Surgical Treatment of Wolff-Parkinson-White Syndrome Combined with AV Nodal Reentrant achycardia in a Patient with Ebstein`s Anomaly - A report of one case - (Ebstein씨 심기형에 동반된 Wolff-Parkinson-White 증후군 및 방실결절 회귀성 빈맥에 대한 수술치험 1례 보고)

  • A 17 year old female patient with Ebstein`s anomaly received surgical treatment for WPW syndrome and AV nodal reentrant supraventricular tachycardia[SVT] Electrophysiologic study revealed that an anomalous pathway was located in the right posterolateral portion and antegrade dual AV nodal pathway responsible for AV nodal reentrant tachycardia. The patient was underwent surgery on February 18, 1987. Intraoperative mapping was used to define the location of accessory pathway. The accessory pathway was cryoablated through the epicardium. Simultaneously discrete cryoablation around the perinodal area was performed to prevent AV nodal reentrant SVT. The atrialized right ventricle of Ebstein`s anomaly was plicated with 11 pledget mattress sutures under the cardiopulmonary bypass. Two and half years after surgery, the patient has no evidence of WPW syndrome or supraventricular tachycardia.

  • Supraventricular tachyarrhythmias are readily characterized and understood, but the surgical procedures for their correction are complex and not easily mastered. Conversely, ventricular tachyarrhythmias are frequently difficult to characterize and localize electrophysiologically and their basic mechanisms are poorly understood. The role of the surgeon in the treatment of cardiac arrhythmia has changed dramatically during the past decade. This report is a case of 26 years old male with supraventricular tachyarrhythmia. The result of endocardial electrophysiologic study demonstrated accessory pathway connecting left atrium to left ventricle which located at left atrial free wall about 4 cm apart from the coronary sinus orifice. The accessory bundle interruption has been successfully accomplished utilizing the internal open heart technique. The operation consisted of dissection of the atrioventricular fat pad and division of all the superficial fibers going from the ventricle to the annulus. Following this, cryoablation made with cryoprobe at - 60` for 90 seconds. The accessory pathway was successfully ablated without specific problems.

  • A 21-year-old man with atrioventricular[AV] reentry tachycardia secondary to concealed accessory AV connection underwent surgical division of two accessory pathways following failure of radiofrequency catheter ablation. pathways were located in the left free wall area.Before cardiopulmonary bypass, the epicardial mapping confirmed the existence and localization of two accessory pathways. The patient was approached through a left atriotomy with a dissection of the left free wall area beginning with an internal mapping was carried out after separation from cardiopulmonary bypass to confirm the absence of retrograde conduction of accessory pathway. Five weeks after surgery, the electrophysiololgic study demonstrated no retrograde conduction through two accessory pathways.

  • Male accessory glands and ejaculatory duct of Drosophila melanogaster are reproductive organs which synthesize secretory seminal proteins. Several products of these organs involved in egg laying, receptivity, and sperm stability or storage were isolated from their lumens. Despite their secretory process play an important role, exocytosis pathway in these organs is not well known. In the present study, we characterized secretory protein profiles and determined their secretory mechanisms. Eight accessory gland secretory proteins and two ejaculatory duct secretory proteins were detected in their lumens. All these proteins were constitutively synthesized in these organs and secreted to their lumens. Secretion of newly synthesized proteins initiated at about 1 h after synthesis, and reached the peak at 4 h after synthesis. It seems that secretion of the proteins may occur via constitutive exocytosis pathway.

  • Purpose: The aim of the present study was to evaluate the characteristics of electrophysiologic studies (EPS) and radiofrequency ablation (RFA) performed in subjects aged less than 30 years with Wolff-Parkinson-White (WPW) syndrome, particularly pediatric patients under 18 years of age, based on our experience. Methods: Two hundred and one consecutive patients with WPW syndrome were recruited and divided to 3 groups according to age: group 1, 6 to 17 years; group 2, 18 to 29 years; and group 3, 30 to 60 years. The clinical, electrophysiological, and therapeutic data for these patients were evaluated by a retrospective medical record review. Results: A total of 73 (36%) of these patients were <30 years of age. Although there were more males than females in group 2 (male:female, 31:11), there was no sex difference in group 1 (male:female, 16:15). Left accessory pathway was detected less frequently in group 1 (32%, 10/31) than in group 2 (57%, 24/42) and group 3 (63%, 81/128) (P=0.023 and P=0.002, respectively). Conclusion: The present study describes several different electrophysiological characteristics in children and adolescents with WPW syndrome. Therefore, when EPS and RFA are performed in children and adolescence with WPW syndrome, we recommend that these characteristics be considered.

  • The late-stage doxorubicin biosynthesis pathway acting enzyme (DoxA) from Streptomyces peucetius CYP129A2 exhibited substrate promiscuity towards the stilbene group of compounds such as resveratrol. DoxA along with two accessory enzymes ferrdoxin reductase and ferredoxin from spinach hydroxylated resveratrol at the 3'-position in vitro to produce piceatannol. The product was identified by HPLC-PDA and high-resolution HR-qTOF-ESI/MS analyses in positive mode. The ESI/MS fragments resembled the hydroxylated product of resveratrol.

  • This paper present a algorithm for determination of the frontal QRS axis. Determination of electrical QRS axis helps In the differential diagnosis of wide QRS tachycardia and of hemiblock and In the localization of an accessory pathway. At first detecting R-point data and S-point data and two data is sumed and this data is determind such as positive or negative. Reference data is calculated by 9-point derivertives that is less affected by noise. Secondly, using data of lead2 calculate a morphology, this value is threshold for executing determination algorithm. This process is main body of this algorithm. As this algorithm have a six pattern of the axis that coded by minnesota ending method, the axis is determined more precisely than any other algorithm using 3 leads and affirm a relation of a axis and hemiblock and tachycardia.

Abnormal Perfusion on Myocardial Perfusion SPECT in Patients with Wolff-Parkinson-White Syndrome (Wolff-Parkinson-White 증후군 환자의 심근 관류 이상)

  • Purpose: Abnormal myocardial perfusion may be caused by ventricular preexcitation, but its location, extent, severity and correlation with accessory pathway (AP) are not established. We evaluated perfusion patterns on myocardial perfusion SPECT and location of AP in patients with WPW (Wolff-Parkinson-White) syndrome. Materials and Methods: Adenosine Tc-99m MIBI or Tl-201 myocardial perfusion SPECT was performed in 11 patients with WPW syndrome. Perfusion defects (PD) were compared to AP location based on ECG with Fitzpatrick's algorithm or electrophysiologic study and radiofrequency catheter ablation. Results: Patients had atypical chest discomfort or no symptom. Risk of coronary artery disease (CAD) was below 0.1 in 11 patients using the nomogram to estimate the probability of CAD. Coronary angiography was performed in 4 patients (mid-LAD 50% in one, normal in others). In 4 patients, AP localization was done by electrophysiologic study and radiofrequency catheter ablation (RFCA). Small to large extent ($11.0{\pm}8.5%$, range:$3{\sim}35%$) and mild to moderate severity ($-71{\pm}42.7%$, range:$-2l7{\sim}-39%$) of reversible (n=9) or fixed (n=1) perfusion defects were noted. One patient with right free wall (right lateral) AP showed normal. PD locations were variable following the location of AP. One patient with left lateral wall AP was followed 6 weeks after RFCA and showed significantly decreased PD on SPECT with successful ablation. Conclusion: Myocardial perfusion defect showed variable extent, severity and location in patients with WPW syndrome. Abnormal perfusion defect showed in most of all patients, but it did not seem to be correlated specifically with location of accessory pathway and coronary artery disease. Therefore myocardial perfusion SPECT should be interpreted carefully in patients with WPW syndrome.

Thermal Property Measurement of Swine Atrium

  • Thermal conductivity, thermal diffusivity were measured in the atrium of a swine heart. Radiofrequency (RF) catheter ablation in an atrium has rapidly emerged at the treatment of symptomatic reentrant arrhythmia associated with accessory pathway or Atrioventricular (AV) node conduction. The thermal properties of an atrium are definitely necessary for these treatments because, in thermal treatments, conductivity and diffusivity are significant factors in the relationship between the applied RF power and the resulting atrium temperature rise. Thermal properties were measured using a self-heated thermistor probe. Thermistor probes were inserted into the tissue of interest and were used to supply heat within the tissue as well as to monitor the temperature rise in the tissue. The measurements were performed at temperatures of 25, 37, $50^{\circ}C$. Atrium thermal conductivity ranged from 5.17$\pm$0.12 mW/cm$^{\circ}C$ at $25^{\circ}C$ to 5.33$\pm$0.08 mW/cm$^{\circ}C$ at $37^{\circ}C$. Atrium thermal diffusivity ranged from 0.00132$\pm$0.00007$cm^2$/sec at $25^{\circ}C$ to 0.00138$\pm$0.00003 $cm^2$/sec at $50^{\circ}C$. This paper also present the thermal property comparison of both chambers of a heart (ventricle and atria).
