  • Tremor is an most common abnormal movements disease which is to-and-fro shaking. It is caused by problems -with the nerves supplying certain muscles. It can affect the whole body or just certain areas. Parkinson's disease, Brain disease like injury of Frontal lobe, Cerebellum and Brainstem, drug and alcohol caused this symptom. It could be occured by physiological and Essential cause. In this paper, authors report three cases who showed tremor after Cerebral infarction. And we suggest that 'Ssanghwa-Tang' should be helpful for tremor patients.

  • We report a case of acute intermittent porphyria presenting with variable symptoms and signs such as hypertention, polyneuropathy, syndrome of inappropriate secretion of antidiuretic hormone and cerebral infarction. A 47 year-old female patient entered hospital with abdominal pain followed by generalized seizure. She was diagnosed to have acute intermittent porphyria in consequence of Watson-Schwartz test and ${\delta}$-ALA in 24 hours urine, but died of respiratory failure.

Neuroprotective Effect of Resveratrol on Acute Brain Ischemia Reperfusion Injury by Measuring Annexin V, p53, Bcl-2 Levels in Rats

  • Background : Cerebral ischemia is as a result of insufficient cerebral blood flow for cerebral metabolic functions. Resveratrol is a natural phytoalexin that can be extracted from grape's skin and had potent role in treating the cerebral ischemia. Apoptosis, a genetically programmed cellular event which occurs after ischemia and leads to biochemical and morphological changes in cells. There are some useful markers for apoptosis like Bcl-2, bax, and p53. The last reports, researchers verify the apoptosis with early markers like Annexin V. Methods : We preferred in this experimental study a model of global cerebral infarction which was induced by bilateral common carotid artery occlusion method. Rats were randomly divided into 4 groups : sham, ischemia-reperfusion (I/R), I/R plus 20 mg/kg resveratrol and I/R plus 40 mg/kg resveratrol. Statistical analysis was performed using Sigmastat 3.5 ve IBM SPSS Statistics 20. We considered a result significant when p<0.001. Results : After administration of resveratrol, Bcl-2 and Annexin levels were significantly increased (p<0.001). Depending on the dose of resveratrol, Bcl2 levels increased, p53 levels decreased but Annexin V did not effected. P53 levels were significantly increased in ishemia group, so apoptosis is higher compared to other groups. Conclusion : In the acute period, Annexin V levels misleading us because the apoptotic cell counts could not reach a certain level. Therefore we should support our results with bcl-2 and p53.

Awake craniotomy removal of a corticospinal tract developmental venous anomaly hemorrhage: A case report

  • Developmental venous anomalies (DVAs) are composed of mature venous vessels that lack malformed or neoplastic elements. Although the hemorrhage risk is considered negligible, some patients may have neurological symptoms attributable to acute infarction or intracranial hemorrhage secondary to thrombosis, in the absence of a coexisting cavernous malformation. We report the case of a 42-year-old patient who presented with acute left-hand paresis secondary to a subcortical hemorrhage. This bleeding originated from a DVA in the corticospinal tract area and was surgically drained through an awake craniotomy. To accomplish this, we used a trans-precentral sulcus approach. After the complete removal of the coagulum, small venous channels appeared, which were coagulated. No associated cavernoma was found. Although the main DVA trunk was left patent, no signs of ischemia or venous infarction were observed after coagulating the small venous channels found inside the hematoma cavity. Two weeks after the procedure, the patient's hand function improved, and he was able to resume desktop work. DVA-associated hemorrhage within the cortico-spinal tract could be safely removed with modern awake mapping techniques. This technique allowed the patient to rapidly improve his hand function.

Acute cerebral infarction associated with thrombocytopenia in primary Sjogren's syndrome : A Case Report (저혈소판증을 동반한 급성 대뇌경색을 보인 원발성 쇼그렌 증후군 1례)

  • Sjogren's syndrome is an autoimmune disease characterized by dry mouth and neutropenia. Although it does not commonly involve the central nervous system, Sjogren's syndrome sometimes affects small vessels through microangiopathic alterations. A 34-year-old woman was hospitalized for left upper quadrantanopia and a tingling sensation in the left hemibody. Brain magnetic resonance imaging revealed acute infarction in the right posterior cerebral artery territory. In laboratory tests, antinuclear (FANA2+) and anti-DNA antibodies (anti-SS-A (Ro)) were detected. Salivary gland scintigraphy revealed moderately decreasedexcretion of saliva. Based on these findings, we concluded she had Sjogren's syndrome. As in this patient, large vessel involvement in Sjogren's syndrome is far less common. Furthermore, it is difficult to administer antiplatelet drugsto patients with thrombocytopenia in Sjogren's syndrome. This is a case of the patient with Sjogren's syndrome that involved thrombocytopenia and large vessel invasion who was treated with antiplatelet drugs and hydroxychloroquine.

Arteriovenous Fistula on Forehead after Autologous Fat Injection: A Case Report (이마부위 자가 지방이식 후 발생한 동정맥 샛길 치험례)

  • Purpose: Autologous fat injection is ideal for patients who wish to add contour, projection and gross volumization of the aging, atrophic face and is claimed to be a safe procedure. However, there are several case reports in the literature where patients have suffered from acute visual loss and cerebral infarction after facial fat injection. This paper will explore a rare case of vessel related complication, an arteriovenous fistula that occurs after fat injection on forehead. Method: A 28 - year - old female who showed a non - tender, soft $1.0{\times}2.5cm$ sized mass on forehead for 3 weeks. A thrill could be detected on the totuous dilatated vessel - like structure around the mass. She had a fat injection on forehead for soft tissue augmentation 3 months prior to developing the mass. 3 - dimensional brain CT angiography showed arteriovenous fistula. Results: The fistula is totally excised with ligation of feeding vessels. Pathology report showed an atypical vessel which had intimal thickening, myxoid degeneration and thrombus formation. There were no evidences of recurrence at least for 2 months of follow - up. Conclusion: An occurrence of arteriovenous fistula after autologous fat injection is very rare. After perforation of artery and vein by coincidence, blood extravasates with the formation of a hematoma capsule and a pseudocapsule around it. The hematoma capsule would expand and clot would reabsorb resulting in a cavity leading to fistula formation. Other vessel related complications like acute visual loss or cerebral infarction are very severe. Therefore, surgeons should be cautious during facial fat injection to avoid vessel injuries.

The Relationship between Oriental Medical Diagnosis and Arteriosclerosis by Carotid-Ankle Vascular Index(CAVI) in Acute Cerebral Infarction Patients (CAVI를 이용한 급성기 중풍환자의 Arteriosclerosis와 한방변증의 관련성 연구)

  • Objectives : This study aimed to clarify the relationship between the Oriental medical diagnosis and arteriosclerosis by measuring carotid-ankle vascular index(CAVI) in acute cerebral infarction patients. Method : One hundred thirty-one subjects were recruited from the patients admitted to the Internal Medical Department at Kyunghee University Oriental Medical Center from April 2007 to August 2008. We sorted cerebral infarction patients and assessed one hundred fourteen patients' CAVI data. We diagnosed dampness-phlegm by Oriental medical diagnosis and evaluated stroke type by single or multiple infarctions. then, we analyzed their characteristics with type of stroke, risk factor, lifestyle, metabolic syndrome and dampness-phlegm diagnosis. Result : 1. On the demographic variables of the patients, age, hypertension, hyperlipidemia, multiple infarction group and metabolic syndrome and dampness-phlegm group were significantly higher in the high CAVI score group than in the control. 2. According to the significant difference in the dampness-phlegm group, we analyzed dampness-phlegm related index for pattern identifications by CAVI score. As a result, dark circles, insomnia, headache, white coating tongue. slippery pulse, and rough pulse were significantly higher in the high CAVI score group then in the control. 3. In multivariate analysis, age, hypertension, multiple infarction and dampness-phlegm groups showed a close relationship with the high CAVI score group. Conclusions : According to the analysis, significance between dampness-phlegm diagnosed patients group and high CAVI score were clarified. Moreover, multiple location infarctions also have a relationship with high CAVI score in cerebral infarction patients. These results can be utilized in the future as a basis material.

Performance of Activities of Daily Living in Adult Hemiplegic Patients (성인 편마비 환자의 일상생활동작 수행능력 평가)

  • The purpose of study was that the functional gains achieved by stroke patients after rehabilitation in the acute phase have been documented. Sixty stroke inpatients who were at least 3.57 month postonset participated;32 males and 28 females;mean age, 57.4 years. By the classification of diagnosis, 20 subjects were cerebral infarction and 27 subjects were cerebral hemorrhage. Activities of daily living(ADL) performance was investigated using the Klein-Bell ADL Scale. ADL test was practiced within physical therapy room. The outcome variables were first analyzed using frequency statistics. The functional gains of the patients with sex and hemiplegic site were then using an ANOVA. Mean score of dressing, elimination, mobility, bathing/hygiene, eating, and telephone use, 26.6, 56.0, 41.4, 36.5, 82.0, and 56.7. Mean score of total, 43.0. There was no significant difference in sex and hemiplegic site.

Change of Magnetic Motor Evoked Potentials in Hemiparesis due to Cerebral Infarction (뇌경색에 의한 편측부전마비에서 자기운동유발전위의 변동)

  • Background and Objectives : The Motor evoked potentials (MEP) study may be useful in the evaluation of the degree of impairment in the motor nervous system and in the determination of the prognosis. The purpose of this study is to evaluate the status of central nervous system in acute and subacute state of cerebral ischemia by comparing the changes of MEP in the initial and follow-up study. Methods : Twenty patients with hemiparesis caused by ischemic stroke were recruited for this study. We tested MEP within 7 days and followed-up after 14 days after symptom onset. The cerebral motor cortex area, cervical area for upper extremity and lumbar area for lower extremity were stimulated by transmagnetic stimulator. The central motor conduction time(CMCT) was measured with the difference in MEP caused by stimulating the vertical area and spinal area. The CMCT of hemiparetic patients were classified into three groups-normal, delayed, and no evoked MEP groups. Results : The CMCT in hemiparetic side of acute ischemic stroke patients were singnificantly delayed (P < 0.05) compared with the control group. The CMCT of hemiparetic side in the follow-up study showed no sinificantly difference in comparison to the control group. The prognosis of motor improvement was better in the groups of delayed MEP than the groups of no evoked MEP. Conclusion : The CMCT of hemiparetic and contralateral sides were delayed in acute ischemic stroke, compared with control group and were returned to normal boundaries in subacute state. But in the most cases with no MEP response in the initial study, also showed no MEP response in the follow-up study. The recovery occurred in the subacute state in cases with mild hemiparesis, whereas recovery did not occur in the subacute stage in case with severe hemiparesis.

Clinical Review on Complications of Stroke Patients Admitted Oriental Medical Hospital

  • Background and Purpose : In the acute stage of stroke, many medical and neurological problems complicate and affect the clinical course of patients. according to previous reports. Although some of them may be predicted, few data exist about them. So we were to investigate the characteristics of complications in hospitalized patients due to stroke. Methods : We retrospectively examined the clinical notes of patients admitted in Won Kwang oriental medical hospital after stroke. Two observers inspected the clinical notes using predefined diagnostic criteria and recorded the type, timing, and frequency of complications that occurred during hospitalization. Results: Total 78 subjects were included in this study. Medical complications(88%) were more frequently complicated than neurological ones(65%). The most common medical complication was constipation(29%) followed by fever(28.2%) and overflow incontinence associated with dysuria(28%). The most common neurological complication was dysphagia(23.7%) and the second was agitation or Insomnia(36%) and the third was headache(21%). Conclusion : Complications after acute stroke were commonly observed. And There were more frequent medical complications than neurological ones. So we should have much knowledge about medical complications and treat them actively. The differences between our study and previous studies are attributable to the different methods including patient selection and diagnostic criteria.
