본 연구는 자폐범주성장애(ASD) 아동의 또래 간 상호작용 촉진을 위한 동기화 기반 그룹 음악치료 사례연구이다. 중재 프로그램은 ASD 아동의 사회기술 발달에 있어 운동조절이 미치는 요인을 확인한 선행연구 결과를 반영하여 구성되었다. 총 5명의 ASD 아동이 대상자로 참여하였으며, 중재는 주 2회씩 총 8회기로 진행되었다. 결과분석을 위해 중재 안에서 관찰된 공동 행동 참여, 동시적 움직임, 눈맞춤 시도, 상호작용 시도 행동의 발생률과 중재 과정에서 나타난 의미 있는 음악행동 변화를 분석하였으며, 세션 내 관찰행동의 변화가 외부 환경에서도 유사하게 관찰되는지 확인하기 위하여 대상자의 부모에게 사전·사후 부모용 또래 놀이행동 척도(PIPPS-P) 검사를 실시하였다. 분석 결과, 중재에 참여한 대상자 모두 또래 간 동기화된 움직임 및 또래에 대한 자발적인 상호작용 시도 행동, 또래 놀이행동 척도 점수가 증가했음을 알 수 있었다. 이러한 결과는 ASD 아동의 지각-운동 연합에 있어서의 문제를 반영한 동기화 기반 그룹 음악치료가 ASD 아동 간 동기화된 움직임을 유도하는 데 효과적임을 보여주는 것이다. 본 연구는 동기화에 기반을 둔 음악치료적 개입이 어떻게 ASD 아동의 또래 상호작용을 촉진할 수 있는지 임상 현장에서 구체적으로 적용 가능한 기초자료를 제공하였다는 점에서 그 의의가 있다.
본 연구에서는 학령전기 자폐스펙트럼장애(ASD) 아동을 대상으로 인지-운동 통합 기반 악기연주 중재를 제공하여 초기 사회기술이 향상하는지를 알아보고자 하였다. 대상자는 평균 만5.6세의 ASD 아동 총 9명로, 개별로 30분씩 주 2회, 총 12회의 세션에 참여하였다. 중재는 자기 감각 증진 및 자기조절(1단계), 상호작용에 대한 의도 형성(2단계), 상호적 활동에 대한 수용(3단계), 상호적 맥락에서의 움직임 조정(4단계), 공동행동 수행(5단계)의 5단계로 구성되었다. 중재 전후 초기 사회-의사소통 평가 척도(ESCS) 내 과제를 참고해 공동주의 행동 및 상호작용 행동을 측정하였고, 참여 아동의 보호자가 사회적 반응성 척도(SRS-2)를 평정하도록 하였다. 또한, 타인(연구자)에 맞추어 움직임(드럼 연주)의 타이밍을 조절하는지 측정하는 사회적 동기화 과제를 실시하였다. 연구 결과, 참여 ASD 아동은 초기 사회기술에 있어 유의한 향상을 보였고, 사회적 동기화 과제에서도 동기화 반응 속도를 제외한 동기화 정확도 및 지속시간에 있어 유의한 향상이 나타났다. SRS-2 척도에서는 유의한 차이가 나타나지 않았는데 이는 장기간 개입이 필요함을 시사한다. ASD 아동의 초기 사회기술 발달에 있어 인지-운동 통합이 중요한 역할을 하는 점을 고려할 때, 본 연구 결과는 인지-운동 통합 기반 악기 연주가 사회적 정보를 인지적으로 처리하고 이를 바탕으로 사회적 맥락에서 움직임을 조정하는 과정의 측면에서 ASD 아동의 사회기술 발달 증진을 위한 효과적인 방안이 될 수 있음을 시사한다.

Objectives : The aim of this study was to estimate the prevalences of electroencephalographic (EEG) abnormalities and epilepsy in children and adolescents with autism spectrum disorder (ASD). In addition, we intended to identify demographic and clinical correlates of epilepsy in ASD. Methods : A total of 140 children and adolescents (age $7.3{\pm}4.8yrs$, 106 boys) with ASD underwent EEG from January 2010 to December 2013 at Asan Medical Center. Medical records were reviewed for demographic information, clinical characteristics, psychiatric diagnoses and comorbidities, EEG findings and neurological diagnoses. Results : The prevalences of EEG abnormalities and epilepsy in children and adolescents with ASD was 62.1% and 38.6%, respectively. In subjects with seizure-like movements, EEG abnormalities and epilepsy were more frequent than those without seizure-like movements (EEG abnormalities : 92.5% vs. 43.7%, p<.001 ; epilepsy : 90.6% vs. 5.7%, p<.001). ASD subjects who had epilepsy were older (p=.001), had lower full scale intelligence quotient (p<.001) and took more antipsychotics (p=.006) than those who did not. Conclusion : The prevalences of EEG abnormalities and epilepsy in our sample were similar to those from Western countries. Our results suggested a possible association of older age, lower intelligence quotient, and antipsychotics use with epilepsy in ASD. Conduct of further prospective study in a larger sample is needed.
Background: The results differ among published studies regarding exposure to meconium and the risk of developing autism spectrum disorders (ASDs). Purpose: The present study pooled all of the epidemiologic studies retrieved from broader databases on the association between meconium exposure and risk of developing ASD in children. Methods: The Web of Science, PubMed, Scopus, and Google Scholar databases were searched without language restrictions for articles published between their inception to February 20, 2020, using relevant keywords. The pooled odds ratios (ORs) and their 95% confidence intervals (CIs) were calculated as random-effect estimates of the associations among studies. A subgroup analysis was conducted to explore any potential sources of heterogeneity among studies. Results: The pooled estimate of OR reported a weakly significant association between meconium exposure and ASD development in children (OR, 1.13; 95% CI, 1.03-1.24). There was low heterogeneity among the articles reporting risk for ASD among children (I2=19.3%; P=0.259). The results of subgroup analysis based on meconium exposure showed a significant association between a meconium-stained neonate and ASD development (OR, 1.18; 95% CI, 1.11-1.24). Couclusion: Meconium exposure was weakly associated with an increased risk of ASD. However, more evidence based on large prospective cohort studies is required to provide conclusive evidence about whether meconium exposure is associated with an increased risk of ASD development.
본 연구는 고기능 자폐스팩트럼장애(ASD) 아동을 대상으로 학교에서 보편적으로 경험하게 되는 사회적 상황을 기반으로 구성된 음악극을 적용하였을 때 의사소통기술에 어떠한 변화를 보이는 지 알아보고자 진행되었다. 본 연구에 적용된 음악극은 실제적 상황에 대한 이해, 음악 안에서의 목표 행동 및 언어적 표현의 습득, 역할극을 통한 상황적 맥락 내 목표 행동의 실제적 적용을 유도하는 단계적 구조로 구성되었다. 대상자는 평균 연령 9세의 ASD 아동 3명이며, 주 2회 40분씩 총 8회기의 그룹 중재가 진행되었다. 결과분석을 위해 또래 및 성인(즉, 연구자) 대상 의견 개시 및 조율 행동, 대상자 간 자발적 상호작용 행동을 관찰하였으며, 사회적 기술 평정척도 실시를 통해 중재 전후 일상생활에서 나타나는 사회적 행동에 변화가 있는지 비교하였다. 분석 결과, 모든 대상자는 또래 대상 의견 개시 및 조율 행동과 성인 대상 자발적 상호작용 행동이 증가한 것으로 나타났다. 반면 또래에게 나타나는 자발적 상호작용 행동과 성인 대상 의견 개시 및 조율 행동에 있어서는 대상자 언어 및 발달 수준에 따라 개별적인 차이가 있었다. 이러한 결과는 사회적 상황 기반 음악극이 ASD 아동이 경험하는 사회적 환경과 유사한 맥락을 제시함으로써 실제적으로 요구되는 의사소통 행동과 타인과의 상호작용 시 자신의 행동을 보다 효과적으로 조절하도록 촉진할 수 있는 가능성을 뒷받침한다. 또한, 본 연구는 ASD 아동의 사회적 의사소통 향상을 위한 중재의 범위를 확장하고, 다각적 전략을 제시했다는 데 그 의의가 있다.

Autism spectrum disorder (ASD) is a developmental disorder associated with cognitive and neurobehavioral disorders. It affects the person's behavior and performance. Autism affects verbal and non-verbal communication in social interactions. Early screening and diagnosis of ASD are essential and helpful for early educational planning and treatment, the provision of family support, and for providing appropriate medical support for the child on time. Thus, developing automated methods for diagnosing ASD is becoming an essential need. Herein, we investigate using various machine learning methods to build predictive models for diagnosing ASD in children using facial images. To achieve this, we used an autistic children dataset containing 2936 facial images of children with autism and typical children. In application, we used classical machine learning methods, such as support vector machine and random forest. In addition to using deep-learning methods, we used a state-of-the-art method, that is, automated machine learning (AutoML). We compared the results obtained from the existing techniques. Consequently, we obtained that AutoML achieved the highest performance of approximately 96% accuracy via the Hyperpot and tree-based pipeline optimization tool optimization. Furthermore, AutoML methods enabled us to easily find the best parameter settings without any human efforts for feature engineering.

Objectives: Among the many intervention programs for children with autism spectrum disorder (ASD), the Early Start Denver Model (ESDM) is one of the few approaches that has succeeded in demonstrating clinical efficacy in randomized control trials. Here, we investigate the clinical efficacy of ESDM intervention in young children with ASD in a community setting within Japan. Methods: All subjects were children with ASD who received ESDM intervention during the study period. Each ESDM session lasted 75 min and occurred once per week for at least 12 weeks. The outcome measures consisted of the Kyoto Scale of Psychological Development (K-test), Aberrant Behavior Checklist-Japanese version (ABC-J), and the Clinical Global Impression-Severity scale (CGI-S). Results: Twenty-seven subjects (29.4±6.4 months old) received ESDM intervention that lasted for 8.0±2.6 months on average. The score on Language and Social developmental quotient on the K-test increased significantly after the intervention. The total scores on the ABC-J and CGI-S significantly decreased after completion of the ESDM intervention. Conclusion: Our results suggest that ESDM intervention could reduce the severity of distinct clinical features of ASD, such as impairments in social interaction and communication assessed by the K-test, and maladaptive behavior rated by the ABC-J and CGI-S. We believe that the ESDM adapted to each institution might become one of the standard options for children with ASD in Japan.
Autism Spectrum Disorder (ASD) is characterized by persistent deficits in social communication and restricted, repetitive patterns of behavior and interest. Sleep problems are not uncommon in children with autism spectrum disorders. Symptoms of insomnia are the most frequent sleep problems in individuals with ASD. Sleep problems can cause significant difficulties in the daily life of children with ASD and their families. Genetic factor, deregulations of melatonin synthesis, extraneous environmental stimuli and psychiatric and medical conditions may cause sleep problems. The first line treatment of sleep problems in ASD includes managements for potential contributing factors and parent education about sleep hygiene care for child and behavioral therapy. Supplementation with melatonin may be effective before considering other medications, such as risperidone, clonidine, and mirtazapine.

This review explores the complexities of autism spectrum disorder (ASD), primarily focusing on the significant eating challenges faced by children and adolescents with this neurodevelopmental condition. It is common for individuals with ASD to exhibit heightened sensitivity to various sensory aspects of food such as taste, texture, smell, and visual appeal, leading to restricted and less diverse diets. These dietary limitations are believed to contribute to an imbalance in the gut microbiota. This review elaborates on how these eating problems, coupled with the distinctive characteristics of ASD, might be influenced by and, in turn, influence the gut-brain axis, a bidirectional communication system between the gastrointestinal tract and the brain. This discussion aims to shed light on the multifaceted interactions and potential implications of diet, gut health, and neurological development and function in children and adolescents with ASD.
Purpose : The purpose of this study was to investigate the differences in social quotient and sensory processing function between completed-toileting and incomplete-toileting children with autism spectrum disorder (ASD). We aims to provide a basis for the training of children with autism spectrum disorders. Methods : The subjects of this research were children with ASD, aged from 4 to 7, receiving sensory integration therapy at the H-Child Development Research Center in Busan, South Korea. After recruiting 30 parents who had children with ASD, we interviewed them and we divided their children into completed-toileting group and incomplete-toileting group based on the past researches. We assessed them using the Social Maturity Scale and Sensory Profile. Spearman correlation was used to analyze between the sensory processing score and social quotient. Mann-Whitney U test was used to compare the Sensory Profile scores of the two groups. Results : The social quotient was correlated with the sensory processing score in the most of component as for auditory processing, vestibular processing, touch processing, multisensory processing, orosensory processing, except for visual processing. The groups of completed-toileting and incomplete-toileting group showed statistically significant intergroup difference in sensory seeking, emotional reactive, oral-sensory sensitivity, and inattention/distractibility in the sensory factor region. No statistically significant difference from intergroup was observed in any modulation areas, nor in behavioral & emotional response domains. Conclusion : A significant intergroup difference was observed in the most of sensory processing areas, except for modulation areas and behavioral & emotional response domains. The result revealed that toileting is significantly correlated with sensory processing. Therefore, this study suggests that the sensory processing function should be considered in the toilet training of children with ASD.
