침술과 피어싱으로 발생한 만성 C형 간염 1예 (A Case of Chronic Hepatitis C Acquired through Ear Piercing and Acupuncture)

  • 산발성 C형 간염 환자의 약 40%에서 감염경로가 확실치 않다. 이러한 환자들에서 HCV 감염이 수혈, 투석, 수술, 수직감염 및 성적 접촉 등의 잘 알려진 감염 경로 이외에도 침, 귓불 천공 등의 경피적 경로를 통해 전파 될 수 있음을 간과하지 말아야 할 것이다. 저자들은 간염의 가족력이나 수술과 수혈 과거력이 없는 10세 여아에서 피어싱과 침술로 인해 발생한 만성 C형 간염을 진단하고, pegylated interferon과 경구 ribavirin으로 치료한 증례를 경험하였기에 문헌고찰과 함께 보고 한다.

십자절개침법을 이용한 멜라닌 세포성 모반 치료의 치험 1례 (A Case of Melanocytic Nevi which was Treated with Crucial Incision Acupuncture Therapy)

  • Objectives : The objective of this study was to report the efficacy of the Crucial Incision Acupuncture Therapy(CIAT) for removing melanocytic nevi. Methods : CIAT is penetration acupuncture to remove a lesion by sticking needles like cross-shape. We used the CIAT for treating one patient with melanocytic nevi, and then we investigated efficacy and side effect for about 5 months. Results : In 16 melanocytic nevuses treated with CIAT, one of the nevuses recurred after about 3 months. And hyperpigmentation lasted for about 2-5 months was caused after the CIAT. Conclusions : In Korean medicine, the CIAT might be used for treating melanocytic nevi. But additional studies might be needed for improvement of treatment effect.

침구소난요지(鍼灸素難要旨)에 대한 연구(硏究) (A study on zhenjiusunanyaozhi(鍼灸素難要旨))

  • "zhenjiusunanyaozhi(鍼灸素難要旨)" is composed of three volumes and published in 1529 by Gao Wu(高武). Gao Wu(高武) is skillful in astronomy, the art of war and the law as well as a medical practitioner in Ming Dynasty. The books he wrote "zhenjiujuying(鍼灸聚英)", "zhizhi(直指)", "douzhenzhengzong(痘疹正宗)", "shexuezhinan(射學指南)", "zhenjiujieyao(鍼灸節要)". "zhenjiusunanyaozhi鍼灸素難要旨" is written by classifying the origin of acupuncture and moxibustion. In other words, it is edited by classifying the contents related to acupuncture and moxibustion out of the ancient Chinese medical book "yellow emperor's canon of medicine and yellow emperor eighty-one difficult" in which are composed of 3 volumes as follows, Volume 1 says the main diseases on "the nine acupuncture needles figure" (九針圖), "the reinforcing and reducing the meridian" (補瀉), "the needle depth" (針刺深淺), "the five shu points - metal, wood, water, fire, earth" (正,滎,輸,經,合) based on 18 chapters in terms of acupuncture in "yellow emperor eighty-one Difficult "難經"", in which it quotes the annotation of "the difficulty by the original meaning "難經本義"" written by Hua Shou(滑壽) in Yuan Dynasty. Volume 2 is composed of 2 parts. Part 1 says the method of treatment on 36 Chapters, the method of acupuncture use in the Linshu "靈樞" and the Suwen "素問" such as "the rule of acupuncture use" (用針方宜), "the nine-pin method" (九針式) and "the nine-pin to only use the time appropriate to consider nature of Heaven, Earth and person" (九針應天地人時以起用) etc., Part 2 says "the five difficult acupuncture(五亂刺)", "the rise and fall of energy and blood(氣血盛衰)". "the pain tolerance(耐痛)" and ect., in which are in terms of method of treatment collected the original texts of 59 chapters on acupuncture to each disease and of 8 chapters on moxibustion in the Linshu "靈樞" and the Suwen "素問". Volume 3 includes 10 chapters in which consist of "the stabbing to disease in 12 meridians (十二經病刺)", "the eight extra meridian disease (寄經八脈病)", "the twelve meridians(十二經脈)", "the fifteen collaterals (十五絡脈), the twelve meridian muscles (十二經筋)", "the acupoint (孔穴)" and etc. This is the book edited comprehensively by classifying the contents on the theory of acupuncture and moxibustion and the circulations of meridians in "yellow emperor's canon of medicine and yellow emperor eighty-one difficult" and there is no case story in particular except his comments in person. This study is for the purpose of helping researching and developing acupuncture and moxibustion and applying their clinical training.

내관(內關)-공손(公孫) 자침이 스트레스 상태의 정상성인 자율신경계에 미치는 영향 (Effects of Acupuncture on Autonomic Nervous System in Normal Subjects under Mental Stress)

  • Objectives: The objective of this study was to assess the effects of acupuncture applied at PC6 and SP4 points on heart rate variability (HRV) in normal subjects under mental stress Methods: 24 healthy male subjects were recruited and randomized to a treatment group (12 subjects) or a control group (12 subjects). Both groups performed a mental arithmetic stress test and then acupuncture needles were inserted on PC6 and SP4 for the study group. In the control group, they took sham acupuncture as a control. Heart rate variability (HRV) was measured during the whole study period by FM-150 (digital Holter ECG recorder). Results: In the both groups, low frequency (LF) power and ratio of low frequency to high frequency (LF/HF) was significantly increased after mental stress. Acupuncture induced significant decrease in low frequency (LF) power and ratio of low frequency to high frequency (LF/HF) which were increased after mental stress test. But sham acupuncture induced no changes. Results: In both groups, low frequency (LF) power and ratio of low frequency to high frequency (LF/HF) significantly increased after mental stress. Acupuncture induced significant decrease in low frequency (LF) power and ratio of low frequency to high frequency (LF/HF) which increased after mental stress test. In contrast, sham acupuncture induced no changes. Conclusions: Acupuncture on PC6 and SP4 could be useful to decrease sympathetic activity and balance autonomic nervous system for those who are under stress.

흰쥐의 족삼리 및 태충 전침자극에 따른 뇌대사활성의 변화 (Alterations of Cerebral Metabolic Activation Following Electro-Acupuncture Stimulation on ST36 and LR3 Acu-Points in Rats)

  • Objective : The meridian theory in oriental medicine explains that each acu-point has a characteristic functional effect. It will be supposed that an acupuncture stimulation on different acu-point evokes different activation on different areas in the central nervous system(CNS) according to the meridian theory. On this supposition, our group tried the semi-quantitative [14C]2-deoxyglucose([14C]2-DG) autoradiography on the acupuncture stimulation to the hindlimb acu-points of Sprague-Dawley rats. Methods : A venous catheter for the intravenous administration of isotope was equipped in the right external jugular vein on 3 days prior to the [14C]2-DG study. On the day of the study, two acupuncture needles were inserted into the ST36(Zusanli) or LR3(Taichong) on the left hindlimb. Electro-acupuncture stimulation (2 Hz, 5 ms, 1~3 mA, 15 minutes) started just before the i.v. injection of [14C]2-DG ($25{\mu}Ci/rat$). The brain and the spinal cord were removed and processed for the [14C] 2-DG autoradiography. Results : The EA stimulation on ST36 reveals over 120% metaboilc activation in Arcuate nucleus, Anterior pretectal nucleus, Dorsal cochlear nucleus, Interposed cerebellar nucleus, and Nucleus of Darkschewitsch. The EA stimulation on LR3 reveals over 120% metaboilc activation in Lateral habenula nucleus, Medial vestibular nucleus, Ventromedial thalamic nucleus, Anteroventral thalamic nucleus, Anterior cingulate cortex, Dentate gyrus, Antero cortical amygdaloid nucleus, Anterior pretectal nucleus, and Dorsal tegmental nucleus compared with the non EA stimulation control group. Conclusion : These results demonstrate that the different acu-points evoke the different activations in brain areas. And with this functional brain mapping study, a new scientific elucidation for the basis of the acupuncture-meridian theory in oriental medicine through differences of activated area in CNS according to the each acupuncture point.

레이저자극과 수기자극에 대한 경락전기반응 특성비교 (Comparison of meridians electric response property for laser and acupuncture stimulation)

  • 최근에 레이저를 임상치료에 응용하는 사례가 증가하고 있다. 그러나 전기적 관점에서 레이저 자극이 경락에서 어떠한 전기적 반응을 유도할 수 있는지에 대한 연구가 미흡하고, 명확한 치료효과에 대한 임상보고가 발표되지 않고 있다. 본 논문에서는 레이저자극과 수기자극이 경락전위 형성에 미치는 영향을 비교 관찰하여 전기적 관점에서 침술과정 및 침술효과에 대한 객관적 근거를 제시하고자 하였다. 수양명대장경상의 삼간혈(LI3)을 각각 자극했을 경우, 삼간혈(LI3)과 합곡혈(LI4)에서의 전위변화를 측정하였다. 그 결과, 레이저 자극 시, 평균 피크전위는 $7.53{\pm}3.44{\mu}V$로 매우 낮게 나타났고, 자극전후 전위패턴에 차이가 없어서 레이저 자극에 대한 유효한 전기반응으로 간주하기 어렵다. 접지조건에 대한 수기자극에서는 평균피크전위가 $2.65{\pm}1.53mV$로 매우 높게 측정되었고, 개인별, 접지조건에 따라 전위크기와 패턴이 다양하게 나타나고 전위패턴은 주로 캐패시터의 충방전 전위와 매우 비슷하게 나타났다. 또한, 절연자침의 경우에서는 접지조건에 관계없이 평균 피크 전위가 $0.25{\pm}0.16mV$로 수기자극 전위에 비하여 매우 낮게 나타났으며, 이는 침자극에 대한 유효한 전기 반응으로 간주하기 어렵다. 따라서, 전기적 관점에서 침술과정과 침술효과는 시술자와 피시술자간의 생체이온전하의 이동에 의한 에너지 교감현상임을 확인하고, 수기자극은 경락의 전기반응을 유도하는 반면, 레이저 자극은 경락의 전기적 반응을 유도하기 어렵다는 것을 확인하였다.

중풍으로 인한 마비성 조음장애 환자의 침술 후 말소리의 음향학적 평가 연구 (Acoustic Evaluation of acupuncture therapy effects on post-stroke dysarthria)

  • Stroke makes several physical deficits. Dysarthria is one of the most difficult problems in conventional medicine because of the weakness of neuromotor control. The purpose of this study is to find the acoustic characteristics of acupuncture therapy effects on post-stroke dysarthria. Seven patients with stroke(infarction or hemorrhage) were selected by CT or MR imaging. The authors applied acupuncture therapy by inserting needles into 8 acupuncture points, ipsilateral ST4, ST6 and contralateral LI4, ST36 on facial palsy side, and CV23, CV24, bilateral "Sheyu" for 4 weeks. Speech sample were composed of five simple vowels /a,e,i,o,u/ and meaningless polysyllabic words CVCVC(C: stops, affricated, fricative sounds, v: /e/). .VOT, total duration of each speech samples and vowel formant (F1&F2) were analyzed on Spectrogram. The results are as follows: 1. VOT of bilabial and velar stops was decreased post treatment. The VOT of bilabial glottalized pre and post treatment were statistically significant (p < 0.05). 2. Total duration of polysyllabic words was decreased post treatment. Decrement of total duration containing the bilabial was statistically significant (p<0.05). 3. First formant of round vowel /o/ pre and post treatment was statistically significant (p<0.05).

고빈도 120 Hz 전침이 Carrageenan으로 유발된 흰쥐의 Prostaglandin E2와 척수 N-Methyl-D-Aspartate Receptor 발현에 미치는 영향 (The Effects of 120Hz Electroacupuncture on the Prostaglandin E2 and Spinal N-Methyl-D-Aspartate Receptor Expression in the Carrageenan-Injected Rat)

  • Objective : The role of high frequency 120 Hz electroacupuncture(EA) in carrageenan-induced pain was studied by examining the alnalgesic effects, and prostaglandin $E_2(PGE_2)$ levels measurement and spinal N-methyl-D-aspartate(NMDA) receptor expression. Inflammation was induced by an intraplantar injection of 1% carrageenan into the right hind paw. Method : Bilateral EA stimulation with 120 Hz were delivered at those acupoints corresponding to Zusanli and Sanyinjiao in man via the needles for a total of 30 min duration in carrageenan-injected rats. Results : EA stimulation showed significant analgesic effects as measured by analgesy-meter at all time points tested compared with controls. Three hours after carrageenan injection, PGE2 levels were measured by commercial kit. EA significantly inhibited PGE2 production in the right paw. The number of NR1 and NR2A, NMDA receptor, immunoreactive neurons was significantly increased in the superficial dorsal horn(laminae I-II) and nucleus proprius(laminae III-IV) of ipsilateral spinal cord at L4-5. But the number of carrageenan-induced NR1 and NR2A immunoreactive neuron, especially NR1 immunoreaction in the superficial dorsal horn, was reduced by 120 Hz EA stimulation. Conclusions : These results indicate that NMDA receptors may mediate transmission of nociceptive information originating in tissue inflammation of hind paw and high frequency 120 Hz EA stimulation have an alleviating action against local inflammatory pain.

족삼리(足三里) 자침이 고혈압환자의 혈압과 내피세포 의존성 혈관확장반응에 미치는 영향 (Effects of Acupuncture at ST36 on Blood Pressure and Endothelial Dependent Vasodilation in Hypertensive Patients)

  • Objectives : The objective of this study was to assess the effect of acupuncture applied at the ST36 point on blood pressure and endothelial dependent vasodilation in hypertensive patients. Methods : 24 hypertensive patients were recruited and randomized to a study group (12 subjects) or a control group (12 subjects). Both groups took FMD (endothelial-dependant, flow-mediated dilation) measurement and then acupuncture needles were inserted at ST36 for the study group. In the control group, they took sham acupuncture as a control. FMD was rechecked after 10-min acupuncture treatment. Blood pressure was measured before and after acupuncture treatment. Results : FMD increased significantly in the study group after acupuncture (9.5${\pm}$2.0% to 11.1${\pm}$2.2%), but not in the control group. In both groups, there were no changes in blood pressure and heart rate. Conclusions : Acupuncture on ST36 appears to improve endothelial dysfunction of hypertensive patients and this might result from inducing activation of endothelium-derived nitric oxide.

프랑스의 동아시아 전통의학 현황 (History of Oriental Traditional Medicine in France)

  • Understanding the present condition of Oriental traditional medicine in different country is demanded for the Korean Traditional Medicine to prepare his orientation in future. Also it is not possible to separate the present condition of OTM from history of oriental medicine as a whole. This is valid for the introduction of TCM in the west. The first information have arrived in Europe by the terrestrial and maritime travelers accounts. The diplomatic way, with special reference to Jesuit fathers, has brought cultural and theoretical precise data. The physicians of East India Company and, later, of expeditionary forces, have supplied with practical data. Five periods follow one another: 1) Period when the source reached and are used in Europe. This first step is intimately involved with the use and comparison of similar factor between the two medical systems ; 2) "Discovery" and use of needles ; 3) latent period when techniques coming from acupuncture source were developed ; 4) "Chinese Acupuncture" correctly speaking ; 5) modern period or investigating period, when Eastern and Western people are trying to correlate data of traditional medicine with those of western science. In particular the actual situation from 2000.
