소아에서 피하매몰 중심정맥포트가 전복된 사례에 대한 경험 - 2예 보고 - (Inversion of Implantable Central Venous Port in Children - 2 Cases Report -)

  • A 3-year-old girl with a primitive neuroectodermal tumor (PNET) and a 6-yearold girl with acute lymphoid leukemia were referred to us because of problems with their implantable central venous ports (Port-A-Cath$^{(R)}$). On physical examination, the ports were upside-down, so a needle could notbe inserted through the membrane of the port. Right lateral side view of the chest radiogram confirmed port inversion in both cases. At operation, the ports were inverted and the transfixing sutures were totally absorbed. The ports were rotated 180 degrees and anchoring sutures placed.

Induction of cell death by 2,4,3',5'-tetramethoxystilbene in human acute promyelocytic leukemia (HL-60) cells and its mechanism.

  • We have previously shown that 2,4,3',5'-tetramethoxystilbene (TMS), a synthetic trans-stilbene analogue, is one of the most potently selective inhibitor of human cytochrome P450 1B1 in vitro and in vivo. In the present studies, the apoptotic effects of TMS were investigated in HL-60 cells. The effects of TMS on the proliferation of HL-60 cells were determined with MTT assay. TMS exhibited cytotoxicity with an $IC_50$ value of 37 nM. Cotreatment with TMS and etoposide, a well-known anticancer drug significantly enhanced the cytotoxicity. (omitted)

  • L -ascorbic acid (LAA) was shown to modulate the in vitro growth of leukemic-colony forming cells from patients with acute myelogeneous leukemia (AML). Dendritic cells (DCs) were successfully cultured from the leukemic blasts by us and others. The effects of LAA on the ex vivo cultured leukemic-DC were studied. Plastic adherent cells from the leukemic blasts were cultured with GM-CSF and IL -4 (each 103 U/$m\ell$) with or without LAA (300 ${\mu}$M) for 7 days and harvested. (omitted)

  • Chronic invasive aspergillosis is a life-threatening disease, especially in immunocompromised patients. The diagnosis and treatment of tracheal aspergillosis (TA) are challenging because of its rarity and nonspecific clinical presentations. The treatment standard of TA has been medical treatment like other forms of invasive aspergillosis, but patients with medically resistant TA require surgical intervention. We demonstrated a successful surgical outcome of chronic invasive TA in a 16-year-old patient with immunocompromised status related to acute myelocytic leukemia.

  • This study is a randomized pretest-posttest design that aims to investigate the effect of early entrepreneurship education on cognitive and non- early entrepreneurship education, non-cognitive skills, creativity, self-efficacy, Bizworld cognitive skills of male sixth-grade primary school students. A total of 45 students were selected by multi-stage random sampling method and were assigned randomly to experimental, active-control and control groups. The experimental group received entrepreneurship education based on the Bizworld entrepreneurship program. The results indicate that early entrepreneurship education had an effect on non-cognitive skills (such as risk taking propensity, creativity, self-efficacy, persistence and need for achievement. It seems that early entrepreneurship education is a proper strategy to develop children's non-cognitive skills in late years of primary school. These skills will affect children's individual, educational, social and occupational future and can have long term benefits for students, families and society.

소아 급성 특발성 혈소판 감소성 자반증에서 골수흡인 검사의 적응 (Indication of Bone Marrow Aspiration in Acute Idiopathic Thrombocytopenic Purpura in Children)

  • 급성 특발성 혈소판 감소성 자반증은 소아에서 비교적 흔한 혈액학적 질환 중의 하나이다. 확진을 위하여 대개 골수흡인 검사를 시행하는데 이는 백혈병, 재생 불량성 빈혈과 같은 다른 심각한 혈액학적 질환을 배제하고 또한 스테로이드 치료를 고려할 경우 진단이 늦어 질 수 있다는 점 때문이다. 그러나 근래에는 전형적인 소견을 보이는 경우에 치료 시 면역글로부린 정주요법이 주로 사용되고 있어 골수흡인 검사의 필요성에 대하여 의문이 제기되고 있다. 본 연구는 이 질환을 의심하는 경우에 골수흡인 검사의 적절한 시행을 위한 적응증을 알아보고자 시행하였다. 1984년 1월부터 2000년 12월까지 영남대학교 의과대학 부속병원 소아과에 내원하여 급성 특발성 혈소판 감소증이 의심되어 골수흡인 검사를 시행하였던 3개월 이상 15세 이하의 환아 120명을 대상으로 내원시 진찰소견, 병력, 혈액학적 검사 소견, 말초혈액도말 검사소견을 기준으로 전형적인 경우와 비전형적인 경우로 구분하여 후향적으로 조사하였다. 급성 특발성 혈소판 감소성 자반증이 의심되어 골수흡인 검사를 시행한 환아 120예 중 전형적인 경우가 66예, 비전형적인 경우가 54예였다. 골수흡인 검사 결과 전형적인 경우는 66예 전예에서 급성 특발성 혈소판 감소성 자반증으로 확진되었으며, 비전형적인 경우는 급성 특발성 혈소판 감소성 자반증이 52예(96%), 재생 불량성 빈혈이 1예(2%), 골수 이형성증이 1예(2%)로 나타났으나 백혈병으로 진단된 경우는 없었다. 본 연구의 결과로 소아 급성 특발성 혈소판 감소성 자반증은 전형적인 병력, 진찰소견 및 말초혈액학적 검사 소견만으로도 진단이 가능하므로 확진을 위한 골수흡인 검사는 필요하지 않고 비전형적인 소견을 보이는 경우는 드물지만 혈소판 감소를 동반하는 다른 혈액학적 질환일 가능성도 있으므로 골수흡인 검사를 시행하여 확진을 하는 것이 바람직하다고 하겠다.
