Incidence of apical crack formation and propagation during removal of root canal filling materials with different engine driven nickel-titanium instruments

  • Objectives: To determine the incidence of crack formation and propagation in apical root dentin after retreatment procedures performed using ProTaper Universal Retreatment (PTR), Mtwo-R, ProTaper Next (PTN), and Twisted File Adaptive (TFA) systems. Materials and Methods: The study consisted of 120 extracted mandibular premolars. One millimeter from the apex of each tooth was ground perpendicular to the long axis of the tooth, and the apical surface was polished. Twenty teeth served as the negative control group. One hundred teeth were prepared, obturated, and then divided into 5 retreatment groups. The retreatment procedures were performed using the following files: PTR, Mtwo-R, PTN, TFA, and hand files. After filling material removal, apical enlargement was done using apical size 0.50 mm ProTaper Universal (PTU), Mtwo, PTN, TFA, and hand files. Digital images of the apical root surfaces were recorded before preparation, after preparation, after obturation, after filling removal, and after apical enlargement using a stereomicroscope. The images were then inspected for the presence of new apical cracks and crack propagation. Data were analyzed with ${\chi}^2$ tests using SPSS 21.0 software. Results: New cracks and crack propagation occurred in all the experimental groups during the retreatment process. Nickel-titanium rotary file systems caused significantly more apical crack formation and propagation than the hand files. The PTU system caused significantly more apical cracks than the other groups after the apical enlargement stage. Conclusions: This study showed that retreatment procedures and apical enlargement after the use of retreatment files can cause crack formation and propagation in apical dentin.

Factors Affecting the Pulp and Root Healing of Root Fractures in Immature Permanent Teeth (미성숙 영구치의 치근 파절 시, 치수 및 치근 치유에 영향 미치는 요인들에 대한 분석)

  • The aim of this study was to evaluate the factors affecting the healing of root fracture in immature permanent teeth and the prognosis of root fracture by statistically analyzing the relationship with the pulp and root healing. The radiographs of 51 root-fractured maxillary anterior permanent teeth were collected. In radiograph, locations of root fracture, apical foramen width and width of diastasis between the fragments were measured. The value of the studied parameters were compared by independent t-test and rogistic regression test. In conclusion, there was no difference in the prognosis of pulp healing according to the location of root fracture. However, root healing occurs well as the root fracture is located at the root apex (p < 0.05). Lastly, the smaller the width of diastasis between the fragments after reduction, the better the pulp healing was (p < 0.05).

Finite element analysis of maxillary incisor displacement during en-masse retraction according to orthodontic mini-implant position

  • Objective: Orthodontic mini-implants (OMI) generate various horizontal and vertical force vectors and moments according to their insertion positions. This study aimed to help select ideal biomechanics during maxillary incisor retraction by varying the length in the anterior retraction hook (ARH) and OMI position. Methods: Two extraction models were constructed to analyze the three-dimentional finite element: a first premolar extraction model (Model 1, M1) and a residual 1-mm space post-extraction model (Model 2, M2). The OMI position was set at a height of 8 mm from the arch wire between the second maxillary premolar and the first molar (low OMI traction) or at a 12-mm height in the mesial second maxillary premolar (high OMI traction). Retraction force vectors of 200 g from the ARH (-1, +1, +3, and +6 mm) at low or high OMI traction were resolved into X-, Y-, and Z-axis components. Results: In M1 (low and high OMI traction) and M2 (low OMI traction), the maxillary incisor tip was extruded, but the apex was intruded, and the occlusal plane was rotated clockwise. Significant intrusion and counter-clockwise rotation in the occlusal plane were observed under high OMI traction and -1 mm ARH in M2. Conclusions: This study observed orthodontic tooth movement according to the OMI position and ARH height, and M2 under high OMI traction with short ARH showed retraction with maxillary incisor intrusion.

A COMPARISON OF THERMOPLASTICIZED INJECIABLE GUTTA-PERCHA TECHNIQUES IN RIBBON-SHAPED CANALS : ADAPTATION TO CANAL WALLS (리본 형태의 근관에서 열연화주입법의 근관벽에 대한 적합도에 관한 연구)

  • The aim of this study is to compare the adaptability of thermoplasticized injectable gutta-percha technique to the canal walls in ribbon-shaped canals. Thirty resin models simulated ribbon-shape canals were instrumented to #40 using .06 taper Profile systems. Three groups of each 10 resin models were obturated by the lateral condensation technique(LC) and the two thermoplasticized injectable gutta-percha technique; Ultrafil Endoset+Obtura II(EO) and Ultrafil Firmset(UF), respectively. After resin model were kept at room temperature for 4 days, they were resected horizontally with micro-tome at 1, 2, 3, 4 and 5mm levels from apex. At each levels. image of resected surface were taken using CCD camera under a stereomicroscope at $\times$40 magnification and stored. Ratio of the area of gutta-percha was obtained by calculating area of gutta-percha cone to the total area of canal using digitized image-ana-Iyzing program. The data were collected then analyzed statistically using One-way ANOVA. The results were as follows. 1 At 1mm levels, there was no statistically significant difference in the mean ratio of gutta-percha among the groups. 2. At 2mm level, EO showed the highest mean ratio of gutta-percha (p<0.05) and there was no significant difference between LC and UF. 3. At 3, 4, 5mm levels, EO and UF had significantly greater mean ratio of gutta-percha than LC(p<0.05) and there was no significant difference between EO and UF. In conclusion, the thermoplasticized injectable gutta-percha techniques demonstrated relatively favorable adaptability to canal walls than lateral condensation technique in ribbon-shaped canals except for 1mm level.

Hardware Architecture and its Design of Real-Time Video Compression Processor for Motion JPEG2000 (Motion JPEG2000을 위한 실시간 비디오 압축 프로세서의 하드웨어 구조 및 설계)

  • In this paper, we proposed a hardware(H/W) structure which can compress and recontruct the input image in real time operation and implemented it into a FPGA platform using VHDL(VHSIC Hardware Description Language). All the image processing element to process both compression and reconstruction in a FPGA were considered each of them was mapped into a H/W with the efficient structure for FPGA. We used the DWT(discrete wavelet transform) which transforms the data from spatial domain to the frequency domain, because use considered the motion JPEG2000 as the application. The implemented H/W is separated to both the data path part and the control part. The data path part consisted of the image processing blocks and the data processing blocks. The image processing blocks consisted of the DWT Kernel for the filtering by DWT, Quantizer/Huffman Encoder, Inverse Adder/Buffer for adding the low frequency coefficient to the high frequency one in the inverse DWT operation, and Huffman Decoder. Also there existed the interface blocks for communicating with the external application environments and the timing blocks for buffering between the internal blocks. The global operations of the designed H/W are the image compression and the reconstruction, and it is operated by the unit or a field synchronized with the A/D converter. The implemented H/W used the 54%(12943) LAB(Logic Array Block) and 9%(28352) ESB(Embedded System Block) in the APEX20KC EP20K600CB652-7 FPGA chip of ALTERA, and stably operated in the 70MHz clock frequency. So we verified the real time operation. that is. processing 60 fields/sec(30 frames/sec).

  • The structures to be demolished have become diverse in types from reinforced concrete to steel. The demand for demolition of steel structures is recently increasing in Korea. Most of flexible linear-shaped charges for steel demolition are now imported from foreign countries. To determine the optimum parameters of design far domestic development of flexible linear-shaped charges, some basic experiments have been carried out and their results are summarized as follows; Copper is shown to be superior to aluminium and lead as a liner material. It is also proved that the optimum apex angle of liner is 90$^{\circ}$ in comparision with 45$^{\circ}$, 60$^{\circ}$ and 120$^{\circ}$ Adequate thickness of liners, standoff distance in terms of quantity of explosives are also examined. Explosives and liners are required to be plasticized in order to improve the bond between explosives and various shapes of steel structures.

Induction of Shoot Primordium in Culture of Garlic (Allium sativum L.) (마늘 배양에 있어서 신초원기 유도)

  • Cultivated garlic, Allium sativum L. is economically important for leaves and bulbs, which historically were used in Korea for spices and condiments of Korean food as well as medicine crops. This experiment was carried out to investigate the effect of development and differentiation on culture of A. sativum (cv: white 6) by explant position, hormone composition and sucrose concentration in culture media. Culture method was investigated to induce shoot primordium. Culture efficiency was better with lower tissue of foliage leaf in explant position and on the medium with NAA 0.02 + BAP 1.0 mg/l in hormone composition than any other. Precocious shoot and callus were induced from shoot apex. Shoot was efficiently differentiated on 4,000 mg/l sucrose with increasing concentration of BAP. Shoot primordium was also induced with liquid rotary culture by histological observation. Rhizoid was induced from callus tissue cluster on medium with NAA 0.02 + BAP 2.0 mg/l.

A PHOTOELASTIC ANALYSIS OF STRESS DISTRIBUTIONS AROUND FIVE DIFFERENT TYPES OF ENDOSSEOUS IMPLANTS ACCORDING TO THEIR STRUCTURES (5종 골내 임플란트의 구조에 따른 주위의 응력분산에 관한 광탄성학적 연구)

  • This study was performed for the purpose of evaluating the stress distributions around five different types of implants according to their structures. The stress distribution around the surrounding bone was analysed by two-dimensional photoelastic method. Five epoxy resin models were made, and vertical and lateral forces were applied to the models. A circular polariscope was used to record the isochromatic fringes. The results of this study were summerized as follows : 1. Threaded type implants showed more even stress distribution patterns than cylinderical type implants when vertical and lateral forces were applied. 2. The stress concentrated patterns were observed at the neck portion and middle portion of the cylindrical type implants comparing with threaded type implants when vertical force was applied. 3. Model 1 and model 4 which are tthreaded type implants showed similar stress distribution patterns at the middle and apical portions and more stress was concentrated at the neck porion of model 1 comparing with model 4 when vertical force was applied. The stresses around model 1 were more evenly distributed when lateral force was applied. 4. More stress was concentrated at the neck and middle portion of cylindrical type implants than threaded type implants when lateral force was applied. 5. Model 1 showed the most even stress distribution patterns when lateral force was applied and stress distribution did no occured at the apical portion of modedl 2 when lateral force was applied. 6. There were almost no differences in stress concentrated patterns with or without having hollow design. And the stress concentrated patterns were observed at the corner of apex in model 5 which has hollow design when vertical force was applied.

Nerve Injury from Overfilled Calcium Hydroxide Root Canal Filling Paste for Maxillary Lateral Incisor Endodontic Treatment (상악 측절치 근관치료 중 수산화칼슘 호제근충제 과충전으로 인하여 발생한 신경손상의 치험례)

  • Calcium hydroxide root canal filing paste (vitapex) is widely used as canal filling paste for infected canal. However, chemical burn is possible because of the high alkali base of calcium hydroxide. A 57-year old woman was admitted to our clinic for consistent dull pain and paresthesia in the left upper lip, zygoma and buccal cheek area, which developed during an endodontic treatment of the left lateral incisor. Radiographic finding showed radiopaque material, which exits from the left incisor root apex, and was within the left canine and first premolar buccal soft tissue. The overfilled Vitapex extended to the soft tissue was surgically curetted. The result of the surgical curettage was favorable. Though slight hypoesthesia on the upper lip was still remained, paresthesia on zygomatic and buccal cheek area was completely recovered. As far as we know, this is the first case report of infraorbital nerve damage from overfilled Vitapex material.

Effects of orthodontic mini-implant position in the dragon helix appliance on tooth displacement and stress distribution: a three-dimensional finite element analysis (교정용 미니임플랜트 식립 위치에 따른 dragon helix의 효과에 대한 유한요소해석)

  • Objective: The purpose of this study was to investigate the stress distribution on the orthodontic mini-implant (OMI) surface and periodontal ligament of the maxillary first and second molars as well as the tooth displacement according to the OMI position in the dragon helix appliance during scissors-bite correction. Methods: OMIs were placed at two maxillary positions, between the first and the second premolars (group 1) and between the second premolar and the first molar (group 2). The stress distribution area (SDA) was analyzed by three-dimensional finite element analysis. Results: The maximal SDA of the OMI did not differ between the groups. It was located at the cervical area and palatal root apex of the maxillary first molar in groups 1 and 2, respectively, indicating less tipping in group 2. The minimal SDA was located at the root and furcation area of the maxillary second molar in groups 1 and 2, respectively, indicating greater palatal crown displacement in group 2. Conclusions: Placement of the OMI between the maxillary second premolar and the maxillary first molar to serve as an indirect anchor in the dragon helix appliance minimizes anchorage loss while maximizing the effect on scissors-bite correction.
