Purpose: This study was designed to construct a structural model for explaining model health-related quality of life in acquired people with physical disabilities. Method: The hypothetical model of this study was consisted of 6 latent variables and 14 observed variables. Exogenous variables included in this model were physical status and economical level. Endogenous variables were social attitudes, family function, self-esteem, and health-related quality of life. Data were collected from 226 acquired people with physical disabilities residing in Seoul and Kyunggi-do from January to February, 2005. The collected data were analyzed using SAS 8.2 version and LISREL 8.32 version program. Results: The results of the fitness test of the modified model were follow as; ${\chi}^2=67.479$ (df=50, p=.05), GFI=.959, AGFI=.914, SRMR=.049, NFI=.961, NNFI=.979, CN=249.244. Health-related quality of life was influenced directly by physical status, economic level, and social attitudes and accounted for 88.8% of the variance by these factors. Conclusion: These results suggest that physical status is the most significant effect on health-related quality of life, and social attitudes and economic level are important factors having influences on health- related quality of life. Therefore improving physical status and economic level, and modifying negative attitudes are necessary to increase health-related quality of life of acquired people with acquired physical disabilities.
Purpose: This study was conducted to identify the factors influencing performance confidence by knowledge and experience of respiratory-assistant therapy in paramedic students. Methods: A descriptive survey study used a convenience sample comprising 387 students from September 15 to October 31, 2016. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson's correlation coefficient, and regression analysis. Results: Knowledge was statistically significant by college(3-year), acquired certification, and observation experience in respiratory-assistant therapy. Performance confidence was statistically significant by sex, academic grade($3.0{\leq}$), acquired certification, and observation and performance experience in respiratory-assistant therapy. There was a significant correlation between knowledge and acquired certification, clinical practice, and major satisfaction. There was a significant correlation between performance confidence and acquired certification, clinical practice, major satisfaction, and knowledge. Regression analysis revealed that performance confidence accounted for 22.9% of the variance by academic grade(3.0~3.99), acquired certification, and major satisfaction. Conclusion: For paramedic students in clinical practice and in-school classes, it is important to improve performance confidence by combining training instruction with experience and education of respiratory-assistant therapy.
Acquired communicated fistula between the esophagus and respiratory system are infrequent and they are caused by carcinoma, trauma, infection and traction. This report reviews the feature of acquired esophagobronchial fistula[that developed spontaneously]. Patient is 34 year old man with excellent result by surgical intervention. The surgical procedures consist of division and repair of the fistula. Clinically and radiologically, the patient is free from coughing after drinking, substernal distress, esophagorespiratory fistula, and esophageal stricture after surgical treatment.
Pneumonia represents a spectrum of diseases that range from community-acquired to health care-associated pneumonia. Despite advances in diagnosis, antimicrobial therapy, and supportive care, pneumonia remains an important cause of morbidity and mortality, particularly in elderly patients and in those with significant comorbidities. Community-acquired pneumonia (CAP) is the leading cause of death from infectious disease in Korea. This article provides a synopsis of recent studies regarding various types of pneumonia, with a focus on CAP.
During a 4 year period from 1987 to 1991, 152 patients underwent open heart surgery for congenital heart disease and acquired heart disease. Over all operative mortality was 6.6%. There were 76 congenital anomalies consisting of 73 acyanotic and 3 cyanotic and 76 acquired heart disease. The age distribution of 152 cases was 3 months to 62 years. Mean age was 13.0 years in congenital heart anomalies and 38.9 years in acquired heart disease patients.
One hundred eleven cases of open heart surgery had been performed at the department of thoracic and cardiovascular surgery in College of Medicine Hallym Univ. from November 28, 1984 to August, 1991. There were 68 cases of congenital heart disease and 43 cases of acquired heart disease. Among the 68 cases of congenital heart diseases, 65 cases of acyanotic group and 3 cases of cyanotic group were noted. The majority of acquired heart diseases were valvular heart disease. The congenital heart disease consisted of 33 ASD, 27 VSD, 6 PS, 3 TOF. The sex ratio of congenital and acquired heart diseases was represented as 1.4: 1 and 1; 1.4 respectively. The age distribution of congenital heart diseases ranged from 11 months to 50 years old and that of acquired heart diseases from 15 to 61 years old. The overall operation mortality was 1 case[0.9%].

Acquired hypohidrosis is a rare condition of unknown pathogenesis, while idiopathic cholinergic urticaria is relatively common. We report the case of a 18-year-old male with idiopathic cholinergic urticaria and acquired hypohidrosis. He presented with an intermittent pruritic eruption precipitated by mild activity, such as mild exercise or laughing, for last 4 years. He was diagnosed with cholinergic urticaria associated acquired hypohidrosis, successfully treated by herbal medicine applied Gejimahwanggakban-tang(桂枝麻黃各半湯).
Human can update the memory with new information not forgetting acquired information in the memory. ART(Adaptive Resonance Theory) does not need to change all information. The methodology of ART is followed. The ART updates the memory with the new information that is unknown if it is similar with the memorized information. On the other hand, if it is unknown information the ART adds it to the memory not updating the memory with the new one. This paper shows that ART is able to classify sensory information of a certain object. When ART receives new information of the object as an input, it searches for the nearest thing among the acquired information in the memory. If it is revealed that new information of the object has similarity with the acquired object, the model is updated to reflect new information to the memory. When new object does not have similarity with the acquired object, the model register the object into new memory
Pneumonia is frequently encountered in the clinical fields, both as a cause for admission and as a complication of the underlying disorder or as the course of treatment. Pneumonia is the second most common hospital-acquired infection and is associated with the highest morbidity and mortality rates among hospital-acquired infections. The guidelines for the management of hospital-acquired pneumonia by the American Thoracic Society include identifying individuals who have recently received antibiotics therapy or have been in medical facilities; these individuals are at higher risk for infection with multiple drug resistant organisms. Individuals, who have acquired pneumonia according to this clinical scenario, have what is known as healthcare-associated pneumonia (HCAP). Patients with HCAP should be considered to have potentially drug-resistant pathogens and should receive broad spectrum empiric antibiotic therapy directed at the potentially resistant organisms. In this paper, the diagnosis, risk factors, and treatment of HCAP are discussed.
From June 1984 to Aug. 1986, 90 cases of open heart surgery were performed at the department of cardiothoracic surgery of Kosin Medical College. There were 63 cases of congenital cardiac anomalies and 27 cases of acquired heart diseases. The sex ratio of congenital and acquired heart diseases were represented as 1.4:1 and 1:1.5 respectively. The age distribution was ranged from 7/12 to 56 years old. Among the 63 congenital cardiac anomalies, 49 cases of acyanotic group and 14 cases of cyanotic group were noted. In 49 cases of acyanotic group, 32 VSD, 15 ASD, 1 PS, and 1 RCA-LV fistula were noticed. In 14 cases of cyanotic group, 7 TOF, 1 TOF (Dextrosardia) combined with IVC interruption, 1 Triology of Fallot, 2 Pentalogy of Fallot. 1 DORV, 1 TA with PDA, VSD, ASD and Left SVC and 1 TAPVC were included. Of the 27 acquired heart diseases, 13 mitral, 3 aortic , 6 bival, 3 triplevalvular diseases and 2 LA myxomas were noted. Overall mortality were 11 cases, which included 2 cases of acyanotic heart diseases, 6 cases of cyanotic heart diseases and 3 cases of acquired hear diseases.
