. starts with interest in lifestyles of individuals and their families and tries to understand the subjective existences of participants in accessible ways and draw the experiences after becoming one-side blind. It cyclically uses deductive verification process through inductive method and establishing hypothesis using materials. Results: According to the results of this study, unilateral acquired blindness studies, due to shattered life, they did not know what to do. Also, discomfort from struggling in a big tunnel and even will to live were found. trying to go out to the world, seeing the new world, and trying to encourage myself, strong attachment to life was shown to by saying, appeared. Each includes sub-topics such as feeling abandoned after confirmed the blindness, feeling disappointed to doctors, family, and friends, trying to live with hope, struggling in a tunnel with thinking how to live, closing the mind from the world, seeing outside the world in the midst of struggling, trying to forget the past with the will of life, having hope to live with care of family, and trying to keep the rest vision. Conclusion: Firstly, in nursing aspect for their adaptation, programs for disable people and nursing intervention focused on their families should be developed. Secondly, since it can be economic and psychological burden for their families and acquaintances, it is necessary to support the blind so that they can find fitted rehabilitation programs and come back to society. Thirdly, active participation of health care providers may influence social interest the improvement of national welfare policy for the unilateral acquired blind.
