Reduction and Fixation Methods for Fractured Anterior Maxillary Sinus Wall Using Suture Tie (봉합결찰법을 이용한 골절된 상악동 전벽의 정복과 고정)

  • The anterior maxillary sinus walls are the most frequently injured sites in midfacial fractures. The maxillary sinus is a difficult surgical site for reduction and fixation due to its narrow surgical field, and has a chance of developing sinusitis when sufficient treatment is not given. In this study, the methods developed by the authors for managing such are introduced. Two small openings were made on both sides of the fracture line, then a suture knot was tied instead of wiring for reduction and fixation. Then an absorbable mesh was applied on top of the fracture site, with a suture knot for additional fixation. This method was applied on an actual patient, and it was a convenient method despite the narrow surgical field that was provided. The authors believe that using suture knots to fixate fractured segments and absorbable mesh is relatively convenient and economically efficient when it comes to the reduction and fixation of the maxillary sinus wall fracture with several fragments.

  • Sutures support wound healing during the initial phase. As the tensile strength of a wound increases, the need for the presence of sutures becomes less important. For this reason, absorbable suture materials have been sought. Nowadays, commonly used suture materials are Chromic Catgut, Coated Vicryl, and PDS. Among these materials, Coated Vicryl is the most popular. Studies were conducted to compare the handling properties, reliability of knots, wound healing, and tissue reactivity(Inflammation, Fibroblast proliferation, Collagen deposition, Giant cell reaction, Absorption) between AILEE vicryl and ETHICON vicryl. We used twelve purebred New Zealand white rabbits, and biopsied the tissue at three, seven, fourteen, and thirty days post implantation. The results showed that both were supple and easy to handle and tie, and gross and histologic differences were not apparent.

THE STUDY ON RIDGE PRESERVATION FOR IMPLANT SITE DEVELOPMENT (임플란트 식립을 위한 치조제 보존술에 관한 연구)

  • Purpose : The aim of this study was to investigate healed bovine bone particles ($Bio-Oss^{(R)}$) and absorbable collagen sponge ($CollaPlug^{(R)}$) applied extraction socket site at 4-6 months' post-extraction. Material and methods : From August, 2004 to October, 2005, 17 sockets in 5 adult patients were selected out of the patients whose received ridge preservation using bovine bone particles and absorbable collagen sponges at Dept. of oral and maxillofacial surgery in Samsung Medical Center. There were 5 male patients, ages 30 to 58 years. Immediate postoperation and 4-6 months after operation study models were compared to evaluate the ridge dimension by measuring vertical height and horizontal width of alveolar ridge. Results : The measurements at 4-6 months revealed, in the ridge dimension, a loss of vertical height of 0.91${\pm}$0.40mm and horizontal width of 1.25${\pm}$0.58mm. There was no adverse reaction. Conclusion : This study suggests that treatment of extraction sockets with graft materials and collagen sponges is valuable in preserving alveolar bone in extraction sockets and preventing alveolar ridges defects.

POST-OPERATIVE SKELETAL STABILITY OF THE MAXILLA TREATED WITH LE FORT I AND U-SHAPED OSTEOTOMIES IN SIMULTANEOUS MAXILLOMANDIBULAR ORTHOGNATHIC SURGERY (양악 악교정 수술에서 르포트 I형과 U-자형 복합 골절단술 후 상악골의 안정성에 관한 임상적 연구)

  • Postoperative skeletal stability was evaluated in combination of Le Fort I and U-shaped osteotomies for superior repositioning of maxilla in bi-maxillary surgeries in 30 consecutive patients. The fifteen patients underwent Le Fort I osteotomy alone and the other fifteen patients underwent Le Fort I and U-shaped osteotomies. In all patients, the maxilla was first osteomized and fixed with absorbable plates system. A bilateral sagittal split ramus osteotomy (BSSRO) of the mandible was then carried out and fixation was performed using absorbable plates. Maxillo-mandibular fixation with rubber ring was used for two weeks post-operatively in all patients. Lateral cephalograms were obtained pre-operatively, 1 day post-operatively, 6 months after surgery. The changes in anterior nasal spine (ANS), point A, upper incisior (U1), and point of maxillary tuberosity (PMT) were examined. The maxillas in the fifteen patients of both examination group were repositioned nearly in their planned positions during surgery and no significant post-operative changes in the examined points of the maxilla were found. These results suggest that a combination of a Le Fort I and U-shaped osteotomy is a useful technique for reliable superior repositioning of the maxilla. The post-operative change in the maxilla using this combination osteotomy was comparatively stable.

  • The structural specification of the rockfall prevention fence installed on the road section is computed according to the hypothetic rockfall energy, and the absorbable energy of the rockfall prevention fence is defined as the sum of the absorption energies of the wire rope, one of the components, the steel support and the wire netting. But the results of the field tests confirmed that the absorption energy of the rockfall prevention fence is not the sum of the energies sustainable by the components, but it is affected by the absorbable energy of part of the components. This shows there are problems with the method of computing the absorption energy of the rockfall prevention fence of the road. So in this paper, as a way of solving the problems, the effects of improving the performance through positional changes of the existing components and the effects of improving the performance through structural changes by expanded metal are checked through tests, and a plan was proposed to solve the problems with the standards of installing the conventional rockfall prevention fence.

Comparison Study of the Use of Absorbable Materials as Internal Splints with Airway Silicone Splint and Absorbable Materials as Internal Splints Alone

  • Background: Packing after closed reduction of nasal fracture causes uncomfortable nasal obstruction in patients. We packed the superior meatus with synthetic polyurethane foam (SPF) to support the nasal bone, and packed the middle nasal meatus with a nasal airway splint (NAS) and SPF. The aim of this article is prospectively to compare the subjective patient discomfort of SPF (Nasopore Forte plus) packing alone and SPF with NAS. Methods: We compared the prospectively subjective patient discomfort of SPF packing alone (group A) and SPF with NAS (group B) via visual analog scale (VAS; 0, no symptom; 100, most severe symptom). Results: At first postoperative day group B showed significant lower scores in dry mouth, sleep disturbance, conversation difficulty. However at third postoperative day, VAS scores of each group had no statistically significant differences. Moreover at fifth postoperative day group A had statistically significant lower scores for nasal pain, dry mouth than the group B. Conclusion: Combination method of using NAS and SPF have some advantage on the patient comfort from first postoperative day to third postoperative day.

  • Either absorbable or nonabsorbable sutures can be used in meniscal repair. Some surgeons prefer nonabsorbable sutures because they believe that nonabsorbable sutures improve meniscal healing as compared with absorbable sutures. But, permanent existence of sutures in the lesion could make problems. We report a case of Inside-Out meniscus repair that nonabsorbable sutures came out of the joint capsule tearing the substance of the meniscus.

  • Purpose: This study was conducted to analyze the vibration absorption effect for the agricultural product transportation-trailer equipped with vibration absorbable connecting hitch, leaf spring suspension, and shock absorber simultaneously (HLS), comparing with the trailer equipped with vibration absorbable connecting hitch only(H), trailer equipped with connecting hitch and leaf spring suspension (HL), and existing trailer with no vibration absorption device (E). Methods: Vertical accelerations were measured at driver seat and front, middle, rear parts of trailer bottom with no load for 4 types of transportation-trailer, and analyzed using FFT analyzer. Results: The magnitude of average vibration accelerations occurred up to 20 Hz, at this low frequencies the severe damage for agricultural products could be represented, was lower (maximum 6 times) for HLS trailer compared with H trailer. And vibration absorption effect for HLS trailer was also higher up to 40-80 Hz and 80-100 Hz, but the difference was less. At driver seat, the vibration absorption effect was high severely for HL and HLS trailer, and the magnitude of vibration acceleration was showed less difference in comparing at trailer bottom. Conclusions: From the test results, it could be recommended that the agricultural products transportation trailer should be equipped with vibration absorption device of HLS.

  • An osteochondral fracture is considered to be an injury involving the cartilage and subchondral bone. Acute traumatic osteochondral fractures can be related to joint instability because abnormal joint motion causes shearing and rotatory stress. Acute osteochondral fractures are frequently missed or misdiagnosed as a pure soft tissue injury. Thus, surgeons' proactive attention is highly required as articular cartilage has limited potential for self-repair and these lesions may develop osteoarthritis. In order to minimize the progression of post-traumatic osteoarthritis, it is important to properly identify and treat osteochondral fractures. Yet, little is known about the operative management of acute osteochondral fractures of the talus. We report here on a case of a middle-aged male with acute osteochondral fractures of the bilateral lateral talar dome. We applied different operative methods on each side with regard to fragment size and stability. A favorable clinical outcome was obtained at 18 months follow-up.

  • The success of implants essentially depends on a sufficient volume of healthy bone at the recipient site during implant placement. In patients who have the severe alveolar bone resorption or pneumatized maxillary sinus, it should be performed that bone regeneration procedure before implant placement. Development of barrier membrane makes it possible that predictable result of alveolar bone reconstruction. Many kind of materials used for barrier membrane technique are introduced, non-absorbable or absorbable membranes. But, when operation site was ruptured with membrane exposure, bacterias can be grow up at the bone graft site. Then morphology and migration of fibroblast will be changed. It works as a negative factor on healing process of bone graft site. In oral and maxillofacial department of Chonbuk national university dental hospital, we use variable suture technique like as subgingival suture, vertical mattress suture, simple interrupted suture, if need, tenting suture after GBR or block bone graft. Within these suture technique, wound healing was excellent without complication, so now we take a report of suture technique in reconstruction of alveolar bone surgery.
