다발성 탐침술을 이용한 견관절 급성 석회화 건염의 치료 (Treatment of Acute Calcific Tendinitis of the Shoulder with Barbotage)

  • Purpose : To analyze the outcome after barbotage and subacromial corticosteroid injection in acute pain attack with calcific tendinitis of the shoulder. Materials and Methods : Twenty-two patients with acute calcific tendinitis were analyzed with average follow-up of Twenty-two months. The shape and size of calcific deposits were classified. The outcome was assessed by UCLA shoulder score and pain in visual analogue scale(VAS). Results : Sixteen patients(73%) had permanent relief of pain. The results assessed by UCLA score were excellent in fourteen patients and good in two patients. Average VAS for pain was 0.6 ± 0.73 (range:0-2). No complication was enccpuntered, but six patients underwent surgical treatment due to lack of improvement or recurrence. Conclusion : Barbotage as a primary treatment in acute calcific tendinits of the shoulder is simple and effective, and therefore should be tried before surgical intervention.

  • Calcific tendinitis is the leading cause of shoulder pain. Among patients with calcific tendinitis, 2.7%-20% are asymptomatic, and 35%-45% of patients whose calcific deposits are inadvertently discovered develop shoulder pain. If symptoms are present, complications such as decreased range of motion of the shoulder joint should be minimized while managing pain. Patients with acute calcific tendinitis respond well to conservative treatment and rarely require surgery. In contrast, patients with chronic calcific tendinitis often do not respond to conservative treatment and do require surgery. Clinical improvement takes time, even after surgical treatment. This review article summarizes the processes related to the diagnosis and treatment of calcific tendinitis with the aim of helping clinicians choose appropriate treatment options for their patients.

  • Calcific tendinitis of the shoulder joint is common disease causing acute pain, mainly involving the supraspinatus or infraspinatus muscle, and less frequently the teres minor or subscapularis muscle. This study reports on the satisfactory arthroscopic removal of calcium deposits as well as infraspinatus and supraspinatus muscle repair without relapse via minimal incision using suture anchors. This was a case of atypically extensive calcific tendinitis involving the infraspinatus muscle, with a bursal side partial rupture of the supraspinatus muscle in a 61-year-old female whose chief complaint was chronic pain of the right shoulder exacerbated by limited movement.

  • Differential diagnosis of posterior neck pain is very challenging based on symptoms and physical examination only. Retropharyngeal calcific tendinitis is a rare and frequently misdiagnosed entity in various causes of neck pain. It results from calcium hydroxyapatite deposition in the longus colli muscle which is characterized by severe neck pain, painful restriction of neck movement, dysphagia, and odynophagia. We herein report a case of a patient with acute retropharyngeal calcific tendinitis, who complained of posterior neck pain, initially diagnosed and treated as a myofascial neck pain syndrome.

골내 침범을 동반한 견관절 석회화 건염 - 증례 보고 - (Calcific Tendinitis of Shoulder Associated with Intraosseous Involvement -A Case Report-)

  • 목적: 골내 침범이 동반된 비전형적인 견관절 석회화 건염 1예를 보고하고자 한다. 대상 및 방법: 만성적인 좌측 견관절 동통의 급성 악화와 운동 제한을 주소로 내원한 59세 여자 환자로 단순 방사선 상 견봉하 석회 침착과 자기 공명 영상 상 감염이나 종양과 유사하게 골 피질을 침범하고 석회 물질이 골 내부로 연장되어 있었다. 관절경하 석회 제거술과 골내 석회의 소파술, 건 결손부에 봉합 나사못을 이용한 회전근 개 봉합술을 시행하였다. 결과: 환자의 급성 견관절 통증은 술 후 즉각적인 호전을 보였고 순조로운 재활 경과와 관절 운동범위의 회복을 보였다. 술 후 5개월에 촬영한 단순 방사선 상 석회 침착의 재발은 관찰되지 않았다. 결론: 견관절 석회화 건염은 주위 골조직으로 연장된 다양한 영상 소견을 나타 낼 수 있으며, 질환을 정확히 판별함으로써 불필요한 검사나 치료를 피할 수 있을 것으로 생각된다.

  • Acute calcific tendinitis (ACT) is a benign painful inflammatory disorder characterized by resorptive process of calcific deposits following the formation of calcium hydroxyapatite crystals in the tendons. It can occur at various sites, especially in the shoulder or hip joint. ACT involving the lateral epicondyle of the humerus and the cervical spine is very rare. Few reports have demonstrated successive ACT at different sites. We report three cases of successive ACT in women, occurring at the subscapularis followed by the lateral epicondyle, flexor carpi ulnaris followed by the supraspinatus, and longus colli followed by the iliopsoas, respectively.

  • 고관절 주변에 발생하는 석회화 건염은 그 발생빈도가 극히 드믈지만 증상이 발생하면 매우 심한 통증으로 일상 생활이 힘들어진다. 저자들은 급성 고관절 통증과 관절 운동 장애를 호소하는 환자 4례에서 석회화 건염을 진단하고 초음파 감시하 다발성 천공술을 시행하여 만족할 만한 결과를 경험하였기에 이를 보고하고자 한다.

Arthroscopic Treatment for Calcific Tendinitis of Origin of Long Head of Triceps

  • A 55-year-old female experienced acute left shoulder pain without specific trauma. Radiography showed calcific deposits in the inferior part of the glenoid fossa. Magnetic resonance arthrography showed calcific deposits in the origin of the long head of triceps brachii muscle. Conservative treatment failed to resolve the symptoms; therefore, arthroscopic surgery was performed. The patient experienced immediate and dramatic pain relief, and normal shoulder motion was demonstrated 1 year after surgery. In conclusion, although rare, calcific tendinitis of the triceps brachii muscle, which causes shoulder pain, should be included in the differential diagnosis of acute shoulder pain. Arthroscopic surgery is a treatment option for chronic cases and those resistant to conservative treatment.

  • Painful periarticular calcification most commonly occurs within the rotator cuff of the shoulder and rarely around the elbow, hip, foot, and neck. As acute inflammatory reaction develops, severe pain, exquisite tenderness, local swelling, and limitation of motion with pain occur. In case of calcific tendinitis of the shoulder, it can be easily diagnosed according to the symptoms and with x-ray. However, in lesions of the hip, as it is a rare location and usually involves pain in the posterolateral aspect of the thigh, which can simulate radicular pain from a lumbar intervertebral disc, it could be difficult to diagnose. Hence, physicians usually focus on lumbar lesions; therefore, misdiagnosis is common and leads to a delayed management. Here, we report the case of a 30-year-old female patient with calcific tendinitis of the rectus femoris that was successfully managed with ultrasound-guided steroid injection. This study offers knowledge about the rectus femoris calcification.

초음파를 이용한 급성 극상근 석회화 건염의 스테로이드 국소 주사 요법 (Ultrasound-Guided Local Steroid Injection Therapy for Acute Calcific Tendinitis of Shoulder)

  • 목적: 견관절의 극상건에 발생한 급성 석회화 건염에 대하여 초음파를 이용한 스테로이드 국소 주사 요법을 시행하고 임상적 결과를 알아보고자 하였다. 대상 및 방법: 2007년 1월부터 2009년 4월까지 증상이 있는 50세에서 70세 사이의 견관절 극상건의 급성 석회화 건염 20례를 대상으로 하였으며, 평균 연령은 58.2세, 평균 추시 기간은 18개월(12개월~24개월)이었으며, 우측이 18례이며, 모두 극상건에 칼슘 침착이 있었다. 석회화 부위에 5-12MHz 고해상도 초음파 영상을 이용하여 18게이지 바늘로 다발성 천공을 한 후 건 주변에 2% lidocaine 1cc와 depomedrol 40mg/ml 1cc를 혼합한 국소주사를 시행하였다. 치료효과의 평가 항목으로 UCLA견관절 평가 양식, 운동 범위, VAS 점수를 이용하였다. 통계학적 검정은 SAS를 이용하여 ANOVA와 Tukey's post-hoc test로 유의 수준 5%에서 유의성을 조사하였다. 결과: 주사 직 후 10분이내에 전예에서 압통점이 소실되었으며, UCLA견관절 평가 양식에서 시술 전에는 15례에서 보통(fair), 5례에서 불량(poor)의 양상을 보였으나, 시술 후 16례에서 우수(excellent), 4례에서 양호(good)의 결과를 보였으며 보통(fair)이나 불량은(poor)은 관찰되지 않았다. 시술 후 전례가 양호 이상의 결과를 보였고, 운동 기능의 제한은 없었으며, VAS 점수는 시술 전 평균 8.9(8~10)이었으나 시술 후 평균 0.5(1~2)로 유의하게 향상되었다(p<0.0001). 합병증은 1례도 없었다. 결론: 초음파를 이용한 다발성 천공술 후 스테로이드 국소주사 요법은 극상근 석회화 건염의 급성 통증기에 이용할 수 있는 유용한 치료 방법 중의 하나이다.
