Purpose : The time spent using smart devices is constantly increasing, particularly in recent times. Using smart devices for a long time with an incorrect posture may lead to cerebral palsy (CP), instability, and abnormal muscle tone. Therefore, we aimed to investigate the relationships among cervical instability, deep neck flexor (DNF) activity, range of motion (ROM), and muscle tonus. Methods : Fifty subjects with CP participated in this study, and they were physiotherapists at W Hospital in Daejeon. Those who voluntarily participated in the research were selected as candidates who fulfilled the selection criteria. According to an instability test, 25 subjects were assigned to the instability and control groups. All subjects first underwent the instability test to be allocated to the appropriate group. Those in the instability group tested positive on the instability test. The Neck Disability Index (NDI), ROM, muscle tone, and DNF activity were measured to evaluate their relationships. The DNF strength and endurance were measured using a cranio-cervical flexion test. The upper trapezius (UT), sternocleidomastoid (SCM), and suboccipital (SO) muscle tones were measured using a contact soft tissue tone measuring instrument. The statistical significance level was set to .05. Results : There were significant differences in the flexion, extension, and rotation of the cervical ROM (CROM) between the two groups (p<.05). The SCM, UT, and SO muscle tones were significantly different between the two groups (p<.05). The DNF strength and endurance showed a significant difference between the two groups (p<.05). Conclusion : We found that there were significant increases in the CROM and muscle tone and decrease in the DNF strength and endurance in the instability group. This indicated that cervical instability is affected by the DNF strength and endurance. We may recommend DNF exercises in cases of cervical instability in clinical environments.
Purpose: In this paper, we propose a study of deep learning algorithms that estimate and predict sarcopenia by exploiting the high penetration rate of smart devices. Method: To utilize deep learning techniques, experimental data were collected by using the inertial sensor embedded in the smart device. We implemented a smart device application for data collection. The data are collected by labeling normal and abnormal gait and five states of running, falling and squat posture. Result: The accuracy was analyzed by comparative analysis of LSTM, CNN, and RNN models, and binary classification accuracy of 99.87% and multiple classification accuracy of 92.30% were obtained using the CNN-LSTM fusion algorithm. Conclusion: A study was conducted using a smart sensoring device, focusing on the fact that gait abnormalities occur for people with sarcopenia. It is expected that this study can contribute to strengthening the safety issues caused by sarcopenia.

Objectives:There is increasing evidence that the cerebellum plays an important role in cognition and psychiatric symptoms as well as motor coordination. The concept of cognitive dysmetria has been making cerebellar function in schizophrenia the focus of current studies. In other words, disruption in the corticocerebellum-thalamic -cortical circuit could lead to disordered cognition and clinical symptoms of schizophrenia. The purposes of this study were to determine cerebellar dysfunction in male schizophrenic patients semiquantitatively with ICARS and to investigate the clinical and cognitive correlates of ICARS in patients. Methods:We compared the scores of cerebellar neurologic sign using ICARS in 47 male patients with a DSM-IV-TR diagnosis of schizophrenia with 30 gender and age-matched healthy control subjects. The semiquantitative 100-point ICARS consists of 19 items divided into 4 unequally weighted subscores:posture and gait disturbances, kinetic functions, speech disorders and oculomotor disorders. All subjects were also assessed with cognitive function test. Cognitive functions were evaluated by Korean-Mini Mental Status Examination (K-MMSE), Verbal fluency test, and Clock drawing test. The patients were administered Korea version of Positive and Negative Symptom Scale(K-PANSS) to assess the symptom severity. Results:Schizophrenic patients had significantly higher scores on the ICARS than control subjects with posture and gait disturbances, kinetic functions, and oculomotor disorders. They also showed more significant impairments in cognitive function tests than control subjects. There was a significant correlation between ICARS and negative symptoms of patients. In cognitive function test, Clock drawing test was significantly associated with negative symptoms. In addition, Clock drawing test was negatively correlated with the total score of ICARS. Conclusion:In this study, we confirmed that schizophrenic patients have significant impairments in cognitive and cerebellar function, and that those were related with negative symptoms of schizophrenic patients. These results support a role of the cerebellum in schizophrenia. It is meaningful that we used a structured, and reliable procedure for rating neurological soft signs, ICARS. We hope that future prospective studies using a similar design help that rate of neurological sign should have been visible with the progression of illness.

The purpose of this study is to analyze the phases of back pain occurring on pregnant women, and to raise the necessity of the pain management on the basis of the former analysis. The objective group is 284 pregnant women who visited department of OBGY of hospital located both in Seoul and Andong. The results are as follows; 1. 204(71.8%) pregnant women were suffering from back pain. The ratio of pain occurrance in terms of the duration of pregnancy shows that 78.3% within 3 pregnant months, 68.4% between four and six months and 72.1% over seven month. Most common ares of pain was low back area rating 60.5%. 2. The most painful postural for the suffering women was lying on the back rating 35.9% and the next painful posture was standing rating 34.4%. And the most painful movement was to maintaining continues movement(57.8%). Twisting back rates the second(17.2%). These two variables were relevant each other(p<0.05). 3. 46.7% of pregnant women were experiencing nocturnal pain. Among women experienced the pain before pregnancy, 39.9% were suffering during the pregnancy. The occurrence of nocturnal pain was related to the pain before and after the pregnancy(p<0.05). 4. 58.8% of pregnant women who experience back pain take the pain for granted as a normal proceeding of pregnancy while 3.9% recognize the symptom as an abnormal. Pain recognition in accordance with the phases does not show much difference(p<0.05). 5. It is shown that the more one delivers the number of babies, the faster back pain occurs(p<0.01). 6. 32.3% of the pain-suffering women have family member(s) having back pain. Family member(s) of the women who does not experience the pain don't have the pain either. This case reports 46.1%. Statistically, these two variables are relevant(p<0.01). 7. 43.0% of back pain experiencing women does not have any particular management plan against the pain. 20.7% is exercising as for prevention. Women who recognize the necessity of some means of therapy for their pain marked 42.9%. The majority(65.8%) of women responded exercise gymnastic work-out are most appropriate pain management. The above results show that a great number of pregnant women is experiencing back pain, however, they properly managed. This span suggests that appropriate advocacy and education for pregnant women is necessary. It is recommended that positive contribution can be made to better health of pregnant women when pain management by physical therapist is given.
Primary aldosteronism is a disease that the stimulus for the excessive aldosterone production resides within the adrenal gland. It was first described by conn in 1955. And many cases were reported by physicians at present in the world. But it is relatively rare in Korea, probably due to lack of attension and medical facilities. Only about 13 cases have been reported at present. The clinical, biochemical features in 1 case of primary aldosteranism caused by adrenal hyperplasia that was diagnosed at Yeungnam University Hospital was observed and the following result were obtained. 1. Clinical feature: The present case was 27-year-old woman who was admitted due to general weakness and easy fatigability. The above mensioned chief complaints occurred 8 months prior to admission when she delivered of second baby by cesarian section. Symptoms such as above chief complaints, intermittent muscle paralysis and cramping were noticed. Trousseau's sign was also present. The average blood pressure ranged from 170/90 to 200/120. 2. Biochemical abnormalities: Severe hypokalemia lower than 2.5 mEq/L was presented and 24 hours urine potassium showed markedly increased urinary loss.(228 mEq/day). Plasma renin activity was decreased under normal range with furosemide administration.(Basal renin; 0.01ng/ml/hr, stimulated renin 0.12ng/ml/hr). Saline suppression test revealed markedly elevated levels of aldosterone higher than normal range. (Basal aldosterone; 320.68pg/ml stimulated aldosterone; 451.86pg/ml). And posture test showed decreased plasma renin activity and increased plasma aldosterone level. - PRA(ng/ml/hr)=Bsal: 0.05(0.15~2.33), Stimulated: 0.22(1.31~3.95) - Aldosterone(pg/ml)=Bsal: 242.77(10~160), Stimulated: 432.09(40~310) 3. Adrenal CT scan revealed no abnormal findings. 4. Treatment and course: Spironolactone was given at OPD with regular follow-up. Her blood pressure ranged from 150/90 to 160/100 and symptoms were improved. The effect of treatment was satisfactory and further follow up would be performed.

Rett syndrome is a progressive neurological disorder that occurs exclusively in females. The syndrome is characterized by regression of language, motor development, and stereotypic hand movement. Autistic behavior, breathing irregularities, gait dyspraxia, scoliosis, and seizure are also accompanied. The cause of Rett syndrome is unknown, however, it is believed that the X-chromosome might playa significant role in the development of the syndrome. Patients with this syndrome have unusual oral and/or digital habits such as abnormal chewing pattern, bruxism, hypersalivation, micrognathia, high vaulted palate, tongue protrusion with lower posture of tongue, hand biting, digit-hand sucking. Dentists who are aware of distinct manifestations of Rett syndrome will be able to aid in early diagnosis and treatment of the syndrome. Prior to dental treatment for a patient with the Rett syndrome under sedation or general anesthesia, one should assess the degree of hypersalivation, apnea, severity of autism, expected life span. Early recognition of the syndrome and also dental treatment with established strict preventive guidelines for patients with the Rett syndrome may obviate the necessity of sedation or general anesthesia. Two cases with the Rett syndome were reported. Both patients had most of the above mentioned typical manifestations of the syndrome. Dental treatment for the case 1(8-year-old) including caries control, stainless steel crown, sealant application was performed under general anesthesia. The case 2 could not be undergone the dental treatment due to poor general conditions.
The study aimed to examine the state of visual acuity tests in preschoolers and determine the factors affecting the deterioration of their vision. A total of 172 pair, child aged 5-7 years and either one of their parents, were recruited from childcare centers or kindergarten between August 2015 and January 2016. Questionnaires were administered to the parents, and the children's visual acuities were measured. Using forward selection in logistics regression analysis, factors affecting vision deterioration were elucidated. Of 172 children, 128 (74.4%) had undergone a visual acuity test previously and 77 (44.8%) showed a deterioration in their vision. Children who had undergone their first visual acuity test after the age of 7 years (OR=7.425, CI=2.844- 19.385) and showed more abnormal eye symptoms, such as squinting or tilting the head to see or falling down frequently (OR=3.084, CI=1.202-7.914) and whose age was younger (OR=10.335, CI=3.252-32.848), were more likely to develop deterioration of vision. Children who had a posture such that they looked up at the computer monitor from below (OR=.075, CI=.022-.255), were less likely to show deterioration of vision. It can be inferred that early visual acuity tests is essential to detect deteriorated vision of preschoolers. Therefore, it is necessary to educate parents, personnels in day care center, and health clinics about the importance of early visual acuity tests and close observation of specific behaviors related to vision deterioration. Development of precautionary intervention program of vision deterioration in preschoolers and examination of its effects are needed.

Study Design: Literature review. Objective: Ultrasound-guided injections are a common clinical treatment for lower lumbosacral pain that are usually performed before surgical treatment if conservative treatment fails. The aim of this article was to review ultrasound-guided injections in the lumbar and sacral spine. Summary of Literature Review: Ultrasound-guided injections, unlike conventional interventions using computed tomography or C-arm fluoroscopy, can be performed under simultaneous observation of muscles, ligaments, vessels, and nerves. Additionally, they have no radiation exposure and do not require a large space for the installation of equipment, so they are increasingly selected as an alternative method. Materials and Methods: We searched for and reviewed studies related to the use of ultrasound-guided injections in the lumbar and sacral spine. Results: In order to perform accurate ultrasound-guided injections, it is necessary to understand the patient's posture during the intervention, the relevant anatomy, and normal and abnormal ultrasonographic findings. Facet joint intra-articular injections, medial branch block, epidural block, selective nerve root block, and sacroiliac joint injections can be effectively performed under ultrasound guidance. Conclusions: Ultrasound-guided injections in the lumbar and sacral spine are an efficient method for treating lumbosacral pain.
Parkinson's disease (PD) is a progressive neurodegenerative disease that mainly affects motor system with clinical features such as bradykinesia, rigidity, tremor and abnormal posture. PD is characterized by the death of dopaminergic neurons in the substantia nigra pars compacta, which is associated with accumulation of oxidative stress and dysregulation of intracellular signaling pathway. Quercetin-3-O-glucuronide (Q3GA), a major metabolite of quercetin, has been reported to have neuroprotective effects. In this study, we examined the neuroprotective effect of Q3GA against 1-methyl-4-phenyl pyridinium ($MPP^+$)-induced neurotoxicity of PD and the underlying molecular mechanisms in SH-SY5Y cells. MTT and LDH assay showed that Q3GA significantly decreased $MPP^+$-induced cell death, which is accompanied by a reduction in poly (ADP-ribose) polymerase (PARP) cleavage. Furthermore, it attenuated $MPP^+$-induced intracellular reactive oxygen species (ROS) with the reduction of Bax/ Bcl-2 ratio. Moreover, Q3GA significantly increased the phosphorylation of Akt and cAMP response element binding protein (CREB), but it has no effects on the phosphorylation of extracellular signal-regulated kinase (ERK). Taken together, these results demonstrate that Q3GA significantly attenuates $MPP^+$-induced neurotoxicity through ROS reduction and Akt/CREB signaling pathway in SH-SY5Y cells. Our findings suggest that Q3GA might be one of the potential candidates for the prevention and/or treatment of PD.
