한국의료패널데이타를 이용한 외래 환자 손상의 사회경제적 비용 추계 (Estimating social and economic costs for outpatient injuries by using Korea medical panel data)

  • 손상 사고 중독과 관련해 환자의 의료비 증가는 국가건강보장체계의 지속가능성을 제고하기 위한 중대한 과제이며, 제도의 개선이나 건강보험의 관리 운영 효율화를 통한 의료비 증가를 억제할 필요가 있다. 따라서 손상 발생과 사망률이 높은 우리나라에서 손상 문제를 사회적으로 강조하기 위해서는 손상의 사회경제적 비용을 추계하는 것이 필요하다. 본 연구는 2008년도 한국의료패널 조사 중 외래 손상 사고 중독 환자를 대상으로 이용실태와 의료비지출을 전반적으로 파악하여, 손상으로 외래 서비스 이용 시 환자의 직접비용과 생산차질 등으로 인한 생산손실비용을 추계하였고, 궁극적으로 손상의 사회경제적 손상 비용을 추계하였다.

A Study on the Importance of Uninsured (Indirect) Cost Item of Workplace Accidents

  • Estimation of accident cost is a sound and great safety indicator on determining accurate occupational safety and health prevention. Just like in Korea, Heinrich ratio analysis of (1:4) between direct and indirect costs has been become widely used in safety management because of its simplicity. In this study four major categories of uninsured (indirect) cost items and 18 sub-categories of uninsured (indirect) cost items were identified. To determine and validate the importance and necessity of the results of a literature review an expert or professional surveyed had been analyses using the SPSS 18.0, where in the participants whose expertize is in the field of compensation and safety. Based on the results of survey all participants all uninsured (indirect) cost items classified was important and necessary when accidents occurred. Despite recognition of expert on the classification of uninsured (indirect) cost items, it is quite difficult to make generalization for all kind of costs in occupational accident case due to different nature of business for each industry.

A Survey on the Implementation of Musculoskeletal Disorders Prevention Program in Shipbuilding Industry

  • Objective: In 2010, the rate of injuries and illnesses in the shipbuilding industry was 1.20, about 1.74 times higher than the average of overall industries(0.69). Musculoskeletal disorders(MSDs) became a major problem since 2000 in shipbuilding industry. The purpose of this study was to provide efficient implementation management through evaluating the economic effectiveness of the Musculoskeletal Disorders Prevention Program(MDPP). Successfully implementing the MDPP in the workplace may decrease the risk of MSDs and therefore reduce MSDs related costs. Methods: Occupational accident index (frequency rate, severity rate, and frequency severity indicator), estimated direct costs, and total occupational accident occurrence rate were provided by analysis of MSDs, statistical analysis of MDPP decreed companies, and data analysis of implementation. Results: The study shows the number of MSDs for MDPP decreed companies decreased 76.2% from 2,319 in 2004 to 551 in 2008. Above all, the number of MSDs decreased 78.7% in Shipbuilding industries. Forty companies (85.1%) showed a decreasing tendency and 27.7% of those had an $R^2$ > 0.7. Similar results were found in severity rate and frequency severity indicator. From these results, it appears there are a number of advantages in implementing the MDPP. Conclusion: MDPP effectiveness was demonstrated by analysing implementation and occupational accident index. A more effective and efficient implementation would take into account business sector, size, policy, and organization of the company.

안강망어업 작업 중 안전사고 저감의 경제적 효과 분석 (An economic feasibility analysis of the reduction of accident at fishing operation in stow net fishery)

  • This study aimed to analyze the economic feasibility of the reduction of working safety accidents in stow net fishery through the prevention education to fishing crews. The benefit was evaluated by estimating insurance payment amount that was reduced through prevention education. The decreasing rate of working safety accidents was presumed to follow the experience in Japan. The cost was assumed as the cost of textbook development. Results indicated that IRR was estimated to be 79.9% in case of scenario assuming no additional operating costs while IRR was 56.3% in case of scenario assuming 10% additional operating costs. In addition, the economic feasibility would be secured when annual operating costs increased to 274 million won according to the result of sensitivity analysis.

비보험비용의 발생 단계 분류에 관한 연구 (A Study on the Classification of Uninsured Cost Occurrence)

  • Although prior researches have been investigated the impact of insured cost(direct cost) on employers and employees, little work has attempted to categorize the items of unsecured cost(indirect cost) by accidents. On this basis, the goal of this study was to achieve a better understanding of the nature of accident cost of unsecured cost. Specifically, this study aimed to categorize the unsecured cost items according to the domestic industry circumstances and use these results for a basis of other accident cost related studies. The results of this study are as follows: (1) accident development steps were categorized as twelve items for improved management according to each step of accident development (2) the points of occurrence and termination of the unsecured cost were identified for the improved management according to each step of accident development and (3) characteristics of each item in unsecured cost were studied and identified for a better control of accident costs. These results provide a basis for further researches on the unsecured cost.

무료심판변론제도의 도입에 관한 연구 (A Study on the Introduction of Free Counsel System for the Maritime Safety Tribunal in Korea)

  • 해양안전심판에는 징계와 권고 또는 명령 등 개인의 권익에 대한 제한이 뒤따르며, 또한 그 결과는 해양사고 관련 민사재판에도 지대한 영향을 미치게 된다. 우리의 해양안전심판제도에는 해양사고관련자가 심판변론인의 도움을 받을 수 있도록 되어 있지만 그 비용이 만만치 아니한 관계로 경제적인 사정에 따라 제약을 받게 되어 있다. 본고에서는 경제적인 사유로 심판변론인의 도움을 받지 못하여 불이익을 당하는 해양사고관련자들을 위하여 사법제도 등의 유사제도를 검토$\cdot$ 연구한 결과 다음과 같은 방법에 의한 무료 심판변론제도의 도입을 제안한다. 1. 심판법령에 "강제변론주의"와 "국선변론인제도"를 도입하고 그 비용을 국가가 부담 2. 위 제도를 도입하되, 국선변론인을 해사관련단체 소속 임직원 중에 선임하고 비용은 소속단체에서 부담 3. 심판원 산하단체로서 금융지원 등 구조활동을 수행할 "해양안전심판협회" 설립

자동차사고 손상유형과 상해에 관한 실사고 연구 (A Real-world Accident Study on Vehicle Damage Types and Occupant Injury)

  • Base on insurance vehicle collision and bodily injury claim reports, 23,655 cases of vehicle to vehicle accidents occurred in Korea 2010 are investigated in order to understand vehicle damage severities, repair costs and occupant injury types. The results of our statistical analysis reveal that minor damages with small dent or scratches on vehicle body panels which is assumed to imply during very low speed crashes are major portion of accident severities types. The most vulnerable body regions due to the real-world accident are neck. The 86.3% of total injured driver in minor rear damaged vehicles has reported neck pains and they are followed by whole bodies and head but with much lower occurrence rates.

  • This study develops modeling processes for alignment optimization considering characteristics of intersections using genetic algorithms and GIS for road alignment optimization. Since existing highway alignment optimization models have neglected the characteristics of intersections, they have shown serious weaknesses for real applications. In this paper, intersection costs include earthwork, right-of-way, pavement, accident, delay and fuel consumption costs that are sensitive and dominating to alignments. Also, local optimization of intersections for saving good alignment alternatives is developed and embedded. A highway alignment is described by parametric representation in space and vector manipulation is used to find the coordinates of intersections and other interesting points. The developed intersection cost estimation model is sufficiently precise for estimating intersection costs and eventually enhancing the performance of highway alignment optimization models. Also, local optimization of intersections can be used for improving search flexibility, thus allowing more effective intersections. It also provides a basis for extending the alignment optimization from single highways to networks. The presented two artificial examples show that the total intersection costs are substantial and sensitive to highway alignments.

국가항공안전비용 산출방법론에 관한 연구 (Study on Calculation Methodology for National Aviation Safety Cost)

  • The world air transport industry has grown steadily with quantitative expansion. The volume of air transport in the world is skyrocketing with the open-sky trend. Air passengers from or to South Korea has shown 5% of annual growth for the last couple of decades which caused South Korea now ranking in 18th in the World for air transport market size. Quantitative expansion of Air transport affects in policy making of air transport operators, such as airports, airlines or authorities, directly and indirectly. Especially, Aviation Safety field should be supported by policy regime with the growth of air transport volume, assured resources for continuous monitoring is standing out as a vital factor. This study is to estimate social costs caused by aviation accidents and investment costs for aviation safety by airports, airlines and authorities as operators. Estimated investment costs for aviation safety verified by comparing and analysing them. Precedent studies were reviewed to refer research methodology to calculate aviation accident costs and safety costs of operators. Safety costs of operators was calculated with literature researches and interview surveys among professionals of each operators in rational range.

순서형 프로빗 모형을 적용한 공사장 교통 사고심각도 분석 (Developing the Accident Injury Severity on a Field of Construction Work Using Ordered Probit Model)

  • The traffic accidents at a construction site, which happen due to construction vehicles' frequent access to a construction site, its subsequent conflicts with ordinary vehicles and pedestrians, and inappropriate installation & management of traffic security facilities, have not many proportions in all traffic accidents, but obviously, the accident damage is quite serious when comparing the level of the fatal per one accident. This research conducted an analysis of traffic accident injury severity using Ordered Probit Model in relation to 241 traffic accident cases that occurred caused by construction sites among the traffic accidents that took place in Seoul and Gyeoggi-do region for two years from 2006 until 2007. As a result, the significant variables enough to explain traffic accident injury severity were analyzed to be the state of road surface, linear shape of an accident spot & whether the damaging car belongs to the vehicle for construction, and whether vehicles have access to a construction site at the time of an accident. Through this, this research found out some fact as follows: first, there need to be more aggressive management of the vehicles for construction and a year-round placement of the manpower who can control vehicular access to a construction site. Second, it is necessary to get drivers to recognize the fact that there exists a construction site on the construction section which is on the border of curved roads in advance to prevent a traffic accident, helping to reduce socioeconomic loss & costs incurred by a traffic accident.
