Etiologic Classification of Recurrent Spontaneous Abortion (습관성 유산의 원인적 분류)

  • Etiologic classification was performed in 155 patients with recurrent spontaneous abortion history. The incidence was 9.3% among 1658 pregnant women at Hanyang university hospital during the same period. In etiologic classification, 37 cases of on-going patients for diagnostic evaluation were excluded. Of the remained 118 patients, the patients with unknown etiology were 32 cases (27.1%), and 86 cases were classified into each etiologies using appropriate diagnostic modalities. Anatomic causes were the largest etiology, which revealed 46.6% (55 among 118 patients). The next etiology was immunologic cause, which revealed 24.6% (29 patients). Of the 86 patients who have at least one cause, 40.7% (35 among 86) have two or more etiologies. Furthermore, six cases (7%) have 3 combined etiologies. The uterine synechia and/or incompetent internal os of the cervix, namaly anatomic causes, revealed highest combination ratio than other etilogies. The popular abortion technique in Korea, D&C, seems to be main factors for these anatmic causes. The combined causes rather than single cause of recurrent spontaneous abortion were unique situation in Korea especially with anatomic etiologies. From this point of view, we think anatomic etiologies should be rule out first in the evaluation of the patients. For this purpose, we believe hysteroscopy would be helpful as a diagnostic work-up and treatment modality in the management of recurrent spontaneous abortion patients in Korea.

Oral Contraceptives, Abortion and Breast Cancer Risk: a Case Control Study in Saudi Arabia

  • Background: Several studies have examined the relationship between oral contraceptive pill (OCP) use, abortions and breast cancer, with mixed results. Hormonal changes associated with OCP use and abortion may increase risk of breast cancer over time, but there is a lack of studies studying this association in Saudi Arabian women. Materials and Methods: We thererfore conducted a case control study in 192 women (92 as cases and 100 as controls), aged 30 to 65, and collected information on variables including examples related to study objectives and those which may confound findings. The Chi square test was used to detect associations between various factors and risk of breast cancer. Results: We found no evidence of interaction between history of abortion or frequency of abortion and breast cancer risk (Chi square=0.422, p =0.420 and 1, p =0.169) respectively. Oral contraceptives did not confer risk for breast cancer overall (OR=0.276, 95%CI 0.092-0.829, p=0.524), while long term use of OCP was associated with increased risk of breast cancer (OR=0.297, 95%CI 0.158-0.557, p=0.001), with higher association for those who used 10 years or more of OCPs (OR=0.282, 95%CI 0.095-0.835, p=0.02). Age at first use of OCPs had no effect on breast cancer risk (p=0.452) or age at diagnosis (p=0.074). Conclusions: Prolonged use of OC (more than 10 years) may be associated with increased risk of breast cancer in Saudi women. Larger population based studies are needed to confirm this finding in this population.

A Study of Lupus Anticoagulants and Anticardiolipin Antibodies in Patients with Infertility and Recurrent Spontaneous Abortion (불임환자와 반복자연유산 환자에서 루프스 항응고인자와 항카디오리핀 항체에 대한 연구)

  • Objective : To report the prevalence of lupus anticoagulants and anticardiolipin antibodies in patients with recurrent spontaneous abortion and infertility. Material and Method: Lupus anticoagulants and anticardiolipin antibodies were analyzed by Diluted Russell's Viper Venom Test (DRVVT) and solid phase enzyme immunoassay, respectively. Results : In 200 patients with infertility, there were 6 cases (3%) with positive lupus anticoagulants or anticardiolipin antibodies. Of these, 3 patients (1.5%) showed positive lupus anticoagulants and anticardiolipin antibodies, respectively. In 120 patients with recurrent spontaneous abortion, there were 13 cases (10.8%) of positive lupus anticoagulants or anticardiolipin antibodies. Of these, one patient (1%) showed lupus anticoagulants and 12 patients (10%) showed anticardiolipin antibodies. But in two groups, there was no cases with positive lupus anticoagulants and anticardiolipin antibodies. Conclusion: Lupus anticoagulants and anticardiolipin antibodies are definite cause of recurrent spontaneous abortion. There has been a speculation that they might be associated with infertility and repeated IVF failures. But it was found that the role of lupus anticoagulants and anticardiolipin antibodies in these cases are not clear.

  • It was reported that the etiologies of recurrent spontaneous abortion are immunologic factors, endocrinologic problems, anatomical abnormalities, genetic abnormalities, infection, and unexplained factors. Among those etiologic factors, genetic abnormalities occur in about 5% of the couples who experience recurrent spontaneous abortions, and most common parental chromosomal abnormality contributing to recurrent abortion is balanced translocation. The advent of in vitro fertilization (IVF), the development of skills associated with the handling of human embryo, and an explosion of knowledge in molecular biology have opened the possibility of early diagnosis of genetic disease in preimplantation embryos. Therefore preimplantation genetic diagnosis (PGD) is indicated for couples, infertile or not, at risk of transmitting a genetic disease. A case of successful pregnancy and term delivery by PGD using fluorescence in situ hybridization (FISH) technique in patient with recurrent spontaneous abortion due to balanced translocation is presented with brief review of literatures.

  • Medication used during pregnancy may affect the growth of fetus and maintenance of pregnancy so that it may cause fetal deformity or abortion. Before 1940 it was recognized that only genetic factor could affect the incidence of fetal deformity and the teratogen was protected by placenta barrier. But since the report that Thalidomide caused phocomelia was announced in l962 and 1962, it was acknowledged that the placenta barrier was imperfect. In oriental medical care, there were so many prescription used during pregnancy for nausea, threatened abortion, recurrent spontaneous abortion and it was acknowledged that those medication did not harm both maternity and fetus. Most of them are composed of the material that was not classified as prohibition during pregnancy. But we thought that it should be demonstrated through objective methods that these materials do not affect the incidence of fetal deformity or abortion and have the effect of preventing abortion and maintenance of pregnancy. As the first step of that study we researched 78 prescription and each materials, their kinds and using frequency, used for illness and symptoms during pregnancy written in Dongeuibokam(東醫寶鑑) so that we got to know the tendency about what materials are used for each illness and symptoms.

A Cross-Sectional Study of Nutrient Intakes by Gestational Age and Pregnancy Outcome(I) (우리나라 임신부의 임신 시기별 영양 섭취상태 및 임신결과에 대한 횡적 조사 연구(I))

  • To assess the effect of an antenatal nutritional status on pregnancy outcome, especially neonatal birty weight, one-day 24hr-recall and two-day recording methods for dietary survey and interview for general and obstetric characteristics of each subject were completed and pregnancy outcome was recorded by phone after delivery. 147 pregnant women attending routinely public health centers in Ulsan were divided into 1st trimester(n=36), 2nd trimester(n=102), 3rd trimester(n=71) by LMP(Last Menstrual Period) because some subjects attended repeatedly in different trimester. The subjects were aged 27.9$\pm$2.9 as mean and the level of education was senior high school and more. 20.4% of subjects experienced spontaneous abortion and 30.0% experienced induced abortion in previous pregnancy. Mean intakes of all nutrients except ascorbic acid were significantly different but dietary composition of energy intakes was not different between trimester. Mineral of calcium, iron and zinc did not meet the RDA for pregnancy outcome was about 20%, which consists of spontaneous abortion (3.4%), caesarian section(15.6%), premature delivery(0.7%) and still births(0.7%). The mean birth weight of neonates is 3.31kg the rate of neonatal birth weight below 10th percentile was 8.4% and the rate of low birth weight(<2.5kg) was 3.1%. By analysis of nutrient factors that influence on the neonatal birth weight (NBW), iron intake correlated negatively and zinc intake correlated positively with NBW in 1st trimester but fat and iron intakes correlated with NBW positively in 3rd trimester. Prepregnancy weight, gestational age at delivery and No. of induced abortion had a positive effects on NBW and No. of spontaneous abortion and te severity of morning sickness had a negative effects on NBW.

  • The purpose of this paper was the analysis of Buddhistic views of fertilization, abortion and birth referred to in the Code of Buddhism. The findings of my analysis of the allusions of the Code are as follows. 1. Modern science holds the view that in fertilization the spermatozoom and the ovum are the essential factors, while the Buddhistic view is that here must be the entry of a reincarnated soul ( 中有 ; jung-yu) into fertilization. 2. In Buddhism, fertilization is regarded as the giving of a new life and the receiving a life which consists of the sufferings, both physical and mental in nature, of the individual to be born. 3. Artificial abortion in any case is stictly prohibited because it is regarded as taking life. 4. Birth perse is assumed to consist of suffering for both the mother and the child itself. Therefore, these are analogous to the perinatoloty of modern science.

  • It was effective for a dosage of herbal medicine to treat hyperemesis gravidarum, threatened abortion and prevent hard labor. We prescribed Kakambosengtang for 15 patients of hyperemesis gravidarum which had weakness of the spleen and stomache, Kakamdalsengsam gyoaesamultang for 10 patients of threatened abortion which had weakness of vital energy and blood, Kakamdalsengsan for 12 patients of preventing hard labor. The result was that the whole syndromes improved. And the healthy baby was born. According to the above mentioned results, it is valid that the sick pregnant women would rather take herbal medicine rightly than avoid herbal medicine blindly.

A Study of the Effect of the Socioeconomic Status of Couple on the Induced Abortion in Korea (부부의 사회경제적 지위가 인공임신중절에 미치는 영향에 관한 일 연구)

  • There are two main purposes in this study. First, we compare the effects of wives' characteristics with the effects of the husbands' characteristics on the induced abortion. Second, we analyze whether the determinants of the induced abortion have changed according to parity and conception period. The main findings are follows. First, both wives' and husbands' socioeconomic characters have insignificant effects on the induced abortion at parity 0, in the 1997 and 2000 Korean Fertility Survey data. Second, during the periods of the lowest-low fertility, after 2000 in Korea, wives' employments have positive effects at parity 0 and 1, while husbands' educational levels have negative effects at parity 1 on the induced abortion. The implications are as follows. First, having children had been the universal social phenomenon before 2000 in Korea. however, after 2000, reproductions have become the women's choice, lather than the duty of married women. Women must weight the balance between the benefits and the costs of children so that women's fertility behaviors become a rational choice. Women's employment is the most important factor in these rational calculations. Second, both Western individualism and the traditional Korean familism have significant effects on the fertility behavior and the induced abortions in Korea. This rejects the diffusion theory, which tells that the traditional familism must be replaced by the Western individualism in order to decline the fertility rates in developing countries.

Paramedic students' perceptions of Abortion, and Suicide (응급구조학과 학생들의 낙태와 자살에 대한 인식)

  • The purpose of this study was to examine the perception of abortion and suicide for 115 paramedic students in 2016. The questionnaire consisted of perception of abortion(8items), the perception of suicide(58items), related 6items of the abortion and related 2 items of the suicide. The collected data were analyzed through t-test, Chi-squared test and ANOVA, using SPSS 21.0. As a result, the total perception score against abortion was $20.57{\pm}4.12$ and the total score of suicide perception was $-14.16{\pm}15.76$. The most answer was that pregnant women were abortion decision makers in both male and female students, and abortions were the highest when they might be unwanted pregnancies. The group who had thought about suicide was higher of perceptions of suicide than the group who did not. The female students who had suicide attempt or suicide experience of close acquaintances showed higher suicide perception score than those who did not. Therefore, A bio-ethics education curriculum should be prepared to establish ethics that respects life for future paramedic students who must make decisions at the boundary between death and life. It is necessary to raise the level of positive suicide perceptions through continuing counseling and education focusing on students who have experienced suicide by family members or acquaintances.
