A Study on the Final Probabilistic Safety Assessment for the Jordan Research and Training Reactor (JRTR 연구용원자로에 대한 최종 확률론적 안전성평가)

  • This paper describes the work and the results of the final Probabilistic Safety Assessment (PSA) for the Jordan Research and Training Reactor (JRTR). This final PSA was undertaken to assess the level of safety for the design of a research reactor and to evaluate whether it is probabilistically safe to operate and reliable to use. The scope of the PSA described here is a Level 1 PSA, which addresses the risks associated with core damage. After reviewing the documents and its conceptual design, nine typical initiating events were selected regarding internal events during the normal operation of the reactor. AIMS-PSA (Version 1.2c) was used for the accident quantification, and FTREX was used as the quantification engine. 1.0E-15/yr of the cutoff value was used to deliminate the non-effective Minimal Cut Sets (MCSs) when quantifying the JRTR PSA model. As a result, the final result indicates a point estimate of 2.02E-07/yr for the overall Core Damage Frequency (CDF) attributable to internal initiating events in the core damage state for the JRTR. A Loss of Primary Cooling System Flow (LOPCS) is the dominant contributor to the total CDF by a single initiating event (9.96E-08/yr), and provides 49.4% of the CDF. General Transients (GTRNs) are the second largest contributor, and provide 32.9% (6.65E-08/yr) of the CDF.

  • Objectives : Suicide is a serious sentinel event in healthcare organization. Suicide and suicidal attempt are fatal and long lasting mental and physical damage to themselves, family, and medical staffs. To develope the system to prevent suicidal accident in hospital, we reviewed and analysed one case of suicidal sentinel event. Methods: The risks of suicidal sentinel event were evaluated and analysed through the root cause analysis and failure mode effects analysis. Result: We found several root causes such as initial assessment of oldest patient and security issues. Couple of action plans to fix the problems were done immediately. According to failure mode, we evaluate the risk priority number to modify the action plans. Conclusion: To reduce the risk of sentinel events, we reviewed the suicidal event and established the new system and action plan to prevent sentinel events.

The Levels of Impacts of Events, Depression and Anxiety among Injured Workers (산업재해 근로자의 사건충격, 불안 및 우울)

  • Purpose: The aim of this study was to investigate the impact of accidental events, depression, and anxiety among workers with industrial accidents in South Korea. Methods: The participants were 510 workers with industrial accidents. Data were collected by personal interviews with structured questionnaires for three months from August to October in 2005. For analyses, frequencies and means were utilized. Results: The participants' average age was 44.9 years, and about 91%of them were male. Also, they had a greater risk of poverty after being injured. The most frequent cause of accidents was fall (32.2%), and the most frequent injured body area was extremities (73.9%). For around a half of the participants, the treatment period was 12 months or shorter. The participants were at great risk of experiencing a negative impact due to events, depression, and anxiety. Conclusion: It is necessary to develop: (a) strategies for injured workers to be financially stable during recuperation; and (b) supporting systems for them not to suffer and exacerbate mental health problems after being injured.

  • The purpose of this paper is to conduct ETA on six items of ESS: the whole system, battery, BMS, PCS, ESS and cable. To achieve that, ESS work flow and its components are categorized. Based on performance, human, environmental, management, and safety, this paper drew initiation events (IE) and end states (ES). ETA is applied to the main functions of each item, and the end states that may occur in one initiation event are suggested. In addition, detailed classification was performed to induce various end states on the basis of the suggested initiation events ; loss of grid electricity of ESS, loss of battery electricity(DC) of battery, impairment of electric function of BMS, loss of grid electricity(AC) of PCS, loss of data of EMS, Mechanical damage of cable, event sequence analysis conducted on the basis of event trees. If the suggested IEs and ESs are applied on the basis of ESS event cases, it is expected to prevent the same kinds of accident and operate ESS safely.

Probability subtraction method for accurate quantification of seismic multi-unit probabilistic safety assessment

  • Single-unit probabilistic safety assessment (SUPSA) has complex Boolean logic equations for accident sequences. Multi-unit probabilistic safety assessment (MUPSA) model is developed by revising and combining SUPSA models in order to reflect plant state combinations (PSCs). These PSCs represent combinations of core damage and non-core damage states of nuclear power plants (NPPs). Since all these Boolean logic equations have complemented gates (not gates), it is not easy to generate exact Boolean solutions. Delete-term approximation method (DTAM) has been widely applied for generating approximate minimal cut sets (MCSs) from the complex Boolean logic equations with complemented gates. By applying DTAM, approximate conditional core damage probability (CCDP) has been calculated in SUPSA and MUPSA. It was found that CCDP calculated by DTAM was overestimated when complemented gates have non-rare events. Especially, the CCDP overestimation drastically increases if seismic SUPSA or MUPSA has complemented gates with many non-rare events. The objective of this study is to suggest a new quantification method named probability subtraction method (PSM) that replaces DTAM. The PSM calculates accurate CCDP even when SUPSA or MUPSA has complemented gates with many non-rare events. In this paper, the PSM is explained, and the accuracy of the PSM is validated by its applications to a few MUPSAs.

Case Studies of Firefighter Burns Safety Accident during Fire-fighting Activities (현장 소방활동 중 소방공무원 화상사고 사례 분석 연구)

  • The purpose of this study is to analyze four cases of firefighter burns in various fire scenes and to find prevention measures to decrease firefighter injuries. Among the analysis reports prepared by the National Fire Research Institute of Korea from 2016 to 2020, four burn-related accidents are summarized and the main causes are conveyed. The four accidents include second-degree burns from using extinguishers during containment of fires; nine firefighters burned due to re-ignition in the LPG car repair shop; two firefighters injured with third-degree burns from using fire extinguishers during life-saving events in residential housing; and injuries from the radiant heat of the tank BLEVE near the factory fire. These cases are comprehensively investigated in their respective scenes and analyzed based on the fire site investigation reports from the fire department and related theoretical explanations of risk for each accident scene. In the third case study, some experimental research is conducted to evaluate the risk involved with the use of safety gloves. This is evaluated by reviewing Fire Tactics and Standard Operational Procedures (SOP) to determine improvements and recommendations for an efficient firefighting response. Results show that the main causes of burn accidents are the insufficient use of personal protective equipment (PPE), such as safety gloves, and the failure to follow firefighting tactics or SOPs. Through the accident investigation and assessment, it is concluded that to reduce the frequency of burn accidents, the performance of firefighting equipment, SOPs, protection tactics, and safety policy systems require improvement.

  • The main purpose of this study was to develop fuzzy models in order to decide system failure rate in industrial accident prevention. The purposed linguistic approach uses the Zadeh's concept of a linguistic variable with value which are not number. The problem of measurement Is the assignment of numbers to reprresnt properties of the involved events, object, or situation. Thus, in this study, part standard compatibility function was used.

Advanced Freeway Traffic Safety Warning Information System based on Surrogate Safety Measures (SSM): Information Processing Methods (Surrogate Safety Measures(SSM)기반 고속도로 교통안전 경고정보 처리 및 가공기법)

  • This study presents a novel traffic information system which is capable of detecting unsafe traffic events leading to accident occurrence and providing warning information to drivers for safer driving. Unsafe traffic events are captured by a vehicle image processing-based detection system in real time. Surrogate safety measures (SSM) representing quantitative accident potentials were derived, and further utilized to develop a data processing algorithm and analysis techniques in the proposed system. This study also defined 'emergency warning area' and 'general warning area' for more effective provision of warning information. In addition, methodologies for determining thresholds to trigger warning information were presented. Technical issues and further studies to fully exploit the benefits of the proposed system were discussed. It is expected that the proposed system would be effective for better management of traffic flow to prevent traffic accidents on freeways.

The Counter-Terrorism Measures for International Sports Events in Korea (한국의 국제스포츠 행사에 대한 대 테러 전략)

  • Recently, alarmed by the frequent international terrorism or safety accidents, the host countries of world sports events are focusing on security activities for the sake of the participants, facilities, events and competitions. They are alarmed because in case any happenings like international terrorism or safety accident, the contest itself can be criticized to be a failure as much as the international reliability of the host nation may well crash. We can see that any failure in coping with terrorism or safety matters can lead to a nation-wise crisis in the case of Indonesia where the October 12th of 2002 terrorist bomb attack in Bali damaged the image of the nation followed by the similar case in Philippine (Oct. 20th, 2002) where the same terrorist attack dramatically scared away the tourists to the nation. Korea is scheduled to hold the World Championship in Athletics in Daegu Metropolitan City in 2011. Also, it is slated to host various world sports events such as Yeosu International Exposition and Incheon Asian Games. In these contexts, this study analyzes counter-terrorism cases related with the recent international sports events that have been organized in a variety of manners in the era of globalization. This study aims to show alternatives for the safety management in these events. In other words, it is focusing on giving directions to the safety policies of the nation -which may become the future hub of north-east Asia and the world - for more perfect guard and defense, and counter-terrorism activities in all the conferences, sports events and international festivals where any private defense and guard companies are allowed to cooperate with the police force or public security agencies.

Intelligent DB Retrieval System for Marine Accidents Using FCM (FCM을 이용한 지능형 해양사고 DB 검색시스템 구축)

  • Marine accidents have always caused huge economic losses, as well as environmental pollution. Prevention of marine accidents has become a focus of argumentation. The analysis of past accident cases, reviewing the experience and lessons, is important and necessary for preventing marine accidents. With the same subject above, the Korean Maritime Safety Tribunal provides for past marine accidents' written judgments and analysis of judgment and associated retrieval system on its homepage. In these systems, the name of the ship, accident occurrence time, accident pattern or related keywords are used as search conditions. However, most of the marine events' happening were not due to a single reason, but multiple ones. In addition, one marine event could often come under several categories. In this case, now the retrieval systems' DB is used on the Korean Maritime Safety Tribunal homepage was built based on single category and failed to be able to retrieve according to multiple reasons or multiple categories. In order to solve this problem, a more practical retrieval approach might be needed. Therefore, in this paper, a new retrieval system will be proposed, which using the linguistic label to describe the cluster after analyzing the relational properties between marine accidents and clustering by FCM algorithm, and then adding an interface to allow users to get the results they want through choosing multiple reasons or multiple categories.
