We report the case of a seven-year-old boy with an ingested foreign body, which was retained within the appendix for a known duration of ten months, ultimately requiring appendectomy. The ingested foreign body was incidentally discovered by abdominal x-ray at an emergency room visit for constipation. Despite four bowel cleanouts, subsequent x-rays showed persistence of the foreign body in the right lower quadrant. While the patient did not have signs or symptoms of acute appendicitis, laparoscopic appendectomy was performed due to the risk of this foreign body causing appendicitis in the future. A small metallic object was found within the appendix upon removal. This case highlights the unique challenge presented by foreign body ingestions in non-verbal or developmentally challenged children and the importance of further diagnostic workup when concerns arise for potential retained foreign bodies.

Background and Objective : Erythema nodosum-like lesions in patients with Behcet's disease is a reactive dermatosis that occurs in response to various stimuli, such as bacterial, fungal and viral infections, drugs and malignancies. We observed and treated a case of erythema nodosum-like lesions with Behcet's disease. We present this case because there were no former reports of erythema nodosum-like lesions with Behcet's disease treated by herbal medicine. Methods : The patient was a 28-year-old. The chief complains were erythema, nodose, edema, burning sensation, tenderness on both legs, forearms and feet, arthralgia and genital ulcers. She was treated with Wolbi-Tang for eleven days. We diagnosed the medication according to the results of manual abdominal examination of the patient. We used the Visual Analogue Scale(VAS). Results : After the treatment the grade of VAS was decreased and clinical symptoms were improved. Conclusions : Wolbi-Tang can be used on the treatment of erythema nodosum-like lesions with Behcet's disease.
A 14-month-old, intact female Maltese (case 1) and a 26-month-old, spayed female Dachshund (case 2) had persistent regurgitation over 1 year and 2 years, respectively. In both cases, there were not specific findings in blood examination. Diagnostic imaging and fluoroscopy revealed that gastroesophageal reflux (GER) occurred when the patients ate a barium meal without abdominal contraction. They were treated with proton-pump inhibitor (esomeprazole), sucralfate and prokinetics (cisapride) while in a standing position for an hour after every meal, according to the gastroesophageal disease (GERD) suggested treatment. The case 1 dog was discharged with improved clinical signs, and the case 2 dog also had been improved clinical signs; however, aspiration pneumonia occurred in case 2, which was thought to be a complication from GERD. After receiving treatment for aspiration pneumonia, the case 2 dog was discharged from the hospital. Since there was no further improvement in symptoms after discharge, the case 1 dog was euthanized by owner's request, and the case 2 is alive, having only mild clinical signs upon the writing of this paper.
Objectives : The aim of this study was to investigate expert opinions on the questions contained in a questionnaire for diagnosing blood stasis accompanying metabolic disorders. Methods : Two rounds of Delphi survey were conducted online targeting on one hundred Korean medicine doctors. Respondents rated the appropriateness of the 30 questions in diagnosing metabolic disorder on a five-point scale, anchored at '5 = very appropriate', '4 = appropriate', '3 = somewhat appropriate', '2 = inappropriate', and '1 = very inappropriate'. Results : The mean score on 30 questions of first and second Delphi survey was 3.26 points and 3.31 points, respectively. The ranking of the top 10 questions that were rated as appropriate for diagnosing blood stasis accompanying metabolic disorder were 'sublingual varices', 'reddish black tongue', 'reddish black gum', 'reddish black lips', ''dark purple palatal mucosa and venous edema', 'night pain, 'ecchymosis of the tongue', 'piercing pain', 'ecchymosis of the skin' and 'prolonged numbness'. Conclusions : The experts agreed that three of the most typical symptoms of blood stasis and the conditions of capillary vessels in the tongue or the oral cavity were highly associated with metabolic disorder, whereas the questions related to abdominal pain lacked an association with metabolic disorders.
Objectives: The purpose of this review is to analyze the domestic study trends on Korean medicine treatment for patients after hysterectomy. Methods: We investigated the studies on Korean medicine treatment for patients who undertaken hysterectomy via searching 4 online databases up to October 2020. After searching studies, we analyzed selected studies. Results: 12 clinical studies were selected and all studies were case reports. The most common cause of hysterectomy was myoma of uterine, followd by adenomyosis. The major complaints of post-hysterectomy were abdominal pain, low back pain and fatigue, general weakness. Herbal medicine was used in all studies. In addition, acupuncture, moxibustion and cupping were performed. The duration of inpatient treatment ranged from 5 to 39 days. All cases showed that chief complaints and accompanying symptoms were improved. Conclusions: Korean medicine treatment is an effective treatment option for patients after hysterectomy. Further large, well-designed studies are needed to establish the foundation of Korean medicine treatment after hysterectomy.
Bartter syndrome is an autosomal recessive hypokalemic salt-losing tubulopathy, and classic Bartter syndrome is associated with mutations in the CLCNKB gene. While chronic hypokalemia is known to induce renal cyst formation in different renal diseases, renal cyst formation in Bartter syndrome is rarely reported. Russian six-year-old identical male twins were referred to our hospital for the evaluation of renal cysts, which were incidentally detected on abdominal sonography due to diarrhea. Both twins had shown symptoms of polydipsia, polyuria, and nocturia since they were one year olds. Vital signs including blood pressure were normal in both twins. Renal sonography revealed nephromegaly, increased echogenicity of renal cortex, and various sized multiple cysts in both kidneys for both twins. Laboratory findings included hyponatremia, hypokalemia, hypochloremia, and metabolic alkalosis. Bartter syndrome with renal cysts were suspected. Genetic analysis for both twins confirmed a homozygous c.1614delC deletion on exon 15 of the CLCNKB gene, which was confirmed as a previously unreported variant to the best of our knowledge. They were managed with potassium chloride, nonsteroidal anti-inflammatory drugs, and angiotensin-converting-enzyme inhibitors. Metabolic alkalosis, hypokalemia, hypochloremia, and polyuria partially improved during the short course of treatment. This is the first report of a homozygous mutation in the CLCNKB gene in an identical twin, presenting with renal cysts.

Objectives: This study examined a clinical case of a paralytic ileus patient diagnosed with Soeumin. Methods: The patient was treated with herbal medicine (Hyangsayangwi-tang, Baekhaogunja-tang, Palmulgunja-tang), infusion solution treatments, acupuncture, moxibustion, and rehabilitative therapy. Their effects were evaluated using the visual analog scale (VAS). Results: After receiving Korean medical treatment for 38 days, the VAS score for dyspepsia was reduced from 8 to 2, the VAS score for nausea and vomiting was reduced from 8 to 2, and the VAS score for abdominal pain was reduced from 7 to less than 1. Conclusions: Korean medicine could be effective in relieving the symptoms of paralytic Ileus.

Cryptococcus neoformans infection usually occurs in patients with advanced human immunodeficiency virus (HIV) infection or with a CD4 T lymphocyte count of <100 cells/µL. Pulmonary and central nervous system infections are the most frequently encountered forms of cryptococcosis; however, colonic cryptococcosis is uncommon. We describe the case of a 41-year-old antiretroviral-naïve man with HIV infection diagnosed eight years prior and intermittent diarrhea for 4 months who presented to the emergency department with a 1-day history of low-grade fever and confusion. Brain magnetic resonance imaging and cerebrospinal fluid analysis revealed normal results; however, he was diagnosed with Pneumocystis jirovecii pneumonia based on chest computed tomography and bronchoalveolar lavage analysis. Trimethoprim-sulfamethoxazole administration was initiated followed by antiretroviral treatment. Although his condition gradually improved, he developed fever and abdominal discomfort, and the diarrhea worsened. Endoscopy revealed a small ulcer in the distal transverse colon. Histopathological examination of a colon tissue sample revealed cryptococcal infection. He improved substantially during liposomal amphotericin B and fluconazole treatment. We encountered a rare case of colonic cryptococcosis that caused chronic diarrhea in a patient with advanced HIV infection. Colonic cryptococcosis should be considered when patients with acquired immune deficiency syndrome present with gastrointestinal symptoms.
This case report is about hemolymph nodes found in a dairy cow whose function is still not fully elucidated. A 4-month Holstein cow presented severe respiratory symptoms and hematochezia for a while with respiratory acidosis and metabolic alkalosis. Coccidiosis was diagnosed and treated immediately, but the cow died from respiratory acidosis and metabolic alkalosis. At necropsy, no abnormal appearance in thoracic and peritoneal organs was observed, but hemolymph nodes were observed being multifocally stuck on omasum serosa and the subcutaneous fascia of abdominal region, and the larger dark red lymph nodes were found along the omasum great curvature. Microscopically, lymphoid depletion and lymphadenitis in the lymph nodes were examined to point systemic infection, and in the hemolymph node, multifocally demarcated pale lesions with macrophage infiltration and fibrin deposition nearby subcapsular sinus. In subcapsular sinus of the hemolymph node, rod to linear gram-negative bacteria were found. Through this study, we might conclude that the hemolymph node is involved in pathogen phagocytosis.

Many modern people are exposed to chronic inflammatory diseases, such as inflammatory bowel disease (IBD), atopic dermatitis and immune disorder. Among those chronic diseases, the incidence ratio of IBD has been increased. IBD, including Crohn's disease and ulcerative colitis (UC), is known to cause abnormal inflammation in intestinal tissue. UC is accompanied by abdominal pain, bloody stool and diarrhea. Many therapeutic agents, such as sulfasalazine, corticosteroids, immunosuppressive agents, have been used for treating UC. However, those agents have side-effects and temporary effects on UC. The aim of this study was to investigate the effect of herbal medicine on UC and relationship between UC and intestinal bacteria according characteristic of herbal medicine. To determine the effect of herbal medicine on UC, various herbal medicine were chosen within oriental medicine category such as cheongyeol and onyeol medicine. In this study, we found carthami fructus, included in cheongyeol medicine, had stronger effect than onyeol medicine. Also, we determined influence of carthami fructus against lactic acid bacteria. Catthami fructus and lingon berry extracts affected the composition of mice intestinal bacteria in mice fecal. The symptoms of UC could be regulate by using herbal medicine, according to characteristic of herbal medicine. Also, herbal medicine might be change body condition to healthy by controlling intestinal bacteria composition. Herbal medicine characteristic could be a therapeutic agent by revealing relationship between intestinal bacteria and UC.
