A Survey on Students' Perception of Clinical Performance Examination (CPX) in College of Korean Medicine Using Student Standardized Patients (학생표준화환자를 사용한 한의과대학 진료수행시험(CPX)에 대한 학생 인식 조사)

  • Objectives : The objective of this survey was to examine students' perception of the clinical performance examination (CPX) in college of traditional Korean medicine using student standardized patients. Methods : College of traditional Korean medicine students who completed the first-semester clinical practice education were selected as the subjects, and they participated in a survey asking questions about the following matters: satisfaction with CPX, self-evaluation, difficulties experienced during the CPX course, level of prior knowledge of clinical practice education, and usefulness of and intimacy with Student Standardized Patients (SSP). Results : Satisfaction with CPX was calculated to be 4.10 (5 being the perfect score), and self-evaluation of CPX was calculated to be 4.12. The subjects chose physical examination as the most difficult item relating to CPX. 80.5% responded positively to the question relating to diverse experiences in practice education, and 52.8% responded positively to the question relating to prior knowledge of diverse standardized patients. 55.6% responded positively to the question relating to performance proficiency of SSP, 63.9% responded positively to the question relating to usefulness of SSP to prepare for CPX, and 69.4% responded positively to the question relating to usefulness of SSP in evaluating CPX, and 55.6% responded positively to the questions relating to intimacy with SSP. It was confirmed that there was a strong quantitative correlation between prior knowledge of clinical practice education and usefulness of SSP, and that there was a strong quantitative correlation between performance proficiency of SSP and usefulness of SSP. It was confirmed that there was a middle-level correlation between performance proficiency of SSP and intimacy with SSP, and that there was a middle-level quantitative correlation between intimacy with SSP and usefulness of SSP. Conclusions : It was confirmed that senior students enrolled in college of traditional Korean medicine were mostly satisfied with the clinical performance examination in college of traditional Korean medicine using SSP, and that usefulness of SSP had a quantitative correlation with prior knowledge of diverse clinical practice educations, performance proficiency of SSP, and intimacy with SSP.

Effects of Valerianae Radix et Rhizoma Methanol Extract on High-fat Diet-induced Hyperlipidemic Mice (힐초근(纈草根) 메탄올 추출물이 고지혈증 유발 생쥐의 지질대사에 미치는 영향)

  • Objectives: This study was designed to investigate the effects of Valerianae Radix et Rhizoma Methanol Extract (VRME) on serum lipid levels in a high-fat diet-induced hyperlipidemic mice. Methods: Each 8 C57BL/6 mice were randomly assigned to normal diet group, high-fat diet control group, high-fat diet plus 100 mg/kg/day of VRME group. In order to induce hyperlipidemia, high-fat diets were supplied to control group and VRME group for four weeks. Normal diet group were supplied with general feed for four weeks. After that control group supplied only high-fat diets as feed, VRME group received oral administration of VRME with high-fat diets for three weeks. and normal diet group were supplied with general feed for three weeks. After seven weeks, the changes in the body weight, the plasma levels of total cholesterol, triglyceride, high density lipoprotein-cholesterol, aspartate aminotransferase (AST), alanine aminotransferase (ALT), blood glucose in serum were measured. Results: In our results, VRME did not affects weight gain, serum AST and ALT in high-fat diet-induced hyperlipidemic mice. Oral administration of VRME lowered levels of total cholesterol and triglyceride, which were elevated by induction of hyperlipidemia. and oral administration of VRME lowered blood glucose significantly. Conclusions: These results suggest that VRME could act as a potent antihyperlipidemic in therapeutics for hyperlipidemia.

  • 연구 배경 최근 한국 및 중국에서 근골격계 질환에 대한 도침술의 활용이 늘고 있다. 하지만 현재까지의 임상연구는 대부분 증례보고 형태에 그치거나, 충분한 근거가 확보되지는 않은 실정이다. 우리는 요천추신경병증환자의 치료에 있어 도침치료와 통상한의치료와의 비교를 통해 도침치료의 유효성, 안전성, 경제성 평가를 시행하기 위해 본 연구를 기획하게 되었다. 연구 방법 본 연구는 예비임상 연구로, 다기관에서 진행되며, 무작위대조군, 활성 대조군을 통한 2개군 병행집단 연구로 평가자 맹검을 시행하게 된다. 이 연구에서는 총 50명의 요천추신경병증 환자를 대상으로 2개군으로 균등하게 분배하여 도침술 또는 통상한의치료를 시행하게 된다(각군당 25명). 도침술 그룹의 경우 협척혈에 도침치료를 시행 받게 되며, 통상 한의치료군은 연구자 판단하에 도침술을 제외한 적절한 한의치료를 시행 받게 된다. 두 개의 그룹은 주당 2회씩 총 3주간 치료를 받게 된다. 일차 유효성 평가는 요통으로 인한 기능장애를 평가하기 위한 Oswestry disability index를 통해 시행한다. 이차 유효성 평가지표로는 numeric rating scale, European Quality of Life 5-Dimension 5-Level, short-form McGill Pain Questionnaire, Roland-Morris Disability Questionnaire scores를 시행하게 된다. 고찰 본 예비임상연구의 결과는 추후 있을 요천추신경병증에 대한 도침치료 및 한의통상치료 비교효과 연구의 유효성, 경제성평가 본 임상연구를 위한 기초 정보 및 가능성을 확인하고 적절한 대상자수 산정에 도움이 될 것이다.

Korean Medicine Treatment for Anterior Inferior Cerebellar Artery Infarction: A Case Report (어지럼증을 주소로 하는 전하소뇌동맥경색 환자에 대한 한방치료 증례보고 1례)

  • Objective: The aim of this study is to report the effectiveness of Korean medicine for a patient with dizziness following anterior inferior cerebellar artery infarction. Methods: The patient was treated with traditional Korean methods including acupuncture, herbal therapy, moxibustion, and vestibular rehabilitation exercise during an admission period of seven days. The patient's dizziness was assessed using a numeric rating scale (NRS) and the Korean Dizziness Handicap Inventory (K-DHI), Korean Activities-specific Balance Confidence (K-ABC), Korean Vestibular Disorders Activities of Daily Living (K-VADL), the modified Rankin scale (mRS), and the Korean version of the Modified Barthel Index (K-MBI). Results: After seven days of combined treatment with traditional methods and vestibular rehabilitation, the patient's dizziness was reduced from NRS 6-7 to NRS 2. In addition, K-DHI decreased from 84 to 22; K-ABC improved from 52% to 78.125%; K-VADL reduced from 175 to 37; the mRS score changed from 4 to 1; and the K-MBI score increased from 86 to 98. No adverse events were observed during treatment. Conclusion: This study suggests that combined therapy of Korean medicine and vestibular rehabilitation can be effective treatment for anterior inferior cerebellar artery infarction patients.

  • The purpose of this study is to present an appropriate management plan as a supplement to the scientific evidence of the currently operated distancing system for preventing COVID-19. The currently being used mathematical models are expressed as simultaneous ordinary differential equations, there is a problem in that it is difficult to use them for the management of entry and exit of small business owners. In order to supplement this point, in this paper, a method for quantitatively expressing the risk of infection by people who gather is presented in consideration of the allowable risk given to the gathering space, the basic infection reproduction index, and the risk reduction rate due to vaccination. A simple quantitative model was developed that manages the probability of infection in a probabilistic level according to a set of visitors by considering both the degree of infection risk according to the vaccination status (non-vaccinated, primary inoculation, and complete vaccination) and the epidemic status of the virus. In a given example using the model, the risk was reduced to 55% when 20% of non-vaccinated people were converted to full vaccination. It was suggested that management in terms of quarantine can obtain a greater effect than medical treatment. Based on this, a generalized model that can be applied to various situations in consideration of the type of vaccination and the degree of occurrence of confirmed cases was also presented. This model can be used to manage the total risk of people gathered at a certain space in a real time, by calculating individual risk according to the type of vaccine, the degree of inoculation, and the lapse of time after inoculation.

A Case Series of Polycystic Ovary Syndrome with Improved Hyperandrogenism Treated with Korean Medicine (한방 치료로 고안드로겐혈증을 개선한 다낭성 난소 증후군 연속증례연구)

  • Objectives: This study aims to analyze the medical record of eight cases of polycystic ovary syndrome (PCOS) in a local Korean medicine clinic, in order to provide evidence on Korean medicine treatment of PCOS and to suggest the direction of future studies. Methods: The medical records of eight cases diagnosed with PCOS based on 2003 Rotterdam Criteria and whose total testosterone level was 0.53 ng/ml and over were retrospectively analyzed. The primary outcome was total testosterone level, and the secondary outcomes included luteinizing hormone (LH) and follicle stimulating hormone (FSH) levels, LH/FSH ratio, weight, last mentrual period (LMP), numerical rating scale (NRS) score of dysmenorrhea, and other associated symptoms pre- and post-treatment. Korean medicine treatment consists of the complex herbal prescription consisting of Yukmijihwang-tang, Baekho-tang, Daesiho-tang, and Gyejibokryeong-hwan, modified according to symptoms; acupuncture and electroacupuncture on 中脘 (CV12), 下脘 (CV10), 關元 (CV4), 石門 (CV5), 合谷 (LI4), 太衝 (LR3), 三陰交 (SP6), and 懸鍾 (GB39); and indirect moxibustion on 神闕 (CV8). Results: The average total testosterone level lowered statistically significantly after the treatment. The average FSH and LH levels lowered with insignificance, while the average LH/FSH ratio and weight lowered statistically significantly. During the treatment period of three to six months, every patient had the menstrual cycle shortened except for one case; and two cases with infertility problem succeeded in being pregnant. There was no adverse event. Conclusions: The Korean medicine treatment was found effective in treating PCOS, especially regarding hyperandrogenism, amenorrhea or oligomenorrhea, and infertility.

Case Report of Traumatic Subarachnoid Hemorrhage-Induced Oculomotor Nerve Palsy Treated with Korean Medicine, Including Sweet Bee Venom (외상성 지주막하출혈 이후 발생한 동안신경마비의 봉독 약침을 활용한 한방치료 증례보고 1례)

  • The aim of this study is to report the effectiveness of a traditional Korean medicine treatment for oculomotor nerve palsy induced by traumatic subarachnoid hemorrhage. A 54-year-old male patient with oculomotor nerve palsy induced by traumatic subarachnoid hemorrhage after a traffic accident was treated with Korean medicine, including acupuncture, electroacupuncture, sweet bee venom pharmacoacupuction, moxibustion, and herbal medicine. The effect of treatment on symptoms - ptosis, and eye movement - was evaluated using a ratio of eye opening and eye movement between the paralyzed eye and the normal eye. After 41 days of treatment, the clinical symptoms had improved. The ptosis and eye movement started to show improvement from day 20; at the time of discharge, ptosis had improved by more than 50%, and eye movement showed some improvement as well. This study suggests that Korean medicine may be an effective treatment for oculomotor nerve palsy induced by traumatic subarachnoid hemorrhage.

Case Report of Multiple Cerebral Infarction in Middle Cerebral Artery with Gait Disturbance Treated by Korean Medicine (중대뇌동맥 영역에 발생한 다발성 뇌경색 환자의 보행장애에 대한 한의 복합 치료: 증례보고 1례)

  • Objectives: This study reported about a patient with a right middle cerebral artery infarction whose gait disturbance was improved by Korean medicine treatment. Methods: The patient was treated with a Korean herbal medicine (Gami-yukmijihwang-tang) along with acupuncture, electroacupuncture, moxibustion, cupping, and physical therapy. The treatment effect was evaluated with the manual muscle test (MMT) and the Korean version of the modified Barthel index (K-MBI). The gait of the patient was evaluated by a 10-m walk test (10MWT), the timed up and go (TUG) test, the functional ambulation profile (FAP) score, and the functional ambulatory category (FAC) score. Spatiotemporal parameters were evaluated using a walkway system (GAITRite®, CIR Systems, Inc., USA). Results: After 83 days of traditional Korean medicine treatment, the K-MBI and FAC scores improved from 50 to 70 and from 1 to 4, respectively. The 10MWT and TUG tests also improved from 24.86 to 16.66 sec and from 22.35 to 17.62 sec, respectively. GAITRite® measurements reflected gait improvements: the FAP score improved from 55 to 86 sec; the step time improved from 0.72 to 0.669 sec; the step length improved from 31.076 cm to 41.284 cm; the gait velocity improved from 42.8 cm/sec to 64.1 cm/sec; the cadence improved from 93.6 steps/min to 90.8 steps/min. No adverse effects resulting from treatment or evaluation occurred during the admission period. Conclusions: This study suggests that traditional Korean medicine treatment may reduce symptoms and improve the quality of life in patients with cerebral infarction.

Safety of Oriental Medical Therapy: Drug Fever of Herbal Medicine and Procedure-related Fever (한방치료의 안전성 : 한약에 의한 약열과 시술관련 발열)

  • Objectives: Drug fever and procedure-related fever are the causes of nosocomial fever. Oriental medicine has been practiced in Asia and is now being practiced as an alternative medicine in western countries. No data are available on the incidence of drug fever and procedure-related fever in oriental medical hospitals (OMHs). The aim of this study was to identify the incidence of drug fever related to oriental herbal medicine and oriental medical procedure-related fever. Methods: This was a retrospective study at one OMH of a university medical institute in Seoul, Korea, conducted from June 2006 to June 2013. Results: Overall, 95 episodes of drug fever occurred among 10880 patients treated with herbal medicine (0.89%). Peak body temperature was 38.37±0.58 ℃, and the fever lasted for 1.0 day (range 1.0-17.0 days). Eosinophilia was found in 15 patients (15.79%) and 8 patients developed toxic hepatitis (8.42%). Five patients had a drug-related skin rash (5.26%). The most common ingredients of the herbal medicines associated with drug fever were licorice, Angelica gigas root, and white Atractylodes rhizome. In total, 16 episodes of procedure-related fever (0.20%) occurred in 8125 patients treated with oriental medical procedures. The peak body temperature was 38.26±0.51 ℃, and the fever lasted for 1.0 day (range 1.0-3.0 days). Among various oriental medical procedures, moxibustion was the most common procedure related to fever, followed by acupuncture. Conclusions: The incidence of drug fever and procedure-related fever in oriental medicine is not high compared with the incidence in western medicine.

A Case of a Soyangin Patient with Aggravated Voiding Dysfunction after Pontine Infarction (교뇌경색 이후 배뇨장애가 악화된 소양인 환자의 한의 치료 1례 보고)

  • ■Objective The purpose of this case report is to report the effect of Korean medicine treatment on a patient with pontine infarction complaining of discomfort due to aggravated voiding dysfunction after the onset of the stroke. ■Methods The patient was hospitalized for 44 days and treated with herbal medicine, acupuncture, electroacupuncture, moxibustion, and interferential current therapy(ICT). We checked the amount of self-voided volume and postvoid residual urine volume(PVR), and asked the patient to subjectively evaluate the degree of discomfort due to voiding dysfunction. ■Results After 44 days of hospitalization, the patient responded that the subjective discomfort due to residual urine remained 30% and delayed urine remained 50% compared to the day of admission. The maximum amount of PVR was 234 ml on the 3rd day, and the minimum amount of PVR was checked on 25th and 35th day as 0 ml. ■Conclusions This study suggests that Korean medicine treatment may be effective in the treatment of voiding dysfunction after stroke.
