The Effect of human Immunoglobulin and Chorionic Gonadotropin on the Production of Maternal Blocking Antibody (인혈청(人血淸) 면역글로부린 및 융모성성선자극호르몬이 습관성유산환자의 혈청내 '차단항체' 생성에 미치는 영향)

  • Human chorionic gonadotropin(hCG) and intravenous immunoglobulin(IVIG) treatment were attempted as a novel therapeutic approach for unexplained recurrent spontaneous abortion(RSA). Forty-four and 3 women with a history of RSA were treated with hCG and IVIG, respectively, during pregnancy. Of these patients, serum blocking factor assay was performed before and after each treatment, in 15 patients; 12 cases with hCG and 3 cases with IVIG. The results were as follows: 1. Of 44 women who receive hCG during pregnancy, 24 delivered healthy infants at term, 10 patients suffered repeat abortion, and 10 women are still pregnant under 28 weeks. Over all success rate of hCG treatment was 70.6% (24/34). Although there is no statistical significance, absolute serm blocking level was decreased after treatment(N=12). 2. Of 3 women who receive IVIG during pregnancy, all 3 women are still pregnant under 28 weeks. Serum blocking level was increased after treatment, however, this increment was not statistically significant. Although no conclusion could be extracted from the patients who received IVIG, the therapeutic effect of hCG is comparable to that of the other therapeutic regimens, such as allogeneic leukocytes. It was postulated that actual etiology of unknown RSA would be classified as hormonal origin although combined etiologies are common in Korean women.

  • A survey was couducted by the staff of the Dept. of Preventive Medicine, Kyung Hee University, School of Medicine, from April through May 1971, on such events as delivery, death, abortion and family planning. The survey directed to a total population 6,552 (Male 3,133; Female 3,419) residing in this area (1,262 households) led us to the following findings: 1) Two year averages of crude birth rate, crude death rate and natural increase rates were 24.9, 5.0 and 19.9 respectively. 2) Infant death rates for the years 1969 and 1970 were 13.2 and 5.8 respectively, mean for the two year period being 9.3. 3) Hospital deliveries rated 68.5 per cent, Home deliveries 28.4 per cent and deliveries at midwives' offices 2.7 per cent. 4) Deliveries other than hospital and midwives' office deliveries were found to be attended more often by mothers. 5) About 41.4 per cent of all pregnant women during last two years received prenatal care once or more. 6) The induced abortion rate was 6.7 per cent in 1969 and 7.5 per cent in 1970. 7) The spontaneous abortion rate was 1.1 per cent in 1969 and 1.4 per cent in 1970. 8) Hypertension was the most frequent cause of adult death(21.6%). 9) The rate of current practice in family planning was 43.3 percent of all women.

Constitution of Prescription and Medicinal Effect & Adaptation Diseases of 'Bullsoosan(佛手散)' in Korean Medical Books (한국(韓國) 의서(醫書)에 보이는 불수산(佛手散)의 처방구성(處方構成)과 효능(效能)·주치(主治)에 대한 고찰)

  • Subjects : A literature research on the constitution and medicinal effect & adaptation diseases of "Bullsusan". "Bullsusan" is a herbal prescription composed of Angelicae Gigantis Radix(當歸) and Cnidii Rhizoma(川芎). Objectives : Through the researching on the records of "Bullsusan" in Korean Traditional Medical Books, gain the literature evidence for adaptation to these days child labor as a pre-labor keeping herbal medicine. And have detailed consideration on the constitution of prescription and medicinal effect & adaptation diseases of "Bullsusan". Methods : First, researched the records of "Bullsusan" in Korean Traditional Medical Books which were included at A Series of Korean Medicine(韓國醫學大系) and analysed component ratio, nickname, herbal manufacture and drug processing method, medicinal effect and adaptation diseases. Second, referred related Korean and Chinese researches that examined the medicinal effect and adaptation diseases of "Bullsusan" by scientific experimentation. Conclusions : We found total 46 records of "Bullsusan" from 20 kinds of Korean Traditional Medical Books included at A Series of Korean Medicine. Prescription component ratio of Angelicae Gigantis Radix and Cnidii Rhizoma were 3:2, 1:1, 2:1, 1:1. 3:2 had most 20 records and 1:1 had second 14 records. Especially 1:1 had a tendency of having nickname "Goonguitang", but not must had. First herbal manufacture was powder, it had 8 records. First drug processing method was decocting with water and alcohol, had 19 records. Medical Effects of "Bullsusan" can be induced to next 8, that were "remove get bad blood, give birth new blood", "easy labor by reducing fetal volume", "acceleration of labor", "test of fetal survival, elimination of dead embryo", "elimination of placenta", "revive", "allaying pain", "nourish the blood". From these medical effects, 9 adaptation diseases can be induced. That were "threatened abortion", "womb ache and vaginal bleeding by spontaneous abortion", "pre-labor keeping(prevention of hard labor)", "acceleration of labor", "hard labor", "missed abortion", "postnatal vaginal bleeding, dizziness, asthma, headache, womb ache", "postnatal mastoptosis and mastodynia", "first aid symptom like as dizziness, unconsciousness, stroke caused by excessive bleeding". The medical effect of "acceleration of labor" and "elimination of placenta" have been examined by modern clinical research. The effect of "remove get bad blood, give birth new blood", "allaying pain" and "nourish the blood" have been examined by modern experimental study. But overdosing on "Bullsusan" to pregnant mouse can cause natural abortion, so the proper dose of "Bullsusan" in pregnant period is very important.

The Effects of Progesterone on Cell Mediated Immunity to Trophoblast in Woman with Recurrent Pregnancy Loss (반복 유산환자의 말초혈액 단핵구와 태반항원을 체외 공동 배양시 세포 매개 면역반응에 프로게스테론이 미치는 영향)

  • Progesterone is necessary for successful pregnancy and had immunosuppressive properties. Peripheral blood mononuclear cells (PBMC) from many women with unexplained recurrent spontaneous abortion responded to trophoblast extract in vitro by prolifertion and releasing soluble, heat-labile factors that are toxic to mouse embryos (embryotoxic factors). Accumulating evidence suggests that T Helper (Th)-1 type immunity to trophoblast is correlated with embryotoxic factor production and is associated with pregnancy loss, while Th2-type immunity is associated with successful gestation. The objective of this study was to determine whether progesterone can inhibit Th1-type cytokine secretion (IFN-${\gamma}$, TNF-${\alpha}$) by trophoblast-activated peripheral blood mononuclear cells from 23 nonpregnant women (age 25-35) with unexplained recurrent abortion (median 5, range 3 to 15)who otherwise produce embryotoxic factors in response to trophoblast. We also determined whether progesterone affected Th2-type cytokines (IL-4, IL-10) in this system in vitro and if IL-10 (1,500 pg/mL) could inhibit Th1-type immunity to trophoblast. IFN-${\gamma}$ was detected in 17 of 23 (74%) trophoblast stimulated PBMC culture supernatants ($77.94{\pm}23.79$ pg/mL) containing embryotoxic activity. TNF-${\alpha}$ was detected in 19 (83%) of these same supernatants ($703.15{\pm}131.36$ pg/mL). In contrast, none of the supernatants contained detectable levels of IL-4 or IL-10. Progesterone ($10^{-5}$, $10^{-7}$, $10^{-9}$M) inhibited Th1-type immunity in a dose dependent manner, but had no effect on Th2-type cytokine secretion. The inhibitory effects of progesterone were abrogated with RU486, but did not affect Th2-type cytokine secretion in trophoblast-activated cell cultures. IL-10, like progesterone also inhibited Th1-type cytokine secretion but had no effect on Th2-type cytokines. These data suggest that therapies designed to suppress Th1-type cytokine secretion in women with recurrent abortion who have evidence of Th1-type immunity to trophoblast may be efficacious in preventing pregnancy loss and should be tested in appropriately designed clinical trials.

  • Objectives: The purpose of these cases is to report the effects of Korean medicine on two women with recurrent miscarriage.Methods: One women who had recurrent miscarriage with myoma uteri was 39 years old. She was treated with herbal medicine and acupuncture until intrauterine pregnancy (IUP) 14 wks. the other who had recurrent miscarriage with adenomyosis was 40 years old. she was also treated with these until IUP 11 wks.Results: After treatment, one women delivered successfully by normal full term spontaneous delivery. The other succeeded in normal pregnancy and she is 15 weeks pregnant.Conclusions: Two cases show that Korean Medicine has effects on recurrent miscarriage with old age and underlying diseases.

  • Listeria monocytogenes is one of the important causes of neonatal sepsis and listerial neonatal infection manifests in two forms : Early-onset sepsis syndrome, associated with spontaneous abortion, still birth, preterm labor, granulomatosis infantiseptica, respiratory distress, sepsis, hemodynamic compromise and late-onset listerosis mainly associated with meningitis. Cases of neonatal listerosis reported in Korea have been rare and all were full term newborns. We, herein, report a case of early-onset sepsis due to L. monocytogenes in a extremely low birth weight infant who were born in a critical condition and succumbed in the second day of life despite the intensive care.
