Factors Affecting Quality of life in the Elderly with Chronic Musculoskeletal Pain (근골격계 만성통증 노인의 삶의 질 영향요인)

  • Purpose: This study aimed to identify factors affecting the quality of life of the elderly people with chronic musculoskeletal pain. Methods: The data were collected from 307 older adults aged 65 years or older with chronic musculoskeletal pain, who visited senior welfare centers in two cities. We used self-rated questionnaires including NRS for pain, WHOQOL-BREF for quality of life, Pain Response Inventory for coping responses to pain, and MSPSS for social support. Stepwise multiple regression analysis were performed using SPSS/WIN 23.0 to identify factors affecting the study subjects' quality of life. Results: The regression model explained 43% of quality of life, which was statistically significant (F=34.11, p<.001). Educational level of high school (${\beta}=.13$, p=.006), pain (${\beta}=-.13$, p=.013), restriction of function (${\beta}=-.13$, p=.028), accommodative pain coping (${\beta}=.24$, p<.001), family support (${\beta}=.18$, p<.001), colleague's support (${\beta}=.25$, p<.001), and perceived health status (${\beta}=.25$, p<.001) were identified as influential factors on subjects' quality of life. Conclusion: Developing integrative interventions is necessary to improve accommodative pain coping skills and to engage family and colleague in support for positive perception of older adults' health status and management of symptoms.

Analysis and Design of an Accommodation-Dependent Eye Model Based on Navarro Model (Navarro 모형안에 기반을 둔 조절을 고려한 모형안의 설계 및 분석)

  • Purpose: In this study, we proposed a simple accommodation-dependent crystalline lens with a constant volume and homogeneous refractive index. Methods: We proposed a human crystalline lens with two aspheric surfaces. Two surfaces intersect in two points and straight line between two points was equator(2b). It assumed that the derivative in axial direction was zero at the equator and the radial derivative was zero at the vertex. Proposed human crystalline lens was divided by the equator into the anterior and posterior parts. It was assume that the volume of each part and refractive index of the human crystalline lens were constant during accommodation. Results: For the changes during accommodation, geometrical parameters were determined by different objective distances. Considering the constant volume of each part with the small decrement of the equator, we obtained the paraxial parameters, such as the anterior and posterior vertex radius of curvature and lens thickness. Compared with the experimental data published in the literature, calculated values using simple approximation showed similar change per accommodative stimulus. Conclusions: These results showed that proposed simple approximation using assumption of constant volume and refractive index of the human crystalline lens made it possible to predict changes of geometrical parameters during accommodation.

Analysis of Binocular Abnormality by Eyeball Motion in Ametropia (비정시의 안구운동에 의한 양안시 이상 분석)

  • To evaluate the reliability of binocular vision measurements by phorometry. 90 students volunteered to participate in this study. 25 subjects were males, and 65 were females, they ranged in ages from 21 to 30 years. All subjects had normal ocular and systematic health, and all of them had at least 1.0 visual acuity with their best correction. At negative relative convergence(NRC) measurement in distance, the percentage of subjects is included in expected value(blur point/break point/recovery point) was 78%/61%/67%, divergence excess(DE) was 9%/31%/33%, and divergence insufficience(DI) was 13%/8%/9%, respectively. And positive relative convergence(PRC) measurement, includes expected value was 20%/46%/39%, convergence excess(CE) was 22%/14%/16%, and convergence insufficience(CI) was 35%/40%/45%. AC/A ratios of 42 subjects were normal. 38 were low, and the rest of them high. A low AC/A ratio is usually the result of a small vergence response in relation to accommodation. Negative relative convergence(NRC) at near, includes expected value was 26%/29%/44%, divergence excess(DE) was 61%/33%/24%, and divergence insufficience(DI) was 3%/38%/32%. And PRC at near, includes expected value was 33%/40%/31%, convergence excess(CE) was 61%/23%/42%, and convergence insufficience(CI) was 6%/37%/27%. For the near point of convergence(NPC) test, 58% of their subjects had a break of ${\leq}8cm$ with the accommodative target. In case of NRA(PRA) measurement, the expected value was 41%(33%). Accommodative insufficiency (AI) was 33%(43%), and accommodative excess(AE) was 26%(24%), respectively. AE was related to respectively low values of NRA. AI and CE are associated with high value of NRA, and the dysfunction of convergence excess combined with AE was related to a normal-high values of NRA. PRA in AI was related to a low value, wheres the dysfunction are associated with high values of PRA.

The Influence of Accommodation on Watching Home 3D TV at Close Distance (가정용 3D TV의 근거리 시청이 조절기능에 미치는 영향)

  • Purpose: This study was investigated weather watching 2D and 3D images effecting on accommodative function (AF), and differences between changes of AF by 2D and 3D. Methods: 50 subjects (male 30, female 20) aged 20's to 40's years old ($22.9{\pm}3.93$ years) who are available to watching 3D images were participated for this study. Accommodative amplitude (AA) by near point of accommodation (NPA), accommodative response (AR), positive and negative relative accommodation (PRA, NRA), accommodative facility (AF) were measured before, after watching 2D and 3D images at 1 m distance for 30 minutes respectively. Results: Accommodative amplitude after both watching 2D and 3D images decreased comparing to before watching images, and AA after watching 3D images was significantly lower than after watching 2D images. AR after both watching 2D and 3D images increased comparing to before watching images, but there was no difference between 2D and 3D. PRA and NRA were not significantly different between before, after watching 2D and 3D images. Accommodation speed by AF was increased for before watching ($13.52{\pm}3.32$ cpm) following by for after watching 2D images ($14.28{\pm}3.21$ cpm) and for watching 3D images ($14.90{\pm}3.27$ cpm). Conclusions: Watching images at close distance is effect to accommodation functions, and sequence of AA decrease of before watching images following by after watching 2D images and after watching 3D images may effect to asthenopia with same sequence as AA decrease. The results of increase of AF after watching images, specially 3D images show a possibility of vision therapy and further detail VT studies using 3D images are required in the future.

Comparison between Response AC/A and CA/C Ratio according to Additional Spherical Powers and Prism Powers (가입렌즈 도수와 프리즘 굴절력 변화에 따른 반응 AC/A비와 CA/C비 비교)

  • Purpose : This study was to examine a correlation of response AC/A ratio by additional spherical powers with PD(Pupilary distance) and relative accommodation, and was to investigate correlation of CA/C ratio by prism powers. The mean differences between a reciprocal response AC/A ratio and a CA/C ratio were compared, and were suggested to be used as data in the refractive power and prism prescriptions in the clinical. Methods: The open field autorefractometer (Nvision-K 5001, Shin nippon) and Howell target at 40 cm fixation distance to 62 persons with average $22.62{\pm}2.84$ ages were used to measure the response AC/A ratio according to additional Spherical powers. The CA/C ratios were measured using the DOG card at 40cm according to prism powers. Results: When the response accommodation according to Additional Spherical power changes and the response accommodation according to prism power changes were compared, it was larger than the response accommodation according prism power change. These were significant differences statistically. The correlation of response AC/A ratio and PD is showed as r = -0.158, the CA / C ratio is shown as r = 0.093. The correlation of response AC/A ratio and relative accommodation showed as r = -0.253, the CA/C ratio showed as r = 0.566 that is predictable correlation. The correlation of response AC/A ratio and CA/C ratio showed as r = -0.355 that is low minus correlation (p = 0.000). The difference between a reciprocal response AC/A ratio and a CA/C ratio showed $0.12{\pm}0.06D/{\Delta}$ with a significant difference statistically (p=0.000). Conclusions: The correlation of relative accommodation and CA/C ratio showed that depend on the individual, The more a relative accommodation is, the higher a response accommodation of convergence by convergence stimulus is. The reciprocal response AC/A and CA/C ratio showed significant differences statistically. This can have higher CA/C ratio in patients with low AC/A ratio in clinical as an independent variable. Thus when the abnormal binocular vision was prescribed in the clinical, it is necessary to consider the accommodative response even if the AC/A ratio is a normal range.

Visual Performances of the Corrected Navarro Accommodation-Dependent Finite Model Eye (안구의 굴절능 조절을 고려한 수정된 Navarro 정밀모형안의 시성능 분석)

  • In recent years, there has been rapid progress in different areas of vision science, such as refractive surgical procedures, contact lenses and spectacles, and near vision. This progress requires a highly accurate modeling of optical performance of the human eyes in different accommodation states. A new novel model-eye was designed using the Navarro accommodation-dependent finite model eye. For each of the vergence distances, ocular wavefront error, accommodative response, and visual acuity were calculated. Using the new model eye ocular wavefront error, accommodation dative response, and visual acuity are calculated for six vergence stimuli, -0.17D, 1D, 2D, 3D, 4D and -5D. Also, $3^{rd}\;and\;4^{th}$ order aberrations, modulation transfer function, and visual acuity of the accommodation-dependent model eye were analyzed. These results are well-matched to anatomical, biometric, and optical realities. Our corrected accommodation-dependent model-eye may provide a more accurate way to evaluate optical transfer functions and optical performances of the human eye.

Comparison of clinical techniques to assess objectively accommodative response (타각적 조절 반응 평가의 임상 기술들의 비교)

  • 조절반응의 평가는 옵토메트리 검사의 중요한 일부분이다. 본 연구는 조절반응의 타각적인 검사방법인 자동굴절계와 MEM 검영법 및 Nott 동적검영법으로 측정한 검사결과의 상관성과 차이를 비교하고자 하였다. 본 연구의 취지에 동의하는 19~29사이(평균나이 22.22${\pm}$2.43세)의 정상시력을 가진 27명의 대학생을 선정하여 4D(25cm)의 조절자극이 주어진 상태에서 자동굴절계(Shin-Nippon NVision-K 5001)와 MEM 검영법 및 Nott 동적검영법을 실시하여 조절 반응을 측정하였다. 4D의 조절자극에 대한 조절반응은 자동굴절계(Shin-Nippon NVision-K 5001) 3.70${\pm}$0.25D, MEM 검영법 3.58${\pm}$0.30D 그리고 Nott 동적검영법 3.77${\pm}$0.29D로 각각 측정되었다. 3가지 검사는 상호간에 높은 상관성이 있었다(p<0.05). MEM 검영법은 Nott 동적검영법보다 0.19${\pm}$0.13D 더 낮게 측정되었고(p<0.05), 자동굴절계(Shin-Nippon NVision-K 5001) 보다 0.12${\pm}$0.24D 더 낮게 측정되었다(p<0.05). Nott 동적검영법과 자동굴절계(Shin-Nippon NVision-K 5001)로 측정한 값은 유의한 차이가 없었다(p>0.05). MEM 검영법으로 측정한 조절반응은 Nott 동적검영법이나 자동굴절계로 측정한 값보다 더 낮았다. 저자는 조절반응을 평가할 때, MEM 검영법을 제외한 Nott 동적검영법과 자동굴절계(Shin-Nippon NVision-K 5001)는 상호 대체하여 사용할 수 있다고 주장한다.

Diameter of the retinal blur circle in a artificial hypermetropia (인워적 원시에 따른 망막에서 착란원 크기)

  • In a artificial hypermetropia with the accommodative response, we investigated a diameter of blur circle as a function of test lens refractive power. In a schematic eye model of the hypermetropia, the second focal length along to accommodated power of the crystal lens are calculated as a function of test lens power and, also distance between the retina and exit pupil are calculated as a function of accommodated power. As these results are compared, the size of blur circle on the retina are obtained.

A Study of Accommodative Response on Emmetropia (정시안의 조절반응량에 관한 연구)

  • The purpose of this research was to measurement real accmmnodative response amount of emmetropia using both eyes open-view auto-refractometer. We classified 377(male 146, female 231)eyes showing good naked vision at least 1.0, with a spherical equivalent refraction of under ${\pm}0.50D$ and cylindercal refraction of under -0.75D, from 10 to 59(mean $25.8{\pm}12.4$) years into 5 groups with 10 year interval and analyzed change of the near astigmatism and accommodative response amount to distinction of age. Age comparison, total mean 1.47D, 10 years age group 1.63D, 20 years age group 1.57D, 30 years age group 1.71D, 40 years age group 0.97D, and 50 years age group 0.05D. It was no difference between 10 to 30 years age group, But it showed the decre8.se which is remarkable from above 40 years age group and it does not almost response from above 50 years age group. Sexual difference, the man appeared some highly in male(mean $26.4{\pm}12.3$ years) 1.56D m female(mean $26.4{\pm}12.3$) 1.43D. Near-astigmatism power change, mean +0.12D, 10 years age group +0.06D, 20 years age group +0.16D, 30 years age group +0.09D, 40 years age group +0.05D, and 50 years age group +0,06D. No significant difference of astigmatism powers among the an groups, but from all years age group the astigmatism power appeared more highly far-astigmatism than near-astigmatism. Near-astigmatism axis change in the far-astigmatism changes at under ${\pm}10^{\circ}$ 103 eyes, above ${\pm}10^{\circ}$ rotation clockwise direction 108 eyes, above ${\pm}10^{\circ}$ rotation counter-clockwise direction 101 eyes. It was analyzed near-astigmatism axis change.

A Study on the Stability Fatigue according to the Accommodation Change due to the Use of Smartphone (스마트폰의 사용으로 인한 조절 변화에 따른 안정피로도에 대한 연구)

  • Purpose. In this study, The change of the accommodation function due to the use of a smartphone was objectively evaluated using an Auto Refractokeratometer and was to investigate the effect of accommodation response and microfluctuations of accommodation on stable fatigue. Methods. Twenty subjects (20 males, 20 females) who had no abnormalities in ophthalmic disease and strabismus, diplopia, suppression, convergence and ocular movement, and corrected visual acuity and unaided visual acuity of 0.8 or more were performed. The average age of the participants is (21.78 ± 1.78) years old. The refractive power of the eye was measured using an Auto Refractokeratometer (Speedy-i K-model, Righton Mfg Co. Tokyo Japan), and the equivalent spherical power was automatically calculated from the measured refractive power. The accommodation response amount was calculated for the accommodation stimulation amount in 8 steps in 0.5D increments from + 0.50D to -3.00D in the calculated equivalent spherical power. The microfluctuations of accommodation in the high frequency region was calculated according to the change in the amount of the accommodation stimulus. At this time, the spectral power of the microfluctuations of accommodation was analyzed by Fast Fourier Transformation (FFT). Results. After using the smartphone, it was found that the accommodation response to accommodation stimulation decreased and the microfluctuations of accommodation increased. Conclusions. It can be said that the use of a smartphone affects the accommodation response and the microfluctuations of accommodation, thereby causing a accommodative stable fatigue. Therefore, it is thought that the use of a smartphone for a long time may bring about a change in the accommodation function.
