Study about the Comparison of Korean-Western Medicine on Atopic Dermatitis and Psychological Factors (아토피 피부염과 심리적 요인에 대한 동서의학적 고찰)

  • This study aimed to investigate the latest trends in the relationship between atopic dermatitis(AD) and psychological factors(PF) and to examine it in korean medicine. We searched MEDLINE for this analysis with the title "atopic dermatitis" AND ("psychology" OR "psychological" OR "mental" OR "emotion" OR "anxiety" OR "depression") in recent 5 years and searched OASIS on the title "atopy" OR "psychology" OR "emotion" from 2002 to 2017. We selected 23 papers on MEDLINE, 7 papers on OASIS. In western medicine, Stress causes changes in the adrenal nerves, endocrine, and immunological mechanisms and exacerbates dermatitis, which is explained by HPA axis and sympathetic nerve axis, neurogenic inflammation, and cholinergic signals. In Korean medicine, Stress(神) exacerbates AD by affecting the five organs, especially the heart(心), causing inflammation(火熱). We studied the link between AD and PF in Western and Korean medicine. More research is needed in the future.

Survey on Pattern Identification and Treatment of Chronic Fatigue in Korea Medicine (만성피로의 한의임상진료현황에 대한 조사 연구)

  • The aim of this survey is to investigate the current state of pattern identification, treatment and management on chronic fatigue in the Korean medicine. From May 25, 2017 to June 7, we distributed contents of questionnaires to 17,992 Korean medical doctors via e-mail, and then received answers from 329 people. The surveys were conducted by a specialized research organizations. Researchers were blinded to the participant's personally identifiable information or whether they participated in the investigation. In Korea, almost Korean medicine doctors (94.5%) used pattern identification in the treatment of chronic fatigue patients. The main diagnostic methods were visceral (38.5%), qi-blood-fluid-humor (32.3%), and sasang constitutional pattern identification (14.3%). The high-frequent therapy were herbal medicine (37.0%), acupuncture (27.9%), moxibustion (11.7%), and cupping (8.6%). The primary goals of treatment were Improvement and relaxation of stress condition (18.1%), improvement of quality of life (13.1%), increase of functional vitality (34.0%), and fatigue management (23.3%). The key factors affecting treatment outcomes were the patient's aggressiveness in treatment (23.5%) and his/her lifestyle and environment(22.7%). This study was a pioneering research for chronic fatigue on the aspect of medical service provider in the Korean medicine. We hope that this study can be used as a basis for developing a more appropriate and reasonable practice guidelines for chronic fatigue.

The Clinical Study on the Effect of Chuna Treatment for Patients with Herniated Intervertebral Disc of Lumbar Spine (요추간판탈출증(腰椎間板脫出症) 환자(患者)의 추나치료(推拿治療) 효과(效果)에 대(對)한 임상적(臨床的) 고찰(考察))

  • Objectives : The aim of present study is to estimate clinical outcome of Chuna treatment and Chuna medicine treatment for patients with H.I.V.D(Herniated Intervertebral Disc) of lumbar spine according to the character of patients and the steps of Chuna therapy, and therefore suggest a prospective, desirable Chuna treatment and patient guidance. Methods : We investigated the 258 cases of outpatients who were diagnosed as the H.I.V.D. on lumbar C.T. or MRI and were treated more than seven times with chuna treatment and more than one time with Chuna medicine treatment In Jaseng Oriental Medicine Hospital from September 1, 1999 to December 31, 2000. We evaluated the subjective symptoms by Hudgin's classification. For the sake of the more objective evaluation for before and after treatment, we calculated the Improvement index and ratio of the four parts to estimate the pain and functional disorder In before and after treatment by Macnab's criteria. Results & Conclusion : The improvement index and ratio showed that the conservative therapy with Chuna therapy improved the symptom of H.I.V.D. much effectively, and especially in the patient groups of male, 30-39 years, standing Job, duration of symptom within one week, the 4th step of chuna treatment, the first step of chuna medicine treatment, and the excellent satisfactory assessment of Chuna treatment. Therefore, it is supposed that the conservative therapy with Chuna therapy and patient guidance should be much actively continued.

  • Objectives : The Purpose of this study is to investigate the clinical application of conservative Korean treatment including hip joint MST for Bilateral Sacroillitis with Synovitis in Rt. Hip Joint Caused by Ankylosing Spondylitis. Methods : Patient is hospitalized at Dept. of Korean Internal Medicine, Jaseng Korean Medicine Hospital, diagnosed as Bilateral Sacroilitis with Synovitis in Rt. Hip Joint Caused by Ankylosing Spondylitis, and treated by herbal medicine, acupuncture, moxibustion, and hip joint MST. This study was measured by NRS(Numeric Rating Scale), SLRT(Straight leg raising test), Patrick test, and Blood test(ESR, CRP). Result : After conservative treatment, the patient's pain was controlled and NRS score was decreased. SLRT score also improved. Patrick test unchanged. ESR, CRP score was decreased. Conclusion : As seen in this one case, conservative Korean treatment including hip joint MST has a positive effect to control pain with Bilateral Sacroillitis with Synovitis in Rt. Hip Joint Caused by Ankylosing Spondylitis.

The Domestic Trends of Traditional Korean Medicine Treatments on Degenerative Knee Arthritis (퇴행성 슬관절염의 한의학적 치료에 대한 국내 연구 동향)

  • Objectives : The purpose of this review is to investigate the trends of Traditional Korean Medicine treatments on degenerative knee arthritis. Methods : We investigated the studies on traditional Traditional Korean Medicine (TKM) treatments for degenerative knee arthritis via searching 5 Korean web databases. Results : 31 research papers were found to be analyzed according to their published year, the titles of journals, published institution, the types of study, the number of cases, the types of treatments. 31 papers were published since 1992. The studies on TKM treatments about degenerative knee arthritis were mainly published in The journal of korean acupuncture & moxibustion medicine. 13 Randomized Controlled Trials, 10 Case series, 4 Reviews, 3 Non RCTs, and 1 Case Report had been under research. In 22 cases of the studies, one method was used to treat the symptoms. Whereas in 5 cases of the studies, various TKM treatments were used to treat the symptoms. Visual analogue score(VAS) and patient's subjective complaints were used as primary assessments. Conclusion : In this study, we analyzed the trends of TKM treatments on degenerative knee arthritis. Reviewing the domestic trends of studies on Traditional Korean Medicine treatments on degenerative knee arthritis and examining the strong and weak points of those treatments are essential for the further studies.

Activation of spinal Serotonergic Receptor Contributes to Electroacupuncture Analgesia in Rat with Chronic Pain (만성통증이 유발된 흰쥐에서 관찰된 침진통효과의 세로토닌성 기전)

  • Objectives : Electroacupuncture (EA)-induced analgesia has been known to be mediated through the activation of opioid, noradrenergic and serotonergic receptors. However, little study on serotonergic mechanism has been performed in an animal model of chronic pain. The present study was designed to elucidate the type of serotonergic receptors responsible for EA analgesia in the chronic pain model. Methods : In rats with complete Freund's: adjuvant-induced inflammation and spinal nerve injury, spinal wide dynamic range (WDR) cell responses to graded electrical stimulation of afferent C fiber were recorded before and after spinal application of selective 5-hydroxytryptamine (5-HT) receptor antagonists. EA stimulation (2Hz, 0.5msec, 3mA) was applied to the contralateral Zusanli point for 30 min. Results : In both models of chronic pain, WDR cell responses were greatly inhibited after EA stimulation. EA-induced inhibition of WDR celt responses was significantly attenuated by spinal application of non-selective 5-HT receptor antagonist, dihydroergocristine Of 5-HT receptor antagonists tested, 5-HT1A (WAY 100635) and 5-HT2 (LY53857) receptor antagonists strongly reduced an ability of EA stimulation to inhibit WDR cell responses. However, 5-HT1B (GR55562) and 5-HT3 (LY278584) receptor antagonists also had weak but significant blocking action on EA-induced inhibitory effect on chronic pain. Conclusions : Dorsal hem cell responses, afferent C fiber stimulation, chronic pain, electroacupuncture, serotonergic receptors.

  • Objective : The main idea of this article is to investigate the specialization of Chinese traditional medicine. The ancient shaman played a bridge role between human beings and supernatural things like ghosts. Even though he didn't have the super power, the ancients believed that he could take care of all kinds of illness. Therefore, it can be said that the medicine of Zhou Dynasty was still under the shamanism although it had already started to be specialized and professionalized. And it was the important role of the ancient shaman of that period that gave patients the shamanistic treatment on the ground of the specialized medical knowledge, which was commonly activated during that time. Method : This article is going to look into the detailed aspects of the specialization of Chinese traditional medicine through the some kinds of written attestations of Zhou Dynasty. Result : The medical knowledge of that time stayed in the early stage, so it was simple and raw. Also it had scientific and unscientific characters in itself at the same time. That's why it could be included in the shamanism; the details can be offered through another article of mine entitled A Research of Shamanistic Medical Activities on Written Attestations in the Zhou Dynasty(兩周時期的醫療巫術), which is being written now. Conclusion : From beginning of the Eastern Zhou period, the medicine gradually got to be specialized. And then the specialized medical treatment and shamanistic medical activities began to be divided as the different two occupations. Searching for various written attestations of that period, we can see lots of records about the specialized medicine treatment such as acupuncture and moxibustion and the medical substances.

  • Fever in cancer patients is often due to the following causes: evil qi and toxity stagnancy, disorders of qi and blood, deficiencies of zang and fu organs, and the disorder of yin and yang. The treatments given to cancer patients with a fever are according to five: (a) Excessive inner heat and toxicants: remove heat and the toxicant, induce purgation. We use Cheng-Qi-Tang plus Qing-Wen-Bai-Du-Yin. (b) Tangle of damp and heat, and qi stagnancy: remove damp and heat, smooth the qi channel. We use Gan-Lu-Xiao-Du-Dan or San-Ren-Tang. (c) Obvious blood and heat stagnancy: remove heat and blood stasis. We use Xue-Fu- Zhu-Yu-Tang. (d) Deficiency of spleen qi, inner heat caused by a yin deficiency: nourish spleen qi and yin to remove the inner heat. We use Bu-Zhong-Yi-Qi-Tang or Xiao-Jian-Zhong-Tang. (e) Prominent yin deficiency and hectic fever: replenish yin and remove inner heat. We use Qing-Hao-Bie-Jia-Tang or Chai- Qian-Mei-Lian-San. The pathogenesis of fever in cancer patients is complicated. We can see both deficiency and excess in one differentiation. Therefore, we must make sure of it, then we can get the most effective treatment.

Two Clinical Cases of Active Release Technique with Oriental Medicine Treatments for Sequlae of Tibial Plateau Fracture (경골 고평부 골절 후유증에 능동이완기법을 병행한 한방치료 호전 2예)

  • The purpose of this study is to evaluate the effect of active release technique on sequelae of tibial plateau fracture. Two patients with sequelae of tibial plateau fracture were treated with active release technique. Visual analog scale (VAS), Lysholm knee scoring scale, range of motion were used to measure changes during treatment. After treatment, visual analog scale, Lysholm knee scoring scale, range of motion were improved significantly. Active release technique showed significant improvement to the sequlae of tibial plateau fracture that were showing no signs of improvement for twelve and eighteen weeks. This clinical trial showed that active release technique has meaningful effect on sequlae of tibial plateau fracture and more research should be followed.

Reviewing Research of Eastern-Western Integrative Medicine Studies in Korea (한, 양방 협진치료에 대한 연구 경향 분석: 국내 논문을 대상으로)

  • Objectives The purpose of this study is to analyze research trends on Eastern-Western integrative medicine in Korea. Methods We searched the studies on Eastern-Western integrative medicine in 5 Korean web database (NDSL, KoreanTK, KISS, OASIS, DBPIA). 66 research papers we founded. Results 13 papers were published at 2010. The studies on Eastern-Western integrative medicine were mainly published in the Journal of Society of Preventive Korean Medicine. 24 papers were case report, include 7 studies on neurology, 4 studies on each oncology and dermatology, 2 studies on each gastroenterology and ophthalmology and otorhinolaryngology, 1 study on each obstetrics and gynecology and endocrinology and nephrology. In 24 case reports, 7 kinds of Eastern medicine treatment method and 4 kinds of Western medicine treatment method were existed. In case reports, medication (100%), herbal medicine, acupuncture (95.8%), moxibustion (58.3%), cupping, infusion solution (25%), pharmacopuncture (20.8%), physical therapy (12.5%), laser, injection, rehabilitation (8.3%) were used. Conclusions In this study, we analyzed the trends of Eastern-Western integrative medicine in korea from 2010 to 2017. There were various studies about Eastern-Western integrative medicine. In case reports, Eastern-Western integrative medicine tend to concentrated on treatment not diagnosis. Not only treatment but also diagnosis is needed in Eastern-Western integrative medicine studies.
