Abdominal Actinomycosis Mimicking Acute Appendicitis in Children: a Case Report (급성 충수염처럼 보인 복부 방선균증 1예)

  • Actinomyces are anaerobic, Gram-positive bacteria that are part of the endogenous flora of mucous membranes in humans. Infection caused by these bacteria is termed actinomycosis. The 3 most common types of actinomycosis are cervicofacial, abdominopelvic, and pulmonary. A previously healthy 6-year-old boy presented with the emergency room with fever, vomiting, and abdominal pain and initially diagnosed with acute appendicitis. Exploratory laparoscopy was done. Histologic finding demonstrated acute gangrenous appendicitis complicated by perforation and sulfur granules compatible with actinomycosis. Subsequently, he was diagnosed with abdominal actinomycosis and received long-term antibiotic therapy. Abdominal actinomycosis is uncommon in children and difficult to diagnose because of its nonspecific symptoms and of difficulties in growing Actinomyces in the clinical setting. It is necessary to include abdominal actinomycosis as a differential diagnosis of children presenting with abdominal pain.

Embolectomy in Peripheral Arteries: 3 Cases (말초동맥 전색에 대한 혈전 제거술 (3례))

  • This is a report of three cases of successful embolectomy in peripheral arteries. First case was the patient who received a mitral commissurotomy 8 months ago. In that time, there was no evidence of left atrial thrombosis. He showed an embolism in the middle portion of left brachial artery without complaining of any ischemic pain. Embolectomy was performed 15 days after disappearance of radial pulse and resulted in no return of radial pulse postoperatively. Second case was a case of an embolism in lower portion of right brachial artery. She complained severe ischemic pain and cyanosis in the right forearm and fingers. She was also in the beginning state of cardiac failure, which was suspected from her hypertension associated with cardiomegaly and arrythmia Embolectomy was performed 17 hours after onset of acute pain. Immediate full pulsation of radial artery was obtained after embolectomy and the acute ischemic symptoms subsided gradually. Third case was an embolism in superior mesenteric artery which occured 24 hours after pneumonectomy for right bronchogenic carcinoma and the patient suddenly complained diffuse abdominal colicky pain. 7 hours after attack of abdominal pain. embolectomy with extensive reset ion of the small intestine was performed with uneventful recovery and without complication, such as short bowel syndrome, postoperatively. Histopathologically, the embolus was consisted of a tissue of anaplastic cell carcinoma, which was identical to the tumor of the resected right lung. Histological findings of other emboli of first and second case were old thrombus.

The Effect of Gyejakjimo-tang on c-Fos Expression in Mice Model of Acute Pain (계지작약지모탕(桂枝芍藥知母湯)이 Acetic Acid에 의한 통증에 미치는 영향)

  • Objectives : We want to know the effect of Gyejakjimo-tang in mice model of acute pain.Methods : We investigated writhing reflex in mice with acetic acid-induced abdominal pain using mice, we observed c-Fos protien expression by immunohistochemistry dyeing method in the paraventricular nucleus(PVN) and supraoptic nucleus(SON) of the hypothalamus.Results : All of Gyejakjimo-tang treated group suppressed acetic acid-induced writhing response as acetic acid injuction group, but in 100㎎/㎏ Gyejakjimo-tang treated group and 200㎎/㎏ Gyejakjimo-tang treated groups represernted significance. All of Gyejakjimo-tang treated groups(50, 100 and 200㎎/㎏ Gyejakjimo-tang-treated group), Fos-positive cells in PNV significantly decreased as acetic acid injuction group, and All of Gyejakjimo-tang-treated groups Fos-positive cells in SON significantly decreased as acetic acid injuction group.Conclusions : The present results showed that the mice pre-treated with the aqueous extract of Gyejakjimo-tang showed analgesic effect on acetic acid-induced abdominal pain.

Co-Infection with Cytomegalovirus and Helicobacter pylori in a Child with $M\acute{e}n\acute{e}$trier's Disease

  • $M\acute{e}n\acute{e}$trier's disease is a rare protein-losing gastropathy characterized by hypertrophic gastric fold, foveolar hyperplasia, and hypoproteinemia with resulting peripheral edema. It is clinically evident as nonspecific gastrointestinal symptoms, including abdominal discomfort, nausea and vomiting, abdominal pain, weight loss, diarrhea, and edema. Pediatric $M\acute{e}n\acute{e}$trier's disease usually has an insidious onset and progressive, chronic clinical course and it spontaneously resolves in weeks or months. The pathogenesis of $M\acute{e}n\acute{e}$trier's disease is not clearly understood. $M\acute{e}n\acute{e}$trier's disease is thought to be associated with some gastric infections. But the cause of $M\acute{e}n\acute{e}$trier's disease is unknown, an association with cytomegalovirus (CMV) and Helicobacter pylori has been suggested. In Korea, We present the first a case of pediatric $M\acute{e}n\acute{e}$trier's disease with positive evidence of CMV and H. pylori.

  • Thalamic pain is a primary cause of central post-stroke pain (CPSP). Clinical symptoms vary depending on the location of the infarction and frequently accompany several pain symptoms. Therefore, correct diagnosis and proper examination are not easy. We report a case of CPSP due to a left acute thalamic infarction with central disc protrusion at C5-6. A 45-year-old-male patient experiencing a tingling sensation in his right arm was referred to our pain clinic under the diagnosis of cervical disc herniation. This patient also complained of right cramp-like abdominal pain. After further evaluations, he was diagnosed with an acute thalamic infarction. Therefore detailed history taking should be performed and examiners should always be aware of other symptoms that could suggest a more dangerous disease.

  • Abdominal cystic lymphangiomas arising from the mesentery, omentum and retroperitoneum are rare and occasionally confused with other cause of acute abdomen. Sixteen children who underwent surgery for abdominal cystic lymphangioma between 1984 and 2005 at the Division of Pediatric Surgery, Keimyung University Dongsan Medical Center were evaluated retrospectively. There were 9 boys and 7 girls. Age ranged from 12 days to 13 years (mean age: 4.7years). The cysts were located in the omentum (4 cases),mesentery (9 cases: jejunum: 3, colon: 6). retroperitoneum (2 cases) and pelvic cavity (one case). The cyst content was hemorrhagic (8 cases), serous (5 cases), and inflammatory (2 cases), and chyle (one). Prenatal diagnosis was made in 3. The clinical features were variable, but abdominal pain, mass, and abdominal distention in order. There were 3 emergency operations in patients with complicated cyst, who were suspected of having panperitonitis and volvulus preoperatively. Complete excision was accomplished in all cases. There was one mortalityin a newborn with E.coli sepsis. Intestinal obstruction developed in 2 cases in long-term follow-up. No recurrence was observed. Although intraabdominal lymphangioma arising from mesentery, omentum and retroperitoneum are rare, it should be considered as a possible cause of acute abdomen.

Unilateral Lower Extremity Paralysis after Abdominal Hysterectomy under Continuous Epidural Anesthesia (지속적 경막외마취하의 복식자궁절제술후 발생한 편측 하지 마비 -증례 보고-)

  • Continuous epidural analgesia has been used widely for treatment of acute abdominal postoperative pain. Neurologic complications related to epidural analgesia occur infrequently but may be caused by various chemical, mechanical, ischemic or idiopathic factors. We report a case of abdominal hysterectomy in which unilateral lower extremity paralysis occurred after continuous epidural analgesia.

Acute pancreatitis in hand, foot and mouth disease caused by Coxsackievirus A16: case report

  • Coxsackievirus A16 (CA16), which primarily causes hand, foot, and mouth disease (HFMD), is associated with complications, such as encephalitis, acute flaccid paralysis, myocarditis, pericarditis, and shock. However, no case of pancreatitis associated with CA16 has been reported in children. We report a case of CA16-associated acute pancreatitis in a 3-year-old girl with HFMD. She was admitted because of poor oral intake and high fever for 1 day. Maculopapular rashes on both hands and feet and multiple vesicles on the soft palate were observed on physical examination. She was treated conservatively with intravenous fluids. On the fourth hospital day, she had severe abdominal pain and vomiting. The serum levels of amylase and lipase were remarkably elevated (amylase, 1,902 IU/L; reference range, 28-100 IU/L; lipase, >1,500 IU/L; reference range, 13-60 IU/L), and ultrasonography showed diffuse swelling of the pancreas with a small amount of ascites. The real-time reverse transcription polymerase chain reaction result from a stool sample was positive for CA16. CA16 can cause acute pancreatitis, and should be considered in the differential diagnosis of abdominal pain in children with HFMD.

  • 의사들은 환자들의 건강 상태와 관련하여 다양한 유형의 정보들을 수집하고 분석하여 개별적인 환자들의 진단을 내리게 된다. 의사들이 한 명의 환자와 관련된 다양한 정보로부터 질환을 결정 내리기까지에는 여러 단계에서 다양한 의사결정이 필요하며 매우 복잡한 과정을 거치게 된다. 그러므로 의사들에게 또는 환자들에게 보조적인 도움을 주고자 많은 의료진단 시스템들이 개발되었다. 현재까지 개발된 대부분의 의료 진단시스템들은 특정한 의사의 경험이나 한 유형의 질환에 고정되어 있다. 그래서 환자들이 급성복통과 같은 여러 가지 유형의 질환에 관련되어 있는 증상을 호소할 때 의사들이 적절한 의사결정을 내리기가 쉽지 않다. 본 논문에서는 급성복통과 관련된 여러 가지 유형의 질환을 진단할 수 있는 시스템을 퍼지 논리를 이용하여 설계하고 구현해 본다.

  • 사례 기반 추론(case-based reasoning)은 현재의 문제를 해결하기 위해서 과거에 유사하게 수행된적이 있는 사례를 유추하여, 유추된 사례의 해를 이용하는 기법으로서 규칙 기반 추론과 함께 여러분야에 응용되고 있다. 하지만 사례기반 추론 시 새로운 사례를 해결하기 위하여 사례베이스 안의 모든 사례를 검색해야 하기 때문에 수행시간이 증가되는 문제점을 지니고 있다. 본 연구에서는 규칙 및 K-Means 클러스터링 알고리즘에 의한 사례 기반 추론을 이용한 ADS-DAAP(Advanced Diagnosis System for Diseases associated with Acute Abdominal Pain)를 제안한다. 제안하는 시스템은 기존의 CDS-DAAP(Combined Diagnosis System for Diseases associated with Acute Abdominal Pain)와 비교해 볼 때, 수행시간을 감소시켰다.
